The role of the circadian system in the etiology and pathophysiology of ADHD: time to redefine ADHD?

Attention-deficit/hyperactivity disorder (ADHD) is highly associated with the delayed sleep phase disorder, a circadian rhythm sleep-wake disorder, which is prevalent in 73-78% of children and adults with ADHD. Besides the delayed sleep phase disorder, various other sleep disorders accompany ADHD, both in children and in adults. ADHD is either the cause or the consequence of sleep disturbances, or they may have a shared etiological and genetic background. In this review, we present an overview of the current knowledge on the relationship between the circadian rhythm, sleep disorders, and ADHD.

[Disorders of the regulation of cardiac rhythm in various sleep phases and cycles in organic and functional pathology of the heart].

The purpose of the study was to examine the correlation between individual stages, phases and cycles of sleep, and the heart rate (HR) in patients with organic and functional cardiac disorders. Twenty normal subjects and 103 cardiologic patients were investigated. During sleep, tonic HR increases and dynamic changes decrease in coronary patients. During stage 4 of slow-wave sleep (SWS), HR increases in postmyocardial-infarction patients. The circadian decrease of HR disappears with every sleep cycle. Anginal patients have a disturbed HR pattern in the second SWS stage, the circadian rhythm is

Integrated Analysis of Circadian and Sleep Signatures in Depression and Schizophrenia Using Multi-Day Actigraphy.

Sleep abnormalities and circadian rhythm disruptions are frequently observed in psychiatric disorders such as depression and schizophrenia. However, most previous studies have examined circadian rhythms and sleep separately, limiting understanding of how these processes interact within individuals. This study examined circadian and sleep characteristics in depression and schizophrenia compared with healthy controls using multi-day wrist actigraphy. Circadian rhythms were assessed using parametric and non-parametric measures of rest-activity patterns, and sleep metrics were derived using a vali

The role of the circadian clock in animal models of mood disorders.

An association between circadian clock function and mood regulation is well established and has been proposed as a factor in the development of mood disorders. Patients with depression or mania suffer disturbed sleep-wake cycles and altered rhythms in daily activities. Environmentally disrupted circadian rhythms increase the risk of mood disorders in the general population. However, proof that a disturbance of circadian rhythms is causally involved in the development of psychiatric disorders remains elusive. Using clock gene mutants, manipulations of sleep-wake and light-dark cycles, and brain

Dissecting the genetic architecture of circadian rhythms in human tissues using a quantitative circadian deviation score.

BACKGROUND: Circadian rhythms influence various physiological and behavioral processes, including sleep, metabolism, and immune response. Although key regulatory factors of biological clocks have been identified and genome-wide association studies have pinpointed some genetic variants linked to sleep traits, the genetic architecture underlying circadian rhythms remains incompletely understood. RESULTS: Here, we introduce the circadian deviation score, a novel quantitative trait that measures circadian disruption at the molecular level. Derived from gene expression levels of thousands of circa

Sleep and circadian alterations in people at risk for bipolar disorder: A systematic review.

BACKGROUND: Sleep and circadian abnormalities have been mostly demonstrated in bipolar patients. However, it is not clear whether these alterations are present in population at high risk for bipolar disorder (BD), indicating a possible risk factor for this condition. OBJECTIVE: This systematic review aims to define current evidence about sleep and rhythm alterations in people at risk for BD and to evaluate sleep and circadian disorders as risk factor for BD. METHODS: The systematic review included all articles about the topic until February 2016. Two researchers performed an electronic searc

Personality Traits Interact With Sleep and Biological Rhythm in Patients With Bipolar Disorder.

Sleep disturbances and circadian rhythm changes in bipolar disorder (BD) may have behavioral components as well as biological components. This study aimed to examine the relationship between personality traits, sleep and circadian rhythm in BD. A total of 150 participants with BD, and 150 healthy controls completed the Big Five Personality Test-50 (B5PT-50-TR), Biological Rhythm Interview of Assessment in Neuropsychiatry (BRIAN), Functioning Assessment Short Test (FAST), Pittsburgh Sleep Quality Index (PSQI), Young Mania Rating Scale and Beck Depression Inventory. In the BD group, B5PT-50-TR e

Integrating sleep and circadian health into cardiac rehabilitation: A narrative review and practical framework for implementation.

Cardiac rehabilitation (CR) aims to enhance cardiovascular health and quality of life, as well as survival through structured exercise training, lifestyle education, cardiovascular risk factor management, and psychosocial support. Despite growing evidence linking sleep disorders, poor sleep health, and circadian rhythm disruption to impaired cardiac recovery, reduced exercise adherence, and increased adverse cardiovascular events, systematic assessment and management of sleep and circadian health remain underutilized in CR programs. In this narrative review, we aim to explore and characterize

'The clocks that time us'--circadian rhythms in neurodegenerative disorders.

Circadian rhythms are physiological and behavioural cycles generated by an endogenous biological clock, the suprachiasmatic nucleus. The circadian system influences the majority of physiological processes, including sleep-wake homeostasis. Impaired sleep and alertness are common symptoms of neurodegenerative disorders, and circadian dysfunction might exacerbate the disease process. The pathophysiology of sleep-wake disturbances in these disorders remains largely unknown, and is presumably multifactorial. Circadian rhythm dysfunction is often observed in patients with Alzheimer disease, in whom

Alterations in sleep and the biological clock in mood disorders: State of the art and therapeutic approaches.

Sleep disorders such as insomnia, hypersomnolence, nightmares and circadian rhythm disorders play a critical role across all stages of mood disorders, including Chronos syndrome, and are recognized as significant risk factors for their onset, relapse, and recurrence. Moreover, emerging evidence suggests that the co-occurrence of sleep or circadian disruptions with mood disturbances may represent a more severe and distinct clinical phenotype, likely associated with a poorer prognosis than when each condition occurs independently. Personalized interventions targeting sleep, circadian rhythms, an

Chronobiology of bipolar disorder: therapeutic implication.

Multiple lines of evidence suggest that psychopathological symptoms of bipolar disorder arise in part from a malfunction of the circadian system, linking the disease with an abnormal internal timing. Alterations in circadian rhythms and sleep are core elements in the disorders, characterizing both mania and depression and having recently been shown during euthymia. Several human genetic studies have implicated specific genes that make up the genesis of circadian rhythms in the manifestation of mood disorders with polymorphisms in molecular clock genes not only showing an association with the d

[Sleep changes with aging].

Many factors contribute to the alteration of sleep in older adults. Most of their complaints can be explained by the modifications of the sleep organisation observed in this population. Sleep architecture is altered with aging. Insomnia and excessive daytime sleepiness can reflect an alteration of the circadian rhythm. This population is also frequently exposed to specific sleep disorders such as the restless leg syndrome, periodic limb movements or obstructive sleep apnea syndrome. Sleep disorders in patients with Alzheimer's disease is a daily preoccupation at home or in institution. Circadi

Sleep and Rest-Activity Rhythms in Recovering Patients with Severe Concurrent Mental and Substance Use Disorder: A Pilot Study.

Objective: Mental health and substance use disorders are commonly associated with disrupted sleep and circadian rest-activity rhythms. How these disorders in combination relate to sleep and circadian organization is not well studied. We provide here the first quantitative assessment of sleep and rest-activity rhythms in inpatients with complex concurrent disorders, taking into account categories of substance use (stimulant vs. stimulant and opioid use) and psychiatric diagnosis (psychotic disorder and mood disorder). We also explore how sleep and rest-activity rhythms relate to psychiatric fun

Dysfunction of circadian and sleep rhythms in the early stages of Alzheimer's disease.

Dysfunction of circadian and sleep rhythms is an early feature of many neurodegenerative diseases. Alzheimer's disease (AD) is a progressive neurodegenerative disorder resulting in cognitive and psychiatric disturbances. Although it is largely unclear whether dysfunctions in sleep and circadian rhythms contribute to the etiology of AD or are a consequence of the disease, there is evidence that these conditions are involved in a complex self-reinforcing bidirectional relationship. According to the recent studies, dysregulation of the circadian clock already occurs during the asymptomatic stage

Role of Melatonin in the Management of Sleep and Circadian Disorders in the Context of Psychiatric Illness.

PURPOSE OF REVIEW: We present a review of research on the role of melatonin in the management of sleep and circadian disorders, stressing current overall view of the knowledge across psychiatric disorders. RECENT FINDINGS: Dysregulation of sleep and circadian rhythms has been established in several psychiatric and neurocognitive disorders for long. Recent research confirms this finding consistently across disorders. The secretion of melatonin in schizophrenia and neurocognitive disorders is reduced due to a smaller volume and enlarged calcification of the pineal gland. On the other hand, mela

Role of sleep-wake cycle on blood pressure circadian rhythms and hypertension.

Stages of different depth characterize the temporal organization of sleep. Each stage exerts an effect on blood pressure (BP) regulation and contributes to its 24-h variation. The main determinant of the circadian influences of sleep and wakefulness on BP is the daytime sympathetic and nighttime parasympathetic prevalence, but many other physiologic mechanisms known either to induce sleep or determine arousal may play an important role in the mediation of sleep influences on BP. Alteration of one or more of such mechanisms may be reflected in altered circadian BP rhythms. Sleep- and arousal-re

The human circadian system in normal and disordered sleep.

The human circadian system regulates rhythmicity in the human body and establishes normal sleep and wake phases. The suprachiasmatic nuclei (SCN), located in the hypothalamus above the optic chiasm, make up the human pacemaker known as the circadian or biological clock, but other essential parts of the circadian system include the pineal gland, retina, and retinohypothalamic tract. The importance of light in resetting the intrinsic human circadian cycle, the intrinsic period of which is slightly longer than 24 hours, ensures that the human cycle will stay entrained to the earth's 24-hour daily

Circadian rhythm disorders in the blind.

Non-24-h sleep-wake disorder in blind patients without light perception is an orphan circadian rhythm sleep-wake disorder and is extremely rare in sighted people. Non-24-h sleep-wake disorder is characterized by insomnia and daytime sleepiness alternating with asymptomatic episodes. The frequency of symptomatic periods depends on the daily desynchronization of endogenous circadian pattern of each patient. Diagnosis requires anamnesis, a sleep diary, and actigraphy, if possible; in addition, repeated 24-h measures of circadian markers such as melatonin secretion are also required. Treatment con
