Speech appliances in the treatment of phonological disorders.

Some individuals are unable to achieve correct production of the /r/ phoneme after receiving substantial amounts of treatment. Clark, Schwarz, and Blakeley (1993) developed a removable speech appliance that positions the tongue to produce the /r/ phoneme. Their results indicate that the appliance was successful with a large group of clients. The rationale and issues related to the use of speech appliances are discussed.

Nonlanguage disorders of speech reflect complex neurologic apparatus.

The mute or nearly mute patient who is alert and has good understanding of speech and a right hemiparesis could have Broca's aphasia, akinesia of speech (transcortical motor aphasia), or aphemia. The patient who has Broca's aphasia does not write well, and his speech does not improve greatly with repetition. The speech of a patient with akinesia of speech improves with repetition. The aphemic patient writes normally, but his speech does not improve with repetition. The mute patient whose eyes are open but who is poorly responsive and moves little or not at all could be an akinetic mute (with e

Rate Modulation Abilities in Acquired Motor Speech Disorders.

PURPOSE: The purpose of this study was to describe, compare, and understand speech modulation capabilities of patients with varying motor speech disorders (MSDs) in a paradigm in which patients made highly cued attempts to speak faster or slower. METHOD: Twenty-nine patients, 12 with apraxia of speech (AOS; four phonetic and eight prosodic subtype), eight with dysarthria (six hypokinetic and two spastic subtype), and nine patients without any neurogenic MSD completed a standard motor speech evaluation where they were asked to repeat words and sentences, which served as their "natural" speakin

Continuous Speech for Improved Learning Pathological Voice Disorders.

Goal: Numerous studies had successfully differentiated normal and abnormal voice samples. Nevertheless, further classification had rarely been attempted. This study proposes a novel approach, using continuous Mandarin speech instead of a single vowel, to classify four common voice disorders (i.e. functional dysphonia, neoplasm, phonotrauma, and vocal palsy). Methods: In the proposed framework, acoustic signals are transformed into mel-frequency cepstral coefficients, and a bi-directional long-short term memory network (BiLSTM) is adopted to model the sequential features. The experiments were c

Functional speech disorders: clinical manifestations, diagnosis, and management.

Acquired psychogenic or functional speech disorders are a subtype of functional neurologic disorders. They can mimic organic speech disorders and, although any aspect of speech production can be affected, they manifest most often as dysphonia, stuttering, or prosodic abnormalities. This chapter reviews the prevalence of functional speech disorders, the spectrum of their primary clinical characteristics, and the clues that help distinguish them from organic neurologic diseases affecting the sensorimotor networks involved in speech production. Diagnosis of a speech disorder as functional can be

Orofacial Praxis Abilities in Children with Speech Disorders.

Introduction Phonological development occurs in a gradual manner until the age of 7 years. The phonological system is constructed in a similar way for all children, despite presenting some variations in terms of age, paths taken, or repair strategies used. Objective To compare the orofacial praxis abilities of children with typical phonological development (DFT), children with phonetic-phonological impairment (DFoFe), and children with phonological impairment (DF), using two tests to assess the orofacial praxis abilities. Methods The sample consisted of 82 subjects between 4 and 8 years of age

Speech processing in children with functional articulation disorders.

This study explored auditory speech processing and comprehension abilities in 5-8-year-old monolingual Hungarian children with functional articulation disorders (FADs) and their typically developing peers. Our main hypothesis was that children with FAD would show co-existing auditory speech processing disorders, with different levels of these skills depending on the nature of the receptive processes. The tasks included (i) sentence and non-word repetitions, (ii) non-word discrimination and (iii) sentence and story comprehension. Results suggest that the auditory speech processing of children w

Subtyping Children With Speech Sound Disorders by Endophenotypes.

PURPOSE: The present study examined associations of 5 endophenotypes (i.e., measurable skills that are closely associated with speech sound disorders and are useful in detecting genetic influences on speech sound production), oral motor skills, phonological memory, phonological awareness, vocabulary, and speeded naming, with 3 clinical criteria for classifying speech sound disorders: severity of speech sound disorders, our previously reported clinical subtypes (speech sound disorders alone, speech sound disorders with language impairment, and childhood apraxia of speech), and the comorbid cond

Extensions to the Speech Disorders Classification System (SDCS).

This report describes three extensions to a classification system for paediatric speech sound disorders termed the Speech Disorders Classification System (SDCS). Part I describes a classification extension to the SDCS to differentiate motor speech disorders from speech delay and to differentiate among three sub-types of motor speech disorders. Part II describes the Madison Speech Assessment Protocol (MSAP), an ∼ 2-hour battery of 25 measures that includes 15 speech tests and tasks. Part III describes the Competence, Precision, and Stability Analytics (CPSA) framework, a current set of ∼ 90 per

Toward an acoustic typology of motor speech disorders.

Acoustic methods have progressed to the point that an acoustic typology of the motor speech disorders can be constructed from a parameteric assessment of the speech subsystems (e.g., phonation, nasal resonance, vowel articulation, consonant articulation, intonation, and rhythm). The results of this analysis can be interpreted in respect to global functions in speech (e.g., voice quality, intelligibility, and prosody). This paper reviews studies showing that specific acoustic analyses have demonstrated or potential value toward the overall goal of constructing acoustic profiles of dysarthria an

[Specific reading disorders and the neurobiology of speech].

Overview of views on dyslexia-specific reading disorders. The most important contribution to the problem was the work of S. T. Orton which began more than 60 years ago. He was the first to formulate the neuropsychological theory of dyslexia. His followers use as a basis his idea that the specific background of dyslexia are cerebral mechanisms with decisive functional relations of the two cerebral hemispheres, but they abandon the factor of dominance of one half of the cerebral cortex. An important contribution was the work of D. Kimura et al. (some 30 years ago) who published findings relating

Electrodynamometric evaluation of nasal escape in speech disorders.

A new method of detecting the degree of nasal air emission in patients with speech disorders during "continuous" speech. The method employs the use of an electronic sensor placed in front of the nasal opening, which then picks up nasal escape and transfer it to an electrical signal that is recorded on an LED digital readout. The degree of nasal air emission, as portrayed on the LED, is then charted, permitting differentiation of the various speech disorders. This newly described method of nasal speech evaluation is of the utmost importance in the rehabilitation of patients with speech disorder

Effect of hearing training on hearing and speech skills in children with speech sound disorders.

PURPOSE: to measure the effect of Brief Computerized Auditory Training (Brief-CAT) on behavioral auditory and speech skills in children with Speech Sound Disorder (SSD). METHODS: 14 children, aged 6 and 9, diagnosed with SSD participated. All children presented one or more altered auditory skills in the behavioral assessment of Central Auditory Processing (CAP). They underwent six sessions of Brief-CAT. CAP's behavioral auditory skills and speech were assessed and a questionnaire was administered pre and post Brief-CAT. Inferential analysis was carried out. RESULTS: Brief-CAT had an effect o

Perceptual Classification of Motor Speech Disorders: The Role of Severity, Speech Task, and Listener's Expertise.

PURPOSE: The clinical diagnosis of motor speech disorders (MSDs) is mainly based on perceptual approaches. However, studies on perceptual classification of MSDs often indicate low classification accuracy. The aim of this study was to determine in a forced-choice dichotomous decision-making task (a) how accuracy of speech-language pathologists (SLPs) in perceptually classifying apraxia of speech (AoS) and dysarthria is impacted by speech task, severity of MSD, and listener's expertise and (b) which perceptual features they use to classify. METHOD: Speech samples from 29 neurotypical speakers,
