Energy intake to meet total energy expenditure improves iron deficiency and metabolic suppression in female long-distance runners: A case series study with dietary intervention.

BACKGROUND: Energy intake (EI) from daily meals, corresponding to total energy expenditure (TEE), is essential for maintaining good physical condition and performance in athletes. Athletes who continue high-intensity training while consuming insufficient energy from their daily diet are at risk for health problems, such as iron deficiency anemia, endocrine dysfunction, and energy metabolism disorders. OBJECTIVES: This study aimed to examine the altered health risks in athletes when EI was increased to meet TEE from a daily diet. DESIGN: Case series study with dietary intervention. METHODS:

Evidence from clinical studies of leptin: current and future clinical applications in humans.

Leptin has been established as the prototype adipose tissue secreted hormone and as a major regulator of several human physiology functions. Here, we are primarily reviewing the findings from studies in humans involving leptin administration. We are describing the metabolic, endocrine and immunologic effects of leptin replacement in conditions of leptin deficiency, such as short-term fasting in healthy individuals, relative energy deficiency in sports (REDS), congenital leptin deficiency (CLD), generalized (GL) and partial lipodystrophy (PL), HIV-associated lipodystrophy (HIV-L) and of leptin

Stress fractures and overuse injuries in children and adolescents.

UNLABELLED: There is an intricate connection between physical activity, bone health, and the susceptibility to stress fractures and overuse injuries. Physical activity has a positive impact on bone strength while a sedentary lifestyle can lead to a heightened risk for injury. The rise of early sports specialization has led to a substantial increase in overuse injuries, particularly in individual sports.Bone Stress Injuries (BSI) represent a category of overuse injuries closely linked to single sport specialization. BSI involves a spectrum of altered bone mechanics, ranging from edema of perios

Food Restriction Impairs Exercise-Induced Bone Mass Acquisition and Causes Iron Metabolic Disturbance with Storage Depletion in Young Female Rats.

Low energy availability is a core feature of Relative Energy Deficiency in Sport that impairs skeletal and hematologic health. However, the contributions of short-term energy restriction and exercise as well as their potential interactive effects remain unclear. This study aimed to clarify the effects of short-term food restriction on exercise-induced bone mass acquisition and its simultaneous impact on iron metabolism in young female rats. Female Sprague-Dawley rats (5 wk old) underwent environmental acclimation and treadmill running exercise training for 1 wk each and randomly grouped into s

Etiology and management of amenorrhea in adolescent and young adult women.

In this article, we will review the etiology and management of amenorrhea in adolescent and young adult women, beginning with the diagnostic work-up and followed by etiologies organized by system. Most cases of amenorrhea are caused by dysfunction of the hypothalamic-pituitary-ovarian (HPO) axis, which is the major regulator of the female reproductive hormones: estrogen and progesterone.  We begin by reviewing hypothalamic etiologies, including eating disorders and relative energy deficiency in sport. Then, pituitary causes of amenorrhea are reviewed, including hyperprolactinemia, empty sella

Body dissatisfaction, disordered eating and exercise behaviours: associations with symptoms of REDs in male and female athletes.

OBJECTIVES: Disordered eating and compulsive exercise behaviours are common among athletes and can increase the risk of relative energy deficiency in sport (REDs). Contrarily, the prevalence of muscle dysmorphia and its relationship with REDs are unknown. This cross-sectional study aimed to evaluate associations of all three with REDs symptoms. METHODS: Elite and subelite Icelandic athletes (n=83, 67.5% females) answered the Low Energy Availability in Females/Males Questionnaires (LEAF-Q/LEAM-Q), Eating Disorder Examination-Questionnaire Short (EDE-QS), Exercise Addiction Inventory (EAI) and

CRITICAL IMPLICATIONS OF FEMALE BONE METABOLISM IN ORTHOPEDICS: STATE OF THE ART.

Female bone metabolism is a dynamic and tightly regulated process that plays a pivotal role in musculoskeletal health and orthopedic outcomes across the lifespan. Bone remodeling depends on the balance between osteoblastic bone formation and osteoclastic bone resorption and is strongly influenced by hormonal status, nutrition, genetics, physical activity, and systemic disease. In women, this balance is profoundly affected by reproductive and aging-related transitions, including adolescence, pregnancy and lactation, perimenopause, menopause, and postmenopause. Maximal peak bone mass is typicall

The Preparticipation Physical Evaluation.

The preparticipation physical evaluation (PPE) is a common reason for young athletes to see a primary care physician. An annual PPE is required by most state high school athletic associations for participation in school-based sports, although there is limited evidence to support its effectiveness for detecting conditions that predispose athletes to injury or illness. In 2019, the American Academy of Pediatrics, with representatives from the American Academy of Family Physicians and other organizations, published updated PPE recommendations (PPE5). According to the guideline, the general goals

Determinants of Sports Injury in Young Female Swedish Competitive Figure Skaters.

Introduction: Although figure skating attracts several hundred thousand participants worldwide, there is little knowledge about physical health and sports injuries among young skaters. The present study aimed to describe the health status of a geographically defined Swedish population of licensed competitive figure skaters and to examine injury determinants. Methods: All licensed competitive skaters in the southeastern region of Sweden were in April 2019 invited to participate in a cross-sectional study using an online questionnaire. Multiple binary logistic regression was used for the examina

Ageing, Sex Differences, and REDs Risk in Endurance Runners: An Integrated Cross-Sectional Study Protocol.

Endurance performance is influenced by age- and sex-specific physiological determinants, while emerging evidence indicates an increasing prevalence of Relative Energy Deficiency in Sport (REDs) among both young and master endurance runners. Despite its clinical relevance, limited data exist on how long-term endurance training modulates REDs risk, skeletal muscle characteristics, and physiological ageing in comparison with inactive individuals. Methods: This cross-sectional study protocol will examine 112 participants stratified by sex, age (20-35 vs. 65-80 years), and training status (enduranc

Disordered eating, exercise dependence, body dissatisfaction, and fueling practices in female CrossFit® and CrossFit® plus endurance participants.

INTRODUCTION: Disordered eating (DE) is primarily observed in endurance athletes and sports that emphasize aesthetics. However, DE prevalence, exercise dependence (EXD), body dissatisfaction, and fueling habits in resistance-trained individuals-particularly CrossFit® athletes-are not well understood. CrossFit® promotes a culture that encourages strong, athletic, and muscular bodies. Both CrossFit® and endurance athletes often strive for low body fat percentages to enhance performance, which may contribute to body dissatisfaction, EXD, and eating concerns. This cross-sectional study investigate

Bone Stress Injuries: Diagnosis and Management.

Bone stress injuries (BSIs) are a spectrum of overuse injuries caused by an accumulation of microdamage, from high physical demands on normal bone or normal physiologic loads on structurally compromised bone. They typically result from overuse in younger patients but are also caused by pathologic bone conditions, including relative energy deficiency in sport, which features decreased bone mineral density. Stress fractures, representing 20% of BSIs, are the most severe type and feature discernable sclerosis or fracture lines on imaging. Without treatment, they can progress to complete fractures

Energy Availability and RED-S Risk Factors in Competitive, Non-elite Male Endurance Athletes.

Relative Energy Deficiency in Sport (RED-S) is predicated on the assumption that low energy availability (EA) induces deficiencies-dysfunction in multiple physiologic systems. However, research on RED-S and EA in male athletes is limited in comparison to women. The aim of this study is to investigate EA and the risk factors for RED-S, and their potential associations in non-elite male endurance athletes. Laboratory assessments for resting metabolic rate (RMR), bone mineral density (BMD), blood hormonal biomarkers and maximal aerobic capacity were conducted on 60 competitive, recreationally tra

A systematic nutrition intervention for low iron status in collegiate distance runners.

Background: Iron is a trace mineral that plays a significant role in oxygen transport and energy production during exercise. In deficiency, iron can have a significant negative impact on sports performance. Due to its relative simplicity, supplementation is a common treatment to combat deficiency. However, there is a paucity of analyses combining supplementation with dietary education as a method of treatment. Objective: To assess the effectiveness of a systematic iron intervention combining nutrition education and supplementation stages to combat low ferritin levels in collegiate runners. Met

Primary functional hypothalamic amenorrhoea in the context of feeding and eating disorders or excessive exercise: Insights from non-athletic and elite athletic populations.

Primary amenorrhoea is defined as the absence of menarche by the expected age, with or without delayed pubertal development in adolescents. It may result from anatomical, genetic, endocrine, and functional causes. Among the latter, we propose the term primary functional hypothalamic amenorrhoea (P-FHA) to describe primary amenorrhoea characterised by hypothalamic-pituitary-ovarian axis suppression in the absence of organic disease. P-FHA is frequently associated with low energy availability (LEA), particularly in individuals with feeding and eating disorders (FEDs) or excessive exercise, often
