Road map to scaling-up: translating operations research study's results into actions for expanding medical abortion services in rural health facilities in Nepal.

BACKGROUND: Identifying unsafe abortion among the major causes of maternal deaths and respecting the rights to health of women, in 2002, the Nepali parliament liberalized abortion up to 12 weeks of pregnancy on request. However, enhancing women's awareness on and access to safe and legal abortion services, particularly in rural areas, remains a challenge in Nepal despite a decade of the initiation of safe abortion services. METHODS: Between January 2011 and December 2012, an operations research study was carried out using quasi-experimental design to determine the effectiveness of engaging fe

Bridging the knowledge gap: accessibility of SRHR information, services and competences of health facilities for adolescents and their care givers in rural Eastern Uganda.

INTRODUCTION: A quarter of Uganda's population is aged 10-19 years and majority live in rural areas. Access to sexual and reproductive health services and rights (SRHR) remains a challenge among this population, contributing to a high teenage pregnancy rate of 24.9% in rural Uganda. The study assessed the accessibility of SRHR information, services and competences of health facilities among adolescents and their care givers in Namutumba and Bugiri districts. METHODS: A cross-sectional study was conducted. 473 respondents were systematically and randomly recruited after informed consent and/ a

Explaining the attitude-behaviour gap in adolescent sexual and reproductive health service utilization: A cognitive dissonance perspective from rural Uganda.

Adolescents in low-resource settings hold positive attitudes toward sexual and reproductive health (SRH) services, yet fail to use them. This study presents a theoretical reinterpretation of mixed-methods data (N = 408 adolescents, aged 13-19) from Uganda's Busoga region, through Festinger's Cognitive Dissonance Theory. Quantitative findings showed health-seeking attitudes predicted SRH service utilization (β = 0.910, p < .001) and future intentions (β = 1.976, p < .001), but 52% of adolescents had no SRH service contact in past year, and mean uptake scores remained below 2.0 on a 5-point scal

Expanding the reach of pre-exposure prophylaxis services in suburban and rural Ontario: an implementation science evaluation of a public health-led model.

BACKGROUND: Despite the proven efficacy of pre-exposure prophylaxis (PrEP) in preventing HIV, disparities among service delivery prevail in Canada, particularly among suburban and rural regions. This study evaluates the reach and effectiveness of a mixed-service delivery PrEP clinic embedded within a public health unit, located in Southeastern Ontario. METHODS: The PrEP clinic possesses an interdisciplinary team that delivers in-person and remote services. In consultation with PrEP clinic staff, we used the RE-AIM Framework to describe client characteristics (reach) and evaluate implementatio

Decentralised hepatitis C testing and treatment in rural Cambodia: evaluation of a simplified service model integrated in an existing public health system.

BACKGROUND: Direct-acting antiviral treatment for hepatitis C virus (HCV) has provided the opportunity for simplified models of care delivered in decentralised settings by non-specialist clinical personnel. However, in low-income and middle-income countries, increasing overall access to HCV care remains an ongoing issue, particularly for populations outside of urban centres. We therefore aimed to implement a simplified model of HCV care via decentralised health services within a rural health operational district in Battambang province, Cambodia. METHODS: The study cohort included adult reside

Understanding the public health consequences of suspending a rural syringe services program: a qualitative study of the experiences of people who inject drugs.

BACKGROUND: Syringe services programs (SSPs) are evidence-based interventions that are associated with decreases in prevalence and incidence rates of HIV and viral hepatitis among people who inject drugs (PWID). SSPs are also effective conduits to deliver overdose prevention resources among PWID. In December 2015, the Kanawha-Charleston Health Department (KCHD) in West Virginia implemented a SSP; however, the program was indefinitely suspended in early 2018 following policy changes that would have forced the program to operate in ways that conflicted with established best practices. Changes in the provision and utilisation of health care services for chronic health conditions during the COVID-19 pandemic in rural northeast South Africa: an interrupted time series analysis.

BACKGROUND: The COVID-19 pandemic has impacted the provision and utilisation of health care services with varying magnitude across settings due to spatial temporal variation in the burden of COVID-19 cases and the roll-out of local COVID-19 response policies. This study assesses changes in the provision and utilisation of health care services for three major chronic health conditions (HIV/AIDS, hypertension, and diabetes) over the pre-COVID-19 and COVID-19 pandemic periods in a rural South African sub-district of Agincourt. METHODS: Segmented interrupted time series regression models are appl

Источник пометил публикацию как отозванную

BACKGROUND: India carries the largest national burden for rabies globally. Coordinating large-scale canine rabies elimination programs is challenging, particularly in rural areas, where the majority of human rabies deaths occur. This study evaluated the feasibility of combining canine rabies vaccination with pre-existing animal-health interventions or public health programs in a rural area of India. MATERIALS AND METHODS: Canine rabies vaccination teams collaborated with a bi-annual bovine foot-and-mouth vaccination program coordinated by the Animal Husbandry Department (AH-collaboration) and

Advancing Interprofessional Primary Health Care Services in Rural Settings for People with Chronic Low Back Disorders: Protocol of a Community-Based Randomized Controlled Trial.

BACKGROUND: Chronic low back disorders (CLBDs) are a substantial burden on individuals and societies, and impact up to 20% of Canadians. Rural and remote residents are approximately 30% more likely to have CLBDs. Reduced access to appropriate team-based health services, including physical therapy, is a key factor that may magnify the impact of CLBD on pain, physical function, overall quality of life, health-related system costs, and individual costs. OBJECTIVE: The purpose of this project is to evaluate the validity, comparative effectiveness, costs, barriers, and facilitators of an interprof

What Contributes to the Activeness of Ethnic Minority Patients with Chronic Illnesses Seeking Allied Health Services? A Cross-Sectional Study in Rural Western China.

Actively seeking health services lies at the core of effective models of chronic disease self-management and contributes to promoting the utilization of allied health services (AHS). However, the use of AHS by ethnic minority Chinese, especially the elderly living in rural areas, has not received much attention. This study, therefore, aims to explore the association between personal characteristics and the activeness of ethnic minority patients with chronic diseases in rural areas of western China seeking AHS. A cross-sectional study was conducted to collect data on the socio-demographic and e

Implementation of occupation-centred practice by occupational therapists in acute adult physical settings: A mixed method study in a regional and rural health service.

INTRODUCTION: Occupational therapy is underpinned by the belief that occupation facilitates health and wellbeing. However, evidence suggests that occupational therapists encounter challenges to implementing occupation-centred practice. The aim of this study was to investigate the uptake, acceptability and impact of a workplace intervention designed to enhance occupation-centred practice of occupational therapists in an acute adult physical context. METHODS: A concurrent mixed methods study using a pre-post design was employed. The setting was a regional and rural health service in Queensland.

Prevalence of Dental Caries and Periodontal Disease, Access to Dental Services and Perception of Oral Health in Adolescents and Adults from a Rural Community in Angola.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection has a direct impact on the gastrointestinal system, as up to 50% of fecal samples from coronavirus disease 2019 (COVID-19) patients contain detectable viral RNA despite a negative rhino-pharyngeal swab. This finding, together with an intestinal expression of angiotensin conversion enzyme 2 protein, suggests a possible fecal-oral transmission for SARS-CoV-2. Furthermore, gastrointestinal (GI) symptoms are common in COVID-19 patients including watery diarrhea, vomiting-particularly in children-nausea, and abdominal pain. Path

Correlates of HIV and STI Testing in Rural Southern Substance Using-Men who have Sex with Men: A Behavioral Model for Health Services Use Approach.

While research shows rural men who have sex with men (MSM) are less likely to get HIV and STI tested compared to urban MSM, research has not identified which groups of rural MSM are more or less likely to get tested. There is a dearth of literature of within-group differences of HIV/STI testing among rural MSM, especially among substance using-MSM who are more vulnerable to HIV/STI infections. This study examined factors associated with HIV and STI testing among rural substance-using MSM in the South, guided by the Andersen's Behavioral Model of Health Services Use. Participants (N = 284) comp

Predictors of male partners' behavioral intentions and cognitive readiness toward maternal health service utilization in rural Tanzania: an application of the theory of planned behavior.

BACKGROUND: Men are not only heads and decision-makers in most families, but they also heavily influence the healthcare access of their female partners and/or children. However, low male spousal accompaniment at health facilities remains a persistent public health challenge in rural Tanzania. While structural barriers are well documented, there is a lack of evidence regarding the behavioral intention factors that drive this men's physical disengagement from clinical spaces. Understanding these internal drivers is critical for designing effective targeted interventions to improve maternal servi

Implementing best practice occupational therapist-led environmental assessment and modification to prevent falls: A qualitative study of two regional and rural public health services in Australia.

BACKGROUND: Occupational therapist-led environmental assessment and modification (EAM) is effective in reducing falls for populations at high risk. Two regional and rural public health services in Queensland devised an implementation strategy to embed best practice occupational therapist-led EAM. METHODS: A qualitative study was conducted to compare the determinants of implementation success across the different health services, using the COM-B model of behaviour change. Six semi-structured interviews were completed with occupational therapists involved at each site, following 12 months of im
