After vasectomy reversal by vasovasostomy or vasoepididymostomy motile sperm appear commonly in the semen even when only nonmotile sperm are present in the intraoperative vasal or epididymal fluid. We studied patients with bilateral congenitally absent vasa deferentia to see if relief of obstruction by implantation of an alloplastic spermatocele also benefits sperm motility in such patients. BACKGROUND: Hydrocele and spermatocele are fluid collections within the scrotum. Due to a lack of therapeutic guidelines, treatment is largely based on clinical experience. In German-speaking countries, the gold standard for treatment is surgical management. However, postoperative complications occur relatively frequently. Hydrocele and spermatocele surgeries were recently included in the outpatient surgical reimbursement catalog as hybrid diagnosis-related groups (DRGs). Reliable data on whether safe outpatient performance is feasible are currently lacking.

Testicular torsion is a rare disease that mostly involves children. Peak incidence is in infancy and in adolescence. Testicular torsion is rarely seen in men over 40 years of age and has only once been accompanied with spermatocele. We report an atypical case of spermatocele in which sonographic findings demonstrated an intraescrotal mass fixed to the testicle, apparently solid and with important vascularity, being these findings suspicious of malignant tumor. We recommend to include complicated spermatocele in the differential diagnosis of ecographically solid masses.

Sclerotherapy with 5% ethanolamine oleate was used to treat 102 hydroceles in 100 patients and 63 spermatoceles in 58 patients. Scrotal ultrasound examination was performed before the sclerotherapy and at each followup visit.
