Management of Respiratory Failure in Hemorrhagic Shock.

Hemorrhagic shock results in acute respiratory failure due to respiratory muscle fatigue and inadequate pulmonary blood flow. Because positive pressure ventilation can reduce venous return and cardiac output, clinicians should use the minimum possible mean airway pressure during assisted or mechanical ventilation, particularly during episodes of severe hypovolemia. Hypoperfusion also worsens dead space fraction. Therefore, clinicians should monitor capnography during mechanical ventilation and recognize that hypercapnia may be treated with fluid resuscitation rather than increasing minute vent

Pathophysiology of Chronic Hypercapnic Respiratory Failure.

Chronic hypercapnic respiratory failure occurs in several conditions associated with hypoventilation. The mechanisms underlying the development of chronic hypercapnia include a combination of processes that increase metabolic CO2 production, reduce minute ventilation (V'e), or increase dead space fraction (Vd/Vt). Fundamental to the pathophysiology is a mismatch between increased load and a reduction in the capacity of the respiratory pump to compensate. Though neural respiratory drive may be decreased in a subset of central hypoventilation disorders, it is more commonly increased in attemptin

The maintenance of total ventilatory requirements through a chronic bronchopleural cutaneous fistula.

In patient with a chronic post-tuberculous bronchopleural cutaneous fistula (BPCF), minute ventilation, dead space, flow rates, arterial blood gas tensions, and oxygen consumption were measured during mouth breathing and after 30 min of steady-state breathing solely through the BPCF. Despite a 390-ml (18%) decrease in dead space when breathing took place through the BPCF, there were no significant changes in minute ventilation or respiratory rate. BPCF breathing was also associated with an increase in airways resistance as reflected by a 300-ml (35%) decrease in the FEV1 and a 16% decrease in

Dead space washout by intentional leakage flow during conventional ventilation of premature infants-an experimental study.

OBJECTIVE: Invasive mechanical ventilation poses a strong risk factor for the development of chronic lung disease in preterm infants. A reduction of the dead space as part of the total breathing volume would reduce the ventilation effort and thereby lower the risk of ventilator-induced lung injuries. In this experimental study, we compared the efficacy of mechanical dead space washout via uncontrolled and controlled leakage flow in their ability to eliminate CO2 during conventional ventilation in preterm infants. METHODS: Three frequently used neonatal ventilators, operating under standard co

Diffusional inhomogeneity: gas mixing efficiency in the new-born lung.

1. The results of 105 nitrogen washouts on seventy-six new-born babies are presented; the mean lung volume (functional residual capacity, FRC) of the seventy-six babies measured at more than 1 hr of age was 77.5 +/- 2.9 (S.E. of mean; S.D. = 25.4). 2. The mean gas mixing efficiency was 0.466 +/- 0.0097 (S. E. of mean; S.D. = 0.096). The reasons for the difference between this figure and the normal for adults of 0.66--0.76 are discussed, and it is suggested that the very much greater turnover rate of gas within the lungs is responsible for diffusion inefficiency. 3. Calculations indicate that t

Ventilatory stimulation with relative external dead space loading does not impact sex differences in exertional dyspnoea.

Sex differences in respiratory responses to ventilatory stress may contribute to exercise tolerance and dyspnoea perception, with potential implications across health and respiratory disease. We hypothesized that dead space loading (DSL) normalized to 15% of forced vital capacity would increase exertional dyspnoea, exacerbate inherent sex-based differences in the respiratory system's response to exercise, and elicit higher diaphragmatic and extra-diaphragmatic respiratory muscle activity in females than in males. Forty healthy, non-smokers (20 males, 20 females; 24 ± 5 years old; maximal O2 up

Effect of additional dead space using end-tidal CO2 measurement on ventilating preterm infants: An experimental study.

BACKGROUND: Dead space is the part of the airway where no gas exchange takes place. Any increase in dead space volume has a proportional effect on the required tidal volume and thus on the risk of ventilation-induced lung injury. Inserts that increase dead space are therefore not used in small preterm infants. This includes end-tidal CO2 measurement. OBJECTIVE: The aim of this study was to investigate the effect of the end-tidal CO2 measurement adapter on ventilation. METHODS: In an experimental setup, an end-tidal CO2 measurement adapter, three different pneumotachographs (PNT-A, PNT-B, PNT

Inert gas mixing in the upper and central airways of man.

The dispersion of an inert bolus of helium (He) or sulfur hexafluoride (SF6) was used as a direct, non-invasive measure of longitudinal gas mixing in the conducting airways of three human subjects. Penetration volume was determined as the milliliters of air inspired after bolus injection, and by computing the increase in mixing between bolus penetrations of 30 and 90 ml, mixing was characterized in the intervening upper airway compartment. Mixing in a central airway compartment was similarly evaluated as the increased mixing between bolus penetrations of 90 and 150 ml. To investigate mixing me

Invasive ventilation at the boundary of viability: A respiratory pathophysiology study of infants born between 22 and 24 weeks of gestation.

BACKGROUND: Invasive ventilation of infants born before 24 weeks of gestation is critical for survival and long-term respiratory outcomes, but currently there is a lack of evidence to guide respiratory management. We aimed to compare respiratory mechanics and gas exchange in ventilated extremely preterm infants born before and after 24 weeks of gestation. METHODS: Secondary analysis of two prospective observational cohort studies, comparing respiratory mechanics and indices of gas exchange in ventilated infants born at 22-24 weeks of gestation (N=14) compared to infants born at 25-27 weeks (N

[Dynamics of the coupling of ventilation and circulation in the lungs of cats exposed to elevated environmental temperatures].

The 1.5-hr exposure of cats to air temperature 50 degrees C led to an increase in respiration rate, ventilation of the respiratory dead space and in intensity of water evaporation from the upper respiratory tract. Increase of the body temperature up to 39.4 degrees C induced no drop in the lung gas exchange efficiency because of the balanced growth of the lung ventilation and blood flow. Further increase of the body temperature (up to 41.9 degrees C) injured the gas exchange function as revealed by a drop in the arterial blood oxygenation and acid--alkaline imbalance (the respiratory alkalosis

Hypercapnic respiratory failure - review.

This review summarizes the issue of acute hypercapnic respiratory failure. Acute respiratory failure is a condition in which the respiratory system is unable to fulfill its basic function, i.e. enriching the blood with oxygen and excreting carbon dioxide. Chronologically, we divide it into acute and chronic, and according to the manifestation into hypoxemic or hypoxemic with hypercapnia. Multiple factors, such as reduced ventilation and increased dead space, contribute to the development of hypoxemic-hypercapnic (global) respiratory failure. Both the patient's clinical presentation and laborat

Postoperative pulmonary atelectasis and collapse, and its prophylaxis with intravenous bicarbonate.

Of 181 patients undergoing major abdominal surgery 116 developed chest complications associated with a metabolic acidosis, low Pco2, depressed tidal volume, increased respiratory rate, but no increase in minute volume. In a matched group of 116 patients given intravenous bicarbonate postoperatively only 15 developed chest complications. This suggests that respiratory physiological dead space decreases in patients with pulmonary collapse and atelectasis following surgery. Acidotic respiration proved inefficient in the postoperative period, and intravenous bicarbonate had a very pronounced effec

Postural activity of the diaphragm is reduced in humans when respiratory demand increases.

1. Respiratory activity of the diaphragm and other respiratory muscles is normally co-ordinated with their other functions, such as for postural control of the trunk when the limbs move. The integration may occur by summation of two inputs at the respiratory motoneurons. The present study investigated whether postural activity of the diaphragm changed when respiratory drive increased with hypercapnoea. 2. Electromyographic (EMG) recordings of the diaphragm and other trunk muscles were made with intramuscular electrodes in 13 healthy volunteers. Under control conditions and while breathing thro

Misinterpretation of carbon dioxide monitoring because of deadspace of heat and moisture exchanger with a filter in pediatric anesthesia: A case report.

RATIONALE: When patients are intubated and treated with mechanical ventilation, the upper respiratory tract is bypassed by the flow of dry and cold air. To prevent disturbances of airway homeostasis, a heat and moisture exchanger filter (HMEF) has been applied to breathing circuit. PATIENT CONCERNS: A 4-month-old male infant was ventilated with the pediatric HMEF. We report the impact of ignoring the direct influence of a filter containing deadspace in pediatric mechanical ventilation. DIAGNOSES: The breathing circuit with HMEF leads to unexpected complications such as mechanical obstruction

The effects of breath-holds and Muller manoeuvres on upper airway carbon dioxide concentration in humans.

BACKGROUND: Obstructive sleep apnoea is caused by collapse of the upper airway. The presence of CO(2) in the upper airway lumen evokes a number of reflexes which favour upper airway re-opening, and we have proposed previously that CO(2) would build up in the upper airway following airway collapse and that this would contribute to reflex airway re-opening. However, it is not known if CO(2) can transfer from the alveoli to the anatomical dead space of the upper airway during apnoea. OBJECTIVES: To determine if alveolar CO(2) can enter the upper airway during breath-holds and Muller manoeuvres.

Effect of reducing anatomic dead space on arterial PCO2 during CO2 inhalation.

Carotid body-denervated (CBD) ponies have a less than normal increase in arterial PCO2 (PaCO2) when inspired CO2 (PICO2) is increased, even when pulmonary ventilation (VE) and breathing frequency (f) are normal. We studied six tracheostomized ponies to determine whether this change 1) might be due to increased alveolar ventilation (VA) secondary to a reduction in upper airway dead space (VD) or 2) is dependent on an upper airway sensory mechanism. Three normal and three chronic CBD ponies were studied while they were breathing room air and at 14, 28, and 42 Torr PICO2. While the ponies were br
