Odontoid process pathologic fracture in spinal tuberculosis.

Craniovertebral junction tuberculosis is a rare lesion in which treatment remains controversial. Options range from conservative treatment to surgery, independently of any associated neurological threat. We here report the first case of pathologic odontoid fracture in a context of spinal tuberculosis, complicated by unusual neurological evolution. The patient presented with non-contiguous multifocal tuberculosis, of which there have previously been only 6 reported cases.

Traumatic lumbosacral pseudomeningocele associated with spinal fracture.

BACKGROUND CONTEXT: Traumatic pseudomeningocele and avulsion of lumbosacral nerve roots are rare in the absence of fractures of the pelvic ring or hip dislocation. Clear prognoses for the root avulsion or pseudomeningocele do not exist. PURPOSE: To present a patient who developed a posttraumatic pseudomeningocele after lumbosacral injury. STUDY DESIGN: Case report and review of the literature. METHODS: A patient who sustained traumatic pseudomeningocele of the lumbar spine was treated nonsurgically and followed over a 2-year period. RESULTS: Nerve root deficit largely recovered, although a

[Exertion, spinal fracture, intervertebral disk displacement].

If causes which cannot be clearly defined as traumatic, result in vertebral fractures or disc ruptures, the expert frequently finds himself confronted with considerable problems. He must carefully establish the relation between what is due to the trauma and what must be attributed to the pre-existing damage. This requires the pre-existing nox to be objectively cleared, especially with regard to osteoporosis. The diagnosis "osteoporosis" should be subjected to particularly careful investigation, especially if the patient is a male. X-ray photographs, taken at an early stage, are insufficient. A

Acute management of spinal fractures: What you need to know.

Spinal injuries occur frequently and influence the diagnosis and treatment of concurrent traumatic conditions. Understanding of spinal trauma patterns, clinical assessment and documentation, imaging options, and classification systems is essential for making multidisciplinary treatment decisions. The initial evaluation focuses on identifying neurological deficits, assessing mechanical stability, and recognizing red-flag symptoms requiring urgent intervention. Furthermore, essential concurrent nonspinal injuries, as well as multiple spinal fractures, should be ruled out. Whole-body or whole-spi

Spinal augmentation for vertebral body fractures in the elderly population.

Osteoporotic vertebral body fractures (OVBFs) are a highly prevalent pathology in the United States elderly population and can result in height loss and kyphotic deformity. While minimally invasive surgical techniques such as percutaneous kyphoplasty (PKP) and vertebroplasty (PVP) are commonly used, the efficacy of these techniques in older individuals is unknown. By examining the current body of evidence, we aim to assess the efficacy of these procedures within the context of appropriate grading of levels of evidence. This narrative review was conducted by searching multiple databases, includ

Pregnancy and Lactation-Associated Osteoporotic Spinal Fractures: A Case Report.

BACKGROUND Pregnancy and lactation-associated osteoporosis (PLO) is a rare condition characterized by fragility fractures occurring during late pregnancy or lactation. It is associated with pregnancy- and lactation-related bone metabolic disorders and risk factors such as low body mass index (BMI). Given the absence of standardized guidelines and limited clinical awareness, PLO is often misdiagnosed or diagnosed late. The patient described in this report experienced both delayed treatment and misdiagnosis. The findings suggest that bisphosphonates have limited efficacy in improving bone minera

Fragility spinal fractures among cirrhotic liver transplant candidates in Croatia.

INTRODUCTION: Existing data on fragility spinal fractures prevalence in liver transplant candidates are scarce and inconsistent. This may be due to other comorbidities, besides hepatic osteodystrophy (HO), that contribute to bone loss and fragility fracture prevalence in chronic liver disease (CLD). OBJECTIVES: The aim of this study was to investigate the prevalence of spinal thoracic and lumbar fragility fractures among cirrhotic, non-chronic kidney disease (CKD), non-diabetic liver transplant candidates and to explore their relationship with clinical characteristics, laboratory markers and

Floating Thoracic Spine After Double, Noncontiguous Three-Column Spinal Fractures.

BACKGROUND: Double, noncontiguous, 3-column spinal injuries are a rare phenomenon most often caused by high-energy trauma. The resulting multilevel, fracture-dislocation injuries represent 2 separate 3-column lesions and produce a floating spine segment between the 2 fracture dislocation sites. Only a few cases of these rare, posttraumatic injuries have been reported previously; however, all of these included a combination of injuries in the cervical, thoracic, lumbar, and/or sacral spine. CASE DESCRIPTION: We present the first report of a case of double-level spinal injury isolated to the th

Early percutaneous fixation of spinal thoracolumbar fractures in polytrauma patients.

INTRODUCTION: Care of polytrauma patients is complex and requires that a particular treatment sequence be followed during what is typically a short period of time. Early, temporary stabilization of injuries (damage control orthopedics [DCO]) is a validated strategy for the care of polytrauma patients. Application of this concept to spinal fractures has also led to good outcomes for patients. The recent development of percutaneous thoracolumbar fixation could improve the initial care of these vulnerable patients even more. The purpose of this study was to evaluate preliminary results in a serie

Risk factors affecting progressive collapse of acute osteoporotic spinal fractures.

UNLABELLED: We prospectively investigated related clinical and radiological risk factors for progression following acute osteoporotic spinal fractures. Fracture location, morphological feature (mid-portion), and involvement of vertebral posterior wall were statistical significant risk factors considering increase of height loss and kyphotic angle, and occurrence of intravertebral cleft sign as a progressive collapse. INTRODUCTION: This study was designed to investigate the clinical and radiological risk factors related to progressive collapse of acute osteoporotic spinal fractures (OSF). MET

Spinal fractures in recreational bobsledders: an unexpected mechanism of injury.

STUDY DESIGN: Comparative effectiveness review. OBJECTIVE: To determine the comparative effectiveness and safety of percutaneous minimally invasive versus open spine surgery for fractures of the thoracolumbar junction. METHODS: A systematic review of the English-language literature was undertaken for articles published between 1970 and March 15, 2012. Reference lists of key articles were also systematically checked. We attempted to identify all articles that reported on the effectiveness and/or safety comparing minimally invasive surgery (MIS) with open surgery for thoracolumbar fractures in

Helmet use and associated spinal fractures in motorcycle crash victims.

BACKGROUND: The effect of helmet use on the incidence of cervical and thoracic fractures sustained in motorcycle crashes remains controversial. METHODS: We retrospectively reviewed the incidence of these fractures in helmeted and nonhelmeted crash victims at a single Level I trauma hospital with a well-defined system for evaluating spinal fractures. RESULTS: Of 422 motorcycle crash victims treated during 3 years, 190 had a traumatic brain injury (TBI) and 75 sustained some form of spinal fracture. CONCLUSIONS: Based on the statistical analysis, there was no relationship between helmet use a

Upper thoracic spinal fractures in trauma patients - a diagnostic pitfall.

The diagnosis of upper thoracic spinal fractures in multiple-trauma patients remains a challenge. The clinical findings are often difficult to detect, especially in the presence of other (extremity) fractures, head injuries or in patients on respiratory support. The findings of chest radiographs and plain spinal films are described in a series of 23 patients with an upper thoracic spinal fracture. Radiographs were retrospectively reviewed by an orthopaedic surgeon and a skeletal radiologist. Fractures were classified according to Magerl and type A1 and A2 compression fractures were excluded. Operative treatment of spinal fractures with the AO internal fixator.

A multicentered prospective trial to investigate the usefulness of the AO spinal internal fixator for the treatment of thoracolumbar fractures and fracture-dislocations was undertaken. This pedicle screw-rod system was effective in stabilizing a variety of unstable fracture patterns. It was effective in decompressing the canal of retropulsed bony fragments associated with burst fractures. By the use of this implant, sagittal plane deformity was easily corrected.

[Spinal metastases and metastasis-induced pathological fractures of the spine].

The vertebral column is a common site of metastatic disease to the bone. The incidence is very high and varies with tumor type. Radiation therapy is effective in the treatment of most cases of spinal metastases. Pathologic fractures of the vertebral body occur in 30-50% of the patients affected, indicating an operative intervention. The ultimate aim of the operation is to increase the patients' quality of life. The tumor mass is normally located in the vertebral body. Therefore, only with an anterior approach can excellent decompression of the spinal cord and reduction of the tumorous tissue b
