Do respiratory comorbidities limit the diagnostic usefulness of ultrasound-guided needle aspiration for subpleural lesions?

BACKGROUND: The usefulness of ultrasound-guided needle aspiration for subpleural lesions has been reported. However, no reports have evaluated its usefulness and safety in patients with respiratory comorbidities such as chronic obstructive pulmonary disease (COPD) and interstitial pneumonia (IP), which can increase the risk of iatrogenic pneumothorax. In this study, we evaluated the influence of chronic respiratory diseases (CRDs) on the usefulness and safety of ultrasound-guided needle aspiration for subpleural lesions. METHODS: Between January 2000 and September 2011, we examined 144 patien

Identifying the association between post-extubation laryngeal edema and aspiration in acute respiratory failure patients.

BACKGROUND: Following greater than or equal to 48-h intubation, one-third of acute respiratory failure (ARF) patients aspirate. Laryngeal edema has been identified as a potentially modifiable risk factor of post-extubation aspiration. The aim of this case-control study was to characterize the anatomical distribution of post-extubation laryngeal edema and to correlate anatomical laryngeal edema severity with aspiration risk in ARF survivors. The study also assessed whether patient weakness influenced the relationship between laryngeal edema and aspiration risk. METHODS: Flexible endoscopic eva

To take a different approach: Can large language models provide knowledge related to respiratory aspiration?

OBJECTIVE: To investigate the performance (accuracy, comprehensiveness, consistency, and the necessary information ratio) of large language models (LLMs) in providing knowledge related to respiratory aspiration, and to explore the potential of using LLMs as training tools. METHODS: This study was a non-human-subject evaluative research. Two LLMs (GPT-3.5 and GPT-4) were asked 36 questions (32 objective questions and four subjective questions) about respiratory aspiration in English and Chinese. Responses were scored by two experts against gold standards derived from authoritative books. The a

Relationship Between Laryngeal Sensation, Length of Intubation, and Aspiration in Patients with Acute Respiratory Failure.

Dysphagia is common in hospitalized patients post-extubation and associated with poor outcomes. Laryngeal sensation is critical for airway protection and safe swallowing. However, current understanding of the relationship between laryngeal sensation and aspiration in post-extubation populations is limited. Acute respiratory failure patients requiring intensive care unit admission and mechanical ventilation received a Flexible Endoscopic Evaluation of Swallowing (FEES) within 72 h of extubation. Univariate and multivariable analyses were performed to examine the relationship between laryngeal s

The use of noninvasive ventilation in acute respiratory failure associated with oral contrast aspiration pneumonitis.

Noninvasive ventilation (NIV) has been used to treat patients with acute respiratory failure, including cases of pneumonia. We used this technique in the management of an 83-year-old patient with acute respiratory failure secondary to inadvertent administration of oral contrast material into the lung, and who did not want to be intubated. NIV resulted in immediate improvement of respiratory status. The patient was weaned from NIV over the next 24 hours and eventually discharged from the hospital.

The Diagnostic Accuracy of the Pitch Glide to Identify Aspiration in Patients with Respiratory Diseases: A Pilot Study.

INTRODUCTION: Initial research has been conducted to determine the diagnostic accuracy of the pitch glide during the clinical swallow evaluation to identify aspiration in adults after stroke. Findings suggest that reduced pitch glide can predict aspiration in patients with dysphagia after stroke. This study aimed to identify the diagnostic accuracy of the pitch glide in detecting aspiration, pharyngeal residue and hyo-laryngeal excursion during swallowing in adults with respiratory diseases. MATERIAL AND METHODS: 17 adults (9 males and 8 females) with dysphagia who had a background of respira

An optimized methodology for whole genome sequencing of RNA respiratory viruses from nasopharyngeal aspirates.

Over the last decade, the number of viral genome sequences deposited in available databases has grown exponentially. However, sequencing methodology vary widely and many published works have relied on viral enrichment by viral culture or nucleic acid amplification with specific primers rather than through unbiased techniques such as metagenomics. The genome of RNA viruses is highly variable and these enrichment methodologies may be difficult to achieve or may bias the results. In order to obtain genomic sequences of human respiratory syncytial virus (HRSV) from positive nasopharyngeal aspirate

Respiratory failure patient with lung cancer diagnosed by transesophageal bronchoscopic ultrasound-guided aspirates.

A 71-year-old man, who had received long-term oxygen therapy for respiratory failure caused by chronic obstructive pulmonary disease, had an enlarged mediastinal lymph node for one year. As his lung function was poor, we tried performing endobronchial ultrasound-guided transbronchial needle aspiration under non-invasive positive pressure ventilation for diagnosis but could not obtain sufficient specimens. Later, we performed an endoscopic ultrasound with bronchoscope-guided fine-needle aspiration (EUS-B-FNA) using a transesophageal approach. Rapid on-site cytology revealed that adequate specim

Evaluation of a PCR/ESI-MS platform to identify respiratory viruses from nasopharyngeal aspirates.

Acute respiratory tract infection is a major cause of morbidity and mortality worldwide, particularly in infants and young children. High-throughput, accurate, broad-range tools for etiologic diagnosis are critical for effective epidemic control. In this study, the diagnostic capacities of an Ibis platform based on the PCR/ESI-MS assay were evaluated using clinical samples. Nasopharyngeal aspirates (NPAs) were collected from 120 children (<5 years old) who were hospitalized with lower respiratory tract infections between November 2010 and October 2011. The respiratory virus detection assay was

Effects of PEEP and tolazoline infusion on respiratory and inert gas exchange in experimental meconium aspiration.

Meconium aspiration syndrome often produces respiratory failure in the neonate. We utilized the multiple inert gas elimination technique to study the effects on respiratory and inert gas exchange of the application of positive end expiratory pressure or continuous infusion of tolazoline HCl. The application of PEEP, with the optimal level of PEEP defined for each animal, produced a decrease in AaDO2 and pulmonary shunt, without an increase in blood flow to low VA/Q areas, or an increase in dead space. Tolazoline infusion, at 2 mg/kg/hour, had no apparent effect on AaDO2 or shunt, or magnitude

The Ighmbp2 -R604X mouse recapitulates the severe SMARD1 clinical symptoms of aspiration, respiratory and feeding deficits.

Spinal muscular atrophy with respiratory distress type 1 (SMARD1) and Charcot Marie Tooth type 2S (CMT2S) are due to mutations in immunoglobulin mu binding protein two (IGHMBP2). We generated the Ighmbp2 -R604X mouse (R605X-humans) to understand how alterations in IGHMBP2 function impact disease pathology. The IGHMBP2-R605X mutation is associated with patients with SMARD1 or CMT2S. The impact of this mutation is substantial, Ighmbp2 R604X/R604X mice have a decreased lifespan (6 days) and weight, and failure to thrive consistent with SMARD1 symptoms. Significant respiratory changes were present

Acute Hypercapnic Respiratory Failure from Foreign Body Aspiration in a 16-Month-Old: A Case Report.

INTRODUCTION: Acute hypercapnic respiratory failure secondary to foreign body aspiration is a rare but severe complication seen in pediatric patients. Foreign body aspiration is one of the leading causes of death in children and requires prompt intervention and stabilization when definitive bronchoscopy is not readily available. CASE REPORT: We describe the case of a 16-month-old male who developed acute hypercapnic respiratory failure following the aspiration of a foreign body. On presentation to the emergency department, the child was in respiratory distress, appeared cyanotic, and had seve

Short-term corticosteroid therapy in aspiration pneumonitis complicated by acute respiratory distress syndrome: A case report.

RATIONALE: Aspiration of gastric contents (Mendelson syndrome) can cause chemical pneumonitis and quickly progress to acute respiratory distress syndrome (ARDS). Evidence for corticosteroids in this setting is limited, and guidelines are unclear. We describe a postpartum case of aspiration pneumonitis evolving to ARDS that improved after a short course of methylprednisolone. PATIENT CONCERNS: A 27-year-old woman developed severe dyspnea, chest tightness, and hypoxemia after a cesarean section performed under general anesthesia. DIAGNOSES: On arrival at Cho Ray Hospital, she was tachypneic wi
