Sudden nocturnal death in young males from ventricular flutter.

Two young males who had no organic heart disease died unexpectedly during the night. In one patient, the monitor ECG showed a sinoatrial block and the electrophysiologic study revealed a sinoatrial conduction time at the upper limit of normal and a prolonged PA interval. The surface ECG showed left axis deviation. Ventricular ectopic beats were confined to the night when he died from ventricular flutter. The ECG of the other patient was normal except for the change in QRS configuration found when the preceding RR interval was prolonged, suggesting a phase 4 block in intraventricular conduction

[Sinus dysfunction and nodo-hisian conductive disorders. Indication for definitive cardiac stimulation].

A consecutive series of 80 patients has been the object of intracavitary electrophysiological studies, including a study of sinus function and of node/His conduction, before implantation of a definitive cardiac pacemaker. Investigation of the sinus yielded normal or insignificant results in 32 cases. It revealed certain criteria of "major" sinus dysfunction (TRSC 1000 ms; more than compensatory return cycles where there was SAP) in 39 cases, made up of 18/20 in group I (documented evidence of sinus malfunction), 10/14 in group II (sinus bradycardia less than or equal to 55/mn), 9/44 in group I

Effect of fentanyl on cardiac automaticity and conduction: direct or mediated action?

The effects of fentanyl at a dose of 100 micrograms.kg-1 on cardiac automaticity, refractoriness and conduction have been studied in a model of heterotopic heart transplantation which combines a denervated heart (donor) and a non-denervated heart (recipient) in each of 20 dogs employed. Once the surgical procedure had been completed, cycle length, sinoatrial conduction time, antegrade and retrograde A-V node block points, cycle length at which 1:1 conduction ceases to exist and the antegrade effective refractory period of the A-V node and ventricles were measured. These parameters were determi

Isolated atrial myopathy and persistent severe cardiac conduction disorders presented as juvenile stroke: A case report.

Myocarditis can be complicated by various conduction abnormalities, including supraventricular and ventricular arrhythmias. It is rare to see conduction disorders as the first and only presentation of myocarditis, which may be reversible or persistent after the acute phase of myocarditis. Here, we present a case of probable myocarditis with preserved right and left ventricular function, isolated atrial cardiomyopathy, and severe conductive disturbances, including sinoatrial node dysfunction, atrial flutter, interatrial block, transient loss of atrial electromechanical activity and persistent c

Acute hyperkalemia and failure of pacemaker stimulus.

Acute hyperkalemia may induce well-known serious cardiac arrhythmia. However, ventricular aberration including concealed conduction may also occur. We report the case of a 35-year-old woman who had a previous history of late-operated ventricular septal defect communication and DDD pacemaker was admitted for dyspnea. During hospitalization, an acute hyperkalemia induced sinoatrial block despite correct pacemaker programming. Sodium bicarbonate allowed to restore sinus rhythm. Our report highlights that acute hyperkalemia may increase thresholds of pacemaker stimulus and physicians should be awa

Idiopathic familial atrial cardiomyopathy with diffuse conduction block.

Isolated atrial cardiomyopathy is rare and familial idiopathic atrial cardiomyopathy has only been described once. We describe a case characterized by severely reduced atrial contractile function, intermittent atrial electrical standstill, diffuse interatrial, intraventricular and atrioventricular block, complicated by systemic embolism. Whilst this condition is undoubtedly rare, other cases may be being mistaken for simple sinoatrial disease.

A new indirect method for measurement of sinoatrial conduction time and sinus node return cycle.

We developed a new indirect method for the measurement of sinoatrial conduction time (SACT) and the sinus node return cycle (SRC) with a transvenous catheter technique. Two early premature stimuli, at intervals 50 msec longer than the effective refractory period (ERP), were given to the right atrium. These early stimuli were followed by eight constant stimuli. The interval of the constant stimuli was a little shorter than the basic cycle length (BCL). The return cycle A1Ar was measured and plotted on the abscissa; the next interval ArA3, was measured and plotted on the ordinate. This was calle

Local opiate receptors in the sinoatrial node moderate vagal bradycardia.

Met-enkephalin-arg-phe (MEAP) interrupts vagal bradycardia when infused into the systemic circulation. This study was designed to locate the opiate receptors functionally responsible for this inhibition. Previous observations suggested that the receptors were most likely located in either intracardiac parasympathetic ganglia or the pre-junctional nerve terminals innervating the sinoatrial node. In this study 10 dogs were instrumented with a microdialysis probe inserted into the sinoatrial node. The functional position of the probe was tested by briefly introducing norepinephrine into the probe

[Lengthening of the direct sinoatrial conduction time: a criterion of sinus node dysfunction].

In order to determine the significance of prolongation of the direct sinoatrial conduction time (DSACT), an attempt was made to record the sinus node potential in 110 patients which was successful in 84 cases. The DSACT was normal in 45 cases (Group A) and prolonged (over 130 ms) in 39 cases (Group B). The symptomatology, standard ECG and the results of investigation of sinus node function by atrial stimulation of the two groups were compared. The DSACT was prolonged in all 13 patients with paroxysmal sinoatrial block or the sick sinus syndrome, in 71% of 15 patients with permanent sinus brady

Management of arrhythmias in patients with Takotsubo cardiomyopathy: Is the implantation of permanent devices necessary?

BACKGROUND: Arrhythmias are frequent in Takotsubo cardiomyopathy (TTC) and a major determinant of outcome. OBJECTIVE: The purpose of this study was to provide a rationale for management strategies, particularly for permanent device implantation given the reversible nature of TTC. METHODS: Treatment strategies of arrhythmias including ventricular fibrillation (VF), ventricular tachycardia (VT), asystole, pulseless electrical activity, and complete atrioventricular (AV) or sinoatrial block were assessed in a bicentric cohort of consecutive patients with TTC (n = 286) with a mean follow-up peri

[Sick sinus syndrome in childhood. A case report (author's transl)].

The case of an eight-year old child suffering from sick sinus syndrome (SSS) is reported. The patient showed S-A conduction disturbances (Mobitz 1 sinoatrial exit block) as noted in two sequential 24-hour recording periods, and a pathologic response to atropine test. An involvement of the A-V node was also present, made clear by a first and a second degree Mobitz 1 block. The clinical, laboratory and noninvasive data excluded an organic cardiopathy. Idiopathic SSS was, therefore, diagnosed. The rare occurrence of this syndrome in the childhood, and the usefulness of Holter monitoring in its de

Vagal control of pacemaker periodicity and intranodal conduction in the rabbit sinoatrial node.

Effects of brief vagal bursting on pacemaker periodicity and intranodal conduction were studied by recording transmembrane potentials in isolated rabbit sinoatrial preparations. Postganglionic vagal terminals were activated by applying brief (50- to 150-msec) trains of pulses (duration, 100 mu sec; frequency, 200 Hz) to the endocardial surface of the node. Sympathetic effects were prevented by superfusion with Tyrode's solution containing propranolol (1 microgram/ml). Vagal trains applied singly every 10 seconds produced brief hyperpolarizations in "true" pacemaker and transitional cells, and

Left Bundle Branch Block: A Reversible Pernicious Effect of Lacosamide.

A 95-year-old male with a medical history of focal epilepsy presented with transient ischemic attack (TIA)/pre-syncope like symptoms. He was on lacosamide (LCM) and levetiracetam. On evaluation, he was found to have left bundle branch block (LBBB), sinus pause of three seconds, and 1st degree atrioventricular (AV) block. After holding LCM, electrocardiogram changes were reversed to baseline (before commencing LCM). In conclusion, to the best of our knowledge, this is the first case of reversible LBBB along with sinoatrial (SA) node and AV node dysfunction in an elderly male on LCM therapy.

[Method of cardiac stimulation in sick sinus syndrome].

The condition of the conducting system of the heart and the sinus node recovery time were studied in 27 patients with the cardiac rhythm pathology under consideration. On the grounds of the resultant data the question concerning the implantation of this or that type of pacemaker (operating in a fixed regimen or "on demand") and-the positioning of the fixing electrode in the atria or ventricle was resolved. The advantages and shortcomings of each of the suggested methods for surgical treatment of sick sinus node are discussed.

[Functional autonomic blockade in the diagnosis of functional sinus node disorders].

The influence of the autonomic nervous system on sinus node automatism was assessed in 61 patients with suspect sinus node dysfunction. Cardiac frequency and corrected sinus node recovery time (CSNRT) were determined before and after functional autonomic blockade with intravenously administered propranolol (0,2 mg/kg) and atropine (0,04 mg/kg). A pathologic CSNRT was found in 59% of patients before and in 54% after autonomic blockade. In 44% pathologic CSNRT occurred during both conditions. In these patients intrinsic sinus node disease can be assumed. In 15% of patients an initially pathologi

Sinus node arrest and sino--atrial block.

Current epidemiological data suggest that sinus node dysfunction is a relatively uncommon condition in the pilot age group. Symptomatic pauses of any mechanism suggest disqualification from flying duties. Asymptomatic pauses (representing sino-atrial block or sinus node arrest) demand further investigation and regular follow-up. Idiopathic pauses greater than 2.5 s on an ambulatory ECG is abnormal and is likely to deny flying status.
