The effect of posture on asynchronous chest wall movement in COPD.

Chronic obstructive pulmonary disease (COPD) patients often show asynchronous movement of the lower rib cage during spontaneous quiet breathing and exercise. We speculated that varying body position from seated to supine would influence rib cage asynchrony by changing the configuration of the respiratory muscles. Twenty-three severe COPD patients (forced expiratory volume in 1 s = 32.5 ± 7.0% predicted) and 12 healthy age-matched controls were studied. Measurements of the phase shift between upper and lower rib cage and between upper rib cage and abdomen were performed with opto-electronic ple

Estimation of eighth costal cartilage in surgical timing of microtia reconstruction.

There is controversy over the optimal timing of microtia reconstruction. The eighth costal cartilage, which is used to shape the helix framework, can be one of the key factors determining surgical timing of microtia reconstruction. Nevertheless, it is difficult to predict the length of the eighth costal cartilage preoperatively. The aim of the present study was to suggest clinical predictors of the length of the eighth cartilage by assessing the correlation between the actual length of the eighth cartilage and preoperative measurements of the cartilage length using three-dimensional rib-cage c

Biomechanical investigation of impact induced rib fractures of a porcine infant surrogate model.

This study investigated the structural, biomechanical and fractographic features of rib fractures in a piglet model, to test the hypothesis that fist impact, apart from thoracic squeezing, may result in lateral costal fractures as observed in abused infants. A mechanical fist with an accelerometer was constructed and fixed to a custom jig. Twenty stillborn piglets in the supine position were impacted on the thoracic cage. The resultant force versus time curves from the accelerometer data showed a number of steps indicative of rib fracture. The correlation between impact force and number of fra

Effects of the Nuss procedure on chest wall kinematics in adolescents with pectus excavatum.

No data are available on the effects of the Nuss procedure on volumes of chest wall compartments (the upper rib cage, lower rib cage and abdomen) in adolescents with pectus excavatum. We used optoelectronic plethysmography to provide a quantitative description of chest wall kinematics before and 6 months after the Nuss procedure at rest and during maximal voluntary ventilation in 13 subjects with pectus excavatum. An average 11% increase in chest wall volume was accommodated within the upper rib cage (p=0.0001) and to a lesser extent within the abdomen and lower rib cage. Tidal volumes did not

Changes in chest wall compartment volumes on induction of anaesthesia with eltanolone, propofol and thiopentone.

Changes in the expiratory dimensions of the rib cage and abdomen on induction of anaesthesia with eltanolone, propofol and thiopentone were measured in 76 patients using respiratory inductance bands. Calibration of the respiratory inductance plethysmograph was by simultaneous flow measurement with a pneumotachograph. Movement of the vertebral column was restrained with a rigid mattress to allow volume change to be estimated more accurately from rib cage and abdominal dimensions. Rib cage volumes decreased by a median of 125 ml, while the median change in the abdominal compartment was 0. These

Body type and speech breathing.

Diameter changes of the rib cage and abdomen were recorded during tidal breathing and speech production in 12 adult male subjects grouped on the basis of prominence on three body type components: relative fatness, relative musculoskeletal development, and relative linearity. Data were charted to solve for lung volume, volume displacements of the rib cage and abdomen, and muscular mechanism. Tidal breathing differed across subject groups with regard to depth, rate, and chest wall configuration. Subjects rated high in relative fatness breathed deeper, slower, and with a greater chest wall deform

Structural change of the thorax in chronic obstructive pulmonary disease.

This study examines structural changes of the thorax in hyperinflated subjects with chronic obstructive pulmonary disease (COPD). Age-matched normal subjects were used for comparison. Thoracic dimensions were determined using anteroposterior and lateral chest radiographs performed at total lung capacity, functional residual capacity, and residual volume. Rib cage dimensions (lateral diameter, rib angle, anteroposterior diameter) and diaphragm position were determined at each lung volume. There were no significant differences in rib cage dimension between the COPD and normal subjects for all lu

Chest wall mechanics: effects of acute and chronic lung disease.

Data from the literature show that lung tissue properties affect the chest wall compliance, Ccw, which is the change in lung volume, Vl, with respect to the pleural pressure, Ppl. to analyze the difference between acute and chronic lung tissue changes, we used a mathematical model that describes the static, nonlinear mechanics of the ventilatory system in terms of its major elements: rib cage; abdomen; diaphragm and lung. With this model we derived the relationship between chest wall, rib-cage and diaphragm compliances. Although the Vl-Ppl relation is independent of lung mechanics, the volume

Morphometric analysis of variation in the ribs with age and sex.

Rib cage morphology changes with age and sex are expected to affect thoracic injury mechanisms and tolerance, particularly for vulnerable populations such as pediatrics and the elderly. The size and shape variation of the external geometry of the ribs was characterized for males and females aged 0-100 years. Computed tomography (CT) scans from 339 subjects were analyzed to collect between 2700 and 10 400 homologous landmarks from each rib. Rib landmarks were analyzed using the geometric morphometric technique known as Procrustes superimposition. Age- and sex-specific functions of 3D rib morpho

Cervical magnetic stimulation of the phrenic nerves in bilateral diaphragm paralysis.

Cervical magnetic stimulation (CMS) produces a greater twitch transdiaphragmatic pressure (TwPdi) than electrical stimulation. This may be because CMS produces rib cage muscle activation, thus producing an inspiratory action independent of the diaphragm. Alternatively, CMS could merely stiffen the rib cage, allowing the diaphragm to act efficiently, by contracting against a stable rib cage. To examine these two hypotheses we studied five patients with isolated bilateral diaphragm paralysis using CMS and bilateral electrical phrenic stimulation (BES). TwPdi, twitch esophageal pressure (TwPes),

[The role of surgeon in treatment of chronic respiratory failure after pulmonary tuberculosis operations].

We analyzed pulmonary functions and rib cage-phrenic motions on 296 cases of chronic empyema patients who had surgical treatment. Pulmonary functions were better preserved in patients with a single step than in those with multi-step operations, and in those who had more than five operations the impairment was inevitable. Both %VC and rib cage-phrenic motions improved by decortication, and by air-plombage, but in those who developed multi-rib fractures after the latter procedure, ipsilateral chest wall motion almost disappeared. Since some restriction of rib cage-phrenic motion is inevitable af

Gastric Outlet Obstruction Secondary to Severe Thoracolumbar Scoliosis.

A 78-year-old woman with a history of idiopathic thoracolumbar scoliosis presented with signs, symptoms, and imaging findings consistent with a gastric outlet obstruction secondary to the rib cage impinging on the pylorus of the stomach. She underwent an operative intervention and intra-operative findings were consistent with severe scoliosis with the right rib cage impinging on the pylorus, causing gastric outlet obstruction. A laparoscopic procedure was performed to pexy the greater curvature of the stomach to the left upper quadrant and a percutaneous endoscopic trans-gastric jejunostomy wa

Preoperative rib cartilage imaging in 3-dimensional chest computed tomography for auricular reconstruction for microtia.

BACKGROUND: There are several methods that may be used to confirm the status of rib cartilage, such as physical examinations or chest radiography, for subjects with microtia. However, these methods are limited because of clinicians' inability to gain accurate information about the rib cartilage. We performed 3-dimensional chest computed tomography to preoperatively evaluate the accuracy of rib cartilage imaging. METHODS: A total of 37 patients preparing for auricular reconstruction using a rib cartilage graft underwent preoperative 3-dimensional rib cage computed tomography (3-D rib CT). The

Plethysmographic comparison of breathing pattern in heaves (recurrent airway obstruction) versus experimental bronchoconstriction or hyperpnea in horses.

BACKGROUND: Horses with recurrent airway obstruction (heaves) are described as exhibiting "increased abdominal effort," but it is unknown whether this translates to an effective contribution to ventilation. HYPOTHESIS: We hypothesized that heaves is characterized by asynchrony between rib cage and abdominal motions, and that the abdominal component is the major contributor to ventilation. ANIMALS: The rib cage versus abdominal motion in naturally occurring heaves (n = 15) was compared to controls at rest (n = 7) and during hyperpnea because of lobeline treatment, and the effects of histamine

Nonhomogeneous alveolar pressure swings: effect of different respiratory muscles.

We tested the hypothesis in dogs that during isovolumic inspiratory efforts the degree of nonhomogeneity of alveolar pressure changes is influenced by regional differences in respiratory muscle activity. Pressure distal to an occluded right lower lobe bronchus (PRLL) was compared with pressure in the occluded airway opening (PaO) during spontaneous inspiratory efforts (S), during right (RP), left (LP), and bilateral (BP) phrenic nerve stimulation, and during spontaneous inspiratory efforts after cutting both phrenic nerves (SCP), Upper and lower rib cage and abdominal dimensional changes were

Diaphragmatic work of breathing in premature human infants.

Present methods of assessing the work of breathing in human infants do not account for the added load when intercostal muscle activity is lost and rib cage distortion occurs. We have developed a technique for assessing diaphragmatic work in this circumstance utilizing measurements of transdiaphragmatic pressure and abdominal volume displacement. Eleven preterm infants without evidence of lung disease were studied. During periods of minimal rib cage distortion, inspiratory diaphragmatic work averaged 5.9 g X cm X ml-1, increasing to an average of 12.4 g X cm X ml-1 with periods of paradoxical r

Development of populational female thorax sizes and body habitus categories using computed tomography (CT) images.

INTRODUCTION: Positioning of the breast during mammography examination is critical to producing optimum quality images. However, the variation in female thorax and body habitus may affect mammography positioning for best image quality. This study aims to establish populational female thoracic (rib cage) sizes and quantify female body habitus categories. METHODS: A retrospective analysis of 347 female computed tomography (CT) chest axial scans was retrieved from an open access database to establish female rib cage sizes. Dimensions of the rib cage were measured digitally across six cross secti

Volume quantification of chest wall motion in dogs.

We employed high-speed multisliced X-ray-computed tomography to determine the relative volume contributions of rib cage (delta Vrc) and diaphragmatic motion (delta Vdi) to tidal volume (VT) during spontaneous breathing in 6 anesthetized dogs lying supine. Mean values were 40 +/- 6% (SE) for delta Vrc and 62 +/- 8% of VT for delta Vdi. The difference between VT and changes in thoracic cavity volume was taken to represent a change in thoracic blood volume (2 +/- 3% of VT). To estimate how much of delta Vrc was caused by diaphragmatic contraction and how much of delta Vdi was caused by rib cage m

Respiratory muscle pressure development during breath holding in apnea divers.

INTRODUCTION: We sought to characterize the patterns of active pressure development of the inspiratory and expiratory rib cage muscles (P(rcm,i) and P(rcm,e)), the diaphragm (P(di,i)), and the expiratory abdominal muscles (P(abm,e)) during maximal "dry" breath holding in trained apnea divers (n = 8). METHODS: Respiratory contractions were assessed via esophageal and gastric manometry. It was expected that inspiratory/expiratory pressures would progressively increase in both magnitude and frequency during the struggle phase, and that inspiratory rib cage muscle pressures would rise at a rate e
