Safety and effectiveness of microdebrider bronchoscopy for the management of central airway obstruction.

BACKGROUND AND OBJECTIVE: Microdebrider bronchoscopy is a relatively new modality for the management of central airway obstruction (CAO) of both benign and malignant origin. Our objective was to describe our experience with this technique, with special attention to its safety and effectiveness. METHODS: We retrospectively reviewed cases of therapeutic bronchoscopies using microdebrider for CAO from two institutions (M.D. Anderson Cancer Center and Michael E. Debakey VA Medical Center, Houston) from August 2008 through February 2012. RESULTS: We identified 51 cases. Malignant CAO was detected

Safety of urate-lowering therapies: managing the risks to gain the benefits.

Gout is a metabolic disorder of purine metabolism and uric acid elimination. Over time, acute gout can develop into a chronic, disabling arthropathy, often associated with multiple comorbidities. Gout patients have often been undertreated, partly because of the clinician's perceived risks of a therapy outweighing its potential benefits. The approval of new therapies to treat hyperuricemia in gout has led to a new understanding of gout management and medication safety regarding new and old therapies. This review focuses on potential safety issues of currently available urate-lowering therapies

Safety and efficacy of intrathecal ziconotide in the management of severe chronic pain.

Ziconotide is a conopeptide intrathecal (IT) analgesic which is approved by the US Food and Drug Administration (FDA) for the management of severe chronic pain. It is a synthetic equivalent of a naturally occurring conopeptide found in the venom of the fish-eating marine cone snail and provides analgesia via binding to N-type voltage-sensitive calcium channels in the spinal cord. As ziconotide is a peptide, it is expected to be completely degraded by endopeptidases and exopeptidases (Phase I hydrolytic enzymes) widely located throughout the body, and not by other Phase I biotransformation proc

An evaluation of the safety and efficacy of administering rofecoxib for postoperative pain management.

UNLABELLED: The goal of our study was to evaluate the analgesic efficacy and safety of administering rofecoxib (1 mg/kg), a cyclo-oxygenase (COX)-2 selective nonsteroidal antiinflammatory drug, before pediatric tonsillectomy. Sixty-six patients, aged 3-11 yr, scheduled to undergo tonsillectomy received either placebo or rofecoxib (1 mg/kg). There were no significant differences between the two study groups with respect to demographics and blood loss. We found that the pain scores were significantly lower in the rofecoxib group compared with the control group at 2 h (P < 0.05) and 24 h (P < 0.0

Review of safety in endoscopic laser foraminoplasty for the management of back pain.

OBJECTIVE: The purpose of this study was to analyze the incidence and gravity of reported complications that arise in spinal surgery and assess the comparative safety, or otherwise, of endoscopic laser foraminoplasty (ELF). BACKGROUND DATA: Chemonucleolysis, decompression, discectomy, and fusion have long been cited as treatments for chronic low back pain. Over recent years newer, less invasive surgical techniques have become available, one such being ELF. Although minimally invasive, the beneficial outcome must be interpreted in relation to concerns regarding the safety of the procedure and

MRI Safety and Disaster Management for MR-Simulator and MR-Linacs.

Magnetic resonance imaging (MRI) is increasingly integrated into radiation oncology through dedicated MRI simulators and MR guided radiotherapy systems, improving target delineation and enabling adaptive treatment strategies. However, MRI introduces a distinct set of safety risks compared with conventional radiotherapy equipment, including hazards related to static magnetic fields, radiofrequency energy deposition, gradient switching, and cryogenic systems. Despite the growing adoption of MRI in radiation oncology, most reported safety incidents and lessons learned originate from radiology set

Quality and Safety Imperatives in the Identification and Management of Hereditary Cancer Syndromes.

OBJECTIVES: We sought to provide an evidence review of the rapidly expanding technologies and sciences that have moved the field of hereditary cancers from niche to mainstream in current cancer care. This shift mandates that all oncology clinicians have foundational knowledge regarding the identification, testing, and management of patients and families at risk for hereditary cancer. Many cancers have established germline biomarker testing recommendations (breast, ovarian, pancreatic, metastatic prostate, colon), and in many of these cancer types, somatic biomarker tests are also considered st

Safety and Efficacy of Radiofrequency Ablation in Management of Various Pancreatic Neoplasms.

Background/Objectives: Pancreatic neoplasms, including adenocarcinoma, pancreatic neuroendocrine tumors (pNETs), intraductal papillary mucinous neoplasms (IPMNs), and high-grade cystic lesions, often require surgical resection as a form of curative treatment. However, comorbidities and high-risk features may preclude surgery. Endoscopic ultrasound-guided radiofrequency ablation (EUS-RFA) has emerged as a minimally invasive alternative with proven cytoreductive efficacy in solid tumors. This case series evaluates the safety and efficacy of EUS-RFA in patients with various unresectable, non-meta

Safety of intravitreal chemotherapy in the management of retinoblastoma: A systematic review of the literature.

Intravitreal chemotherapy is used as a salvage therapy for retinoblastoma with persistent or recurrent vitreous seeding after primary treatment. To assess the safety of this technique, we conducted a systematic review of all studies reporting ocular toxicity data. Forty-eight trials involving 2751 eyes were included. The most common complications were cataract, retinal toxicity, and vitreous hemorrhage. However, severe and permanent adverse events were limited, while the risk of extraocular dissemination, a significant concern, was practically eliminated through preventive techniques. Globe sa
