Echocardiography in Pandemic: Front-Line Perspective, Expanding Role of Ultrasound, and Ethics of Resource Allocation.

The grave clinical context of the coronavirus disease 2019 (COVID-19) pandemic must be understood. Italy is immersed in the COVID-19 pandemic. Most of the world will soon follow. The United States currently has the most documented cases of COVID-19 of any nation. Severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2)-associated acute cardiomyopathy is common in critical care patients and is associated with a high mortality rate. Patients with COVID-19 frequently require mechanical support for adequate oxygenation. A severe shortfall of ventilators is predicted. Of equal concern is the pr

Homemakers and heights. Intra-household resource allocation and male stature in the Netherlands, 1860-1930.

In this article we test the hypothesis that the secular increase in heights in the course of the second half of the nineteenth century was associated with the rise of the breadwinner-homemaker household. In these 'modern' households, women raised the living standards (quality and quantity of food, hygiene and care) for all members, especially the children. We model the assumed contributions to the family budget by age and gender of household members, and find that a strong imbalance between consumers and producers in the household put severe strains on effective resource allocation, leading to

Burn Injury Cutaneous Functional Units: Allocating Occupational Therapy Resources and Influencing Practice.

Burn injury severity is currently determined by estimating the total body surface area (TBSA) burned, but this method fails to capture the severity of subsequent functional consequences. Alternatively, cutaneous functional units (CFUs)--skin segments that accommodate for movement and commonly become contracted--can be used to more accurately estimate burn severity based on functional consequences. Bilateral hands account for 81.3% of the body's CFUs, though only account for 5% of TBSA. However, hand burn survivors can have worse physical outcomes (ie, contractures, deformities) after burn inju

COVID-19 in West Africa: regional resource mobilisation and allocation in the first year of the pandemic.

The world continues to battle the ongoing COVID-19 pandemic. Whereas many countries are currently experiencing the second wave of the outbreak; Africa, despite being the last continent to be affected by the virus, has not experienced as much devastation as other continents. For example, West Africa, with a population of 367 million people, had confirmed 412 178 cases of COVID-19 with 5363 deaths as of 14 March 2021; compared with the USA which had recorded almost 30 million cases and 530 000 deaths, despite having a slightly smaller population (328 million). Several postulations have been made

Simultaneous Liver-Kidney Allocation Policy: A Proposal to Optimize Appropriate Utilization of Scarce Resources.

The introduction of the Mayo End-Stage Liver Disease score into the Organ Procurement and Transplantation Network (OPTN) deceased donor liver allocation policy in 2002 has led to a significant increase in the number of simultaneous liver-kidney transplants in the United States. Despite multiple attempts, clinical science has not been able to reliably predict which liver candidates with renal insufficiency will recover renal function or need a concurrent kidney transplant. The problem facing the transplant community is that currently there are almost no medical criteria for candidacy for simult

The Attentional Resource Allocation Scale (ARAS): psychometric properties of a composite measure for dissociation and absorption.

BACKGROUND: Differences in attentional processes have been linked to the development and maintenance of psychopathology. Shifts in such processes have been described by the constructs Dissociation and Absorption. Dissociation occurs when external and/or internal stimuli are excluded from consciousness due to discrepant, rather than unitary, manifestations of cognitive awareness [Erdelyi MH. 1994: Int J Clin Exp Hypnosis 42:379-390]. In contrast, absorption can be conceptualized by a focus on limited stimuli, to the exclusion of other stimuli, because of unifying, rather than discrepant, manife

Social Status and Clinical Resource Allocation by a Large Language Model: An Evaluation of 30,618 Decisions.

Objective: The objective was to quantify whether demographic and social attributes that were irrelevant to stated clinical need, prognosis, and expected benefit altered resource-allocation decisions made by a general-purpose large language model (LLM). Methods: We conducted a cross-sectional audit of the gpt-5-chat-latest API model alias on 8 October 2025, across seven clinical vignettes, generating 30,618 forced-choice comparisons between patient profiles. Profiles varied across a full-factorial combination of eight demographic and social attributes while clinical need, prognosis, and expecte

Optimal Allocation of Societal HIV Prevention Resources to Reduce HIV Incidence in the United States.

Objectives. To optimize combined public and private spending on HIV prevention to achieve maximum reductions in incidence.Methods. We used a national HIV model to estimate new infections from 2018 to 2027 in the United States. We estimated current spending on HIV screening, interventions that move persons with diagnosed HIV along the HIV care continuum, pre-exposure prophylaxis, and syringe services programs. We compared the current funding allocation with 2 optimal scenarios: (1) a limited-reach scenario with expanded efforts to serve eligible persons and (2) an ideal, unlimited-reach scenari

Surgical Transfer Decision Making: How Regional Resources are Allocated in a Regional Transfer Network.

BACKGROUND: Tertiary care centers often operate above capacity, limiting access to emergency surgical care for patients at nontertiary facilities. For nontraumatic surgical emergencies there are no guidelines to inform patient selection for transfer to another facility. Such decisions may be particularly difficult for gravely ill patients when the benefits of transfer are uncertain. METHODS: To characterize surgeons' decision-making strategies for transfer, a qualitative analysis of semistructured interviews was conducted with 16 general surgeons who refer and accept patients within a regiona

Bridging energy and proteome allocation models allows prediction of the resource cost of traits across environments.

Many traits divert resources from reproduction and are costly due to the physiological trade-offs organisms face. These traits might require the allocation of different resources, e.g., energy expenditure or production of macromolecules, and it is unclear how their costs can be compared given their different units. Moreover, the effect of environmental conditions and genetic background on the cost of traits is not well understood. Two different frameworks have been proposed to quantify the resource costs of traits, focusing on either energy allocation or proteome allocation. These frameworks i

The Mitochondrial AtPHT3;3 Transporter Is Required for Resource Allocation During Late Embryo Development and Seed Maturation.

Mitochondrial transport processes are critical for oxidative phosphorylation; however, the physiological roles of many members of the plant mitochondrial carrier family remain poorly characterized. In Arabidopsis thaliana, three PHOSPHATE TRANSPORTER 3 (PHT3) proteins are identified as potential mitochondrial phosphate carriers, but only PHT3;1 and PHT3;2 have been investigated. Here, we examined the least studied member of this family, AtPHT3;3, which exhibits elevated expression in reproductive tissues. After confirming the subcellular localization of PHT3;3 to mitochondria, we performed inv

Dual-Strategy Resource Allocation in an Invasive Clonal Plant to Counter Foliar Herbivory for Rapid Recovery.

Clonal integration (i.e., sharing resources via interconnected ramets) is recognized as a key mechanism enabling clonal plants to mitigate herbivory. However, how the magnitude and direction of this response vary between invasive and native clonal plants under spatially heterogeneous herbivory remains unresolved. In this study, we subjected the invasive alligator weed, Alternanthera philoxeroides, and its native congener, sessile joyweed A. sessilis, to varying feeding position treatments by the flea beetle Agasicles hygrophila (damage to apical ramets, basal ramets, or both ramets) and herbiv

Sexual mixing patterns in men who have sex with men: network approaches for smart resource allocation.

BACKGROUND: Age-based sexual mixing patterns in men who have sex with men (MSM) can greatly inform strategic allocation of intervention resources to subsets of the population for the purpose of preventing the greatest number of new HIV infections. METHODS: Egocentric network data collected from MSM participating in annual HIV sentinel surveillance surveys were used to assess age-dependent mixing and to explore its epidemiological implications on the risk of HIV transmission risk (among those HIV-infected) and HIV acquisition risk (among those not infected). RESULTS: Mixing in this sample of

Risk Factor-Guided Early Discharge and Potential Resource Allocation Benefits in Patients with Traumatic Subarachnoid Hemorrhage.

OBJECTIVE: We sought to develop screening criteria predicting the lack of poor neurologic outcomes in patients presenting with traumatic subarachnoid hemorrhage (tSAH) and to evaluate their potential to improve resource allocation in these cases. METHODS: We retrospectively reviewed patients presenting with tSAH to the emergency department (ED) of a tertiary-care institution from 2016 to 2018. We defined good neurologic outcomes as patients with stable/improving neurologic status, who did not require neurosurgical intervention, had no expanding bleed, and needed no hospital readmission. Strategic resource allocation for malaria elimination in endemic settings: a systematic review of cost-effectiveness evidence.

INTRODUCTION: Malaria remains a significant global health burden, especially in endemic regions where efficient use of limited resources is critical. Economic evaluations provide essential evidence to guide strategic resource allocation and optimise intervention outcomes. This systematic review synthesises recent evidence (2018-2025) on the cost-effectiveness of malaria prevention and treatment interventions in endemic settings. MATERIALS AND METHODS: A systematic search of Scopus, Web of Science, and PubMed identified English-language studies published between 2018 and 2025. Eligible studies

Distributed Event-Triggered Algorithm Designs for Resource Allocation Problems via a Universal Scalar Function-Based Analysis.

In this article, we are concerned with distributed algorithm designs for resource allocation problems via event-triggered communication. The target is to search an optimal resource allocation scheme such that the summation of objective functions is minimized. Due to communication efficiency and privacy concerns, distributed algorithms with event-triggered communications are proposed in this article. The communication is only permitted or triggered if variation of gradient of the local objective function exceeds a threshold. By constructing a novel technical lemma and a universal scalar functio

Guideline implementation for breast healthcare in low- and middle-income countries: breast healthcare program resource allocation.

Breast cancer is serious public health problem in countries of all resource levels. Although major advances in the detection and treatment of the disease have occurred in higher income settings, similar progress has been slow or scarce in most low- and middle-income countries (LMCs). The poorer outcomes in LMCs may relate to the limited capability of their healthcare systems (HCS) to provide successful early detection, diagnosis, and treatment of breast cancer. Impediments to better outcomes include insufficient numbers of appropriately trained healthcare workers, limited access to screening/t

How to synergize natural resource supply-demand mismatching to allocate the most suitable NbS measures?

The application of Nature-based Solutions (NbS) measures can enhance the supply capacity of natural resources and provide essential support for sustainable development. However, NbS planning often lacks a scientific basis that integrates local ecological conditions with socio-economic needs. In response to the ability of different types of NbS to provide a variety of natural resource supply remedies, this study proposed a methodology which is applied to the Yangtze River Economic Belt (YREB) in China, considering four major NbS types (climate-oriented, food-oriented, recreation-oriented, and w
