Here, we report the diagnosis and treatment of an extraoral cutaneous sinus tract originating in a mandibular second molar with a C-shaped root canal system. The patient was referred to our department by a dermatologist after a series of unsuccessful treatments, including antibiotics. Diffuse radiolucency on a preoperative radiograph revealed that earlier root canal treatment had been only partially successful. Consequently, we performed retreatment of the root canal comprising removal of the former restoration and gutta-percha, cleaning and shaping, and passive irrigation with sodium hypochlo

AIM AND BACKGROUND: Debriding and disinfecting complex anatomies within the root canal system pose a major challenge during root canal therapy. Even with current chemomechanical techniques, debris and bacterial remnants are commonly left behind, which are generally believed to increase the risk of endodontic failure. This case details the use of a new technique to debride complex apical anatomy in a maxillary molar.
CASE REPORT: A 48-year-old female presented to the clinic with a chief complaint of increasing pain in her tooth. Clinical examination of the right first maxillary molar (#3) reve

This review article describes the methods and clinical recommendations for reinforcing traumatized anterior immature teeth with pulp necrosis treated with mineral trioxide aggregate (MTA) apexification. Traumatic injury can cause pulp necrosis and incomplete root formation in immature teeth. MTA apexification is the treatment of choice for necrotic immature teeth, particularly during the middle or late stages of root development. MTA apexification has a high success rate; however, failures due to cervical or root fractures occasionally occur. The risk of fracture is higher in immature teeth wi

The basic principles on which successful endodontic therapy depends have been commonly accepted as (1) debridement, (2) sterilization and (3) obturation. Each of these principles is subjected to critical evaluation. Because of the morphological complexity of most root canals complete debridement is not possible. No sampling technique is yet available to ensure total sterility of the prepared root canal nor is it possible to ensure that the root canal system is hermetically sealed by the filling material. Although it has been shown that 43 per cent of biopsies from the periapical area of endodo

This study evaluated the effects of two canal preparation methods on leakage in endodontically treated teeth, comparing the relative leakage rates for conventionally prepared teeth and those instrumented and obturated with the Ultradent Endo-Eze system. Two groups (n=11) of freshly extracted human premolars (and some canines) with closed apices and single roots were used. The roots in the first group were instrumented with conventional stainless steel hand files and obturated with gutta-percha and Sealapex. The experimental group were instrumented and obturated using the Ultradent Endo-Eze sys

A case is presented in which combined endodontic and orthodontic therapy was performed in traumatically injured teeth 12 months after an accident. Calcium hydroxide treatment was used to halt any possible resorption during orthodontic treatment. The teeth were repositioned in a desirable manner without any complication by orthodontic treatment. Root canal obturation was accomplished after the completion of active orthodontic treatment. Recall examination 12 months after completion of root canal treatment showed clinical and radiographic evidence of healing.

AIM: To report the healing of a large periapical lesion following non-surgical root canal treatment.
SUMMARY: A 12-year-old male with a large periapical lesion around the apices of his mandibular incisors is described. The lesion formed after trauma to the mandibular anterior teeth 5 years previously. During root canal treatment, chlorhexidine gluconate was used for irrigation, and calcium hydroxide was used both for the intracanal dressing and as a base of the root canal sealer. Periapical healing was observed 3 months after obturation and continued at the 12-month review. Key learning point

BACKGROUND: Bioceramic-based root canal sealers are increasingly important in root canal treatment because of their biocompatible properties. This study aimed to evaluate postobturation pain incidence and intensity after root canal obturation with NeoSealer Flo, MTA-Bioseal, and GuttaFlow bioseal calcium silicate-based sealers and AH Plus epoxy resin-based sealer in patients with asymptomatic irreversible pulpitis or necrotic pulp with chronic apical periodontitis.
METHODS: A total of 120 participants with single-rooted teeth were included and randomly divided into four groups according to th

Endodontic treatment was performed on 25 pulpally involved mandibular primary molars in 4 to 9-year-old children; the root canals were obturated with a new root canal filling material consisting of a mixture of calcium hydroxide, zinc oxide, and 10% sodium fluoride solution, using hand-operated lentulo-spirals. All cases were evaluated clinically every 3 months and also radiographically every 6 months to assess the success of the treatment; we also examined the resorption of the root canal filling material from the root canals and the status of overpushed material, if any, as the tooth resorbe

Taurodontism is a morpho-anatomical abnormality in the shape of a tooth. An enlarged pulp chamber, apical displacement of the pulpal floor and no constriction at the level of the cemento-enamel junction are the characteristic features. Special care is required in all aspects of endodontic treatment of a taurodontal tooth, including identifying the orifice, canal exploration, cleaning, shaping and obturation of the root canal. This case report describes the successful endodontic treatment of a taurodontic mandibular second premolar with five root canals.

A case of endodontic treatment of a mandibular first premolar exhibiting a total of four distinct root canals and four apical foramina is described. This occurrence in mandibular first premolar has rarely been reported in the endodontic literature. Endodontic treatment that considers the anatomic variation of root canal morphology is important to ensure a favorable healing outcome, and its identification could be enhanced by careful examination using a dental operating microscope. Obturation of root canals using a warm vertical compaction technique with a highly-radiopaque root canal sealer, s

BACKGROUND: Effective root canal obturation depends on intimate sealer adaptation to dentin, yet intracanal moisture at the time of obturation may alter sealer penetration according to material chemistry. Sealers with different chemical compositions may respond differently to residual intracanal moisture, particularly epoxy resin-based sealers and calcium silicate-containing resin-based sealers.
AIM: Study evaluates effect of different intracanal moisture conditions on the penetration of MTA Fillapex and AH Plus in dentinal tubules utilising confocal laser scanning microscopy.

Fracture of an endodontic instrument during an endodontic procedure is a common complication, but displacement of the separated instrument either inside or by the side of the inferior alveolar canal is very rare. This case report presents nonsurgical retreatment of a symptomatic, previously endodontically treated mandibular left first molar tooth with internal root resorption. The unique feature of this case report is the presence of a separated endodontic instrument entirely beyond the root apex and located deep within the body of the mandible. Under rubber dam isolation, access cavity prepar
