Optimized 3D-FLAIR sequences to shorten the delay between intravenous administration of gadolinium and MRI acquisition in patients with Menière's disease.

OBJECTIVES: The aim of this study was to shorten the 4-h delay between the intravenous administration of gadolinium and MRI acquisition for hydrops evaluation using an optimized 3D-FLAIR sequence in patients with Menière's disease. METHODS: This was a single-center prospective study including 29 patients (58 ears), recruited between November 2020 and February 2021. All patients underwent a 3-T MRI with an optimized 3D-FLAIR sequence without contrast then at 1 h, 2 h, and 4 h after intravenous administration of gadobutrol. The signal intensity ratio was quantitatively assessed with the region

Inner ear pathologic features following mumps infection. Report of a case in an adult.

Temporal bone studies in an adult with a moderately severe, bilateral sensorineural hearing loss revealed bilateral cochlear changes 13 years after mumps infection. The organ of Corti was completely absent in the greater part of the superior horizontal basal limbs, with occasional hair cell loss throughout the rest of the cochlea. The outer sulcus cell area was degenerated. The stria vascularis was normal, as was the tectorial membrane, except for small hyaline droplets. The number of nerve fibers was extremely decreased in the spiral bony lamina of the basal turns. Basophilic material, possib

Do the organs of the labyrinth differentially influence the sympathetic and parasympathetic systems?

It has long been recognized that the vestibular system plays a major role in autonomic control. The nature of this control remains in dispute, however, as some evidence points to a vestibularly mediated parasympathetic activation, whereas other evidence points to a sympatho-excitatory role for labyrinthine outputs. A theoretical explanation is offered that attempts to resolve this issue by postulating that the utricles exert a predominantly sympatho-excitatory influence via their interactions with brain noradrenergic pathways, while the semicircular canals (and possibly saccules) increase para

MRI of endolymphatic hydrops in patients with intralabyrinthine schwannomas: a case-controlled study using non-enhanced T2-weighted images at 3 T.

OBJECTIVE: Like other vestibular schwannomas developing in the internal auditory canal, intralabyrinthine schwannomas (ILS) may present with similar symptoms as in endolymphatic hydrops. Two different studies have described MR saccular hydrops in ~ 30% of internal auditory canal vestibular schwannomas, but this association has never been studied in ILS before. The aim of this work is to study the prevalence of a saccular dilation in ILS, on a T2-weigthed sequence at 3 T, compared to a control group. MATERIAL AND METHODS: All patients presenting with typical ILS between January 2008 and Octobe

Robotic-assisted excision of a prostatic utricle cyst in a 12-month boy with proximal hypospadia and 45X0/ 46XY karyotype.

INTRODUCTION: Prostatic utricle cysts result from incomplete regression of Mullerian duct structures and occur most frequently in males with perineal or peno-scrotal hypospadias. Utricular cysts may present with various signs and symptoms including urinary tract infection, pain and post-void incontinence, a palpable abdominal mass or recurrent epididymitis. MATERIAL AND METHODS: We present a 12 months boy we treated due to proximal hypospadias in two stages, in association to a dysplastic undescended testis that was excised. The karyotype of the patient was 45X0/46XY and after successful hypo

Aminoglycoside Damage and Hair Cell Regeneration in the Chicken Utricle.

In this study, we present a systematic characterization of hair cell loss and regeneration in the chicken utricle in vivo. A single unilateral surgical delivery of streptomycin caused robust decline of hair cell numbers in striolar as well as extrastriolar regions, which in the striola was detected very early, 6 h post-insult. During the initial 12 h of damage response, we observed global repression of DNA replication, in contrast to the natural, mitotic hair cell production in undamaged control utricles. Regeneration of hair cells in striolar and extrastriolar regions occurred via high rates

[Streptomycin toxicity on dark cells of utricles in guinea pigs].

Thirty guinea pigs were divided into three groups. The first group received intramuscular injection of streptomycin sulfate 400 mg/kg/day for one week; the second group received the same dosage for two weeks; the third group served as the control. The cochlea was dissected after treatment and the utricle observed under electron microscope. The nystagmus lasted 11.3 sec. clockwise and 12.75 sec. counter-clockwise before treatment, 10.75 sec. and 9.25 sec. after one week and 6.0 sec. and 5.5 sec. after two weeks. Histopathologic study showed the normal dark cells in the control group. In the one

Effect of macular ablation on frequency and latency of motion-induced emesis in the squirrel monkey.

Three previously motion-emetic sensitive squirrel monkeys were rendered refractory to a standard motion-emetic regimen by a two-stage utriculosacculectomy procedure which preserved the cristae ampullares of semicircular canals. Three non-operated control squirrel monkeys tested on the same motion-emetic regimen time schedule as the operated animals remained motion-emetic sensitive with regard to incidence, frequency, and latency of motion-induced emetic responses. Following a sham surgical procedure (stapedectomy) performed on two of the latter animals and one additional new animal, the emetic

Cochleostomy or endolymphatic sac surgery for advanced Meniere's disease.

The operation of cochleostomy is described. The surgical technique is vital for the true assessment of results. If the cochlear duct is punctured, the risk to hearing is unacceptably high. If only the osseous spiral lamina is punctured, the results are acceptable in the short term, though the rationale cannot be explained. In the management of difficult patients with more advanced Meniere's disease, some variant of cochleostomy should probably be included in the therapeutic options. The results of endolymphatic sac surgery are significantly better in a matched group of patients.

Patients with CHARGE association: a model to study saccular function in the human.

The term CHARGE association refers to a combination of congenital malformations, the mnemonic CHARGE designating the most frequently occurring anomalies in the constellation. "C" indicates coloboma of the retina, "H" heart defects, "A" choanal atresia, "R" retarded growth and/or central nervous system anomalies, "G" genital hypoplasia, and "E" ear anomalies and/or deafness. The inner ear anomaly consists of a specific form of labyrinthine dysplasia that includes Mondini dysplasia of the pars inferior (cochlea and saccule) and complete absence of the pars superior (utricle and semicircular cana

A New Theory for Ménière's Disease: Detached Saccular Otoconia.

Ménière's disease is an inner ear disorder characterized by vertigo attacks, fluctuating and progressive hearing loss, tinnitus, and aural fullness in the affected ear. The pathophysiology of Ménière's disease remains elusive. Theories so far are anatomical variation in the size or position of the endolymphatic sac and duct, viral inflammation or autoimmune involvement of the sac, or a genetically determined abnormality of endolymph control. Animal studies on blocking the ductus reuniens and endolymphatic duct have produced hydrops in the cochlea, saccule, and utricle. Cone beam computed tomog

Neural hearing loss in a child with poliomyelitis: a histopathological study.

The temporal bones of a 26-month-old white female with a paralytic syndrome clinically and pathologically identical to poliomyelitis were examined. The aetiological agent was unknown although a non-poliomyelitis enterovirus infection seemed likely. There was a complete absence of the cochlear neurons and substantially reduced peripheral and central axons with loss of some inner hair cells but preservation of outer hair cells. Scarpa's ganglion, and the geniculate ganglion were partially atrophied. The saccule and utricle were mildly dilated and Reissner's membrane of the apical turn was bulgin

Magnetic resonance imaging for evaluation of otic labyrinth pathology.

The auditory (cochlear) labyrinth, the cochlea, holds the hearing sensory receptors. The vestibular labyrinth contains the sensory receptors for balance, which lie in the three semicircular canals and in the otolithic organs (the saccule and utricle). Pure tone audiometry and brain stem evoked audiometry can help to differentiate a peripheral cochlear disorder from central lesion. The type of nystagmus, severity of postural instability, and neurological evaluation can help to differentiate a peripheral vestibular disorder from a central vestibular lesion. The decision whether to perform an app

Bast's valve: what we know and what we don't.

PURPOSE: To synthesize current knowledge on the utriculo-endolymphatic valve (UEV, Bast's valve)-a flap-like structure at the junction of the utricular and endolymphatic ducts and to address the central question of whether it represents a passive anatomical remnant or an active regulator of endolymphatic homeostasis with implications for disorders such as Ménière's disease. METHODS: A critical narrative review of anatomical, imaging, experimental, computational, developmental, comparative, and pathological studies was conducted. Literature was identified through PubMed/MEDLINE, Embase, Web of

The Superiority of the Otolith System.

BACKGROUND: The peripheral vestibular end organ is considered to consist of semi-circular canals (SCC) for detection of angular accelerations and the otoliths for detection of linear accelerations. However, otoliths being phylogenetically the oldest part of the vestibular sensory organs are involved in detection of all motions. SUMMARY: This study elaborates on this property of the otolith organ, as this concept can be of importance for the currently designed vestibular implant devices. Key Message: The analysis of the evolution of the inner ear and examination of clinical examples shows the
