[Respiratory virus infections in children].

Eighty to ninety percent of pathogens responsible for acute respiratory infections in children are viruses, but despite advances in virology these organisms are isolated in only 20 to 45 percent of the cases. Studies conducted outside hospitals have provided epidemiological data. The virus most frequently encountered is the respiratory syncytial virus. The main clinical feature of these respiratory viral infections is obstruction of the bronchioles, and their immediate or delayed danger is the risk of chronic obstructive bronchitis. Treatment is symptomatic, but specific antiviral agents, nota

[Clinical studies on the secondary bacterial infection in respiratory syncytial virus infection of children].

Secondary bacterial infection was studied on 231 children admitted with Respiratory Syncytial virus (RSV) infection in the 10 years since 1987. Of the 231 children, 56 (24.2%) had dual bacterial infection possibly due to secondary bacterial invasion. The diagnoses of bacterial disease were sepsis (2), pyothorax (2), pneumonias (41), otitis media (7), nasopharyngitis (2) and urinary tract infection (2). Dual bacterial infections were more frequent in infants and children over 6 months than in infants younger than 6 months. The main etiologic agents were Staphylococcus aureus and enteric gram-ne

Oesophageal foreign body: an unusual cause of respiratory symptoms in a three-week-old baby.

Impaction of a foreign body in the oesophagus is common in children. If the event is not witnessed by an adult or the object is radiolucent the diagnosis is difficult as respiratory symptoms may predominate. We report a three-week-old child with respiratory symptoms who had both a respiratory syncytial virus infection and a radio-lucent oesophageal foreign body. This case is the youngest yet reported and serves as a remainder of the difficulties in the diagnosis of this important condition.

Respiratory syncytial virus pneumonia in a lung transplant recipient: case report.

A 29-year-old man underwent bilateral lung transplantation and received maintenance immunosuppressive therapy. He was readmitted 11 months later with symptoms of cough, sneezing, and rhinorrhea. The physical examination was normal. Laboratory results were significant for a reduction of FEV1 and an interstitial infiltrate on chest films. The patient had recently undergone bronchoscopy for rejection surveillance, and 2 days before admission the bronchoalveolar lavage cultures returned positive for respiratory syncytial virus. The patient was treated with aerosolized ribavirin with complete resol

Viral pneumonia in the immunocompromised adult with neoplastic disease: the role of common community respiratory viruses.

Until recently, the viruses most often identified in lower respiratory tract infections among immunocompromised adults with neoplastic disease have been the herpesviruses (cytomegalovirus, herpes simplex virus, varicella-zoster virus, human herpesvirus-6) and adenovirus. These viral pneumonias occur with greater frequency and/or severity among these adults than among immunocompetent adults. During the last decade, mounting evidence has suggested that the RNA viruses that commonly cause lower respiratory tract disease in the general population, namely influenza virus, respiratory syncytial viru

Ribavirin therapy of adult respiratory syncytial virus pneumonitis.

Respiratory syncytial virus is a common respiratory tract pathogen in infants. Pulmonary infection in adult and elderly populations can occur with severe and even fatal pneumonitis having been reported in several recent outbreaks. We present a previously healthy adult patient who developed respiratory syncytial virus pneumonia severe enough to require mechanical ventilation. Antiviral therapy with aerosolized ribavirin was successfully undertaken and the patient recovered completely. Respiratory syncytial virus infection should be considered in the differential diagnosis of atypical adult pneu

Effectiveness of topically administered neutralizing antibodies in experimental immunotherapy of respiratory syncytial virus infection in cotton rats.

Initial studies of the prophylactic effect of parenterally administered respiratory syncytial virus (RSV)-neutralizing antibodies in cotton rats indicated that virus replication in lung tissues was restricted when animals with preexisting antibody titers in serum of 1:100 or more (as measured by plaque reduction) were challenged intranasally with 10(4) PFU of virus. Subsequently, a therapeutic effect of parenterally administered RSV antibodies (present in human gamma globulin) was demonstrated in both cotton rats and owl monkeys. Parenteral inoculation of RSV-infected cotton rats or owl monkey

Isolation and Identification of a Genotype C Bovine Parainfluenza Virus Type 3 and Its Pathogenicity in Albino Guinea Pigs.

Bovine parainfluenza virus type 3 (BPIV3) is a significant pathogen in cattle, causing a range of respiratory symptoms from mild to severe. In this study, we isolated a BPIV3 (BPIV3/SX/2021) strain using Madin-Darby bovine kidney cells from a lung tissue sample obtained in Shaanxi province. The full length of the BPIV3/SX/2021 strain is 15,474 nt, encoding six structural and three accessory proteins. A phylogenetic analysis revealed that SX2021strain belongs to BPIV3 genotype C, and the isolate shares the highest sequence homology with the other genotype C strains isolated in China. A genetic

The pulmonary clearance of Pasteurella hemolytica in calves infected with bovine parainfluenza-3 virus.

The purpose of this study was to determine if parainfluenza-3 virus in calves interfered with normal pulmonary bacterial clearance. Three groups (A, B and C) of four calves each were exposed to an aerosol of parainfluenza-3 virus. Three days later the four hour pulmonary clearance of Pasteurella hemolytica was determined on the first group (A), seven days later on the second (B) and 11 days later on the third (C). Group A had a mean pulmonary bacterial retention of 3.6 +/- 3.5%. Group B was 83.1 +/- 35.9% and Group C was 41.2 +/- 30.9%. The results demonstrate that parainfluenza-3 virus interf

Infection with a respiratory virus before hematopoietic cell transplantation is associated with alloimmune-mediated lung syndromes.

BACKGROUND: Alloimmune-mediated lung syndromes (allo-LSs) are life-threatening complications after hematopoietic cell transplantation (HCT). Respiratory virus (RV) has been suggested to play a role in the pathogenesis. OBJECTIVE: We studied the relation between RV DNA/RNA detection in the upper/lower airways before HCT and the occurrence of allo-LSs. METHODS: We retrospectively analyzed all HCT recipients between 2004 and 2014, in whom real-time PCR for RV was performed in nasopharyngeal aspirates (NPAs) and bronchoalveolar lavage (BAL) fluid before HCT. The main outcome of interest was the

Epidemiology and aetiology of acute bronchiolitis in Hong Kong infants.

The epidemiological, clinical and virological features of 1220 children with acute bronchiolitis admitted to the Prince of Wales Hospital, Hong Kong, from 1985 to 1988 are reported. They accounted for 6.6% of total paediatric admissions and provided a case incidence of bronchiolitis requiring admission to hospital of approximately 21 per 1000 children 0-24 months of age. The clinical course and outcome was in general benign. The average hospital stay was 5 days and there were no deaths. Ten per cent of patients were repeatedly admitted to hospital with recurrent wheezing after discharge. Two i

Current perspectives on respiratory syncytial virus infection.

Respiratory syncytial virus (RSV) is the most important cause of acute viral bronchiolitis and pneumonia in infants and children. Management of RSV infection has changed dramatically over the past 5 to 7 years because of (1) the advent of widely available rapid tests for its specific identification and (2) use of the antiviral agent ribavirin (Virazole). Although RSV infection remains a life-threatening disease, particularly in certain high-risk groups, appropriate management and judicious use of ribavirin should help physicians reduce its morbidity and mortality.

Acute bronchiolitis in infancy. The relationship to later recurrent obstructive airways disease.

Fifty-one infants admitted to hospital because of acute bronchiolitis were followed until 2 years of age. Sixty per cent had three or more attacks of bronchopulmonary obstruction, and the remainder two or less attacks. The children with three or more attacks also had more respiratory infections. They had younger parents and their close relatives more often had respiratory atopic illness. The children with three or more attacks wheezed for a longer time after the bronchiolitis. No difference was found for breast-feeding, neonatal respiratory complications, crowded homes, parental smoking and ot

Diagnosis of respiratory syncytial virus infection in the bovine respiratory tract by immunofluorescence.

Direct staining of nasopharyngeal smears with hyperimmune bovine serum raised against respiratory syncytial virus (RSV) and conjugated with fluorescein isothyocyanate was compared with virus isolation from 14 experimentally infected animals. Twenty-four out of 107 specimens examined were positive by the fluorescent antibody (FA) method and 21 out of 107 by virus isolation. Most of the FA positives (81 per cent) were detected after the ninth day of infection whereas 80 per cent of virus isolations were made before this time. Only one false positive (one out of 57 specimens) was detected by the

Development of SPR biosensor for simultaneous detection of multiplex respiratory viruses.

A surface plasmon resonance (SPR)-based biosensor was developed for specific detection of nine common respiratory virus, including influenza A and influenza B, H1N1, respiratory syncytial virus (RSV), parainfluenza virus 1-3 (PIV1, 2, 3), adenovirus, and severe acute respiratory syndrome coronavirus (SARS). The SPR biosensor was developed by immobilizing nine respiratory virus-specific oligonucleotides in an SPR chip. To increase the biosensor sensitivity, biotin was used to label the PCR primer and further amplify the signal by introducing streptavidin after hybridization. Throat swab specime

Matrix metalloproteinase-9 and tissue inhibitor of matrix metalloproteinase-1 in the respiratory tracts of human infants following paramyxovirus infection.

Respiratory syncytial (RSV) and parainfluenza (PIV) viruses are primary causes of acute bronchiolitis and wheezing illnesses in infants and young children. To further understand inflammation in the airways following infection, we tested for the presence of matrix metalloproteinases (MMP) and natural tissue inhibitors of MMP (TIMP) in primary and established human cell lines, and in the nasopharyngeal secretions (NPS) of human infants infected with RSV or PIV. Using ELISA and multiplex-based assays, MMP-9 and TIMP-1 proteins were, respectively, detected in 66/67 and 67/67 NPS. During PIV or RSV
