Early hemodynamic and renal effects of hemorrhagic shock resuscitation with lactated Ringer's solution, hydroxyethyl starch, and hypertonic saline with or without 6% dextran-70.

BACKGROUND: Considering the renal effects of fluid resuscitation in hemorrhaged patients, the choice of fluid has been a source of controversy. In a model of hemorrhagic shock, we studied the early hemodynamic and renal effects of fluid resuscitation with lactated Ringer's (LR), 6% hydroxyethyl starch (HES), and 7.5% hypertonic saline (HS) with or without 6% dextran-70 (HSD). MATERIALS AND METHODS: Forty-eight dogs were anesthetized and submitted to splenectomy. An estimated 40% blood volume was removed to maintain mean arterial pressure (MAP) at 40 mm Hg for 30 min. The dogs were divided int

The added value of hypertonic saline solution to furosemide monotherapy in patients with acute decompensated heart failure: A meta-analysis and trial sequential analysis.

We assessed the effects of hypertonic saline solution (HSS) plus furosemide versus furosemide alone in patients with acute decompensated heart failure (ADHF). We searched four electronic databases for randomized controlled trials (RCTs) until June 30, 2022. The quality of evidence (QoE) was assessed using the GRADE approach. All meta-analyses were performed using a random-effects model. A trial sequential analysis (TSA) was also conducted for intermediate and biomarker outcomes. Ten RCTs involving 3013 patients were included. HSS plus furosemide significantly reduced the length of hospital sta

Impact of continuous hypertonic (NaCl 20%) saline solution on renal outcomes after traumatic brain injury (TBI): a post hoc analysis of the COBI trial.

BACKGROUND: To evaluate if the increase in chloride intake during a continuous infusion of 20% hypertonic saline solution (HSS) is associated with an increase in the incidence of acute kidney injury (AKI) compared to standard of care in traumatic brain injury patients. METHODS: In this post hoc analysis of the COBI trial, 370 patients admitted for a moderate-to-severe TBI in the 9 participating ICUs were enrolled. The intervention consisted in a continuous infusion of HSS to maintain a blood sodium level between 150 and 155 mmol/L for at least 48 h. Patients enrolled in the control arm were t

[Renoprotective effect of small volumes of hypertonic saline solution in chronic heart failure patients with marked fluid retention: results of a case-control study].

During intensive therapy of chronic heart failure (CHF) patients with marked fluid retention using high doses of i.v. furosemide the additional effect of agents which might exert osmotic attraction of interstitial fluids has been proposed. They are thought to impede the impairment of renal blood supply and glomerular filtration rate, which may be caused by a combined action of cardiac preload acute reduction, hypotension and neurohormonal activation. We therefore assessed in CHF patients with NYHA class III and BNP values from 900 to 1500 pg/ml, who were treated with i.v. furosemide, the predic

Comparison of utilizing a hypertonic saline solution and mannitol to improve brain relaxation during craniotomy in patients with brain tumours: a prospective randomized controlled trial.

Hyperosmolar therapy, specifically the use of mannitol, has been employed to improve brain relaxation, but mannitol use may cause hypovolemia and electrolyte imbalance. Given these risks, hypertonic saline was introduced as an alternative; however, data on its efficacy and safety are limited. Researchers conducted a prospective, double-blind, randomized controlled trial. Sixty-six patients with supratentorial or posterior fossa brain tumours undergoing craniotomy were randomized into two groups. Group M received 20% mannitol at 3 ml/kg, and Group H received 3% hypertonic saline at the same dos

Physician Preferences and Prescribing Patterns for Hypertonic and Isotonic Saline Solutions in Upper Respiratory Symptom Management: A Multinational Clinical Practice Survey Across 13 Countries.

BACKGROUND: Managing respiratory symptoms in patients with comorbidities presents complex therapeutic challenges due to the risk of drug interactions and contraindications associated with conventional treatments, requiring careful consideration of treatment options. Recent interest in natural therapeutic alternatives, especially seawater-based nasal sprays, reflects evolving physician attitudes and unmet clinical needs. Despite widespread clinical application, multinational data on physician perspectives toward isotonic and hypertonic saline therapies remain limited, particularly in patients w

Salt repletion and diuretic response: The role of serum chloride. A post-hoc analysis of the SALT-HF trial on furosemide and hypertonic saline solution administration in ambulatory patients with worsening heart failure.

AIMS: The efficacy of combining hypertonic saline solution (HSS) with loop diuretics in worsening heart failure (WHF) remains uncertain. Hypochloraemia has been associated with reduced diuretic efficacy. Some authors propose that chloride repletion may enhance natriuretic and diuretic responses. This post-hoc analysis of the SALT-HF trial evaluated the effect of single-dose administration of HSS plus intravenous (IV) furosemide versus IV furosemide, stratified by baseline serum chloride. METHODS AND RESULTS: The analysis included 148 ambulatory patients with WHF from the double-blind, randomi

Hyaluronic acid improves "pleasantness" and tolerability of nebulized hypertonic saline in a cohort of patients with cystic fibrosis.

INTRODUCTION: Inhaled hypertonic saline improves lung function and decreases pulmonary exacerbations in people with cystic fibrosis. However, side effects such as cough, narrowing of airways and saltiness cause intolerance of the therapy in 8% of patients. The aim of our study was to compare the effect of an inhaled solution of hyaluronic acid and hypertonic saline with hypertonic solution alone on safety and tolerability. METHODS: A total of 20 patients with cystic fibrosis aged 6 years and over received a single treatment regimen of 7% hypertonic saline solution or hypertonic solution with

Effect of Hypertonic Saline in Intra-Articular Hydraulic Distension for Adhesive Capsulitis.

BACKGROUND: Maintaining the integrity of the capsule along with infusing a sufficient amount of fluid is 1 of the therapeutic concepts in intra-articular hydraulic distension (IHD) for adhesive capsulitis. It has been known that hypertonic saline solution decreases tissue edema and increases the fluid volume within the epidural space, causing microdissection, in epidural adhesiolysis. OBJECTIVE: To investigate the effect of hypertonic saline solution in capsule-preserving intra-articular hydraulic distension (CPIHD) for adhesive capsulitis. DESIGN: Prospective randomized controlled trial. S

Hypertonic-hyperoncotic solutions improve cardiac function in children after open-heart surgery.

OBJECTIVES: Hypertonic-hyperoncotic solutions are used for the improvement of micro- and macrocirculation in various types of shock. In pediatric intensive care medicine, controlled, randomized studies with hypertonic-hyperoncotic solutions are lacking. Hypertonic-hyperoncotic solutions may improve cardiac function in children. The primary objective of this controlled, randomized, blinded study was to evaluate the hemodynamic effects and safety of hypertonic-hyperoncotic solution infusions in children shortly after open-heart surgery for congenital cardiac disease. The secondary objective was
