Age-related changes of elements with their relationships in human cranial and spinal nerves.

To elucidate compositional changes of peripheral nerves with aging, the authors investigated age-related changes of elements and their relationships in the optic, trigeminal, vagus, median, radial, ulnar, femoral, sciatic, tibial, and common peroneal nerves by inductively coupled plasma-atomic emission spectrometry. The subjects consisted of 10 men and 12 women, ranging in age from 65 to 91 yr. It was found that although accumulations of Ca and P occurred only in the trigeminal nerve at old age, it hardly occurred in the optic, vagus, median, radial, ulnar, femoral, sciatic, tibial, and common

Morphological alterations induced by sodium valproate on somites and spinal nerves in rat embryos.

The antiepileptic drug valproic acid is a well-known teratogenic agent; its main target organ is the neural tube, though skeletal malformations have also been described. In our recent work, respecifications of vertebrae were described in rat fetuses after treatment with 400 mg/kg of sodium valproate at specific somitogenic stages. The observed malformations were stage-dependent. Morphological segmental respecification was observed at the level of segments in formation at the moment of exposure and at the level of more posterior segments. Recently, specific alterations in the development of cra

Short-latency somatosensory evoked potentials (SSEPs) of the tibial nerves in spinal cord injuries.

Stimulations of the tibial nerves of 76 spinal cord injured patients revealed short-latency somatosensory evoked potentials (SSEPs) especially P37 recorded from the scalp. The SSEPs findings can be classified by degrees of impairment into 5 different types as follows: Type 1: Absence of P37. This was found in all patients with complete cord lesions as well as patients with loss of joint sense. Type 2: Decreased amplitude and prolonged latency. This was elicited in patients with moderate to severe central cord lesions. Type 3: Prolonged latency only. Type 4: Decreased amplitude only. Type 5: No

Electroacupuncture direct to spinal nerves as an alternative to selective spinal nerve block in patients with radicular sciatica--a cohort study.

We applied electroacupuncture to the spinal nerve root by inserting needles under x ray imaging in three cases with radicular sciatica, as a non-pharmacological substitute for lumbar spinal nerve block. In all three cases, symptoms were markedly reduced immediately after electroacupuncture to the spinal nerve root. The sustained effect was noticeably longer than that of spinal nerve blocks previously performed, in two out of the three cases. We suggest that descending inhibitory control, inhibitory control at the spinal level, inhibition of potential activity by hyperpolarisation of nerve endi

Spinal cord and spinal nerve root involvement (myeloradiculopathy) in tuberculous meningitis.

Most of the information about spinal cord and nerve root involvement in tuberculous meningitis is available in the form of isolated case reports or case series. In this article, we evaluated the incidence, predictors, and prognostic impact of spinal cord and spinal nerve root involvement in tuberculous meningitis. In this prospective study, 71 consecutive patients of newly diagnosed tuberculous meningitis were enrolled. In addition to clinical evaluation, patients were subjected to magnetic resonance imaging (MRI) of brain and spine. Patients were followed up for at least 6 months.Out of 71 pat

Quality of spinal extradural anaesthesia: the influence of spinal nerve root diameter.

Profiles on the progress of spinal extradural anaesthesia were made in 246 patients using various concentrations of lignocaine, bupivacaine and etidocaine. The advance of analgesia through the different dermatomes showed a consistent and orderly spread on the upper lumbar and thoracic segments. This spread was irregular through the lower lumbar and upper sacral segments. The first sacral segment was especially difficult to block with an overall failure rate of 17.53% with presently available concentrations of various local anaesthetics. This failure was abolished with the use of etidocaine 1.5

Entrapped spinal nerve roots in a pseudomeningocoele as a complication of previous spinal surgery.

Pseudomeningocele is a rare but well recognised complication of lumbar surgery (microdiscectomy and laminectomy). Most of the patients tolerate the presence of the cyst well, however some present with back pain and spinal claudication, presumably due to neural compression. We report a case who presented following three operations (microdiscectomy, laminectomy and excision of a pseudomeningocele) with symptoms of spinal claudication and bilateral radicular pain. The cause of her pain was evident only at operation and was due to herniation of nerve roots through the dural defect.

Anatomic description of the basivertebral nerve and meningeal branch of the spinal nerve in the dog.

PURPOSE: The existence of the basivertebral nerve and meningeal branch of the spinal nerve has not been proven in dogs to date. The objectives of this study are to 1) determine whether dogs have a meningeal branch of the spinal nerve (MBSN) and a basivertebral nerve (BVN) and to (2) describe anatomical characteristics of these two nerves. Authors also put forward a discussion on the possible clinical relevance of these findings. MATERIAL AND METHODS: Dissections were performed on six embalmed dogs at the Veterinary Faculty of Barcelona with the use of stereomicroscopy and microsurgery equipme

Spinal nerve origins of the muscular branches of the radial nerve: an electrophysiological study.

BACKGROUND: In injuries of the lower brachial plexus, finger flexion can be restored by nerve or tendon transfer. However, there is no technique that can guarantee good recovery of finger and thumb extension. OBJECTIVE: To determine the spinal nerve origins of the muscular branches of the radial nerve and identify potential intraplexus donor nerves for neurotization of the posterior interosseous nerve in patients with lower brachial plexus injuries. METHODS: An intraoperative electrophysiological study was carried out during 16 contralateral C7 nerve transfers. The compound muscle action pot

Neurophysiologic monitoring of the spinal accessory nerve, hypoglossal nerve, and the spinomedullary region.

This review of hypoglossal nerve, spinal accessory nerve, and spinomedullary region intraoperative monitoring details pertinent central and extramedullary anatomy, an updated understanding of proper free-run EMG recording methods and recent developments in stimulation technique and instrumentation. Mapping and monitoring the floor of the fourth ventricle, especially the vagal/hypoglossal trigone region, are emphasized. Although cranial nerve transcranial electrical motor evoked potential recordings can afford appreciation of corticobulbar/corticospinal tract function and secure a more dependab

Electrophysiological testing of spinal accessory nerve in suspected cases of nerve transection.

INTRODUCTION: In this study we sought to determine whether standard electrophysiological testing of the spinal accessory nerve (SAN) accurately identifies patients who would benefit from surgical repair. METHODS: Sixteen consecutive patients sent for surgical evaluation of unilateral SAN injury were studied clinically and electrophysiologically. RESULTS: All patients demonstrated a low-amplitude SAN compound muscle action potential (CMAP) that required a higher stimulus intensity to obtain it than on the unaffected side. Upper trapezius needle electromyography showed dense fibrillation poten

[Chronic pernicious course in multiple sclerosis with involvement of the optic nerves and total spinal sclerosis (diffuse spinal sclerosis) (author's transl)].

Four cases with the clinical diagnosis of chronic MS are presented, which, histologically, show large solitary demyelinated and gliotic areas involving almost the entire spinal cord over 12-25 segments as well as the optic chiasm. The abortive form of the neuromyelitis optical syndrome in MS is discussed. The additional involvement of the cerebral hemispheres suggests that the clinical and histological picture described is related to Schilder's disease.

Nerve fascicle transfer using a part of the C-7 nerve for spinal accessory nerve injury.

OBJECTIVE Spinal accessory nerve (SAN) injury results in a series of shoulder dysfunctions and continuous pain. However, current treatments are limited by the lack of donor nerves as well as by undesirable nerve regeneration. Here, the authors report a modified nerve transfer technique in which they employ a nerve fascicle from the posterior division (PD) of the ipsilateral C-7 nerve to repair SAN injury. The technique, first performed in cadavers, was then undertaken in 2 patients. METHODS Six fresh cadavers (12 sides of the SAN and ipsilateral C-7) were studied to observe the anatomical rela

Temporal regulation of macrophage polarization by abnormally innervated CGRP + Sensory nerves following spinal cord injury.

Spinal cord injury (SCI) initiates a complex neuroimmune cascade, where the interplay between damaged neural circuits and the inflammatory response critically determines clinical outcomes. A pivotal but poorly understood aspect of this interplay is the aberrant regeneration of CGRP⁺ sensory nerve fibers and their dynamic communication with immune cells. This review systematically delineates the spatiotemporal remodeling of CGRP⁺ fibers post-SCI and elucidates their time-dependent regulation of macrophage polarization. We synthesize emerging evidence on how CGRP signaling, through receptors lik

The future of artificial hibernation medicine: protection of nerves and organs after spinal cord injury.

Spinal cord injury is a serious disease of the central nervous system involving irreversible nerve injury and various organ system injuries. At present, no effective clinical treatment exists. As one of the artificial hibernation techniques, mild hypothermia has preliminarily confirmed its clinical effect on spinal cord injury. However, its technical defects and barriers, along with serious clinical side effects, restrict its clinical application for spinal cord injury. Artificial hibernation is a future-oriented disruptive technology for human life support. It involves endogenous hibernation

Enhancement of multiple cranial and spinal nerves in vanishing white matter: expanding the differential diagnosis.

Abnormal cranial or spinal nerve contrast enhancement on MRI in cases of suspected pediatric leukodystrophy is recognized as an important clue to the diagnosis of either metachromatic leukodystrophy or globoid cell leukodystrophy (Krabbe disease). We report a case of genetically confirmed childhood vanishing white matter with enhancement of multiple cranial and spinal nerves in addition to the more typical intracranial findings. This case expands the limited differential diagnosis of cranial nerve or spinal nerve enhancement in cases of suspected leukodystrophy and may aid in more efficient wo

[Permanent electrostimulation of sacral spinal nerves with an implantable neurostimulator in treatment of fecal incontinence].

Functional deficits of the striated muscular anal sphincter frequently result in faecal incontinence. The therapeutic options for patients without a defined muscular defect are limited. Our patient without defined lesion, but with a clinically relevant reduction of the voluntary force of the anal sphincter resulting in daily loss of stool, underwent an electrostimulation procedure of the sacral spinal nerves. The procedure was divided in three steps: acute percutaneous testing, temporary percutaneous nerve evaluation and permanent electrostimulation phase with an implantable neurostimulation d

Glial bundles in spinal nerve roots. An immunocytochemical study stressing their nonspecificity in various spinal cord and peripheral nerve diseases.

Glial bundles (GBs) in spinal nerve roots in 86 autopsy cases with various spinal lesions were examined using the peroxidase-antiperoxidase technique for glial fibrillary acidic protein (GFAP). In 19 of 22 cases of Werding-Hoffmann disease (WHD), GBs were present in the anterior roots (ARs) but absent in the youngest age group (age less than 1.5 months at death). GBs were numerous in classical cases (age 3-24 months), accompanying severe damage of the anterior horns and roots, but were less prominent in most cases of protracted course (age 2-8.5 years). Thus, development of GBs in the ARs of m
