Prevalence and variability in use of physical and chemical restraints in residential aged care facilities: A systematic review and meta-analysis.

BACKGROUND: Use of physical and chemical restraints are common in residential aged care facilities worldwide. Restraint use can pose harm to residents even causing deaths. OBJECTIVE: To synthesize the prevalence and variability in physical and chemical restraint use, and examine factors that may contribute to this variability of prevalence rates. METHODS: Six health science databases were searched from inception up to 21st January 2020. Quantitative studies investigating restraint use in residential aged care facilities that reported data from year 2000 onwards were included. Meta-analyses o

Synergistic induction of severe hypothermia (poikilothermia) by limbic seizures, acepromazine and physical restraint: role of noradrenergic alpha-1 receptors.

The maintained production of extreme reductions in core temperature (20-22 degrees C) or poikilothermia can be reliably produced by the synergistic interaction of limbic seizures (induced by lithium and pilocarpine), postseizure administration of a single injection of acepromazine, and physical restraint. Administration of the specific and nonspecific dopamine antagonists haloperidol, chlorpromazine, SCH23390, or clozapine did not simulate the effect at clinically effective dosages. Single injections of phentolamine and prazosin but not of propranolol instead of acepromazine following the seiz

Development of an Evidence-Based Instrument to Justify the Use of Physical Restraint in General Adult Ward Settings: A Systematic Review.

BACKGROUND: Physical restraint is commonly applied in the clinical settings despite numerous studies presenting its paucity of efficacy and safety. Despite the various tangible and intangible factors associated with moral and safety issues, nurses must make decisions on restraint use in ethical dilemmas. Health care providers often find it challenging to make appropriate decisions regarding the use of physical restraint in demanding clinical environments without a standard and objective assessment tool. OBJECTIVES: The objectives aimed to identify effective instrument to justify the decision-

Prevention of physical restraints in the acute care setting (PROTECT): study protocol for a cluster-randomised controlled pilot study.

INTRODUCTION: Physical restraints (PR) are regularly used in acute care settings, although evidence for their effectiveness and safety (eg, for prevention of falls) is lacking. Their use is associated with adverse events, such as decreased mobility and injuries for patients. We developed a complex intervention to prevent PR in acute care settings according to the UK Medical Research Council Framework, and investigated the feasibility. The intervention comprised the qualification of key nurses as multipliers and a short interprofessional information session. The intervention has proven to be fe

Reducing the use of physical restraints in home care: development and feasibility testing of a multicomponent program to support the implementation of a guideline.

BACKGROUND: A validated evidence-based guideline was developed to reduce physical restraint use in home care. However, the implementation of guidelines in home care is challenging. Therefore, this study aims to systematically develop and evaluate a multicomponent program for the implementation of the guideline for reducing the use of physical restraints in home care. METHODS: Intervention Mapping was used to develop a multicomponent program. This method contains six steps. Each step comprises several tasks towards the design, implementation and evaluation of an intervention; which is theory a

The effectiveness of physical restraints in reducing falls among adults in acute care hospitals and nursing homes: a systematic review.

BACKGROUND: Physical restraint was introduced as the primary measure to maintain patient safety in preventing falls. However, physical restraints may lead to complications such as functional loss, immobility, delirium, pressure sore, and even falls or injuries. Despite all these negative effects and many alternatives that are available, physical restraint is still commonly used in hospitals and nursing homes. Hence, it is crucial to understand the effectiveness of physical restraints in protecting adult patients from falling. OBJECTIVE: The overall objective was to examine the effectiveness o

Perceptions of physical restraints use in the elderly among registered nurses and nurse assistants in a single acute care hospital.

Physical restraint use among hospitalized older adults remains an important issue. Despite evidence indicating that restraints can be harmful and strict regulatory rules restricting the use of restraints, healthcare practitioners continue to utilize physical restraints, often in the name of safety. The purpose of this study was to examine the perceptions regarding physical restraint use among registered nurses (RNs) and nursing assistants (NAs). The Perceptions of Restraint Use Questionnaire (PRUQ) was used to evaluate nursing staff perceptions. The overall mean score for the PRUQ was 2.8 out

Extreme hypothermia induced by a synergism of acute limbic seizures, physical restraint, and acepromazine: implications for survival following brain injury.

Core temperature was measured in rats 24 hr. after they had been assigned to one of 8 groups in a 3-way analysis of variance design that involved (1) induction of limbic seizures by a systemic injection of lithium/pilocarpine, (2) physical restraint, and (3) administration of acepromazine. An extraordinarily powerful interaction was noted among seizures, physical restraint, and acepromazine-produced hypothermia (24 degrees C) compared to the other 7 treatments (> 35 degrees C). The putative poikilothermic response is commensurate with the loss of mammalian behaviors that follow these seizures.

Updated Dietetic Prescribing Guidelines When Naso-Gastric Tube Feeding Under Physical Restraint Is Required: A Modified Nominal Group Technique Study.

INTRODUCTION: Naso-gastric tube (NGT) feeding under physical restraint is a lifesaving clinical intervention that is sometimes needed in mental health patients, particularly those with restrictive eating disorders. Initial guidance was published in 2020 and subsequently reviewed and updated to support dietitians in this specialist area. METHODS: Using purposeful recruitment, we brought together specialist mental health dietitians from England, Wales, Scotland and Northern Ireland who work in inpatient child and adolescent mental health services or adult inpatient mental health services. A mixed-methods systematic review protocol to examine the use of physical restraint with critically ill adults and strategies for minimizing their use.

BACKGROUND: Critically ill patients frequently experience severe agitation placing them at risk of harm. Physical restraint is common in intensive care units (ICUs) for clinician concerns about safety. However, physical restraint may not prevent medical device removal and has been associated with negative physical and psychological consequences. While professional society guidelines, legislation, and accreditation standards recommend physical restraint minimization, guidelines for critically ill patients are over a decade old, with recommendations that are non-specific. Our systematic review w

Cross-Sectional Study of Rehabilitation Training Intensity and Physical Restraints in Patients With Cardiovascular Disease and Neurocognitive Disorders in Acute Care Hospitals.

Background Previous studies have reported the characteristics of rehabilitation therapy and physical restraint use in patients with neurocognitive disorders during the acute phase, based on three years of clinical records from dementia care team rounds at our hospital. The incidence of neurocognitive disorders varies by disease group, and the degree of physical restraint is significantly associated with the intensity of the rehabilitation therapy. Among the disease groups addressed in this study, we were particularly interested in cardiovascular diseases. In patients with cardiovascular diseas

Impact of a Videoconference Educational Intervention on Physical Restraint and Antipsychotic Use in Nursing Homes: Results From the ECHO-AGE Pilot Study.

OBJECTIVES: US nursing homes care for increasing numbers of residents with dementia and associated behavioral problems. They often lack access to specialized clinical expertise relevant to managing these problems. Project ECHO-AGE provides this expertise through videoconference sessions between frontline nursing home staff and clinical experts at an academic medical center. We hypothesized that ECHO-AGE would result in less use of physical and chemical restraints and other quality improvements in participating facilities. DESIGN: A 2:1 matched-cohort study comparing quality of care outcomes b

Previous-day physical restraint duration as a bedside signal for following-day delirium: a retrospective patient-day analysis in a mixed ICU.

BACKGROUND: Delirium is common in the intensive care unit and associated with adverse outcomes. However, bedside indicators that flag patients at high risk for delirium the following day are lacking. Although physical restraint has been linked to delirium, many prior studies relied on cross-sectional, same-day assessments that cannot exclude reverse causation. This study aimed to evaluate whether cumulative previous-day physical restraint duration was a bedside signal for following-day delirium. METHODS: We conducted a single-center retrospective observational cohort study using patient-days
