Posttreatment imaging of the skull base.

Imaging the skull base after surgery can be challenging because anatomic structures may have been destroyed by an underlying process or removed at surgery. Foreign substances may be introduced to fill a void left by tumor resection, for hemostasis, and to repair dural defects. Previous imaging studies must be available for comparison to understand the characteristics of an underlying lesion. By following the progression of a lesion on subsequent imaging studies, the nature of treatment-related changes and residual or recurrent pathology is best realized.

Neural conservation in skull base surgery.

Surgical treatment of lesions of the skull base carries significant risk to the functioning of the cerebral hemispheres, the brain stem and the cranial nerves. This risk is due both, to problems associated with maintaining an adequate blood flow while exposing and removing the tumor and to direct or indirect trauma to the brain, perineural tissues and cranial nerves. These risks may be reduced if information about possible implications of surgical maneuvers on the cerebral blood flow and on the function of the patients central nervous system and cranial nerves is available and can be monitored

Skull base trauma: diagnosis and management.

The singular anatomical relationship of the base of the skull is responsible for the particular problems that may arise after injury. Extensive dural laceration and severe neurovascular damage may accompany skull base injuries. Trauma to the anterior skull base is frequently related to the paranasal sinuses, and trauma to the middle and the posterior skull base usually affects the petrous bone. Injury to the anterior fossa including the paranasal sinuses may produce CSF leakage, damage the olfactory nerves, optic nerves, and orbita contents. Fractures may affect the carotid canal, injure the i

Imaging of mucormycosis skull base osteomyelitis.

Skull base osteomyelitis (SBO) is typically bacterial in origin and caused by Pseudomonas, although the fungus Aspergillus has also rarely been implicated. SBO generally arises from ear infections and infrequently complicates sinonasal infection. Rhinocerebral Mucor infection is characteristically an acute, fulminant, and deadly infection also affecting the orbits and deep face and is associated with intracranial complications. Bony involvement is uncommon because of the angioinvasive nature of the fungus. More recently, chronic invasive Mucor sinusitis has been described. We report the unusua

Interstitial brachytherapy for skull base tumors.

1. Irrespective of the histology, all tumors responded to 125I permanent BT. 2. There were no intra- or postprocedural complications, making this modality extremely safe. 3. There were no acute or late complications attributable to continuous LDR radiation with 125I permanent BT. 4. The dose distribution can be tailored to fit the tumor shape by seed placement at the desired location and by selecting the seed strength. 5. Unlike any other radiation therapy procedure, with SBT, the placement of the 125I seed or seeds is verifiable at each step of the procedure on the CT screen, making the treat

Skull base osteomyelitis interpreted as malignancy.

Lesions in the skull base may present difficulties in diagnosis primarily because the access needs to be invasive and one has to rely on imaging that may still be misleading. The case presented here illustrates this example in that the patient had abnormalities on computed tomography (CT) and magnetic resonance image (MRI) scans quite convincing of malignancy but which with time proved, essentially through clinical judgment, to be skull base osteomyelitis secondary to malignant external otitis (OME).

Skull base osteitis following fungal sinusitis.

Aspergillus sp. sinusitis is not uncommon in immunocompromised patients but is unusual in patients who are not immunocompromised. The disease may occur as a saprophytic condition, as an allergic sinusitis or as a potentially lethal invasive disease. The differentiation between non-invasive and invasive Aspergillus sp. sinusitis is crucial and this distinction is fully discussed. The treatment options are also considered. Invasive disease requires aggressive treatment with long-term antifungal agents in sufficient doses combined with wide surgical excision. We present a patient who presented wi

Telangiectatic osteosarcoma of the skull base.

We present a patient with a distinctive lesion of the skull base, which, at imaging, showed expansile destruction of the sphenoid bone, hemorrhage with fluid-fluid levels, and contrast enhancement of a solid portion. These features ordinarily suggest either giant cell tumor or aneurysmal bone cyst; however, pathologic examination confirmed instead a rare variant of osteosarcoma of the telangiectatic type. Although imaging findings can indicate any of these possibilities, biopsy is an essential step in arriving at the diagnosis.

[Bitemporal hemianopsia after skull base fracture].

We report a case of bitemporal hemianopsia after skullbase fracture. An 18-year-old male presented with frontal head hit due to a traffic accident. Consciousness level on admission was JCS 10. Initial CT scan revealed traumatic SAH and pneumocephalus. We treated him conservatively. Ten hours after the accident, consciousness level went down, and CT imaging disclosed bilateral frontal hemorrhage. The three dimensional CT (3D-CT) imaging showed two fracture lines from the roof of the ethmoid sinus to the planum sphenoidale. Although his consciousness improved gradually, he complained of rhinorrh

Occult malformations of the skull base.

Occult malformations of the skull base are rare anomalies. They are often not detected until they give rise to complications such as meningitis or cerebrospinal fluid rhinorrhea. We used high-resolution computed tomography, fluorescein endoscopy, cisternography, magnetic resonance imaging, and duraplasty to diagnose occult malformations of the skull base in 4 patients. The four patients had had between 4 and 6 attacks of meningitis. Cerebrospinal fluid rhinorrhea was confirmed in 3 cases. The following occult malformations were found: (1) an encephalocele of the glabella/cribriform plate and a

Primary myxomas of the skull base.

Twelve cases of intracranial primary myxomas are discussed. The mean age at presentation was 34.2 years and seven patients were male. All tumours originated extradurally from the skull base. Most of the patients had a chronic clinical course, characterized by unilateral dysfunction of one or several cranial nerves. It was difficult to determine the pathological diagnosis before operation. Radical excision is the best choice of treatment for intracranial myxoma, but the tumour has a tendency to recur. Intracranial primary myxomas have been rarely discussed in the neurosurgical literature. There

Stereotactic radiosurgery of skull base meningiomas.

Between April 1992 and March 1994, 48 patients with skull base meningiomas were treated at our department. The age of these patients ranged from 10 to 76 years. The male:female ratio was 1:3. Thirty-seven of these patients had primary open surgery for partial removal and subsequent radiosurgical treatment. In 1 patient a recurrent meningioma after so-called total microsurgical removal was treated radiosurgically. Radiosurgery was performed as a primary treatment in 11 patients. The mean tumor volume was 13.7 cm3 (range: 0.8-82 cm3). These tumor volumes could be covered by mean isodose volumes

Meningioma of the posterior skull base.

Combined intratemporal and cerebellopontine angle meningiomas are rejatively rare. There are unsolved problems with the stability of the skull and spine and the lower cranial nerves and there is a marked tendency for the tumor to involve the spinal cord. This article reports on five cases of combined intratemporal and cerebellopontine angle meningiomas.

Swallowing disorders following skull base surgery.

The modern otolaryngologist-head and neck surgeon has the technical ability to perform a wide range of surgical procedures at the skull base. Associated with some of these operations are swallowing deficits secondary to cranial nerve paralyses or anatomic disturbances. The skull base surgeon must physically and emotionally prepare patients preoperatively for these functional disabilities.

Facial reanimation after skull base trauma.

Three basic types of procedures are used by the author to reanimate the face paralyzed by interruption of the facial nerve at the skull base. In order of choice they are direct nerve repair or grafting; hypoglossal-facial nerve anastomosis; and regional facial reanimation (brow lift, implantation of a gold weight or spring in the eyelid, tightening of the lower eyelid, and temporalis muscle transposition to reanimate the mouth). The indications for and expected results of each approach are presented with special emphasis on the usefulness of regional reanimation.

MR imaging of the skull base.

Fifty-four patients with abnormalities primarily involving the base of the skull were evaluated by magnetic resonance (MR) imaging. The results were compared with information obtained by other radiologic studies, primarily X-ray CT. On MR imaging, better anatomic definition of soft tissues in the deep compartments of the nasopharynx was achieved through the high level of tissue contrast discrimination and lack of bone artifacts. The parapharyngeal fat plane, separating pterygoid from pharyngeal musculatures, was consistently demonstrated. However, the inability of MR to image compact bone prov
