[«Verschroben»-phenomenon in schizophrenia and schizophrenia spectrum disorders: aspects of systematics].

AIM: Psychopathological analysis and typological differentiation of «verschroben»-phenomenon as the full-blown syndrome of defect in schizophrenia and schizophrenia spectrum disorders. MATERIAL AND METHODS: Fifty patients, 33 female and 17 male, mean age 44.8±8.5 years, with ICD-10 diagnosis of schizotypal disorder (F21) and residual schizophrenia (F20.5) were included in the study. Patients were examined using psychopathological and psychometric (SPQ-74) methods. RESULTS AND CONCLUSION: There was clinical heterogeneity of the «verschroben» type defect: the differentiation of primary defect …

Abnormal subjective experiences of schizophrenia: evaluation of the Eppendorf Schizophrenia Inventory.

The Eppendorf Schizophrenia Inventory (ESI) is a new clinical questionnaire aimed at self-experienced cognitive disturbances that are considered to be diagnostically specific for schizophrenia. Two ESI validation studies are presented in this report. The aim of study I was to search for objective correlates of the ESI in 100 schizophrenic inpatients. In study II, the diagnostic validity of the ESI was examined by group comparisons (inpatients with first episode or chronic schizophrenia, alcoholism, depression, or obsessive-compulsive disorder, and healthy adults). Study I yielded numerous plau…

Depersonalisation and schizophrenia: Comparative study of initial and multiple episodes of schizophrenia.

INTRODUCTION: The phenomena of depersonalisation/derealisation have classically been associated with the initial phases of psychosis, and it … METHODS: A descriptive, controlled and cross-sectional study of 48 patients diagnosed with paranoid schizophrenia (20 with an initial episode and 28 with multiple episodes). These patients were assessed using scales such as the Cambridge Depersonalization Scale, the Positive and Negative Symptom Scale, and the Dissociative Experiences Scale. RESULTS: Participa …

[Schizophrenia in children and adolescents: relevance and differentiation from adult schizophrenia].

Schizophrenia in childhood is rare (point prevalence <1/10,000 before the age of 12) and most often has insidious onset, severe clinical presentation and adverse course and outcome. The incidence of schizophrenia rises dramatically in adolescence, and its prevalence is estimated at 0.23% in the age between 13 and 18 years. The findings from clinical, neuroimaging, neuropsychological and neurobiological studies support that there is a substantial continuity between childhood, adolescent and adult schizophrenia, despite developmental differences. For this reason, the DSM-IV and ICD-10 criteria f…

Defining and predicting functional outcome in schizophrenia and schizophrenia spectrum disorders.

BACKGROUND: To assess criteria and to identify predictive factors for functional outcome. The criteria should cover all domains proposed by the Remission in Schizophrenia Working Group. METHOD: PANSS ratings were used to evaluate the symptomatic treatment outcome of 262 inpatients with schizophrenia spectrum disorders within a naturalistic multicenter trial. Functional remission was defined as a GAF score >61 (Global Assessment of Functioning Scale), SOFAS score >61 (Social and Occupational Functioning Scale) and a SF-36 mental health subscore >40 (Medical Outcomes Study-Short Form Health Sur…

Molecular mechanism of schizophrenia with reference to disrupted-in-schizophrenia 1 (DISC1).

Disrupted-in-schizophrenia 1 (DISC1) is a gene disrupted by a (1:1) (q42.1;q14.3) translocation that segregates with major psychiatric disorders in a Scottish family. To elucidate how DISC1 confers susceptibility to psychiatric disorders, identification of the molecules, which bind to the domain close to the translocation breakpoint in the DISC1 gene, was performed and fasciculation and elongation protein zeta-1 (Fez1), a novel DISC1-interacting protein, termed DISC1-binding zinc-finger protein (DBZ) and Kendrin were identified. The DISC1-Fez1 interaction is up-regulated by nerve growth factor…

Treatment refractory schizophrenia.

Treatment resistance constitutes a significant dilemma in schizophrenia since it affects a substantial number of patients, their families and the health care professionals involved in their care. Nonresponsiveness needs to be approached as a multidimensional syndrome by specifying which symptoms in the spectrum of positive symptoms, negative symptoms, excitement/hostility, cognitive symptoms, and anxiety/depression are failing to respond to treatment. This review presents some of the clinical, demographic and biological correlates of nonresponse, in addition to compliance issues, psychosocial …

Simple schizophrenia: revisited.

Simple schizophrenia is no longer a part of official American classification schemes, but is a diagnosis still used in other parts of the world. Long considered a traditional schizophrenic subtype, in time the concept became unclear, was used infrequently, and exhibited poor reliability and doubtful descriptive validity. We reviewed charts of 52 patients diagnosed with simple schizophrenia. Our findings confirm the descriptive heterogeneity and diagnostic imprecision of simple schizophrenia.

[Treatment-resistant schizophrenia].

Schizophrenia is a psychiatric disorder that causes severe cognitive, behavioral and social dysfunction, responsible for a shortening of the life expectancy of patients, with an increased risk of suicide, cardiovascular disease and cancer. The management of patient with schizophrenia is global and atypical antipsychotics, antagonizing dopamine pathway, are the first line pharmacological treatment. Clozapine, the first atypical antipsychotic discovered, is currently still the most effective molecule against schizophrenia, while causing less extrapyramidal side effects. Its particular pharmacolo…
