The clinical utility of search coil horizontal vestibulo-ocular reflex testing.

CONCLUSION: Testing of the horizontal vestibulo-ocular reflex (VOR) with head rotations (including head impulses) using the magnetic scleral search coil technique (SCT HHI) provides valuable additional diagnostic information in patients with persistent dizziness, oscillopsia or imbalance. It identifies high and low frequency/acceleration vestibular abnormalities that are frequently missed using other methods. OBJECTIVES: To evaluate the diagnostic utility of SCT measurement of the horizontal VOR in the multidisciplinary neurotology clinic of a tertiary referral centre. PATIENTS AND METHODS:

Impact of endolymphatic hydrops on the function of the horizontal canal during caloric stimulation in Ménière's disease.

PURPOSE: When a dizzy patient with episodic vertigo has an abnormal caloric and a normal video head impulse test (vHIT), this caloric-vHIT dissociation provides vital information for a diagnosis of Ménière's disease (MD). Endolymphatic hydrops (EH), a histological marker of MD, is hypothesized to be involved in the caloric-vHIT dissociation in MD through hydropic duct distension of the horizontal semicircular canal (SC). This study was designed to determine the impact of EH on the function of horizontal SC during caloric stimulation. METHODS: Caloric test and vHIT were used to evaluate the fu

Non-contrast MRI of Inner Ear Detected Differences of Endolymphatic Drainage System Between Vestibular Migraine and Unilateral Ménière's Disease.

OBJECTIVE: We aimed to evaluate the diagnostic performance of some anatomical variables with regard to endolymphatic sac (ES) and duct (ED), measured by non-contrast three-dimensional sampling perfection with application-optimized contrasts using different flip angle evolutions (3D-SPACE) magnetic resonance imaging (MRI), in differentiating vestibular migraine (VM) from unilateral Ménière's disease (MD). METHODS: In this study, 81 patients with VM, 97 patients with unilateral MD, and 50 control subjects were enrolled. The MRI-visualized parameters, such as the distance between the vertical pa

On the vestibular labyrinth of Brachiosaurus brancai.

The extensive remains of large sauropods, excavated in the Upper Jurassic layers of the Tendaguru region of Tanzania, East Africa by Janensch [15], include an intact fossil cast of a vestibular labyrinth and an endocast of the large Brachiosaurus brancai. The approximately 150 million year old labyrinth cast demonstrates clearly a form and organisation congruent in detail to those of extant vertebrate species. Besides the near-orthogonal arrangement of semicircular canals (SCCs), the superior and inferior branches of the vestibulo-acoustic nerve, the endolymphatic duct, the oval and round wind

Drilling technique of the internal auditory canal in vestibular schwannoma surgery assisted by neuronavigated autofocus microscope.

BACKGROUND: The wide anatomical variability of temporal bone structures makes the application of neuronavigation particularly useful. This technical note outlines an IAC drilling technique performed with the assistance of a neuronavigated autofocus microscope to enhance intraoperative anatomical orientation, facilitate tailored bone removal and prevent injuries at the intraosseous petrous structures. METHOD: From January 2023 to January 2024, twenty-one out of thirty-six patients with vestibular schwannoma underwent a retrosigmoid transmeatal approach with IAC drilling assisted by the neurona

[Isolated functional loss of the lateral semicircular canal in vestibular neuritis].

Today, modern tools in vestibular testing are feasible to provide information about functional status of all five peripheral vestibular receptors. Isolated or combined loss of crista and macula receptor function can be determined in the diagnostic process. We describe a seldom case of isolated functional loss of lateral semicircular canal receptor function in a 55-year-old patient. Whereas there was no ispilateral caloric response and video head impulse test revealed a catch-up saccade, air-conducted (AC) cervical and ocular vestibular-evoked myogenic potentials (cVEMP, oVEMP), subjective visu

[Retraction of the endolymphatic membranes in temporal bones of the Wittmaack and Tufts collections].

BACKGROUND: In histologic studies, the volumetric status of the intralabyrinthine fluids is judged by the position of the endolymphatic membranes. Bulging of the membranes, commonly known as endolymphatic hydrops, is assumed to be caused by excess of endolymph. The opposite situation, retraction of the membranes is, however, only incidentally described and relatively little attention has been paid to its significance. Almost one hundred years ago Wittmaack described retraction of the endolymphatic membranes, which has since been considered to be preparation artifact--a concept that essentially

A new hypothesis on the plurifactorial etiology of Meniere's disease.

Meniere's disease is characterized by an extremely long trend: this fact, as in other diseases of very wide chronicity, permits one to formulate a special type of hypothesis regarding the aetiology and the pathogenesis of the disease; it is comprised of many individual factors, of differing chronology and which are connected to each other with successive causality. This hypothesis was founded on a large body of data concerning the multiple histopathological effects on the inner ear of an initially inflammatory disease of the middle ear.

Mutation of the POU-domain gene Brn4/Pou3f4 affects middle-ear sound conduction in the mouse.

Mutagenesis of the POU-domain gene Brn4/Pou3f4 causes defects in the cochlear duct, semicircular canal, temporal bone and stapes footplate. The footplate defect suggested a middle-ear conductive component to the hearing loss associated with this mutation. This was examined by measuring velocity transfer functions at the umbo of wild type and knockout mice during sound stimulation of the tympanic membrane. When the median umbo velocity of test frequencies in the two groups were compared, the mid-range frequencies of the knockout mice showed a statistically reliable reduction in velocity (maximu

Virtual reconstruction of the endocranial anatomy of the early Jurassic marine crocodylomorph Pelagosaurus typus (Thalattosuchia).

Thalattosuchians were highly specialised aquatic archosaurs of the Jurassic and Early Cretaceous, and represent a peak of aquatic adaptation among crocodylomorphs. Relatively little is known of their endocranial anatomy or its relevance for the evolution of sensory systems, physiology, and other aspects of biology. Nevertheless, such data have significance for two reasons: (1) thalattosuchians represent an important data point regarding adaptation to marine life in tetrapods; and (2) as early-diverging members of the crocodylian stem-lineage, thalattosuchians provide information on the evoluti

Ultrastructure of melanocytes in the dark cell area of human vestibular organs: functional implications of gap junctions, isolated cilia, and annulate lamellae.

BACKGROUND: It is known that melanocytes exist in almost all parts of the inner ear, such as the cochlear duct, stria vascularis, Reissner's membrane, modiolus, vestibular organs in the region surrounding the cristae and maculae, semicircular canals, and pars rugosa of the endolymphatic sac. But there have been few studies using human materials, because of the difficulty of obtaining materials. We attempted to investigate the detailed ultrastructure of melanocytes in the vestibular organs of human inner ear. METHODS: Eight surgical specimens obtained from patients with vestibular schwannoma w

Harvest of Vestibular End-Organs under Physiologic Conditions during Labyrinthectomy.

The living human inner ear is challenging to study because it is encased within dense otic capsule bone that limits access to biological tissue. Traditional temporal bone histopathology methods rely on lengthy, expensive decalcification protocols that take 9-10 months and reduce the types of tissue analysis possible due to RNA degradation. There is a critical need to develop methods to access fresh human inner ear tissue to better understand otologic diseases, such as Ménière's disease, at the cellular and molecular level. This paper describes a technique for the harvest of human vestibular en

Multimedia article: laparoscopic left lateral hepatic lobectomy for metastatic colorectal tumor.

BACKGROUND: The growth of experience in laparoscopic surgery, technological improvements in laparoscopic instruments, and the application of laparoscopy to oncology surgery are responsible for the new challenge of laparoscopic liver surgery. Several series of laparoscopic liver resections have been reported, and these series have shown the feasibility of resections. The first anatomical laparoscopic liver resection was a left lateral segmentectomy, reported in 1996 by Azagra et al. due to favorable anatomy of this hepatic segment for a totally laparoscopic approach. METHODS: This video shows

[Canalith repositioning maneuver: proposal of a new therapy for benign paroxysmal positional vertigo of the posterior semicircular canal].

A new therapy is proposed for the treatment of Benign Paroxysmal Positional Vertigo (B.P.P.V.) of the Posterior Semicircular Canal (P.S.C.): the Canalith Repositioning Maneuver (C.R.M.). The need for a new maneuver to treat B.P.P.V. of the P.S.C. arises from the difficulties encountered in daily practice, under particular conditions (i.e. elderly, obese, traumatized patients and in the presence of rachis pain, etc.), to perform the most common rehabilitative techniques such as the Semont Maneuver and Epley's Canalith Repositioning Procedure (C.R.P.). The results achieved using this new techniq

Labyrinthitis ossificans: histopathologic consideration for cochlear implantation.

Labyrinthitis ossificans may be a hindrance to cochlear implantation by making electrode insertion difficult. We performed a histopathologic study of 24 temporal bones with labyrinthitis ossificans from multiple causes. The organ of Corti was graphically reconstructed and the degree of obstruction was estimated for each millimeter of the cochlea. Correlations were calculated between the degree of new bone formation and the cause, patient's age and sex, and time from the original temporal bone insult. Our results demonstrate that complete cochlear ossification is rare. The scala tympani in the

Computed tomography reveals the endocranial anatomy of Crocodylia: Implications for phylogenetic relationships and ecomorphological convergence across Crocodylomorpha.

Living crocodylians (alligators, caimans, gavials and 'true' crocodiles) are the remnants of a broader clade, Crocodylomorpha, that displays high ecomorphological diversity, particularly in skull shape. However, there are several instances of ecomorphological convergence between distant lineages, especially pertaining to the independent acquisition of a long, slender, longirostrine snout. In some instances, it is difficult to determine whether extinct species belong to one longirostrine lineage or another, with this problem especially prevalent with regards to the gavialoid crocodylian clade.

Bmp4 is essential for the formation of the vestibular apparatus that detects angular head movements.

Angular head movements in vertebrates are detected by the three semicircular canals of the inner ear and their associated sensory tissues, the cristae. Bone morphogenetic protein 4 (Bmp4), a member of the Transforming growth factor family (TGF-beta), is conservatively expressed in the developing cristae in several species, including zebrafish, frog, chicken, and mouse. Using mouse models in which Bmp4 is conditionally deleted within the inner ear, as well as chicken models in which Bmp signaling is knocked down specifically in the cristae, we show that Bmp4 is essential for the formation of al

A place principle for vertigo.

OBJECTIVE: To provide a road map of the vestibular labyrinth and its innervation leading to a place principle for different forms of vertigo. METHOD: The literature describing the anatomy and physiology of the vestibular system was reviewed. RESULTS: Different forms of vertigo may be determined by the type of sense organ, type of ganglion cell and location in the vestibular nerve. CONCLUSION: Partial lesions (viral) of the vestibular ganglion are manifested as various forms of vertigo.

Study of a temporal bone of Homo heildelbergensis.

CONCLUSIONS: The characteristic features of the Hh specimen conformed to those of other Pleistocene human fossils, indicating strong cranial structures and a heavy mandible. The mastoid was large and suggested a powerful sternocleidomastoid muscle. The inner ear and tympanic cavities were similar in size and orientation, suggesting that their functions were probably similar. Our observations suggest that the left ear of this Hh specimen was healthy. The large canaliculo-fenestral angle confirms that this ancestor was bipedal. It also strongly suggests that Hh individuals were predisposed to de
