PURPOSE: To make a literature review on spinal stenosis recurrence after a first surgery and edit rules to avoid this complication.
METHODS: We conducted two separate PUBMED searches to evaluate the revision post-stenosis and degenerative scoliosis surgery using the terms: lumbar vertebrae/surgery, spinal stenosis, spine, scoliosis and reoperation. The resulting papers were categorized into three groups: (1) those that evaluated reoperation post-simple decompression; (2) those that evaluated spinal decompression and fusion for short (3 levels or less) or long (more than 3 levels) segment spin

People's understanding of lumbar spinal stenosis has become more and more comprehensive and reasonable, however, there are still many controversies about the concepts of "central lumbar canal" and "lateral lumbar spinal canal", and there is no unified standard at present. In this paper, we redefine and differentiate the two concepts. We believe that some kinds of central canal stenosis caused by bilateral recess stenosis can be completely solved by bilateral percutaneous endoscopic transforaminal discectomy. At the same time, the concept of "lumbar lateral recess" is ambiguous. We redefine it

Lumbar spinal stenosis (LSS) affects more than 200,000 adults in the United States, resulting in substantial pain and disability. It is the most common reason for spinal surgery in patients over 65 years. Lumbar spinal stenosis is a clinical syndrome of pain in the buttocks or lower extremities, with or without back pain. It is associated with reduced space available for the neural and vascular elements of the lumbar spine. The condition is often exacerbated by standing, walking, or lumbar extension and relieved by forward flexion, sitting, or recumbency. Clinical care and research into lumbar

Spinal stenosis is common and presents in a variety of forms. Symptomatic lumbar stenosis occurs in approximately 10% of the population and cervical stenosis in 9% over age 70. Imaging is central to the management decision process and first-choice MR imaging may be substituted with CT and CT myelography. A review of the literature is presented with particular emphasis on the clinical-radiologic correlation in both neurogenic intermittent claudication and cervical spondylotic myelopathy. Advanced techniques promise improvements, particularly with radicular compressive lesions, but remain underu

Spinal lumbar stenosis is a disease that occurs mainly between the 5th and 7th decade of life and can be congenital or acquired. The latter has many etiologies, but a degenerative cause is the most common. Stenosis is a narrowing of the spinal canal diameter caused by many factors such as bulging discs, hypertrophy of flavum ligament, facet capsule thickening and osteophyte formation. The classical symptom of the disease is sciatic pain, that improves with lumbar flexion and worsens with ambulation. Neurological examination is often normal and the most useful imaging test is magnetic resonance

Lumbar spinal stenosis is a common condition seen in patients presenting to physicians who specialize in pain management or perform spine surgery. The designation of "spinal stenosis" without other qualifiers is vague and as such holds little practical value. Classifications have been created in order to more specifically describe the various etiologies as well as the site(s) of narrowing. For this purpose, diagnostic imaging studies are vital. These include myelography, computed tomography (CT), and magnetic resonance imaging (MRI). Each imaging modality has its own inherent advantages and li

Surgical treatment of lumbar spinal stenosis is aimed at decompressing the structures of the spinal canal. Several surgical techniques have been described over the last few years. This article gives a survey of the surgical procedures used for the treatment of spinal stenosis. When comparing and discussing indications and current surgical techniques used for spinal stenosis, one can describe some general tendencies: if the symptoms are severe or disabling and do not respond to appropriate conservative treatment, or if the patient is not able to cope with the pain any longer and views his or he

Neurologic compromise due to degenerative disorders of the lumbar spine are designated by the generic term "lumbar spinal stenosis". Differences in the interpretation of this term exist across the fields of pathology, radiology, and rheumatology, creating significant confusion. Rheumatologists view lumbar spinal stenosis as a functional rather than an anatomic entity and hold that its diagnosis should be based on clinical grounds. As in disk disease, imaging studies lack sensitivity and specificity, are poorly correlated with the level and severity of manifestations and are of no assistance fo

Lumbar spinal stenosis is a clinical-anatomic syndrome. Radiographic evidence of cauda equina compression is necessary but not sufficient to establish the diagnosis. Patients must have a clinical syndrome consisting of back and lower extremity discomfort exacerbated by lumbar extension or relieved by flexion, or evidence of lower extremity neurologic deficits. Symptomatic lumbar spinal stenosis may arise from a variety of specific etiologies and frequently coexists with other pain syndromes.

Spinal stenosis is a common problem in the elderly population. Thanks to better knowledge it is being diagnosed more frequently and in many cases successfully treated by surgery. Clinically, patients with a lumbogluteal and in later stages bilateral sciatic pain syndrome can be differentiated from patients with monoradicular pain. Typically, the posture deteriorates with walking distance. Spinal stenosis must be differentiated from peripheral vascular disorders and compressive disc herniation. The possibilities of conservative therapy are limited and their effect is lasting only in mild cases.

BACKGROUND: Concurrent cervical and lumbar spinal canal stenosis is known as tandem spinal stenosis (TSS). As research on TSS is limited, there is no consensus on the optimal surgical approach to this problem. We evaluated the prevalence and clinical characteristics of TSS in patients with symptomatic lumbar spinal canal stenosis (LCS).
METHODS: The authors performed a retrospective analysis of the outcomes of 565 patients who underwent lumbar surgeries performed for symptomatic LCS. In all the patients, both the cervical and lumbar regions were evaluated preoperatively, and we compared TSS p

Surgical indications for simple decompression in patients with lumbar spinal stenosis are well established. Following these guidelines, surgeons can expect good and excellent outcomes in 75% to 90% of patients. Despite the publication of many studies pertaining to the addition of arthrodesis and instrumentation, the indications for adding these procedures to a decompression are much less clear. Preoperative and intraoperative factors must be carefully considered when contemplating the addition of arthrodesis in the setting of spinal stenosis. In patients with preoperative degenerative spondylo

A 33-year-old man was referred to physical therapy by his primary care physician for low back pain that had been present for 7 months. The neurologic screen with mixed upper and lower motor neuron signs was of concern for spinal cord involvement. The primary care physician was contacted and magnetic resonance imaging ordered. Facet arthrosis and epidural lipomatosis were present, resulting in thoracic and lumbar spinal stenosis. J Orthop Sports Phys Ther 2020;50(6):345. doi:10.2519/jospt.2020.9059.

OBJECT: The authors undertook this study to document the clinical outcomes of microendoscopic laminotomy, a minimally invasive decompressive surgical technique using spinal endoscopy for lumbar decompression, in patients with lumbar spinal stenosis (LSS).
METHODS: A total of 366 patients were enrolled in the study and underwent microendoscopic laminotomy between 2007 and 2010. Indications for surgery were single- or double-level LSS, persistent neurological symptoms, and failure of conservative treatment. Microendoscopy provided wide visualization through oblique lenses and allowed bilateral

Narrow lumbar spinal canal or lumbar spinal stenosis is a not uncommon problem. Thirty-two patients with this disorder have been treated surgically at the Ochsner Medical Center between 1963 and 1973. The best method of diagnosis is a careful history and second is roentgenographic evidence, particularly the myelogram. The surgical treatment consists of total laminectomy and foraminotomy to insure complete decompression of the nerve roots. Eighteen patients have been followed from 14 to 44 months. Sixteen of the 18 considered their operations a success.

The conditions of generalized skeletal dysplasia, hypochondroplasia and achondroplasia, are reviewed. Both of these diseases may be complicated by spinal stenosis. The spinal complications are illustrated by a case history in which the basic disease was hypochondroplasia with secondary cervical spondylosis, narrow foramen magnum and clinical spastic paraplegia.
