Delayed Sciatic Neuropathy Due to Late Migration of Cerclage Wire Following Total Hip Arthroplasty.

Femoral cerclage wiring is commonly used during total hip arthroplasty (THA) to prevent propagation of intraoperative femoral cracks. Neurologic complications related to cerclage wiring are extremely rare, particularly as delayed presentations. We report a 62-year-old woman who developed acute, severe sciatic pain 7 years after uncemented THA in which a prophylactic 18-gauge cerclage wire had been applied for a nondisplaced femoral crack. Lumbar imaging was normal, but pelvic radiographs showed wire breakage. Magnetic resonance imaging demonstrated migration of a wire fragment adjacent to the

Ultrasound-guided nerve hydrodissection for sciatic neuropathy caused by piriformis rhabdomyolysis: A case report.

Peripheral entrapment neuropathy is a rare complication of rhabdomyolysis and lacks successful treatment. In this article, we report a case of sciatic neuropathy caused by piriformis rhabdomyolysis which was successfully treated with ultrasound-guided sciatic nerve hydrodissection. A 27-year-old male patient with left buttock pain and left lower limb weakness received ultrasound-guided nerve hydrodissection via injection of dexamethasone and 5% dextrose water into the area surrounding the left sciatic nerve. After injection, the patient's pain and left lower extremity weakness subsided. In con

Controllable forces for reproducible chronic constriction injury mimicking compressive neuropathy in rat sciatic nerve.

BACKGROUND: Compressive neuropathy is a recurring and challenging disease for patients, regardless of medical or surgical treatment. Neuropathological severity is associated with the force of mechanical compression. Available animal models do not address mechanical issues with reproducible outcomes. We used a chronic constriction injury model to analyze tension-controlled compressive neuropathy and achieve reproducible functional outcomes. NEW METHOD: We refined a modified animal model for chronic constriction nerve injury under controllable compressive tensile strength to target the unilater

Postoperative Sciatic Neuropathy Following Ankle Surgery and Peripheral Nerve Blockade: A Case-Based Review.

Peripheral nerve blocks are commonly used for intraoperative and postoperative analgesia in foot and ankle surgery due to superior pain control and opioid-sparing effects. Although considered safe, neurologic complications can occur and present diagnostic and management challenges for the anesthesiologist. This case-based educational review describes a 72-year-old patient who developed persistent postoperative sciatic-distribution motor and sensory deficits after ankle open reduction and internal fixation performed with adductor canal and popliteal sciatic nerve blocks. The clinical scenario h

Sciatic Nerve Neuropathy Secondary to Sacral Pressure Ulcer Following COVID-19: A Case Report.

We report the case of a previously healthy 25-year-old man who developed severe SARS-CoV-2 infection requiring prolonged mechanical ventilation. Following extubation, he presented with peripheral sensory deficits and a sacral pressure ulcer. He was later admitted to our service with fever and a Grade IV ulcer, which required surgical debridement, negative pressure wound therapy, and definitive reconstruction using a V-Y gluteal flap. Examination of the right leg at admission demonstrated a neuropathic pain pattern, with neurological evaluation and imaging confirming distal sensorimotor polyneu

Tang Bi formula alleviates diabetic sciatic neuropathy via AMPK/PGC-1α/MFN2 pathway activation.

Diabetic peripheral neuropathy (DPN) is one of the most common chronic complications of diabetes mellitus, which affects various regions of the nervous system. Tang Bi formula (TBF) has been proven effective for DPN, while the underlying mechanism remains unclarified. This study aimed to clarifiy the neurprotective mechanism of TBF intervention in DPN through animal and cell models. UHPLC/QTOF-MS and network pharmacology analysis were utilized to identify the bioactive components and potential targets of TBF. DPN models were established in rats and Schwann cells to evaluate the therapeutic eff

Expression of Nrf2 Promotes Schwann Cell-Mediated Sciatic Nerve Recovery in Diabetic Peripheral Neuropathy.

BACKGROUND/AIMS: High glucose-induced oxidative stress and inflammatory responses play an important role in painful diabetic neuropathy by activating the TLR4/NFκB signal pathway. Schwann cells (SCs) are integral to peripheral nerve biology, contributing to saltatory conduction along axons, nerve and axon development, and axonal regeneration. SCs provide a microenvironment favoring vascular regeneration but their low survival ratio in hyperglycemic conditions suppress the function to promote nerve growth. Nuclear factor erythroid 2-related factor 2 (Nrf2) promotes remyelination after periphera

Bilateral Sciatic Neuropathy after an Autologous Breast Reconstruction in a Massive Weight Loss Patient.

Perioperative compression neuropathy is a known potential complication of prolonged surgical procedures. Sciatic postoperative neuropathy has rarely been reported. We present a 34-year-old woman who underwent right breast reconstruction with supercharged (venous anastomosis) transverse rectus abdominis flap and developed bilateral sciatic compression neuropathy. Her history was remarkable for sleeve gastrectomy 2 years earlier resulting in 105 pound weight loss 1 year before breast reconstruction. During the procedure, the patient was in the supine position for 8 hours and in the semirecumbent

Intraoperative cortical somatosensory evoked potentials for detection of sciatic neuropathy during total hip arthroplasty.

Approximately 3% of patients undergoing hip arthroplasty develop postoperative sciatic neuropathy. The factors associated with changes in somatosensory evoked potentials (SSEP) and sciatic neuropathy were examined in patients undergoing hip arthroplasty, to evaluate whether the use of intraoperative SSEP could help reduce the incidence of postoperative sciatic neuropathy. Eighty-eight patients were assigned to either monitored or unmonitored groups. SSEP were recorded following peroneal nerve stimulation, using contralateral stimulation to detect systemic influences on SSEP. Amplitude reductio

Application of angiotensin II to healthy rat sciatic nerve can produce neuropathy without associated vasculopathy.

Elevated angiotensin II (AII) levels have been associated with hypertension, diabetes, and polyneuropathy. It is unknown whether AII applied to healthy nerve can be used to model a confined neuronal injury by producing localized vasculopathy and associated neuropathy. In this study, angiotensin II (2.2 μg/ml) or saline was infused constantly via osmotic pump onto the sciatic nerve of 20 rats for 12 weeks. Nerve conduction studies were repeated every 4 weeks, and sciatic nerve was collected for pathological analysis at 12 weeks. Animals infused with AII showed a significant decrease in nerve fi

Presumed septic sacroiliitis in a puppy with unilateral hind limb lameness and sciatic nerve neuropathy.

A 5-month-old female entire Dachshund presented with an acute onset of left hind limb lameness following intense play. There were concurrent intermittent neurological deficits in the left hind limb, and pain in the lumbosacral region and on dorsal flexion of the tail. Computed tomography (CT) imaging revealed an asymmetric widening of the left sacroiliac joint with loss of cortical margins, accompanied by mild osteolytic changes of the adjacent ilium and sacrum highly suggestive of septic sacroiliitis. There was also perilesional steatitis in the region of the sciatic nerve. An 8-week course o

Antinociceptive and Anti-Inflammatory Effects of Nypa fruticans Wurmb by Suppressing TRPV1 in the Sciatic Neuropathies.

Neuropathic pain is generally characterized by sensory abnormalities such as sensory disorders, hyperalgesia, and allodynia. Recent studies have reported that TRPV1 activation is essential for establishing of inflammation in the neuropathy pain models, showing that the expression of this receptor is increased, and contributing to enhanced thermal sensitivity. Nypa fruticans Wurmb (NF), which was used as a folk remedy, is a plant that is gaining attention due to its various effects. In this study, we investigated the antinociceptive and anti-inflammatory effects of NFE (Nypa fruticans Wurmb ext

Experimental study of the etiology and pathogenesis of sciatic neuropathy under conditions of therapy for ischium fractures.

Surgical (with external fixation device; series I) and conservative (series II) treatment of experimental unilateral transverse fracture of the body and ramus of ischium was carried out in 35 mongrel dogs. Roentgenometry, pathological, and histological studies showed sciatic nerve injury in both series, a complication of the fracture emerging at the moment of primary lesion. Further damage to the nerve due to its compression in an anatomical narrowings with fibrous degenerative tissues and bone fragments under conditions of conservative therapy, tunnel neuropathy, was paralleled by more pronou

Chronic Sciatic Neuropathy in Rat Reduces Voluntary Wheel-Running Activity With Concurrent Chronic Mechanical Allodynia.

BACKGROUND: Animal models of peripheral neuropathy produced by a number of manipulations are assessed for the presence of pathologic pain states such as allodynia. Although stimulus-induced behavioral assays are frequently used and important to examine allodynia (ie, sensitivity to light mechanical touch; von Frey fiber test), other measures of behavior that reflect overall function are not only complementary to stimulus-induced responsive measures, but are also critical to gain a complete understanding of the effects of the pain model on quality of life, a clinically relevant aspect of pain o

Sciatic Neuropathy Caused by Focal Venous Engorgement Associated With Deep Vein Thrombosis: A Case Report.

A 69-year-old woman complained of severe left leg weakness with paresthesia. Electrodiagnostic tests revealed sciatic neuropathy, and imaging studies showed venous engorgement around the sciatic nerve. After anticoagulant therapy and intensive rehabilitation, the patient's muscle strength improved from 1 to 3 on the Medical Research Council scale. The diagnosis of sciatic neuropathy caused by deep vein thrombosis is extremely rare; however, it should be considered in the differential diagnosis of unilateral lower extremity weakness in patients recovering from intensive care.

Transcriptomic profiling of sciatic nerves and dorsal root ganglia reveals site-specific effects of prediabetic neuropathy.

Peripheral neuropathy (PN) is a severe and frequent complication of obesity, prediabetes, and type 2 diabetes characterized by progressive distal-to-proximal peripheral nerve degeneration. However, a comprehensive understanding of the mechanisms underlying PN, and whether these mechanisms change during PN progression, is currently lacking. Here, gene expression data were obtained from distal (sciatic nerve; SCN) and proximal (dorsal root ganglia; DRG) injury sites of a high-fat diet (HFD)-induced mouse model of obesity/prediabetes at early and late disease stages. Self-organizing map and diffe

An experimental painful peripheral neuropathy due to nerve constriction. I. Axonal pathology in the sciatic nerve.

A constriction injury to the sciatic nerve of the rat produces a painful peripheral neuropathy that is similar to the conditions seen in man. The pathology of the sciatic nerve in these animals was examined at 10 days postinjury, when the abnormal pain sensations are near maximal severity. The nerves were examined with (1) complete series of silver-stained longitudinal sections of pieces of the nerve (3 cm or more) that contained the constriction injury in the center, (2) toluidine blue-stained semithin sections taken at least 1 cm proximal and 1 cm distal to the constriction, and (3) EM secti

Octanol alleviates chronic constriction injury of sciatic nerve-induced peripheral neuropathy by regulating AKT/mTOR signaling.

OBJECTIVE: Activation of gap junction channels can induce neuropathic pain. Octanol can limit the conductance of gap junctions containing connexin 43 proteins. Thus, this study focused on the roles of octanol in chronic constriction injury (CCI)-induced peripheral neuropathy in mice and its mechanisms of action. METHODS: Male mice were assigned into control, sham, CCI, CCI + Octanol-20 mg/kg, CCI + Octanol-40 mg/kg and CCI + Octanol-80 mg/kg groups. CCI was performed by applying three loose ligations to mouse sciatic nerve, and the mice with CCI was administered with 20 mg/kg, 40 mg/kg, or 80
