[New aspects on the pathogenesis of patent ductus arteriosus in premature infants].

It is generally accepted that vasodilator prostaglandins are involved in the pathogenesis of persistent ductus arteriosus (PDA) in preterm infants suffering from respiratory distress syndrome (RDS). When studying the prostaglandin metabolism it became apparent that in about 80% of these infants the activity of PGI2 and/or PGE2 was increased. In parallel to weaning infants from the respirator a decrease in prostaglandin activity was observed which was associated with ductal closure. The inhibition of prostaglandin synthesis with indomethacin had the same effect. Considering the various lines of

Differential cardiovascular effects mediated by mu and kappa opiate receptors in hindbrain nuclei.

To further investigate the role of opioid peptides and specific opiate receptor subtypes in central cardiovascular regulation by hindbrain nuclei, mu (D-Ala2,MePhe4,Gly-ol5 enkephalin, DAGO), delta (D-Ala2,D-Leu5 enkephalin, DADL) or kappa (MRZ 2549) agonists were microinjected into hindbrain nuclei of spontaneously or artificially respired, pentobarbital-anesthetized rats. In the nucleus tractus solitarius (NTS), DAGO and DADL (0.3 nmol) elicited pressor responses and tachycardia. MRZ (3.0-16 nmol) depressed blood pressure in spontaneously breathing rats, but accelerated heart rate in artific

[The effect of dead space fraction on weaning from mechanical ventilation in COPD patients].

BACKGROUND: Chronic Obstructive Pulmonary Disease (COPD) is a common disease that tends to occur worldwide. Physiologically, COPD represents a disruption in ventilation and in the exchange of gases in the lungs. Laboratory tests indicate elevated CO2 level, gradual reduction of the levels of oxygen and pH in arterial blood, and a consequent rise in the Dead Space Fraction (DSF) of the lungs. OBJECTIVE: Patients with COPD represent a large portion of those artificially ventilated in the Intensive Care Unit (ICU). In an attempt to develop a prognostic tool for the length of treatment, we compar

Successful treatment of a 12-year-old boy with Guillain-Barré syndrome requiring tracheostomy due to respiratory muscle paralysis: A case report.

Childhood Guillain-Barré syndrome (GBS) occasionally leads to respiratory failure early after onset, requiring long-term ventilation management after tracheal intubation. However, patients requiring tracheostomy management are rare. In the present study, a case of a 12-year-old boy with GBS who required artificial respiration management due to rapid progression of respiratory muscle paralysis is reported. Intravenous immunoglobulin (IVIg) and pulse steroid therapy were provided; however, both were ineffective and tracheostomy was necessary 26 days after onset. A second course of IVIg and pulse

[Mechanical ventilation in infants and children].

Mechanical ventilation is a palliative treatment of acute respiratory failure. It is indicated whenever no immediate and effective causative treatment is applicable. The respiratory pump defect is counterbalanced by an artificial ventilation. The causes of acute respiratory failure extend far beyond the field of pulmonary pathology. The main respiratory characteristics of children are due their high mean metabolic status: they have high minute respiration, fast respiratory rate and I/E ratios that are near to 1. Two categories of respirators are used in the 28 days to 15 years age range. Infan

Mitochondria from human term placenta. II. Characterization of respiratory pathways and coupling mechanisms.

Pathways of electron transport utilized for respiration in human term placental mitochondrial preparations were differentiated and characterized through the use of classical respiratory chain inhibitors and multiple sources of reducing equivalents. Mechanisms of associated energy conservation and utilization were examined in the preparations with uncouplers and inhibitors of phosphorylation. Inhibition by rotenone, antimycin A and cyanide established the classical electron transport chain as the major pathway of respiration with glutamate and succinate as substrates. Approximately 20% of gluta

Ventilator-associated sinus arrhythmia in a preterm neonate--an indicator for a mature autonomic nervous system?

According to control theory, the interactions between respiration and heart rate (i.e. respiratory sinus arrhythmia, RSA, and breath amplitude sinus arrhythmia, BASA) reflect the inner workings of the physiological control systems of respiration and circulation. This paper reports on a preterm neonate (28.5 weeks old, 940 g) who showed the presence of ventilation-associated sinus arrhythmia (VASA) under moderate artificial ventilation. His heart rate variability was entrained to the ventilatory stimulus. VASA and entrainment suggest that the parasympathetic part of the autonomic nervous system

Relationship Between Depressive State and Treatment Characteristics of Acute Cervical Spinal Cord Injury in Japan.

BACKGROUND: Few studies have assessed whether treatment of acute cervical spinal cord injury (SCI) patients contributes to depression. METHODS: Using an administrative database, we assessed patients for whom the diagnosis was unspecified injuries of cervical spinal cord (International Classification of Diseases and Injuries-10th (ICD-10) code; S14.1). We categorized patients with codes for depressive episode (ICD-10 code; F32) or recurrent depressive disorder (F33), or those prescribed antidepressants (tricyclic, tetracyclic, Selective Serotonin Reuptake Inhibitors, Serotonin Noradrenaline Re

Alveolar crystal burden in stone workers with artificial stone silicosis.

BACKGROUND AND OBJECTIVE: An epidemic of silicosis has emerged due to a failure to control risks associated with exposure to high-silica content respirable dust generated while working with artificial stone products. Methods for quantification of alveolar crystal burden are needed to advance our understanding of the pathobiology of silica-related lung injury as well as assisting in the diagnosis, clinical management and prognostication of affected workers. The objective of this study was to develop and validate novel methods to quantify alveolar crystal burden in bronchoalveolar lavage (BAL) f

Respiratory function after lesions in medulla oblongata.

OBJECTIVES: To evaluate the correlation of lesions of the brain as visualized in cranial magnetic resonance imaging (MRI) and the ability of spontaneous respiration. METHODS: In a prospective concept, cranial MRI after traumatic brain injury or spontaneous intracerebral hemorrhage was performed in 250 subjects at an early stage. All MRI findings were correlated with respiratory conditions on the day of examination. Sedation was performed only to facilitate toleration of the artificial ventilation, as and when necessary. Spontaneous respiration could hence be registered clinically. RESULTS: T

Patterns of bulbar respiratory neurons during and after artifical hyperventilation.

Single bulbar respiratory unit activity was recorded in urethane-anesthetized rabbits. During artificial ventilation, reflex contractions of the diaphragm were elicited by the forced deflations of the respirator. Out of 44 neurons, four I, one E and two EI units exhibited a shift of their discharge relative to the contractile phase of the diaphragm, compared to normal respiration. During hypocapnic apnea, some I, IE and E neurons became silent. Other cells belonging to all phase types, however, continued to fire tonically at a rate less than the peak rate in normal bursts. When rhythmic respir

[Controlled hyperventilation with dead space (author's transl)].

We report on a new kind of ventilation-hyperventilation, whereby dead space is added between the patient and the respirator. This kind of ventilation is compared with positive end expiratory pressure ventilation together with intermittent sighs on 10 dogs. Over a period of 80 hours no respiratory or circulatory differences were found in the two groups. There also was no difference in clinical or anatomic histologic findings. It was demonstrated that dogs can be artificially ventilated for a much longer period than estimated. The findings of "respirator lungs" were only shown in an emphysema of

Assessment of half-mask elastomeric respirator and powered air-purifying respirator reprocessing for an influenza pandemic.

BACKGROUND: Health care facilities are considering the use of reusable respiratory protective devices (RPDs) to mitigate a potential N95 filtering facepiece respirator shortage caused by an influenza pandemic. US regulators are also considering stockpiling reusable RPDs for pandemic preparedness, but limited data exist on the effectiveness of cleaning and disinfection of these devices. This study defines reprocessing protocols and evaluates their effectiveness against a pandemic influenza strain in a laboratory setting. METHODS: Five half-mask elastomeric respirator models and 3 powered air-p

Degree of dependence on the ventilator according to sleep states in artificially ventilated premature infants.

Polygraphic recordings were performed in 14 sleeping premature infants receiving ventilation for respiratory distress syndrome. All were clinically stabilized, with normal EEG and neurologic status and differentiated sleep states (coded according to EEG and REM criteria). They all had two respiratory patterns: passive, completely dependent on the ventilator, and active, with autonomous respiratory movements and/or inspiratory diaphragmatic activity added to passive respiration. We found that in infants ventilated at the rate of 18-54/min, respiration was more active and autonomous in active RE

[The unstable thoracic wall: possibilities for treatment].

In the treatment of severe chest injuries with flail chest either positive-pressure mechanical ventilation (and tracheostomy) is necessary or the surgical stabilisation of the chest wall by osteosyntheses of the broken ribs. Fourteen patients (age 27-73) with severe chest injuries and paradoxical respiration were operated and the rib fractures stabilized by means of small reconstruction plates and cerclage wires. Three patients died due to other reasons. There were no serious complications. The results are reported. In severe chest injuries surgical treatment seems to be justified; it reduces

[Patient-noninvasive mechanical ventilation interaction].

Noninvasive mechanical ventilation is one more step in the treatment of patients with acute respiratory failure. In addition to gas exchange disorders, its primary indication to initiate it is the presence of signs of respiratory muscles fatigue. To assure successful mechanical ventilation, the ventilator and patient must be synchronized, that is, the effort the patient makes to start inspiration is recognized by the ventilator and it quickly delivers gas flow, that the flow provided by the ventilator adapts to the flow need of the patient during delivery of gas phase and that the ventilator r

Sperm-Cultured Gate Ion-Sensitive Field-Effect Transistor for Non-Optical and Live Monitoring of Sperm Capacitation.

We have successfully monitored the effect of progesterone and Ca2+ on artificially induced sperm capacitation in a real-time, noninvasive and label-free manner using an ion-sensitive field-effect transistor (ISFET) sensor. The sperm activity can be electrically detected as a change in pH generated by sperm respiration based on the principle of the ISFET sensor. Upon adding mouse sperm to the gate of the ISFET sensor in the culture medium with progesterone, the pH decreases with an increasing concentration of progesterone from 1 to 40 μM. This is because progesterone induces Ca2+ influx into sp
