[Rifaximin for hepatic encephalopathy in children. Case report].

Rifaximin is an antibiotic recently approved for the treatment of hepatic encephalopathy in adults. In children more than 12 year-old, it has been approved for travelers' diarrhea and it is also widely used in inflammatory bowel disease. We report, to our knowledge, the first case of a pediatric patient who received rifaximin for hepatic encephalopathy with good clinical outcome.

Bile acids improve the antimicrobial effect of rifaximin.

Diarrhea is one of the most common infirmities affecting international travelers, occurring in 20 to 50% of persons from industrialized countries visiting developing regions. Enterotoxigenic Escherichia coli (ETEC) is the most common causative agent and is isolated from approximately half of the cases of traveler's diarrhea. Rifaximin, a largely water-insoluble, nonabsorbable (<0.4%) antibiotic that inhibits bacterial RNA synthesis, is approved for use for the treatment of traveler's diarrhea caused by diarrheagenic E. coli. However, the drug has minimal effect on the bacterial flora or the in

Rifaximin: a novel nonabsorbed rifamycin for gastrointestinal disorders.

Rifaximin, a virtually nonabsorbed (<0.4%) rifamycin drug, has in vitro activity against aerobic and anaerobic gram-positive and gram-negative microorganisms. Because rifaximin is nonabsorbed, systemic adverse effects are unusual, and after 3 days of therapy, the fecal level of the drug reaches 8000 microg/g. Moreover, the important selection of resistant mutants by the related drug, rifampin, has not yet been observed for rifaximin. Rifaximin has been demonstrated to reduce the duration of traveler's diarrhea secondary to noninvasive bacterial pathogens and recently has been shown to reduce t

Rifaximin: in vitro and in vivo antibacterial activity--a review.

In vitro inhibitory activity of rifaximin is directed against Gram-positive and Gram-negative, aerobic and anaerobic bacteria. It is effective in the treatment of gastrointestinal infections when given orally because of the high concentration of the drug remaining in the gut lumen. Laboratory investigations have been carried out to assess the in vitro activity of rifaximin on different bacterial strains isolated from both human and domestic animals. The objective of this project is to review the in vitro and in vivo activity of rifaximin against bacterial infection with Gram-negative rods, Gra

[The rifaximin therapy and prophylaxis of episodes of acute diverticulitis].

The authors consider some nosographic aspects of diverticular disease, and refer the personal experience on the treatment of 36 patients with diverticulitis. Beside surgery, that is indicated when complications occur (perforation, abscess, fistulae, occlusion), medical therapy (diet + drugs) could reduce the symptomatology and the risk or progression of complications. The authors present data on the clinical use of rifaximin in the treatment of acute diverticulitis.

Treatment of pyogenic skin infections with rifaximin cream.

The authors carried out a random single blind study on 40 patients with pyodermatitis to assess the efficacy and tolerance of rifaximin cream in pyogenic skin infections. Twenty patients were treated with rifaximin and 20 with oxytetracycline, both products being in the form of cream for dermatological use. Application of rifaximin led to a more rapid and intense regression in the subjective and objective symptoms than the comparison drug, which was also clinically effective. Tolerance of both products was excellent.

Rifaximin - Chitosan Nanoparticles for Inflammatory Bowel Disease (IBD).

BACKGROUND: Inflammatory Bowel Disease (IBD) cannot be controlled easily and the recurrence is the most challenging issue for the physicians. There are various controlled and colon targeted drug delivery systems available for the treatment with limited success rate. Nanoparticles prepared by using the colon targeted polymers such as chitosan may improve the IBD due to their smaller size, unique physico chemical properties and targeting potential. OBJECTIVE: The aim of this investigation was designed to formulate and develop a colon targeted polysaccharide nanoparticles of rifaximin (RFX) by u

Consolidation Therapy of Probiotics With Rifaximin Prevent Relapse in Rifaximin-Responsive Diarrhea-Predominant Irritable Bowel Syndrome: A Multicenter Randomized Clinical Trial.

BACKGROUND AND AIM: Although rifaximin treatment achieves high remission rates in diarrhea-predominant irritable bowel syndrome (IBS-D), current therapeutic approaches remain ineffective in preventing disease recurrence. This study aimed to evaluate the relapse rates with Rifaximin after consolidation course with live Bacillus subtilis and Enterococcus faecium capsules (Medilac-S) and explore gut microbiota changes in these IBS-D patients. METHODS: We performed a multicenter, open-label, randomized trial. Participants responded to 2-week rifaximin were randomly assigned to receive Medilac-S f

Rifaximin in cirrhosis: when therapeutic drift meets antimicrobial stewardship.

This Viewpoint examines the evolving role of rifaximin in cirrhosis, arguing that therapeutic drift has outpaced evidence and antimicrobial stewardship. While rifaximin is effective for secondary prevention of recurrent overt hepatic encephalopathy, randomised trials have failed to demonstrate benefit for primary prevention of hepatic encephalopathy, prevention of acute-on-chronic liver failure, or broader disease modification. Concurrently, emerging microbiological and epidemiological data challenge the perception of rifaximin as a benign gut modulator. Prolonged exposure is associated with s

Rifaximin modulates 5-fluorouracil-induced gastrointestinal mucositis in rats.

OBJECTIVE: This study assessed the protective potential of rifaximin in 5-fluorouracil (5-FU) induced intestinal mucositis in the Wistar rats'. MATERIALS AND METHODS: Twenty-nine Wistar rats were divided into 4 interventional groups of 6 animals (A, B, C and F) and one control group (M) of 5 animals. Groups A, B and C received for three days consecutively rifaximin orally: 50 mg/kg (group A), 100 mg/kg (group B) and 200 mg/kg (group C). In the fourth day, 500 mg/kg of 5-FU was administered intraperitoneally to the groups A, B, C and F. A semi-quantitative histological assessment for duodenum,

Rifaximin vs. lactulose in treatment of minimal hepatic encephalopathy.

BACKGROUND & AIMS: Lactulose and rifaximin have already been shown to improve both cognitive functions and health related quality of life (HRQOL) in MHE patients. We aimed to compare the efficacy of rifaximin with lactulose in reversal of MHE and improvement in HRQOL in cirrhotic patients with MHE. METHOD: This prospective, randomized, open label, non-inferiority trial, was conducted at the Gastroenterology department of a tertiary care institute in Northern India. MHE was diagnosed if any two of the five neuro-psychometric (NP) tests were positive. HRQOL was assessed using the sickness impac

Rifaximin may have a dramatic effect on hepatic encephalopathy.

With a steadily increasing healthcare burden, all physicians must be aware of methods to manage the complications of chronic liver disease, including hepatic encephalopathy. Approved by the Food and Drug Administration in the USA and several European countries as a treatment for recurrent hepatic encephalopathy an increasing number of patients are now receiving rifaximin for this condition. This ‘lesson of the month’ highlights the dramatic effect that rifaximin may have on hepatic encephalopathy.

Rifaximin diminishes neutropenia following potentially lethal whole-body radiation.

Terrorist attacks involving radiological or nuclear weapons are a substantial geopolitical concern, given that large populations could be exposed to potentially lethal doses of radiation. Because of this, evaluating potential countermeasures against radiation-induced mortality is critical. Gut microflora are the most common source of systemic infection following exposure to lethal doses of whole-body radiation, suggesting that prophylactic antibiotic therapy may reduce mortality after radiation exposure. The chemical stability, easy administration and favorable tolerability profile of the non-
