Cardiac autonomic activity during sleep is only very slightly influenced by the emotional state of the patient and, unlike some of the traditional tests of autonomic function, may be studied in all patients. In an attempt to evaluate autonomic function in patients with different neuropsychiatric disorders, two different methods of quantifying the changes in sympathetic and parasympathetic cardiac control during sleep were used: (1) the ratios of consecutive R-wave (R-R) intervals before and after spontaneous body movements; (2) spectral analysis of R-R intervals. It was found that more than on

Sleep breathing disorders (SBD) are related to obstructions resulting from repetitive narrowing and closure of the pharyngeal airway. Their diagnoses and treatment are critically dependent on an accurate identification of and discrimination between types of respiratory events. However, these disorders have been diagnosed using indirect or invasive measurements, which resulted in serious doubts concerning the correct evaluation of breathing events. The forced oscillation technique (FOT) has recently been suggested as a clinical tool able to accurately and non-invasively quantify respiratory obs

Autonomic blockade is commonly employed as a test of sinus node dysfunction. We compared primary and secondary atrial postpacing pauses and postextrasystolic sino-atrial responses before and after autonomic blockade in 56 patients with the clinical diagnosis of sick sinus syndrome. Pharmacological autonomic blockade was achieved by atropine 0.04 mg kg-1 and propranolol 0.2 mg kg-1, i.v. In a group of patients with a normal intrinsic heart rate the number of positive electrophysiologic variables associated with sinus node dysfunction declined after autonomic blockade. In 91% of these patients,

Sleep fragmentation is the predominant factor causing excessive daytime sleepiness in diseases such as sleep apnea and periodic leg movement syndrome. The reference standard for quantifying sleep fragmentation is the arousal index (ArI), which is defined as the average number of arousals per hour of sleep. Arousal scoring is tedious and subjective resulting in considerable inter- and intra-rater variability. Moreover, ArI is only weakly correlated with other indicators of sleep fragmentation such as the total sleep time (TST) and the sleep efficiency (SE). This introduces consistency problems,

Polycystic Ovary Syndrome (PCOS) is a common endocrine disorder affecting up to 13 % of women. Lifestyle interventions are first-line treatments, however attrition in women with PCOS is high. This review summarises current evidence on barriers to lifestyle management in PCOS and suggested strategies for overcoming these challenges, mapped to the Capability, Opportunity, Motivation and Behaviour model.Physical capability for lifestyle changes may be impacted by altered gut hormone regulation and energy expenditure in PCOS. This may contribute to difficulties with weight management. The higher p

Patients with Parkinson's disease (PD) display significant sleep disturbances and daytime sleepiness. Dopaminergic treatment dramatically improves PD motor symptoms, but its action on sleep remains controversial, suggesting a causal role of nondopaminergic lesions in these symptoms. Because the pedunculopontine nucleus (PPN) regulates sleep and arousal, and in view of the loss of its cholinergic neurons in PD, the PPN could be involved in these sleep disorders. The aims of this study were as follows: (1) to characterize sleep disorders in a monkey model of PD; (2) to investigate whether l-dopa

BACKGROUND: A recent case study reported a non-24 h sleep-wake disorder in a patient with myotonic dystrophy type 1 (DM1). Research on intrinsic circadian rhythm sleep-wake disorders (CRSWDs) in DM1 patients is though lacking. This study utilized actigraphy to ascertain the presence of CRSWDs in DM1 patients and establish a comparison with healthy control subjects.
METHODS: Thirty-three patients with genetic diagnosis of DM1 and 33 age- and sex-matched healthy control subjects wore an actigraph and filled out a sleep diary for two consecutive weeks. Sleep, light exposure, and nonparametric me

This review presented an introduction of the visual pathway related circadian rhythm regulation system: the intrinsically photosensitive retinal ganglion cells-suprachiasmatic nucleus-pineal gland-melatonin axis, and discussed the impact of light with different wave length and irradiation received by retina on circadian rhythm and sleep habit. A hypothesis was proposed consequently that the high morbidity of sleep disorder in elderly might be partially attributable to the long-term blue light blocking status induced by age-related cataract. A number of relative literatures were reviewed and a

The percent of rapid eye movement (REM) sleep decreases dramatically between 10 and 30 days postnatally in the rat. The pedunculopontine nucleus (PPN) is known to modulate waking and REM sleep. We recorded intracellularly from 127 neurons in the PPN in 12-21-day rat brainstem slices maintained in warmed, oxygenated artificial cerebrospinal fluid. We identified three types of PPN neurons based on intrinsic membrane properties, type I (LTS), type II (A) and type III (A+LTS), as previously described. The percent of type I neurons increased from 6% at 12 days to 17% by 21 days, while the percent o

Sleep staging and Obstructive Sleep Apnea (OSA) event detection are fundamental aspects of sleep research. However, existing studies have predominantly focused on single-task learning methods, which do not adequately capture the intrinsic relationship between respiratory events and sleep staging. Furthermore, the utilization of EEG signals alone limits the capacity to comprehensively characterize OSA-related physiological dynamics. To address these limitations, this paper proposes SpecExpert-CGC, a novel multi-task co-learning framework based on EEG and ECG signals for simultaneous OSA event d

Smith-Magenis syndrome (SMS) (OMIM #182290) is a rare genetic disorder with a prevalence of 1 in 25 000 live births. Approximately 90% of SMS patients have harbored a 3.7 Mb interstitial 17p11.2 deletion involving the RAI1 gene, while 10% of cases have carried pathogenic variants of the RAI1 gene. SMS is characterized by sleep disturbance, intellectual impairment, developmental delay, craniofacial and cardiovascular anomalies, obesity, self injury, aggressive and autistic-like behaviors. Most SMS patients have sleep disorders such as short total sleep time, frequent night waking, short sleep o

Since the discovery of rapid eye movement (REM) sleep (also known as paradoxical sleep; PS), it is accepted that sleep is an active process. PS is characterized by EEG rhythmic activity resembling that of waking with a disappearance of muscle tone and the occurrence of REMs, in contrast to slow-wave sleep (SWS, also known as non-REM sleep) identified by the presence of delta waves. Here, we review the most recent data on the mechanisms responsible for the genesis of SWS and PS. Based on these data, we propose an updated integrated model of the mechanisms responsible for the sleep-wake cycle. T

Multiple mechanisms may modulate an association between restless legs syndrome/Willis-Ekbom disease (RLS/WED) and cardiovascular disease (CVD), including chronic sleep deprivation, intermittent, periodic limb movements in sleep (PLMS)-related autonomic fluctuations and possible autonomic dysfunction intrinsically associated with RLS per se. The purpose of this paper is to review the existing RLS/WED literature focusing on the pathophysiologic evidence for possible associations between RLS/WED and PLMS with CVD and events (CVE). Specific intrinsic dysautonomic aspects of the disease, which may

OBJECTIVE: To evaluate objective data on sleep quantity/quality and motor activity during night sleep in children with Tourette syndrome (TS).
METHOD: Polysomnography of 17 unmedicated TS children (ages: 7;11-15;5, mean: 11;10 years) without comorbid attention-deficit hyperactivity disorder (ADHD) was compared with 16 age-, sex- and IQ-matched healthy controls. Sleep analyses according to the procedure of Rechtschaffen and Kales were supplemented by counting epochs with short arousal-related movements (<or=15 s), thus allowing to calculate correlations between motor activity and sleep paramet

Circadian rhythms are near 24-h cycles in a number of physiological and behavioural parameters and the underpinning circadian timing systems is one of the key homeostatic regulatory systems in mammalian physiology. Many common psychiatric conditions are associated with disrupted sleep, including a common occurrence of delayed or advanced phase sleep syndromes, which in themselves may be indicative of dysregulated circadian timing in these disorders. In this article we discuss the evidence for abnormal circadian rhythms in seasonal affective disorder, bipolar disorder and attention deficit/hype

Mounting evidence that circadian abnormalities are a risk factor for cancer and for cardiovascular, psychiatric, and other disorders calls for in-depth investigation of intrinsic clock-dependent processes in diurnal animal models phylogenetically close to humans. Rhesus monkey (Macaca mulatta) is the most extensively studied diurnal nonhuman primate. Similar to humans, it features consolidated nighttime sleep and advanced cardiovascular, neuroendocrine, and cognitive responses. However, the intrinsic circadian rhythmicity in this species remains to be fully characterized. Here it is demonstrat

Circadian rhythm sleep disorder, free-running type (CRSD, FRT) is a disorder in which the intrinsic circadian rhythm is no longer entrained to the 24-hour schedule. A unique case of CRSD, FRT in a 67-year-old sighted male is presented. The patient had a progressively delayed time in bed (TIB) each night, so that he would cycle around the 24-h clock approximately every 30 days. This was meticulously documented each night by the patient over the course of 22 years. The patient's CRSD, FRT was associated with severe depression, anxiety, and agoraphobia. The agoraphobia may have exacerbated the CR

The biological clock in mammals is located in the suprachiasmatic nuclei of the hypothalamus. The combined output of multiple neuronal cellular oscillators determines the master circadian rhythm, which paces the myriad periodic functions of the organism, including, to a certain degree, the sleep-wake rhythm. The intrinsic master circadian rhythm, which is slightly longer than 24 hours, is synchronized daily to the extrinsic 24-hour day by the entrainment process, governed mainly by exposure to the environmental light at specific times. The pineal hormone melatonin is a specific and sensitive m

The authors describe a paradoxical reaction to lamotrigine (LTG) treatment in a patient with idiopathic rolandic epilepsy characterized by seizure deterioration, the appearance of new seizure type, and transient cognitive impairment. This phenomenon was present at a low dose after a slow titration and promptly reverted on LTG discontinuation. This rare event may have similarities with carbamazepine-induced seizure worsening caused by the Na++ channel inhibitory effect of the two antiepileptic drugs.

In blind individuals, the absence of light cues results in disturbances of sleep and sleep-related neuroendocrine patterns. The Zeitgeber influence of light on the timing of sleep is assumed to be mediated by melatonin, a hormone of the pineal gland, whose secretion is inhibited by light and enhanced during darkness. Here, we investigated whether a single administration of melatonin improves sleep and associated neuroendocrine patterns in blind individuals. In a double-blind crossover study, 12 totally blind subjects received 5 mg melatonin and placebo orally 1 h before bedtime starting at 230
