Increased Stretching of Mechanoreceptors in Superior Tarsal Muscle Reflexively Contracts Upper Trapezius Muscle as well as Levator Palpebrae Superioris and Occipitofrontalis Muscles as Eye-Eyelid-Eyebrow-Head Coordinated Movements: A Case Series.

To maintain a vertical visual field, fast-twitch fibers in the levator palpebrae superioris muscle (LPSM) stretch mechanoreceptors in the superior tarsal muscle (STM), which contracts the slow-twitch fibers in both the LPSM and the occipitofrontalis muscle (OFM). Exceeding the upgaze limit without head movement increases reflex contraction of the OFM to raise the eyebrows and pull the scalp backward while also causing involuntary contraction of the upper trapezius muscle (UTM) to extend the head backward, resulting in tension-type headaches (TTH) and neck pain. Intact thumb reflex in areflexic Guillain Barré syndrome: A novel phenomenon.

INTRODUCTION: Areflexia is one of the cardinal clinical features for the diagnosis of Guillain Barré syndrome. However, some patients may have sluggish proximal muscle stretch reflexes. Presence of thumb reflex, a distal stretch muscle reflex has not been documented in Guillain Barré syndrome. MATERIALS AND METHODS: We prospectively evaluated thumb reflex in Guillain Barré syndrome patients and age matched controls from April to September 2013. RESULTS: There were 31 patients with Guillain Barré syndrome in whom thumb reflex could be elicited in all (24 brisk, 7 sluggish), whereas all the ot

Revisiting hyper-resistance to muscle stretch in cerebral palsy: muscle hypo-extensibility is more of an issue than hyperreflexia.

Hyper-resistance to passive muscle stretch is a common, debilitating symptom of spastic paresis. Although straightforward to assess, hyper-resistance is caused by a complex interaction of altered tissue properties, stretch hyperreflexia, and involuntary background muscle activation. Identifying the contribution of each underlying component causing hyper-resistance is of great significance for designing treatments. The aim of this study was to investigate the components contributing to ankle plantarflexors' hyper-resistance in spastic cerebral palsy. We compared ankle biomechanical and reflex v

Effect of Low-Frequency Vibration on Muscle Response under Different Neurointact Conditions.

Stretch reflex is an important factor that influences the biomechanical response of the human body under whole-body vibration. However, there is a lack of quantitative evaluation at lower frequencies. Thus, the aim of this study was to investigate the effects of vibration on the stretch reflex and, in particular, to explore the quantitative relationship between dynamic muscle responses and low-frequency vibrations. The gastrocnemius muscle of 45 Sprague-Dawley rats was dissected. Sinusoidal vibrations of five discrete frequencies (2~16 Hz) with peak-to-peak amplitudes of 1 mm were applied to t

Passive, intrinsic and reflex-mediated stiffness in the ankle extensors of hemiparetic patients.

The total mechanical response to a stretch in a contracting muscle is the sum of the response from the properties of the fibres contracting prior to the stretch (intrinsic properties), the response from the passive tissues and the response from the stretch reflex-mediated contraction of the muscle fibres. The passive, the intrinsic and the reflex-mediated mechanical response to a 4 degrees stretch of the ankle extensors in both legs of nine spastic hemiparetic patients and, as control, the left ankle extensors in eight healthy subjects were measured. The stretch was imposed at the top of a mai

The calcium channel blocker nimodipine inhibits spinal reflex pathways in humans.

Voltage-sensitive calcium channels contribute to depolarization of both motor neurons and interneurons in animal studies, but less is known of their contribution to human motor control and whether blocking them has potential in future antispasmodic treatment in humans. Therefore, this study investigated the acute effect of nimodipine on the transmission of human spinal reflex pathways involved in spasticity. In a double-blinded, crossover study, we measured soleus muscle stretch reflexes and H reflexes and tibialis anterior cutaneous reflexes in 19 healthy subjects before and after nimodipine

Effect of muscle biomechanics on the quantification of spasticity.

The impact of muscle biomechanics on spasticity was assessed by comparison of the reflex responses of the elbow and metacarpophalangeal (MCP) flexor muscles in individuals with chronic spastic hemiplegia following stroke. Specifically, methods were developed to quantify reflex responses and to normalize these responses for comparison across different muscle groups. Stretch reflexes were elicited in the muscles of interest by constant velocity ramp-and-hold stretches at the corresponding joint. The muscles were initially passive, with the joint placed in a midrange position. Estimates of biomec

Flexor reflex responses triggered by imposed knee extension in chronic human spinal cord injury.

Hypersensitivity of the flexor reflex pathways to input from force-sensitive muscle afferents may contribute to the prevalence and severity of muscle spasms in patients with spinal cord injury (SCI). In this study, we triggered flexor reflexes with constant velocity knee movements in 15 subjects with SCI. Ramp and hold knee extension perturbations were imposed on one leg while the hip and ankle were held in an isometric position using an instrumented leg brace. Knee, ankle and hip torque responses and electromyograms from six muscles of the leg were recorded following controlled knee extension

5-HT1B/1D agonist CGS-12066B attenuates clasp knife reflex in the cat.

1. The effect of intrathecal injection of the selective serotonin (5-HT)1B/1D receptor agonist CGS-12066B maleate (825 nmol) was assessed on stretch-evoked clasp knife inhibition of hindlimb ankle extensor muscle reflex force in precollicular decerebrate cats in which neural transmission in dorsolateral spinal pathways was blocked bilaterally by focal cooling. 2. During cold block, ramp and hold stretches of the medial gastrocnemius muscle (MG) evoked only a brief reflex excitation that was followed by powerful, long-lasting inhibition (the clasp knife reflex). Hip proprioceptors preferentially modulate reflexes of the leg in human spinal cord injury.

Stretch-sensitive afferent feedback from hip muscles has been shown to trigger long-lasting, multijoint reflex responses in people with chronic spinal cord injury (SCI). These reflexes could have important implications for control of leg movements during functional activities, such as walking. Because the control of leg movement relies on reflex regulation at all joints of the limb, we sought to determine whether stretch of hip muscles modulates reflex activity at the knee and ankle and, conversely, whether knee and ankle stretch afferents affect hip-triggered reflexes. Testing spasticity mechanisms in chronic stroke before and after intervention with contralesional motor cortex 1 Hz rTMS and physiotherapy.

BACKGROUND: Previous studies showed that repetitive transcranial magnetic stimulation (rTMS) reduces spasticity after stroke. However, clinical assessments like the modified Ashworth scale, cannot discriminate stretch reflex-mediated stiffness (spasticity) from passive stiffness components of resistance to muscle stretch. The mechanisms through which rTMS might influence spasticity are also not understood. METHODS: We measured the effects of contralesional motor cortex 1 Hz rTMS (1200 pulses + 50 min physiotherapy: 3×/week, for 4-6 weeks) on spasticity of the wrist flexor muscles in 54 chroni

The activity of human muscle spindle endings in normal motor behavior.

In none of the above instances has the fusimotor system been found to be responsible for the described changes in stretch reflex gain. These findings do not prove that the fusimotor system is incapable of altering the gain of the stretch reflex; they do, however, establish that there are quite adequate mechanisms of gain control completely independent of the fusimotor system. One could reasonably question whether the fusimotor system ever operates primarily as a gain-controlling mechanism. That "more in" would probably result in "more out" cannot be disputed, but this may be of little real fun

Preservation of the masseter reflex in Friedreich's ataxia.

In Friedreich's ataxia, the muscle stretch reflexes are absent or hypoactive in the upper and lower extremities because of pathologic involvement of the dorsal root ganglion cells. Paradoxically, the masseter reflex remains unaffected and may even be hyperactive in some cases. The masseter reflex is hypothesized as unique among stretch reflexes in that its afferent nerve cell body (the mesencephalic nucleus) is located intra-axially within the brainstem rather than in the craniospinal ganglia, where the afferent nerve cell bodies subserving the other stretch reflexes are located. This study su

Terminal H-reflex Measurements in Mice.

The Hoffmann reflex (H-reflex), as an electrical analog to the stretch reflex, allows electrophysiological validation of the integrity of neural circuits after injuries such as spinal cord damage or stroke. An increase of the H-reflex response, together with symptoms like non-voluntary muscle contractions, pathologically augmented stretch reflex, and hypertonia in the corresponding muscle, is an indicator of post-stroke spasticity (PSS). In contrast to rather nerve-unspecific transcutaneous measurements, here, we present a protocol to quantify the H-reflex directly at the ulnar and median nerv

Stretch-Induced Muscle Responses Under Threat Versus Safety.

When in a stressful situation, making fast and accurate decisions is crucial. Previous work has shown that sensorimotor decisions can improve under threat. However, it is unclear if these improvements are achieved by improvements in perceptual or motor performance. Here, we present two hypotheses for how threat might influence motor preparation and use muscular stretch reflexes to test both. The task-unspecific hypothesis predicts that threat promotes motor preparation irrespective of the reach target, through tonic upregulation of the short latency stretch reflex. In contrast, the task-specif

Inhibition and potentiation of the exercise pressor reflex by pharmacological modulation of TRPC6 in male rats.

We determined the role played by the transient receptor potential canonical 6 (TRPC6) channel in evoking the mechanical component of the exercise pressor reflex in male decerebrated Sprague-Dawley rats. TRPC6 channels were identified by quadruple-labelled (DiI, TRPC6, neurofilament-200 and peripherin) immunohistochemistry in dorsal root ganglion (DRG) cells innervating the triceps surae muscles (n = 12). The exercise pressor reflex was evoked by statically contracting the triceps surae muscles before and after injection of the TRPC6 antagonist BI-749327 (n = 11; 12 μg kg-1) or SAR7334 (n = 11;

A Closed-Loop Method to Identify EMG-Torque Dynamics in Human Balance Control.

Human balance control requires continuous modulation of ankle torque by central and spinal activation of the ankle muscles combined with the intrinsic mechanical stiffness of the joint. These components appear together and cannot be measured separately. This work presents a novel multiple-input, single-output, closed-loop identification method that decomposes the ankle torque in human balance control into its central, stretch reflex, and intrinsic components. The method models separate transfer functions for each EMG-torque relation for central and stretch reflex mechanisms and estimates the a

Changes of reflex size in upper limbs using wrist splint in hemiplegic patients.

We evaluated the effect of prolonged wrist extension on H reflex in the flexor carpi radialis (FCR) muscle and tendon jerk (T) reflex in the biceps brachii (BB) muscle of 17 chronic hemiplegic patients. H reflex of the FCR and T reflex of the BB were assessed every 5 minutes within 20 minutes during prolonged wrist extension and post-20 minutes after the extension. As a result, H reflex in the FCR was reduced by passive wrist stretch in 82% of the spastic limbs. The effect was larger in the higher spastic group. In 45% of the spastic limbs, T reflex in the BB also was reduced by passive wrist
