Chronic Symptoms After Vestibular Neuritis and the High-Velocity Vestibulo-Ocular Reflex.

HYPOTHESIS: As the anterior and posterior semicircular canals are vital to the regulation of gaze stability, particularly during locomotion or vehicular travel, we tested whether the high-velocity vestibulo-ocular reflex (VOR) of the three ipsilesional semicircular canals elicited by the modified Head Impulse Test would correlate with subjective dizziness or vertigo scores after vestibular neuritis (VN). BACKGROUND: Recovery after acute VN varies with around half reporting persistent symptoms long after the acute episode. However, an unanswered question is whether chronic symptoms are associa

A Hybrid Extended Least Squares method (HybELS) for Vestibulo-Ocular Reflex identification.

The Vestibulo-Ocular Reflex (VOR) plays an essential role in the majority of daily activities by keeping the images of the world steady on the retina when either the environment or the body is moving. The modeling and identification of this system plays a key role in the diagnosis and treatment of various diseases and lesions, and their associated syndromes. Today, clinical protocols incorporate mathematical techniques for testing the functionality of patients' VORs through the analysis of the patients' responses to various stimuli. We have developed a new tool for simultaneous identification

Do predictive mechanisms improve the angular vestibulo-ocular reflex in vestibular neuritis?

Recovery from vestibular neuritis (VN) is often incomplete which leads to persistent vestibular imbalance during rapid head movements. Patients with unilateral vestibular lesions have a larger gain of the horizontal vestibulo-ocular reflex during active compared to passive head movements. To test whether this gain increase is related to predictive mechanisms we studied 15 patients with VN and 14 control subjects during predictable and unpredictable passive horizontal head impulses in the light and darkness. The vestibulo-ocular reflex showed a significantly shorter latency and higher gain in t

Adaptations and deficits in the vestibulo-ocular reflex after sixth nerve palsy.

PURPOSE: The effects of paralytic strabismus on the vestibulo-ocular reflex (VOR) have not been systematically investigated in humans. The purpose of this study was to analyze the VOR in patients with unilateral peripheral sixth nerve palsy. METHODS: Twenty-one patients with unilateral peripheral sixth nerve palsy (6 severe, 7 moderate, 8 mild) and 15 normal subjects were studied. Subjects made sinusoidal +/-10 degrees head-on-body rotations in yaw and pitch at approximately 0.5 and 2 Hz, and in roll at approximately 0.5, 1, and 2 Hz. Eye movement recordings were obtained using magnetic scler

Effects of adaptation of vestibulo-ocular reflex function on manual target localization.

The goal of the present study was to determine if adaptive modulation of vestibulo-ocular reflex (VOR) function is associated with commensurate alterations in manual target localization. To measure the effects of adapted VOR on manual responses we developed the Vestibular-Contingent Pointing Test (VCP). In the VCP test, subjects pointed to a remembered target following passive whole body rotation in the dark. In the first experiment, subjects performed VCP before and after wearing 0.5X minifying lenses that adaptively attenuate horizontal VOR gain. Results showed that adaptive reduction in hor

Oscillopsia: visual function during motion in the absence of vestibulo-ocular reflex.

OBJECTIVES: To investigate (1) the effects of loss of vestibular function on spatiotemporal vision and (2) the mechanisms which enable labyrinthine defective (LD) patients to adapt to oscillopsia. METHODS: Visual function and eye movements were assessed in seven normal subjects and four LD patients with oscillopsia due to absent vestibulo-ocular reflex. Temporal vision was assessed by measurement of threshold sensitivity for detection of a target which moved across a flickering, spatially uniform background field. Spatial vision was investigated by measurements of threshold sensitivity for th

Vestibulo-ocular reflex deficits to rapid head turns following intratympanic gentamicin instillation.

The response of the vestibulo-ocular reflex following unilateral vestibular deafferentation by gentamicin ablation was studied using transient stimuli. The response to these rapid passive head turns showed a strong asymmetry with permanent, reduced gains toward the side of lesion. These gain reductions have large variation (gains of 0.26 to 0.83), which may result from preferential sparing of regularly firing afferent fibers following gentamicin ablation. Based on the size and nature of the nonlinearity, an explanation based on Ewald's second law was discounted.

Compensation of total loss of vestibulo-ocular reflex by enhanced optokinetic response.

The gain in the optokinetic nystagmus reflex (OKR) of 27 labyrinthine-defective patients was compared with that of 27 subjects without any otoneurological abnormality, who were matched by age and sex. The patients without labyrinth function exhibited significantly higher OKR gains than the control subjects, especially so at higher age. The observed enhancement of OKR gain in the absence of vestibular function is ascribed to the effect of optokinetic training brought about by enhanced slippage of retinal images due to lack of an efficient vestibulo-ocular reflex.

Quantifying the vestibulo-ocular reflex with video-oculography: nature and frequency of artifacts.

Video-oculography devices are now used to quantify the vestibulo-ocular reflex (VOR) at the bedside using the head impulse test (HIT). Little is known about the impact of disruptive phenomena (e.g. corrective saccades, nystagmus, fixation losses, eye-blink artifacts) on quantitative VOR assessment in acute vertigo. This study systematically characterized the frequency, nature, and impact of artifacts on HIT VOR measures. From a prospective study of 26 patients with acute vestibular syndrome (16 vestibular neuritis, 10 stroke), we classified findings using a structured coding manual. Of 1,358 i

Effects of 4-aminopyridine on nystagmus and vestibulo-ocular reflex in ataxia-telangiectasia.

Ataxia-telangiectasia (A-T) is a progressive neurodegenerative disorder with prominent eye movement deficits localizing to the cerebellum. We sought to determine if 4-aminopyridine (4-AP), which putatively enhances the precision of Purkinje neurons, could improve the disorders of eye movements and vestibular function in A-T. The influence of 4-AP on disorders of eye movements and vestibular function was studied in four A-T patients. The effects on the cerebellar control of vestibulo-ocular reflex (VOR) was quantitatively assessed by the decay time constant of per- and post-rotational nystagmus

Suppression of the vertical vestibulo-ocular reflex in older persons at risk of falling.

A case-control design was used to assess eye-head-trunk coordination for community-dwelling elders performing a stand-walk task. Eighteen elders with a high risk of falling were matched to 18 subjects with low risk on the basis of age, living status (living alone or with someone) and category of residence (independent living or assisted living facility). Standard electro-oculography was used in conjunction with an electromagnetic tracking device to measure eye and head-trunk motion, respectively. For the low-risk group, the mean slope of gaze-head and gaze-trunk relationships was significantly

Vestibulocochlear implant (VCI) modulation improves mid and high frequency vestibulo-ocular reflex (VOR).

INTRODUCTION: The vestibulo-ocular reflex (VOR) stabilizes gaze during head movements by generating compensatory eye movements. In bilateral vestibulopathy (BV), loss of vestibular function leads to loss of VOR and consequently, oscillopsia. Currently, no curative treatment exists. Vestibulocochlear implants (VCI) aim to restore vestibular function by modulating electrical stimulation of the ampullary nerves in response to head motion. Although prior studies have demonstrated partial VOR restoration, a comprehensive evaluation across different stimulation conditions remains limited. This study

Poor gait performance is influenced with decreased vestibulo-ocular reflex in poststroke patients.

The vestibulo-ocular reflex (VOR) exerts a significant influence on gait performance. Therefore, a decrease in VOR function could worsen gait impairments in patients with poststroke hemiparesis. The effects of decreased VOR function on gait performance could be further exacerbated by aging-related physical weakness and impaired motor function of the hemiparetic lower limb. The aim of our study was to evaluate the possible synergistic effect of aging and impairment in lower extremity function and the VOR on walking ability of poststroke hemiplegic patients. The VOR was evaluated, using the gaze

The cerebellar nodulus/uvula integrates otolith signals for the translational vestibulo-ocular reflex.

BACKGROUND: The otolith-driven translational vestibulo-ocular reflex (tVOR) generates compensatory eye movements to linear head accelerations. Studies in humans indicate that the cerebellum plays a critical role in the neural control of the tVOR, but little is known about mechanisms of this control or the functions of specific cerebellar structures. Here, we chose to investigate the contribution of the nodulus and uvula, which have been shown by prior studies to be involved in the processing of otolith signals in other contexts. METHODOLOGY/PRINCIPAL FINDINGS: We recorded eye movements in two
