Fracture of the thumb sesamoid bones is rare. We report here a fracture of the ulnar sesamoid in a 25-year-old man caused by a fall on his outstretched hand. He complained of pain and loss of thumb motion. Physical examination revealed tenderness, swelling, and a hematoma over the first metacarpal joint of the right thumb. Radiographs showed a fracture of the ulnar sesamoid which should not be confused with normal variants that occasionally arise from ossifications centers that fail to unite.

Four cases of osteonecrosis of hallucal sesamoids are reported here. Surgical excision of necrotic sesamoid tissue yielded satisfactory results, with the patients reporting no residual pain. Although it has not been frequently addressed in the literature, avascular necrosis of the sesamoid bones should be considered in the differential diagnosis of persistent forefoot pain.

The pathomechanics for the development of the hallux valgus deformity takes place at the first metatarsophalangeal joint--the sesamoid complex. The sesamoid complex consists of seven muscles, eight ligaments, and two sesamoid bones. When the first metatarsal escapes the complex and drifts medially, the sesamoids remain twisted in situ, several of the ligaments "fail," and others contract. The authors propose reduction of the metatarsus primus varus by first metatarsal osteotomy and appropriate ligament releases and plications to restore alignment. A detailed understanding of the pathomechanics

BACKGROUND: Sesamoid displacement is a characteristic feature of hallux valgus; however, its three-dimensional positional changes relative to the surrounding bony structures remain unclear.
PURPOSE: To compare the three-dimensional position of the lateral sesamoid in hallux valgus with sesamoid dislocation and in normal feet using computed tomography.
STUDY DESIGN: Retrospective comparative study.
METHODS: Feet with hallux valgus and sesamoid dislocation (Group A) were compared with normal feet (Group B). Three-dimensional computed tomography reconstructions were used to evaluate the spatia

This paper reviews 26 patients who presented with disorders of one or both sesamoids of the foot. There is a high association between long-distance running and chondromalacia or stress fractures in these bones. Thirty-eight per cent of these patients had variations in the anatomy at this articulation, including asymmetry in sesamoid size, symmetrical sesamoid enlargement, metatarsal condylar hypoplasia, and metatarsal rotational abnormalities.

A case of avulsion fracture of the lateral sesamoid is reported in association with dislocation of themetatarsophalangeal joint of the hallux. The radiographic criteria to establish the diagnosis of fracture in sesamoid bones are outlined. Review of the literature reveals only one other reported case associated with a hallux dislocation, and in that case, the radiographic diagnosis was not clearly established.

Accessory ossicles and sesamoid bones are skeletal variations, more commonly seen in the region of foot and ankle. Most such accessory and sesamoid bones remain asymptomatic. However overuse and trauma can make such feet symptomatic. Knowledge of such bony ossicles is essential in the management of patients presenting with foot pain. Dorsal foot pain can be caused by a symptomatic Os intermetatarseum - an accessory ossicle found between the bases of first and second metatarsals and the medial cuneiform. Its incidence has not been well established because of insufficient appropriate multi-centr

Introduction An awareness of the anatomical parameters of the foot such as the position and orientation of the sesamoid bones can be of great value for the etiology and diagnostic approach to patients with hallux valgus (HV). The purpose of this study was to evaluate the basic anatomical features and measurements related to the characteristics of HV in cadaveric material. Materials and methods The study sample included 12 cadaveric limbs with HV and 10 cadaveric limbs without HV as a control group. We measured the HV angle (HVA), the first to second intermetatarsal angle (IMA), and the first t

By means of roentgenography method with subsequent roentgenogramometry and roentgenodensitometry, morphological peculiarities of structure and mineralization of the hand bones in sportsmen-volley-ball players and in schoolchildren who do not go in for sports, at the age of 14-19 years have been studied. Systematic training in volley-ball produces certain increase in dimentions of the hand bones, prolongation of their synostosis time, intensification in development of the sesamoid bones and increasing amount of mineral salts in their bones.

Subluxation of the metatarsosesamoid joints frequently occurs with the development of hallux valgus deformity, and the restoration of a normal metatarsosesamoid articulation has been proposed as essential for achieving a biomechanically sound operative result. The position of the sesamoid bones on the AP radiograph is used often to assess the pre- and postoperative relationship between the hallucal sesamoids and the metatarsal sulci. We evaluated the validity of this approach. Thirty subjects with hallux valgus and 30 control subjects participated in this study by undergoing both AP and tangen

The microvascular anatomy of the sesamoid complex was investigated in 15 cadaver specimens using histology and tissue clearing (Spalteholz) techniques. It was found that both sesamoids appeared equally well vascularized and the vascular supply to each sesamoid originated from two major sources (proximal and plantar) and one minor source (distal). Proximally, vessels originating from the first plantar metatarsal artery enter the sesamoid at its attachment to the flexor hallucis brevis. In addition, vessels enter the plantar surface of the sesamoid near the midline and arborize throughout the bo

We report two cases of nontraumatic metatarsal pain with sclerosis and fragmentation of the lateral sesamoid bone on roentgenographs and computed tomography images. One patient underwent magnetic resonance imaging (MRI), which showed low signal from the sesamoid bone. These imaging findings suggested osteonecrosis. Histology of the sesamoidectomy specimen confirmed this diagnosis in one patient. Avascular necrosis of the metatarsal sesamoid is an uncommon disorder. The suggestive roentgenographic and MRI findings rule out the other painful conditions of the sesamoid bone. The features are revi

A case of bilateral congenital aplasia of the carpal scaphoid bone is presented. In contrast to previously reported cases of congenital hypoplasia of the scaphoid, this case was not associated with hypoplasia or absence of thenar and forearm muscles, absence of the sesamoid bones of the thumb, abnormalities of the skeleton of the thumb ray or hypoplasia of the forearm bones.

During the 14-year period of 1971-1985, 368 stress fractures in 324 athletes were treated. The series contained 268 fractures in males and 100 fractures in females; 32 fractures occurred in children (less than 16 years), 117 in adolescents (16-19 years), and 219 in adults. Forty-six fractures were incurred by athletes at an international level, 274 by athletes at a national or district level and 48 by recreational athletes. Of the total cases, 72% occurred to runners and a further 12% to athletes in other sports after running exercises. The distribution of the stress fractures by site was: tib

Lesser metatarsal sesamoids are one of the most common accessory bones of the foot and are most commonly seen at the fifth metatarsophalangeal joint. They are rarely seen in other metatarsophalangeal joints. In the literature, there are reports of solitary accessory sesamoid bones seen at lesser metatarsophalangeal joints. We report the case of a 68-year-old woman with lesser metatarsal sesamoids accompanying all of the metatarsophalangeal joints.

Several groups of tetrapods have expanded sesamoid (small, tendon-anchoring) bones into digit-like structures ("predigits"), such as pandas' "thumbs." Elephants similarly have expanded structures in the fat pads of their fore- and hindfeet, but for three centuries these have been overlooked as mere cartilaginous curiosities. We show that these are indeed massive sesamoids that employ a patchy mode of ossification of a massive cartilaginous precursor and that the predigits act functionally like digits. Further, we reveal clear osteological correlates of predigit joint articulation with the carp

Sesamoid bone disorders are disabling conditions with limited treatment options. This case report describes a 17-year-old football player with avascular necrosis (AVN) in both the tibial and fibular hallux sesamoids with a concomitant non-displaced stress fracture of the tibial hallux sesamoid. After a short period of conservative management, the patient underwent open sesamoid core decompression with an application of concentrated bone marrow aspirate and amnion matrix. After postoperative physical therapy, the patient achieved a painless range of motion of the first metatarsophalangeal joint
