Human Basilar Arterial Plaque Detected by Near-Infrared Spectroscopy: Clinical Image.

A patient in their 70s presented with weakness of the left limb and aphasia. Left vertebral angiography showed acute basilar artery occlusion. After mechanical thrombectomy, basilar artery trunk stenosis was evident and catheter-based near-infrared spectroscopy (NIRS) showed lipid-rich atherosclerotic plaque extending to almost 220° of the vessel circumference in the culprit lesion. Loading doses of dual antiplatelet therapy and aggressive medical treatment were started because additional intervention might have increased risk of plaque protrusion and thrombotic reocclusion. The patient presen

Near-infrared spectroscopy monitoring during endovascular treatment for acute ischaemic stroke.

INTRODUCTION: The aim of endovascular treatment (EVT) for acute ischaemic stroke is to relieve the cerebral tissue hypoxia in the area supplied by the occluded artery. Near-infrared spectroscopy (NIRS) monitoring is developed to assess regional cerebral tissue oxygen haemoglobin saturation (rSO2). We aimed to investigate whether NIRS can detect inter- and intra-hemispheric rSO2 differences during EVT. PATIENTS AND METHODS: In this prospective, observational study, patients undergoing EVT for a proximal intracranial occlusion of the anterior circulation between May 2019 and November 2020, were

Correction of global physiology in resting-state functional near-infrared spectroscopy.

Significance: Resting-state functional connectivity (RSFC) analyses of functional near-infrared spectroscopy (fNIRS) data reveal cortical connections and networks across the brain. Motion artifacts and systemic physiology in evoked fNIRS signals present unique analytical challenges, and methods that control for systemic physiological noise have been explored. Whether these same methods require modification when applied to resting-state fNIRS (RS-fNIRS) data remains unclear. Aim: We systematically examined the sensitivity and specificity of several RSFC analysis pipelines to identify the best m

Subject-independent decoding of affective states using functional near-infrared spectroscopy.

Affective decoding is the inference of human emotional states using brain signal measurements. This approach is crucial to develop new therapeutic approaches for psychiatric rehabilitation, such as affective neurofeedback protocols. To reduce the training duration and optimize the clinical outputs, an ideal clinical neurofeedback could be trained using data from an independent group of volunteers before being used by new patients. Here, we investigated if this subject-independent design of affective decoding can be achieved using functional near-infrared spectroscopy (fNIRS) signals from front

Near-Infrared Spectroscopy Imaging for Assessing Skin and Wound Oxygen Perfusion.

Near infrared spectroscopy (NIRS) is a technique whereby light is transmitted to the skin or the surface of a wound. Light is selectively absorbed or reflected in order to calculate the percentage of oxygenated and deoxygenated hemoglobin. By calculating this ratio, the level of tissue oxygen perfusion can be assessed. This article describes an NIRS imaging device and demonstrates its use as a clinical tool to predict future viability of skin flaps, potential for wound healing, and progress of closure following application of a biologic wound product. Several cases are examined.

Measuring visual cortical oxygenation in diabetes using functional near-infrared spectroscopy.

AIMS: Diabetes mellitus affects about 6% of the world's population, and the chronic complications of the disease may result in macro- and micro-vascular changes. The purpose of the current study was to shed light on visual cortical oxygenation in diabetic individuals. We then aimed to compare the haemodynamic response (HDR) to visual stimulation with glycaemic control, given the likelihood of diabetic individuals suffering from such macro- and micro-vascular insult. METHODOLOGY: Thirty participants took part in this explorative study, fifteen of whom had diabetes and fifteen of whom were non-

High-dose antidepressants affect near-infrared spectroscopy signals: A retrospective study.

BACKGROUND: Recent studies have highlighted the clinical usefulness of near-infrared spectroscopy (NIRS) in psychiatry. However, the potential effects of psychotropics on NIRS signals remain unknown. METHODS: We conducted a systematic chart review of 40 depressed patients who underwent NIRS scans during a verbal fluency task to clarify the relationships between psychotropic dosage and NIRS signals. The dosage of psychotropic medications was calculated using defined daily dose (DDD). We investigated the associations between the DDD of psychotropic medications and oxygenated hemoglobin (oxy-Hb)

Does depth of the frontal sinus affect near-infrared spectroscopy measurement?

Near-infrared spectroscopy (NIRS) is a non-invasive method that reflects real-time cerebral oxygenation (rSO2) by the use of two adhesive optodes placed on the forehead of the patient. Frontal sinuses vary anatomically and a large frontal sinus might compromise the NIRS signal since the NIRS optodes are placed at the skin surface superficial to the underlying frontal sinus. The aim of this case-series was to elucidate whether there is a difference in the obligate changes in rSO2 during cardiac surgery between patients with a small as opposed to a large anterior-posterior distance of the fronta

Near-infrared spectroscopy in carotid artery stenting predicts cerebral hyperperfusion syndrome.

OBJECTIVE: Cerebral hyperperfusion syndrome (CHS) following carotid artery stenting (CAS) or carotid endarterectomy (CEA) is rare but often fatal once intracranial hemorrhage has occurred. In particular, CHS occurs significantly earlier after CAS than after CEA. Thus a monitoring method for early detection of CHS is required. Near-infrared spectroscopy (NIRS) provides a noninvasive monitoring technique for assessing regional cerebral oxygen saturation (rSO2). This study evaluated the usefulness of transcranial NIRS during CAS for prediction of CHS. METHODS: Periprocedural rSO2 was monitored i

Event-related functional near-infrared spectroscopy (fNIRS): are the measurements reliable?

The purpose of the present study was to investigate the retest reliability of event-related functional near-infrared spectroscopy (fNIRS). Therefore, isolated functional activation was evoked in the occipital cortex by a periodic checkerboard stimulation. During a 52-channel fNIRS recording, 12 subjects underwent 60 trials of visual stimulation in two sessions. The retest interval was set to 3 weeks. Linear correlations of the contrast t values supplemented by scatter plots, channel-wise intraclass correlation coefficients (ICC) as well as reproducibility indices for the quantity of activated

A near infrared spectroscopy study investigating oxygen utilisation in hydrocephalic rats.

Determination of hydrocephalus and its severity is important for optimal management of the condition. We have used near infrared spectroscopy (NIRS) to assess changes in concentrations of oxygenated (O2Hb), deoxygenated (HHb), total haemoglobin (tHb) and cytochrome c oxidase (Caa3) in normal and hydrocephalic Texas (HTx) rats in response to a 5 min head down tilt and a sodium pentobarbitone (NaPB) challenge. The former was used to test vascular responses and the latter to test metabolic responses. The haemoglobin oxygenation index (HbD) was derived which provides information regarding oxygen u

Localized near-infrared spectroscopy and functional optical imaging of brain activity.

Changes in cerebral blood flow (CBF) and cerebral metabolic rates (CMRO2) have been used as indices for changes in neuronal activity. Near-infrared spectroscopy (NIRS) can also measure cerebral haemodynamics and metabolic changes, enabling the possible use of multichannel recording of NIRS for functional optical imaging of human brain activity. Spatio-temporal variations of brain regions were demonstrated during various mental tasks. Non-synchronous behaviour of cerebral haemodynamics during the neuronal activation was observed. Gender- and handedness-dependent lateralization of the function b

Near-infrared spectroscopy: continuous measurement of cytochrome oxidation during hemorrhagic shock.

OBJECTIVE: Mitochondrial cytochrome a,a3 redox shifts can be determined by near-infrared wavelength reflection. Since near-infrared wavelengths penetrate skin and bone, a potential exists to noninvasively measure mitochondrial oxidation using this phenomenon. The purpose of this study was to compare conventional parameters of resuscitation with regional measurements of spectroscopically derived cytochrome redox state in a hemorrhagic shock model. DESIGN: Prospective, controlled laboratory investigation. SETTING: Animal research laboratory of a university medical center. SUBJECTS: New Zealan

Monitoring of immature rabbit brain during hypoxia with near-infrared spectroscopy.

Continuous monitoring of the cerebral blood flow, oxyhemoglobin, deoxyhemoglobin, total hemoglobin, oxidized cytochrome a, a3, and tissue pH during prolonged CO2 or N2 loading in 2-week-old rabbits was performed by near-infrared spectroscopy, the thermocouple method, and a tissue pH meter. Near-infrared spectroscopy demonstrated decreases in oxyhemoglobin and oxidized cytochrome a, a3 and increases in deoxyhemoglobin and total hemoglobin in the early stage within 5 min, which gradually lessened with time on both 10% concentration of inspired O2 with CO2 and N2. CBF increased with venous retent

The Potential of Near-Infrared Spectroscopy as a Therapeutic Tool Following a Stroke (Review).

The advancement of novel technologies for the rehabilitation of post-stroke patients represents a significant challenge for a range of interdisciplinary fields. Near-infrared spectroscopy (NIRS) is an optical neuroimaging technique based on recording local hemodynamic changes at the cerebral cortex level. The technology is typically employed in post-stroke patients for diagnostic purposes, including the assessment of neuroplastic processes accompanying therapy, the study of hemispheric asymmetry, and the examination of functional brain networks. However, functional NIRS can also be used for th

Imaging the developing brain with near-infrared spectroscopy in cochlear implanted children.

Cochlear implants (CIs) have revolutionized how we treat hearing impairment. Despite major technological and clinical advances, some CI children's language abilities remain below those of their age-matched peers, and there is still considerable individual variability in final outcomes. One important factor underlying this may be individual differences in brain plasticity before and after implantation. However, the neural changes induced in the developing brain by deafness, language deprivation, and the restoration of hearing due to implantation are little understood, in part because the method

Near-infrared Spectroscopy and Skin Tone in Children: A Prospective Cohort Study.

BACKGROUND: Retrospective studies suggest that pulse oximetry overestimates saturation in children from races that may be associated with darker skin tone. Near-infrared spectroscopy (NIRS) relies on similar optical technology, but less is known about the effect of skin tone on NIRS. This study aimed to quantify the effect of skin tone on NIRS performance. METHODS: Consecutive patients under 21 yr old undergoing cardiac catheterization were enrolled (N = 110). Skin tone was measured using spectrophotometry. Regional oxygen saturation was recorded from a Medtronic (USA) INVOS 5100C NIRS device

The Usefulness of Functional Near-Infrared Spectroscopy for the Assessment of Post-Stroke Depression.

Background: Post-stroke depression (PSD) is the most common mood disorder following stroke and is also the main factor that limits the recovery and rehabilitation of patients with stroke. The prevalence of PSD is ~30%. Since there is no gold standard for the diagnosis and evaluation of PSD, it is important to raise awareness of PSD and to establish methods for its evaluation, early diagnosis, and treatment. In the field of psychiatry, functional near-infrared spectroscopy (fNIRS) has been used as a diagnostic tool for the measurement of oxygenated hemoglobin (oxy-Hb). This study aimed to asses
