Extreme Overvalued Belief and the Legacy of Carl Wernicke.

Individuals with extreme overvalued beliefs often carry out abhorrent and inexplicable acts of violence. They hold odd and bizarre beliefs that are shared by others in their culture or subculture. This creates a dilemma for the forensic psychiatrist because the definition of delusion may not be adequate. We discuss the evolution of the term "overvalued idea" first described in a forensic context by neuropsychiatrist Carl Wernicke in 1892. The overvalued idea is invoked in British scholarship as a definition for beliefs seen in anorexia nervosa, morbid jealousy, paranoid litigious states, and o

Psychosis in Huntington's disease: a review and comparison with schizophrenia.

Psychosis is a relatively rare phenomenon in Huntington's disease (HD) yet it occurs more commonly amongst individuals with HD than in the general population. Its presence is associated with significant distress and caregiver burden. This review evaluates the epidemiology, aetiology, phenomenology, neurobiology and treatment of psychosis in HD, drawing comparisons with schizophrenia as an archetypal psychotic disorder. We conducted a detailed literature search and narrative synthesis and found that prevalence estimates of psychosis in HD varied widely (4.1-17.6 %). While generally more common

Thirty years of forensic psychiatric evaluations: Reflections on diagnostic consistency and expert opinion.

Forensic psychiatry lacks objective biological markers and relies on clinical methodologies inherently influenced by the evaluator's cultural background and cognitive biases. This case study, spanning 30 years of clinical and forensic assessments, highlights the challenges associated with achieving diagnostic consistency and objectivity in expert opinions. A 63-year-old man, first detained at 19 for attempted robbery and homicide, underwent multiple forensic psychiatric evaluations from 1989 to 2025. Initial assessments consistently found him not guilty by reason of insanity due to a schizophr

The heritability of cluster A personality disorders assessed by both personal interview and questionnaire.

BACKGROUND: Personality disorders (PDs) as assessed by questionnaires and personal interviews are heritable. However, we know neither how much unreliability of measurement impacts on heritability estimates nor whether the genetic and environmental risk factors assessed by these two methods are the same. We wish to know whether the same set of PD vulnerability factors are assessed by these two methods. METHOD: A total of 3334 young adult twin pairs from the Norwegian Institute of Public Health Twin Panel (NIPHTP) completed a questionnaire containing 91 PD items. One to 6 years later, 1386 of t

The 15q11.2 BP1-BP2 microdeletion syndrome: a review.

Patients with the 15q11.2 BP1-BP2 microdeletion can present with developmental and language delay, neurobehavioral disturbances and psychiatric problems. Autism, seizures, schizophrenia and mild dysmorphic features are less commonly seen. The 15q11.2 BP1-BP2 microdeletion involving four genes (i.e., TUBGCP5, CYFIP1, NIPA1, NIPA2) is emerging as a recognized syndrome with a prevalence ranging from 0.57%-1.27% of patients presenting for microarray analysis which is a two to four fold increase compared with controls. Review of clinical features from about 200 individuals were grouped into five ca

Do autistic traits play a role in the bullying of obsessive-compulsive disorder and social phobia sufferers?

BACKGROUND: Social phobia and obsessive-compulsive disorder (OCD) share several similarities: both are categorized as anxiety disorders, avoidant personality disorder and depression are common in both, they have a similar age of onset and course, and both disorders respond to treatments with serotonin reuptake inhibitors and cognitive behavioural therapy. However, OCD and social phobia differ in respect to their relation to autism spectrum disorders (ASD; i.e. Asperger's syndrome, autism, pervasive disorder not otherwise specified). Findings that suggest a link between OCD and ASD have no para

A Case Report of Leukocytosis During Modified Electroconvulsive Therapy of Paranoid Personality Disorder.

INTRODUCTION: Modified electroconvulsive therapy (MECT) is a viable therapeutic option for patients with mood disorders and schizophrenia. We found that there is a relationship between MECT and leukocytosis. To the best of our knowledge, this is the first case of this problem. There are no relevant guidelines recommending the risk of leukocytosis caused by MECT, nor the method to reduce the risk. We hope to share this case to provide a reference for the prevention and treatment of similar patients with leukocytosis during or after MECT and remind psychiatrists to pay attention to this risk of

Increased motor activity in cycloid psychosis compared to schizophrenia.

Although cycloid psychoses share psychopathological features with schizophrenia, their course and outcome are comparable to those seen in bipolar affective disorder. The diagnostic category is of interest because it has been demonstrated that cycloid psychoses can be diagnosed reliably and distinguished from other psychoses based on treatment response and neurophysiology. Despite this, only few studies have investigated the neurobiological differences between cycloid psychoses and schizophrenia, and although hyper- and hypoactivity have been observed in patients with cycloid psychoses, no stud

Metacognitive training in schizophrenia: from basic research to knowledge translation and intervention.

PURPOSE OF REVIEW: There has been a marked increase in the study of cognitive biases in schizophrenia, which has in part been stimulated by encouraging results with cognitive-behavioral interventions in the disorder. We summarize new evidence on cognitive biases thought to trigger or maintain positive symptoms in schizophrenia and present a new therapeutic intervention. RECENT FINDINGS: Recent studies indicate that patients with paranoid schizophrenia jump to conclusions, show attributional biases, share a bias against disconfirmatory evidence, are overconfident in errors, and display problem

Parental-Bonding and Alexithymia in Adolescents with Anorexia Nervosa, Their Parents, and Siblings.

Anorexia nervosa (AN) patients and their family-members share alexithymia, anxiety, depression, and other psychological symptoms, in the context of altered attachment. These domains have been individually studied in the context of eating disorders; few attempts have been made to study their interaction, especially including family members. In this study, alexithymia, parental-bonding, and psychopathology were assessed in 32 Italian families consisting of an adolescent AN patient, a sibling, and their parents. We aimed to (a) describe a sample of Italian families with a child affected by AN, no

Common genetic and environmental risk for personality disorders and psychotic-like experiences in young adult twins.

INTRODUCTION: Psychotic-like experiences (PLE) have been associated with the subsequent emergence of psychotic disorders as well as several other domains of psychopathology. In this twin study, we estimated the genetic and environmental correlations between PLE and 10 personality disorders (PD). METHODS: Diagnoses of 10 PDs according to the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) and PLE from the Composite International Diagnostic Interview (CIDI) were retrieved for 2793 young adult twins from the Norwegian Twin Registry. Risk for having a PD and PLEs was modeled using

[Neurological disorders in the narrative works of Benito Pérez Galdós].

INTRODUCTION: Benito Pérez Galdós was a realistic writer. As such, he was devoted to displaying reality in its full complexity. He shared the traits of experimental medicine of Claude Bernard (Introduction à l'étude de la médecine expérimentale). The writer was a close friend of famous contemporary doctors (Gregorio Marañón, Manuel Tolosa Latour), had textbooks with neurological content, and was familiar with Charcot. METHODS: With this background, we have foreseen neurological descriptions in his works. To search for them, we have reviewed three paramount novels: Fortunata y Jacinta, La de B

Electroconvulsive Therapy Refusal in Severe Psychosis: Navigating Capacity, Surrogate Decision-Making, and Ethical Duty.

Electroconvulsive therapy (ECT) remains among the most effective interventions in psychiatry for severe depression, catatonia, and treatment-resistant psychosis. Despite its well-established efficacy and safety profile, particularly in medically complex and older patients, its use continues to evoke controversy due to the required medical clearance. Ethical dilemmas arise when patients lack insight or capacity to consent, creating tension between respect for autonomy and the duty of beneficence. We present the case of a 62-year-old woman with a longstanding history of paranoid schizophrenia, g

Multimodal imaging in a family with Cockayne syndrome with a novel pathogenic mutation in the ERCC8 gene, and significant phenotypic variability.

BACKGROUND: Cockayne syndrome is a rare autosomal recessive neurodegenerative disorder caused by mutations of either the ERCC6/CSB or ERCC8/CSA genes. Here, we describe two sisters with Cockayne syndrome caused by compound heterozygous mutations in the ERCC8 gene using multimodal imaging. Significant ophthalmic and systemic phenotypic variability is discussed. MATERIALS AND METHODS: Multimodal imaging was performed in two affected sisters and included electroretinography, optical coherence tomography, ultra-wide-field confocal scanning laser ophthalmoscopy, fundus autofluorescence and fluores

Efficacy of a support group intervention on psychopathological characteristics among caregivers of psychotic patients.

BACKGROUND: Families of individuals with psychotic disorders are playing a major role in the care of their relatives, but report poorer physical competence, elevated symptoms of psychological distress and more psychopathological characteristics. AIM: To compare psychopathological characteristics demonstrated by caregivers of psychotic patients in Greece who participated in support groups organized by non-governmental mental health organizations (NGOMH) compared to caregivers who did not. METHODS: Participants were caregivers of a psychotic patient assigned to either the intervention group (n

Association between circulating levels of C-reactive protein and positive and negative symptoms of psychosis in adolescents in a general population birth cohort.

BACKGROUND: Schizophrenia is associated with elevated levels of circulating C-reactive protein (CRP) and other inflammatory markers, but it is unclear whether these associations extend to psychotic symptoms occurring in adolescence in the general population. A symptom-based approach may provide important clues for apparent trans-diagnostic effect of inflammation, which is also associated with depression and other psychiatric disorders. METHODS: Based on data from 2421 participants from the Avon Longitudinal Study of Parents and Children birth cohort, we examined associations of serum CRP leve

A longitudinal twin study of cluster A personality disorders.

BACKGROUND: While cluster A personality disorders (PDs) have been shown to be moderately heritable, we know little about the temporal stability of these genetic risk factors. METHOD: Paranoid PD (PPD) and schizotypal PD (STPD) were assessed using the Structured Interview for DSM-IV Personality in 2793 young adult twins from the Norwegian Institute of Public Health Twin Panel at wave 1 and 2282 twins on average 10 years later at wave 2. Using the program Mx, we fitted a longitudinal latent factor model using the number of endorsed criteria for PPD and STPD. RESULTS: The stability over time of

Psychopathological and psychiatric evaluation of patients affected by lipodystrophy.

PURPOSE: Lipodystrophy is a collection of rare disorders defined by complete or partial loss of adipose tissue, due to abnormal adipocyte production, function, or distribution; it shares the main metabolic complications with obesity. Aims of the present study were to investigate the psychopathological characteristics of non-HIV lipodystrophic patients in comparison with a group of obese patients, a group of patients affected by oncologic chronic illness, and a control group of healthy subjects. METHODS: All participants were female: 16 non-HIV lipodystrophic women (mean age 42 ± 12 years), 20

[Acute psychotic disorders related to bupropion: review of the literature].

RATIONALE AND OBJECTIVES: Bupropion, or amfebutamone, is an atypical antidepressant also used during tobacco cessation. From a structural standpoint, it resembles amphetamine drugs with psychostimulant effects, and endogenous monoamines. From a pharmacological standpoint, bupropion, and two of its most important active metabolites, inhibit dopamine and norepinephrine reuptake. It has recently been discovered that bupropion may act as a non-competitive cholinergic nicotinic receptor antagonist, and that it may inhibit the activation of reward systems triggered by nicotine. Buproprion's efficacy

Exploring Links Between Psychosis and Frontotemporal Dementia Using Multimodal Machine Learning: Dementia Praecox Revisited.

IMPORTANCE: The behavioral and cognitive symptoms of severe psychotic disorders overlap with those seen in dementia. However, shared brain alterations remain disputed, and their relevance for patients in at-risk disease stages has not been explored so far. OBJECTIVE: To use machine learning to compare the expression of structural magnetic resonance imaging (MRI) patterns of behavioral-variant frontotemporal dementia (bvFTD), Alzheimer disease (AD), and schizophrenia; estimate predictability in patients with bvFTD and schizophrenia based on sociodemographic, clinical, and biological data; and
