PROBLEM: Sperm flagellar protein 2 (SFP2), which was earlier identified using a novel combinatorial approach, was evaluated for its contraceptive potential in mice.
METHOD OF STUDY: Male mice were actively immunized with two synthetic peptides of SFP2. Antipeptide antibody was characterized by Western blot and indirect immunofluorescence. Immune response was monitored, and mating studies were performed 6 and 22 weeks post-immunization.

There is a vast amount of evidence linking the presence of genitourinary infection with infertility. Infection in either partner can result in specific effects on sperm function, accessory sexual gland dysfunction, and the induction of immunologic responses in either partner. Careful investigation for infection should be undertaken in every couple with the complaint of infertility. Treatment needs to be individualized in each instance.

Eighteen patients were reviewed 7 to 23 years after prolonged unilateral testicular torsion. We studied 25 patients with spermatic cord torsion occurring after puberty. The elapsed time between the onset of symptoms and observation varied from 6 months to 7 years. We evaluated the semen of these patients and found that the number of spermatozoa was lower in patients with an atrophic testis (13 patients) than in those with a normal testis or, in those with torsion treated by orchidectomy. The incidence of non-fatal myocardial infarction among 4830 vasectomised men was 0.9 cases per 1000 man-years during 24 420 man-years of observation. This was slightly lower than the rate in 24 150 non-vasectomised men, matched with a vasectomised man for calendar year of birth and duration of observation. Review of medical records for a matched sample of study subjects indicated no measurable confounding by important cardiac risk factors.

Subfertility was evaluated in 425 men. Varicocele were diagnosed in 37.4 per cent of these patients and surgical correction of the varicocele in 68 men resulted in a 65 per cent improvement in semen quality. Idiopathic abnormalities were found in 25.4 per cent of the patients. Since the study group was heterogenous setereotyped or empirical treatment cannot hope to be successful if uniformly applied. Other specific causes of male subfertility are identified and therapeutic options are discussed.

Sperm agglutinins emerging from the reopened part of the vas deferens were found in the seminal fluid of 10 out of 29 men after vasovasostomy. There was a strong association between the presence of these antibodies and the failure to impregnate over a period of 14 to 33 months.
