INTRODUCTION: Failure after operative treatment of complex proximal humeral fractures (PHF) can prove challenging even for experienced surgeons. Reverse shoulder arthroplasty (RSA) seems to offer a satisfactory revision procedure with good clinical outcomes.
MATERIALS AND METHODS: We present a case series of 14 patients, who were treated during a 3.5 years period (from 01/2016 until 06/2019). They all underwent revision surgery with RSA for failed operative treatment of PHF. Their mean age was 68 years (range, 51-84 years). 2 patients (14.3%) had been primarily treated with open reduction and

Proximal humerus fractures are relatively common, with treatment dependent on fracture-specific, patient-specific, and surgeon-specific factors. This study sought to identify preferences among shoulder specialists and orthopedic traumatologists in the treatment of proximal humerus fractures. An anonymous Internet survey of fellowship-trained shoulder surgeons and traumatologists was conducted with radiographs and select computed tomography images of 15 unique displaced proximal humerus fractures. Participants were asked to classify each case according to Neer criteria and choose management fro

BACKGROUND: Currently, little information is available on functional outcome of periprosthetic humeral fractures after shoulder arthroplasty. This investigation aimed to evaluate functional and radiological outcome and patients' satisfaction following this type of injury treated by open reduction and internal fixation.
METHODS: Retrospective chart analysis of patients treated at two level-I trauma centers. Patients were examined clinically and radiologically. Additionally, functional outcome was assessed using the established DASH-questionnaire and standardized examination for calculation of

Complex fractures of the proximal humerus are uncommon injuries and a therapeutic challenge to the orthopaedic surgeon. Successful treatment requires proper evaluation of the patient and analysis of standardized high-quality radiographs. The trauma series of radiographs (including true anteroposterior and lateral views in the scapular plane, and axillary view) is essential for accurate fracture assessment. Generally, joint-preserving reconstructive techniques are emphasized, aiming at restoration of the anatomy of the proximal humerus. In young individuals with excellent quality of the bone fr

Fracture complications of convulsions are reported occasionally, e.g.: mono- or bilateral posterior shoulder luxations and luxation fractures, central dislocations of the hip as well as fractures of the femoral neck or compression fractures of vertebrae. A case is reported where the patient sustained four such lesions simultaneously. The epileptic seizure occurred following the sudden interruption of a neurotropic therapy with Carbamazepin (Tegretal). A general osteodystrophy was not found. The simultaneous presence of four such lesions following one convulsion has never before been reported a

BACKGROUND: Postoperative scapular stress fractures (SSFs) are a formidable problem after reverse shoulder arthroplasty (RSA). Less is known about patients who have these fractures preoperatively. The primary aim of this study was to examine postoperative satisfaction in patients undergoing primary RSA who have preoperative SSF and compared to a matched cohort without preoperative fracture. The secondary aim was to examine the differences in patient-reported outcomes between and within study cohorts.
METHODS: A retrospective chart review of primary RSAs performed by a single surgeon from 2000

Few studies report on periprosthetic humeral shaft fractures after reverse total shoulder arthroplasty (RTSA). The authors evaluated outcomes of 5 patients with this complication who were initially treated nonoperatively. Of 152 patients who underwent RTSA at the authors' institution from 2012 to 2017, 4 experienced periprosthetic humeral shaft fractures. One patient was referred to the authors for fracture treatment. All 5 patients were initially treated nonoperatively. The mean duration of follow-up was 11.5 months (range, 1.5-26 months). The authors analyzed time to fracture union, Single A

The functional outcome of hemiarthroplasty in displaced proximal humeral 3- and 4-part fractures or fracture dislocations in elderly patients is frequently unpredictable and depends on the position of the prosthesis and tuberosity fixation. Reverse shoulder arthroplasty represents an alternative in elderly patients. The purpose of this study was to report the results of a retrospective series of 30 reverse shoulder prostheses in trauma indications. We also compared the results of a less medialized reverse shoulder prosthesis (Arrow) with those reported for the traditional (Delta III) reverse p

This retrospective study was done to find out the outcome of hemiarthroplasty of the shoulder following comminuted proximal humeral fractures in 20 elderly patients. Their average age was 77.6 years. The average follow-up was 33 months. All patients were reviewed in the outpatient clinic using the Constant Score. The median Constant Score was 47.5. None of the patients had severe pain. Four had moderate and 16 had no or mild pain. Range of movement was not good. The activities of daily living were significantly reduced in patients with moderate shoulder pain. Sixteen patients (80%) were satisf

BACKGROUND: Acromion stress fractures (ASF) or scapular spine fractures (SSF) following reverse total shoulder arthroplasty (RTSA) are common complications with impaired clinical outcome. The underlying biomechanical factors remain unclear. The aim of this study was to evaluate basic demographic and radiographic parameters predicting occurrence of different types of ASF/SSF in a large single-center study cohort.
METHODS: A total of 860 RTSA (805 patients) with available minimum follow-up of 2 years were implanted between 2005 and 2018 at a tertiary academic center. All RTSA with subsequent AS

BACKGROUND: The purpose of this study is to compare short-term outcomes, including complication rates, readmissions, and reoperations, between inpatient and outpatient reverse total shoulder arthroplasty (rTSA) performed for proximal humerus fractures.
METHODS: All rTSA performed for proximal humerus fractures from January 2020 to January 2025 were retrospectively analyzed. Patients were evaluated based on whether they had the surgery performed as an inpatient (at least 1 overnight hospital stay) or as an outpatient (discharged on the same day as surgery). The decision for inpatient versus ou

The shoulder girdle, which includes the clavicle, scapula, and proximal humerus, forms a dynamic scaffold for seamless motion and load transfer to the axial skeleton. CT with multiplanar reformatted and volume-rendered images allows precise characterization, measurement, and injury classification and is particularly useful for complex high-energy disruptions encountered in trauma centers. The spectrum of shoulder girdle injuries spans clavicle fractures, acromioclavicular joint separations, floating shoulder patterns, glenohumeral fracture-dislocations, glenoid fossa fractures, proximal humeru

Periprosthetic proximal humeral fractures following shoulder arthroplasty are rare but present significant clinical challenges due to variable fracture patterns, stem stability, and patient factors. This systematic review aimed to summarize contemporary evidence on management strategies, including nonoperative treatment, open reduction and internal fixation (ORIF), and revision arthroplasty, and to evaluate outcomes, complications, and factors influencing fracture healing. A comprehensive search of PubMed, Embase, Scopus, and the Cochrane Library identified eight studies meeting the inclusion

PURPOSE OF REVIEW: This review critically evaluates the literature on acromial stress fractures (ASFs) following reverse total shoulder arthroplasty (rTSA), with a focus on risk factors, prevention, and treatment. By synthesizing contemporary evidence and highlighting areas of ongoing debate, this review aims to provide clinicians with an updated understanding of these complications and identify priorities for future research.
RECENT FINDINGS: ASFs have an approximate incidence of 3-5% following rTSA. Preoperative risk factors include female sex, acromial morphology, rheumatologic disease, ch

Reverse shoulder arthroplasty (RSA) for proximal humerus fractures (PHFs) is traditionally performed with cemented humeral fixation. The purpose of this study was to compare the clinical and radiographic outcomes of cemented and cementless RSA for PHF. Between 2010 and 2014, 38 acute PHFs were treated with RSA and followed for a minimum of 2 years (mean, 37 months; range, 24-66 months). The mean time from fracture to RSA was 7 days (range, 1-30 days). Humeral stems were cemented in 19 shoulders and uncemented in 19 shoulders. Outcome measures included visual analog scale pain scores, range of

INTRODUCTION: Reverse shoulder arthroplasty (RSA) is a key tool in the orthopedic trauma surgeon's arsenal, especially when faced with a proximal humerus fracture in older patients. However, few studies have focused on the glenoid side of RSA in this indication as the implant is placed in a generally healthy scapula.
HYPOTHESIS: Glenoid implants for RSA after trauma are well positioned and do not often cause complications.
MATERIAL AND METHODS: Retrospective multicenter study of 513 patients who underwent RSA because of a proximal humerus fracture. The mean follow-up was 55 months. Radiograp
