Over the past few decades, U.S. cities have changed dramatically, largely because of two major trends: the fall of violence and the rise of urban inequality. Despite the attention given to each of these trends, little research has assessed how they are related to each other. This study is the first to generate causal evidence on the impact of violent crime on economic residential segregation. We document the effect of the crime drop on economic segregation in 500 U.S. cities between 1990 and 2010, using exogenous shocks to city crime rates to identify causal effects. We find that declining vio

OBJECTIVE: Trial of labor after cesarean (TOLAC) offers an opportunity to mitigate maternal morbidity disparities, which are associated with cesarean delivery rates as well as with racial segregation in obstetric care. This study sought to assess the association between the proportion of births to Black individuals at a hospital and the likelihood of low-risk patients undergoing TOLAC and achieving vaginal birth after cesarean (VBAC).
METHODS: This is a retrospective cohort study of 2017-2019 Nationwide Inpatient Sample deliveries with a history of prior cesarean delivery. We categorized hosp

BACKGROUND: Historical redlining, a 1930s residential segregation policy, has been associated with shorter breast cancer survival. It is unknown whether this disparity has changed over time, given the time since redlining increases and subsequent breast cancer advances have occurred. This study evaluated whether associations between historical redlining and all-cause survival of breast cancer have changed from 1995 to 2019.
METHODS: This New York State Cancer Registry-based cohort included 135,827 patients with breast cancer who were diagnosed between 1995 and 2019 and resided in a historical

BACKGROUND: While there is growing evidence of the negative impact of neighborhood segregation on maternal and infant health, the evidence for stillbirth is limited.
METHODS: A retrospective cohort study of live births and stillbirths for 2007-2020 was conducted in greater Houston, TX, to examine the associations of racialized economic segregation, assessed using the Index of Concentrations at the Extremes (ICE), with stillbirth. Here, ICE contrasted census tract-level racial and economic privilege with disadvantage (i.e., high-income non-Hispanic White persons vs. low-income Persons of Color

OBJECTIVE: Racial discrimination has been associated with unhealthy behaviors, but the mechanisms responsible for these associations are not understood and may be related to residential racial segregation. We investigated associations between self-reported racial discrimination and health behaviors before and after controlling for individual- and neighborhood-level characteristics; and potential effect modification of these associations by segregation.
DESIGN: We used data from the longitudinal Coronary Artery Risk Development in Young Adults (CARDIA) study for 1169 African-Americans and 1322

IMPORTANCE: Racial disparity in the use of prostate magnetic resonance imaging (MRI) presents obstacles to closing gaps in prostate cancer diagnosis, treatment, and outcome.
OBJECTIVE: To identify clinical, sociodemographic, and structural processes underlying racial disparity in the use of prostate MRI among men with a new diagnosis of prostate cancer.
DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study used mediation analysis to assess claims in the US Surveillance, Epidemiology, and End Results (SEER)-Medicare database for prostate MRI among 39 534 patients with a diagno

BACKGROUND: Traffic-related air pollutants have been associated with a variety of adverse human health impacts, including cancers. In the United States, numerous studies have documented racial inequities in neighborhood exposures to traffic-related air pollution. Emerging evidence suggests that structural racism may influence neighborhood exposures to air pollutants. However, existing research has largely focused on residential racial segregation, one indicator of structural racism. This study developed a multidimensional measure of structural racism to examine the relationship between structu

BACKGROUND: Racial/ethnic dementia disparities among older adults with end-stage kidney disease (ESKD) are likely driven by environmental and social injustices. We quantified the impact of fine particulate air pollution exposure (PM2.5; environmental injustice) by racial/ethnic segregation (social injustice) on dementia diagnosis in ESKD.
METHODS: We leveraged the US national registry to identify 884 950 older adults (age ≥55 years) who initiated dialysis (2003-2019). Adjusted Cox regression models with shared state-level frailty quantified the adjusted hazard ratio (aHR) of dementia by PM2.5

BACKGROUND AND OBJECTIVES: Increasingly, Black women are aging alone. Yet information about health correlates and the residential context where older Black women are aging-in-place is lacking. The current study examines one aspect of Aging While Black that affects Black women-living alone without close family or kin in the household - among a sample of older Black women (N = 890).
RESEARCH DESIGN AND METHODS: Using hierarchical linear models and data from the 2014/2015 and 2019 waves of the Southeastern Pennsylvania Household Health Survey, living alone status (e.g., Level 1) and area-level i

KEY POINTS: Regardless of race and ethnicity, older adults with kidney failure residing in or receiving care at dialysis facilities located in high-segregation neighborhoods were at a 1.63-fold and 1.53-fold higher risk of dementia diagnosis, respectively. Older adults with kidney failure residing in minority-predominant high-segregation neighborhoods had a 2.19-fold higher risk of dementia diagnosis compared with White individuals in White-predominant neighborhoods.
BACKGROUND: Dementia disproportionately affects older minoritized adults with kidney failure. To better understand the mechanis

IMPORTANCE: Historically redlined neighborhoods may experience disinvestment, influencing their likelihood of gentrification, a process of neighborhood (re-)development that unequally distributes harms and benefits by race and class. Understanding the combined outcomes of redlining and gentrification informs how the mutually constitutive systems of structural racism and racial capitalism affect pregnancy outcomes.
OBJECTIVE: To examine if historical redlining and contemporary gentrification is associated with increased severe maternal morbidity (SMM) odds.

In cross-sectional studies from the United States, single indicators of structural racism have been associated with adverse birth outcomes. Yet little is known about multidimensional measures of structural racism and birth outcomes, or about the impact of such measurement across generations. We constructed a multidimensional measure of structural racism and linked it to a multigenerational birth outcomes dataset of Black women from South Carolina who gave birth between 1989 and 2020. Each family tree had information on grandmothers' (Generation 1, G1) and mothers' (Generation 2, G2) structural

INTRODUCTION: Women and racial minority groups bear a disproportionate burden of stroke. Stroke disparities have been partially explained by gender and race differences in individual-level SES and neighborhood-level education and residential segregation. This study explored whether structural sexism and structural racism, operationalized as structural inequalities between gender groups and between race-ethnic groups across multiple social determinants of health at the county level, could be contributors by investigating their associations with stroke prevalence.
METHODS: County-level age-adju

OBJECTIVE: Previous research suggests that a shortage of nursing home beds in Latino communities and segregation within facilities in urban settings may contribute to low utilization patterns that both Latino and African American elders exhibit. In order to explore structural barriers to nursing home care for African American and Latino families, this study examines the supply and ethnoracial composition of nursing homes in Chicago communities.
DESIGN: With data from the 1990 US Census of Population and Illinois' 1994 Long-Term Care Facility Survey, regression was used to determine if Latino

BACKGROUND: Approximately 15% of women report depressive symptoms during pregnancy, with Black women more likely to report depressive symptoms prenatally compared with White women. Depressive symptoms are associated with small for gestational age infants and preterm birth. Neighborhood affordability, socioeconomic, ethnic, and racial segregation may adversely influence maternal mental health.
OBJECTIVE: We sought to estimate the effect(s) of residential racial composition (ie, percentage of Black residents living within the block group) and neighborhood socioeconomic factors on depressive sym

BACKGROUND: Ethnic enclaves are culturally or ethnically distinct neighborhoods with high concentrations of individuals with shared ethnic origins, immigrants, and/or ethnic-specific businesses. We examined whether residence in Asian American enclaves was associated with the incidence of three screening-amenable cancers, using a composite enclave index across five US states.
METHODS: Using cancer registry data, we identified 74,485 breast, 4,134 cervical, and 35,736 colorectal cancer tumors diagnosed in California, Florida, New Jersey, New York, and Texas from January 1, 2006, to December 31,

The U.S. has merely 4% of the world population, but contains 25% of the world's COVID-19 cases. Since the COVID-19 outbreak in the U.S., Massachusetts has been leading other states in the total number of COVID-19 cases. Racial residential segregation is a fundamental cause of racial disparities in health. Moreover, disparities of access to health care have a large impact on COVID-19 cases. Thus, this study estimates racial segregation and disparities in testing site access and employs economic, demographic, and transportation variables at the city/town level in Massachusetts. Spatial regressio

BACKGROUND: Formerly incarcerated individuals (FIIs) encounter difficulties with covering the cost of dental and medical care, adhering to medication regimens, and receiving fair treatment from health care providers. Yet, no published research has examined modifiable pathways to increase FIIs' health literacy (HL), which is essential for addressing the health needs of this vulnerable population.
OBJECTIVE: The aim of this article is to examine neighborhood characteristics (neighborhood deprivation, racial and economic polarization, and residential segregation) and public assistance program en

Racial, ethnic, and socioeconomic disparities persist in American medicine. Black pregnant people die from pregnancy-related causes at 2 to 3 times the rate of their White counterparts, with cardiovascular disease as the leading contributor. These disparities in cardio-obstetrics arise from intersecting structural forces-economic inequity, residential segregation, limited health care access, implicit bias, and the cumulative physiological toll of structural racism. Effective solutions require a multifaceted action: community health worker programs, group prenatal care, hospital quality improve

OBJECTIVE: Rapid opioid reduction or discontinuation among patients on high-dose long-term opioid therapy (HD-LTOT) is associated with increased risk of heroin use, overdose, opioid use disorder, and mental health crises. We examined the association of residential segregation and health care access with rapid opioid reduction or discontinuation among patients on HD-LTOT and examined effect measure modification of individual-level characteristics.
METHODS: Using 2006-2018 North Carolina private insurance claims data, we conducted a retrospective cohort study of patients who were 18-64 years of
