Gait Recovery in Patients with Stroke with Severe Motor Damage: The Prognostic Role of Sensory Pathway Preservation.

BackgroundThe role of sensory pathways in gait recovery remains unclear. We hypothesized that preservation of the somatosensory pathway may play a key role in gait recovery in patients with stroke and severe motor pathway impairment.ObjectiveThis study aimed to investigate the impact of preserving the somatosensory pathway, spinothalamic tract (STT), and superior thalamic radiation (STR) on gait recovery in patients with chronic stroke and severe damage to motor pathways.MethodsThis retrospective cross-sectional study included 85 patients and investigated the association between functional amb

A case of reversible paraparesis following celiac plexus block.

BACKGROUND AND OBJECTIVES: Permanent and acute reversible paraplegia following celiac plexus block (CPB) have been reported. We report a case of prolonged reversible paraparesis after alcohol celiac plexus block. CASE REPORT: A 72-year-old man with primary multicentric pancreatic tumor and multiple hepatic metastases underwent alcohol celiac plexus neurolysis for severe abdominal pain radiating to the back. The patient had complete pain relief after the block but developed paresthesia of the left leg, which then spread to the right leg. Subsequently, loss of flexion and extension of the muscl

Efficacy of Fiber Tractography in the Stereotactic Surgery of the Thalamus for Patients with Essential Tremor.

Several targets and targeting methods are utilized in stereotactic surgery to achieve tremor suppression for patients with intractable tremor. Recent developments in magnetic resonance imaging, including diffusion tensor imaging, have enabled the setting of appropriate targets in stereotactic surgery. In this retrospective study, the optimal target to suppress tremors in stereotactic surgery was explored using diffusion tensor image-based fiber tractography. Four tracts were focused on in this study, namely: the cerebello-thalamo-premotor cortical fiber tract, cerebello-thalamo-primary motor c

[Lateral medullary infarction presenting with truncal lateropulsion and decreased pain and temperature sensation below T10 sensory level].

A 45-year-old man noted a tendency to fall to the left side on standing following headache and vomiting. Neurological findings were unremarkable except for truncal lateropulsion to the left side. Brain magnetic resonance imaging (MRI) showed a small infarct in the left inferolateral part of medulla. Clinical and imaging findings suggested vertebral artery dissection as the cause of stroke. On hospital day 6, the patient developed decreased pain and temperature sensation in the right side below T10 sensory level and decreased sweating on the left side of the face with miosis of the left pupil.

Lateral medullary infarction presenting only with dermatomal sensory disturbance: a case report.

The triad of Horner's syndrome, ataxia and sensory disturbances clinically identifies patients with lateral medullary infarct (LMI). However, dermatomal temperature and pain sensory loss have rarely been reported in patients with LMI. We describe the case of a patient with LMI presenting solely with temperature and pain sensory disorder below the C5 sensory level. Magnetic resonance imaging findings demonstrated a small superficial infarction in the right lateral medulla oblongata, indicating that the sensory deficit was caused by a restricted infarction of the lateral spinothalamic tract. We

A patient with headache, right upper extremity and right hemithorax hypothermalgesia.

We present a case of partial Wallenberg syndrome also called partial lateral medullary syndrome, a hemorrhagic or ischemic stroke of the area fed by the posterior inferior cerebellar artery and the clinical manifestation depends on the extension of the lesion: dorsal-ventral, medial-lateral and rostrocaudal. Five types have been described. Our patient had headache, hoarseness, right upper extremity, right hemithorax and right upper gluteal hypothermalgesia implicating the involvement of the cervical, the thoracic and part of lumbar fibers of the left lateral spinothalamic tract and the ambiguo

Dermatomal Sensory Manifestations in Opalski Syndrome.

A 31-year-old Japanese woman presented with sudden-onset unstable gait followed by nuchal pain. A neurological examination revealed right-sided limb weakness and decreased pain and thermal sensation on the left side below the level of the L1 dermatome. A lower lateral medullary infarction with ipsilateral hemiplegia, known as Opalski syndrome, caused by spontaneous vertebral artery dissection was diagnosed by magnetic resonance imaging. The spinothalamic tract in the medulla oblongata has a topographic arrangement of sensory fibers, and the dermatomal sensory deficit in this case can be explai

Usefulness of dorsal laser evoked potentials in patients with spinal cord damage: report of two cases.

Stimulation of the dorsal skin with brief laser impulses easily evokes brain potentials (laser evoked potentials, LEPs). Dorsal LEPs were first used to study the conduction velocity in the human spinothalamic tract. In this study the diagnostic usefulness of this technique was assessed by recording dorsal LEPs in two patients with focal spinal cord lesions (one intrinsic and the other extrinsic) and spared lemniscal sensitivities. In both cases, the brain evoked potentials were normal after stimulation of the metamers above the lesion but absent after stimulation of those below. Dorsal LEP rec

A new concept in the electrophysiological evaluation of syringomyelia.

OBJECT: The current neurophysiological assessment of syringomyelia is inadequate. Early-stage syringomyelia is anatomically predisposed to affect decussating spinothalamic fibers that convey pain and sensation primarily. Silent periods have been proven to be a sensitive tool for detecting alterations in this pathway. METHODS: Thirty-seven patients with syringomyelia were included in this prospective study. Routine electrophysiological measurements were applied including somatosensory evoked potential (SSEP) and motor evoked potential (MEP) recordings for all extremities. The silent periods we

Persistent Spinal Pain Syndrome: A Study of Contact Heat-Evoked Potentials.

INTRODUCTION: Persistent spinal pain syndrome (PSPS) type 2 is a chronic condition characterized by low back pain, with or without radiating limb pain, persisting after spinal surgery. The underlying pathophysiology remains unclear, with potential contributions from altered central pain processing mechanisms. OBJECTIVE: This study investigates the role of central sensitization in PSPS using contact heat-evoked potentials (CHEPs), an electrophysiological tool for assessing spinothalamic tract integrity and central pain processing. MATERIALS AND METHODS: The study included 36 healthy controls

[A case of central pain observed in medical medullary infarction].

A 45-year-old man had typical features of the left medial medullar syndrome which were left tongue paralysis, right hemiparesis, and right disturbance of touch and deep sensation sparing the face. He complained of hyperpathia, numbness and pain of the right body and limb sparing the face. MRI disclosed left upper medial medullary infarction, and cerebral angiography revealed occlusion of the left vertebral artery before the posterior inferior cerebellar artery. In general, the touch and deep sensation are carried through the medial lemniscus located in the medial medulla, therefore central pai

The differential synaptic organization of the spinal and lemniscal projections to the ventrobasal complex of the rat thalamus. Evidence for convergence of the two systems upon single thalamic neurons.

The synaptic organization of terminals originating either from the spinal cord (spinothalamic) or from the dorsal column nuclei (lemniscal) was investigated in the ventrobasal complex of the rat thalamus. Wheatgerm agglutinin conjugated to horseradish peroxidase was used as an anterogradely transported axonal tracer, using benzidine dihydrochloride as a chromogen for the identification by electron microscopy of spinal and lemniscal projections to the ventrobasal thalamus. A double anterograde tract tracing strategy, based labeling by wheatgerm agglutinin conjugated to horseradish peroxidase of

Transmastoid presigmoid retrolabyrinthine approach for removal of pontine cavernous malformation: how I do it.

BACKGROUND: Bleeding of brainstem cavernous malformations (BSCM) cause high morbidity and should be treated surgically whenever possible. METHOD: We present a 56-year-old man, who was diagnosed with a BSCM at right pons, which caused functional impairments of dorsal column, spinothalamic tract, cochlear nucleus, and middle cerebellar peduncle. A transmastoid presigmoid retorlabyrinthine approach via the lateral pontine zone (LPZ), with an assistance of imaging guidance and intraoperative neurophysiological monitoring, was performed to completely resect the BSCM. Cordotomy for Intractable Cancer Pain: A Narrative Review.

BACKGROUND: Cordotomy is an invasive procedure for the management of intractable pain not controlled by conventional therapies, such as analgesics or nerve block. This procedure involves mechanical disruption of nociceptive pathways in the anterolateral column, specifically the spinothalamic and spinoreticular pathways to relieve pain while preserving fine touch and proprioceptive tracts. OBJECTIVES: The purpose of this review article is to refresh our knowledge of cordotomy and support its continued use in managing intractable pain due to malignant disease. STUDY DESIGN: This is a review ar

Neurons at the origin of the medial component of the bulbopontine spinoreticular tract in the rat: an anatomical study using horseradish peroxidase retrograde transport.

An anatomical technique based on the retrograde transport of horseradish peroxidase (HRP) was used to investigate the projections of spinal cord neurons to the reticular formations in the rat. Both large and restricted injections were staggered all along the bulbar and pontine levels, involving the nucleus gigantocellularis, the nuclei reticularis pontis, pars oralis and caudalis and in some cases the nucleus raphé magnus. Labeled cells were constantly encountered in the reticular part of the neck of the dorsal horn throughout the whole length of the cord, mainly contralateral to the central c

Updated neurophysiology of itch.

The unique physiological features of histamine-sensitive C-fibers and spinothalamic tract neurons support the hypothesis of itch specific pathway, whereas subsequent studies on cowhage-induced itch have provided evidence against it, suggesting the presence of multiple neural pathways for itch. Not only peripheral pruritogens but also spinal neural receptors are involved in the control of itch, and will be the target of treatment. Itch sensitization in chronic pruritus is another crucial factor that needs to be considered in the treatment. Neuropathic itch is the type of itch that occurs when n

Ventral Column Spinal Cord Stimulation for Refractory Back and Leg Pain: A Case Series.

Spinal cord stimulation is a promising and increasingly used treatment for numerous pain etiologies. However, spinal cord stimulation is inconsistently and incompletely effective for refractory chronic back and leg pain, leaving significant room for advancement. While there has been moderate improvement over the past decade, in general, only half of patients with spinal cord stimulation experience only 50% pain relief. An inadequate response to spinal cord stimulation may be attributed, in part, to stimulation of a suboptimal anatomical target. Spinal cord stimulation is primarily applied to t

Ventral Column Spinal Cord Stimulation for Pain Management: Mechanisms, Clinical Evidence, and Future Directions.

Spinal cord stimulation (SCS) is a form of neuromodulation that delivers electrical impulses to the spinal cord, altering pain signals to provide relief from chronic pain. Since its introduction in the 1960s, SCS has primarily targeted the dorsal columns of the spinal cord. While SCS has proven effective for many neuropathic pain conditions, its limitations have driven exploration of alternative anatomical targets for electrical stimulation. Work in the 1970's suggested that ventral column spinal cord stimulation (VC-SCS) might offer superior pain relief by targeting the spinothalamic tracts (
