Psammomatoid ossifying fibroma (POF), a variant of ossifying fibroma, is a benign fibroosseous lesion typically arising within the nasal cavity, paranasal sinuses, and orbit. Cranial vault involvement is exceedingly rare, with very few cases reported in the literature. The authors report a case of POF in the neurocranium of an 11-year-old child, 4 years after chemotherapy and radiation therapy for acute lymphoblastic leukemia. This case is reported in view of its rarity, novelty of presentation, and the difficulty in diagnosis due to its radiological resemblance to aneurysmal bone cyst or mono

We report a case of a 72-year-old male who presented to the hospital with a chief complaint of diplopia in the setting of a recent onset of urinary incontinence and right-sided back pain. He was subsequently diagnosed with prostate cancer, notably metastasizing to the right sphenoid bone, causing impingement of the oculomotor nerve. Our case is unique in that the patient's initial presentation of prostate cancer was oculomotor nerve palsy with subsequent histologic analysis of the primary tumor showing both small cell neuroendocrine carcinoma along with adenocarcinoma. Also, the initial routin

INTRODUCTION: McCune-Albright syndrome (MAS) is a rare disorder characterized by the classic triad of precocious puberty, polyostotic fibrous dysplasia and café-au-lait pigmented skin lesions. Cystic change is rare in fibrous dysplasia (FD), especially in McCune-Albright syndrome. There were no reports about cyst degeneration in MAS which resulted in abnormal visual acuity and visual fields. Herein, we report a female patient with MAS associated with sphenoid bone cysts which resulted in visual deterioration to describe the computed tomography (CT) and magnetic resonance (MR) imaging findings

Primary extracranial meningiomas are rare tumors that arise outside the cranial cavity without continuity with intracranial structures, and their preoperative diagnosis is often challenging. We report a rare case of a primary extracranial meningioma centered on the sphenoid bone with multicompartmental extension. A 53-year-old woman presented with a gradually enlarging mass in the left temporal region. Imaging revealed a well-defined lesion extending from the sphenoid sinus to the lateral orbit and temporal region. Computed tomography (CT) demonstrated bone thinning and remodeling without dest

BACKGROUND: To study the efficacy of surgery for recurrent T3 nasopharyngeal carcinoma (NPC) and to determine the prognostic significance of various skull base bone invasion.
METHOD: Retrospective view of the surgical outcome for recurrent T3 NPC. Kaplan Meier and log rank tests were used to determine the 5-year overall and disease specific survival. Multivariate analysis was used to identify significant independent prognostic factors that affect the surgical outcome.
RESULTS: Between 1990 and 2017, 208 patients with recurrent T3 NPC were recruited. Salvage surgery was performed via the endo

The orbital roof in adults consists mainly of the frontal bone (FB), with the ala minor of the sphenoid at the posterior margin. In fetuses, these bones have been observed to overlap at the junction. The present study examined sagittal, frontal, and horizontal histological sections from 38 human fetuses at a gestational age (GA) of approximately 7-39 weeks. At GA 7-8 weeks, the ala minor extended anterolaterally from the orbitosphenoid, covering the posteromedial quadrant of the roof until GA 9 weeks and reaching to almost the center of the roof at late-term. Simultaneously, the FB appeared in

We report a patient with a lateral orbital wall fracture that occurred as a result of a blow-out fracture. The patient has extensive pneumatization of the sphenoid sinus, and the air extends into the lateral orbital wall. It makes the lateral orbital wall much thinner unusually, being more vulnerable to a sudden increase in intraorbital pressure. Pure blow-out fracture of the lateral orbital wall has not been reported in the medical literature. Therefore, this is the first case report of a lateral orbital wall fracture occurring as a blow-out mechanism.

Fibrous dysplasia (FD) is characterized by expansile fibro-osseous lesions that may occur in association with endocrinopathies as part of McCune-Albright syndrome (MAS). Craniofacial FD may result in substantial morbidity, including facial asymmetry and compression of vital neurovascular structures. There is a critical need to understand the natural history and risk factors for craniofacial lesion expansion to develop preventative trials and identify candidates for intervention. The purpose of this study was to evaluate expansion rates in periorbital bones (frontal, zygomatic, and sphenoid). P

Skull base architecture is tough to understand because of its 3D complex shape and its numerous foramen, reliefs or joints. It is especially true for the sphenoid bone whom central location hinged with most of skull base components is unique. Recently, technological progress has led to develop new pedagogical tools. This way, we bought a new real-time three-dimensional insight of the sphenoid bone that could be useful for the teacher, the student and the surgeon. High-definition photography was taken all around an isolated dry skull base bone prepared with Beauchêne's technique. Pictures were

A 19-year-old man was admitted to the hospital because of blurred vision, visual field defect, diplopia and hypesthesia of the left face. Neurological examination on admission revealed impairments of the II, III, IV, VI cranial nerves bilaterally and the first branch of the V nerve on the left. X-ray films of the skull showed a marked decalcification of the sella and upper portion of the clivus. Cerebral angiography demonstrated a moderately vascularized, large tumor in the sella-clival region. The tumor was supplied mainly by the branches of the right internal carotid artery, which was occlud

The anterior suprapetrosal approach provides access to the anterior part of the middle cerebral fossa and its various openings: small round, oval and large round foramina. Trephination of the floor of the cerebral fossa reveals the Eustachian tube, the intrapetrosal portion of the internal carotid artery, the pterygomaxillary fossa, and possibly a particularly markedly developed recess of the sphenoidal sinus. This new surgical approach can resolve certain rare but classical therapeutic problems, particularly of excision of tumors in this region: neurinomas of V developing anteriorly, meningio

Basilar invagination (BI) is a congenital malformation of the skull base, in which underdevelopment of the cranial base displaces the odontoid process cranially and posteriorly. Few studies have examined the role of sphenoid bone in BI, despite its central position occupying large portion of the skull base. The objective of this study is to compare the craniometric measurements centered on the sphenoid bone between the two types of BI, and normal controls. This is a cross-sectional study based on MRI and CT databases from authors practice, from 1985 to June 2024. The craniometrics of the sphen

BACKGROUND: Parasellar plasmacytomas are rare neurosurgical entities. Intrinsic characteristics of these tumors, such as adjacent bone erosion and symptoms resulting from invasion and mass effect, may lead to the possibility of a solitary extramedullary plasmacytoma (SEP) as a differential diagnosis.
CASE DESCRIPTION: We present the case of a 39-year-old male with a 1-month history of bilateral decreased visual acuity, retroocular pulsating pain, and chromatic vision loss. A computed tomography scan of the head revealed a parasellar lesion causing chiasmatic compression, as well as clival, or

BACKGROUND: Parathyroid carcinoma accounts for <1% of cases of primary hyperparathyroidism (PHPT). This rare condition may present with severe hypercalcemia and bone complications such as osteoclastomas and pathologic fractures. Here, we present a rare condition of panhypopituitarism resulting from an osteoclastoma in the sphenoid bone that invaded the pituitary fossa due to parathyroid carcinoma. Case Report. A 47-year-old woman previously diagnosed with PHPT underwent a parathyroidectomy 6 years earlier, with histological examination indicating a parathyroid adenoma. After surgery, she conti

This report describes a case of spontaneous cerebrospinal fluid leak through a narrow canal-like fistula in the lateral wall of the sphenoid sinus, which was managed through rigid reconstruction. Rigid reconstruction of the skull base was performed by fitting a pile-shaped bone into the fistula like the pile-driving technique.

BACKGROUND: Ewing's sarcoma (ES) is a malignancy that arises from bones or soft tissue, characterized by primitive small and round blue cells. Primary ES typically occurs in the long bones, vertebrae, or pelvis, and is extremely rare in the skull base.
CASE DESCRIPTION: A 14-year-old girl presented with posterior cervical pain and dysfunction of multiple cranial nerves (CNs). Radiological investigation revealed a solid mass of the petroclival bone extending into the sphenoid sinus. The patient underwent endoscopic transsphenoidal surgery for diagnosis of the pathology, and partial resection w

We report a case of myeloid sarcoma with multifocal skeletal involvement, including the greater wing of the sphenoid bone. A 23-month-old boy presented with left-sided proptosis and fevers, and was found to have an infiltrative mass involving the left sphenoid bone on orbital imaging. Full body imaging further demonstrated multiple bony lesions in the pelvis, thoracic and lumbar vertebrae, bilateral femura, and left humerus, and biopsies of the humerus were consistent with myeloid sarcoma. The patient was started on a standard chemotherapy regimen and is responding well. Myeloid sarcoma presen

Primary spontaneous cerebrospinal fluid leaks occurring in the absence of any apparent cause are rare. Patients may present such leaks simultaneously, or successively, in multiple locations. We report here the sixth case of anterior and lateral skull base defects presented in a single patient. Although rhinoliquorrhea and otoliquorrhea were reported separately in the literature, we postulated that the pathophysiology is the same for the whole skull base; obesity seems to contribute to the development of such primary spontaneous cerebrospinal fluid leak. Consequently, management is similar and

A fairly rare case of coexistence of intracranial meningioma with aneurysm is reported. Scant reports in the literature point out that the lessons are situated of the unilaterally, and the decrease of intracranial pressure after removal of the expanding lesion causes subarachnoid haemorrhage, even it the aneurysm had not ruptured before the operation. This suggests that both lessons should be managed at one step.

A patient with fibromatosis of the skull and a so-called "parophthalmicus syndrome" is described in whom there was hypoplasia and stenosis of the homolateral internal carotid artery and aplasia of the ophthalmic artery. The vascular changes in this phakomatosis are discussed. We suggest that the vascular changes in the carotid be included in the extended parophthalmicus syndrome.
