INTRODUCTION: In past biomechanical studies, repetitive motion of lumbar extension, rotation, or a combination of both, frequently seen in batting or pitching practice in baseball, shooting practice in soccer, and spiking practice in volleyball, have been considered important risk factors of lumbar spondylolysis. However, clinically, these have been identified in many athletes performing on a running track or on the field, which requires none of the practices described above. The purpose of this study was to verify how much impact running has on the pathologic mechanism of lumbar spondylolysis

OBJECTIVE: The aim of this study was to investigate sagittal alignment and compensatory mechanisms in patients with monosegmental spondylolysis (mono_lysis) and multisegmental spondylolysis (multi_lysis).
METHODS: A total of 453 adult patients treated for symptomatic low-grade spondylolytic spondylolisthesis were retrospectively studied at a single center. Patients were divided into 2 subgroups, the mono_lysis group and the multi_lysis group, based on the number of spondylolysis segments. A total of 158 asymptomatic healthy volunteers were enrolled in this study as the control group. Radiogra

STUDY DESIGN: Case report and literature review.
OBJECTIVES AND METHODS: We report a case of subaxial cervical spondylolysis and review 142 reports of this anomaly in the English language literature to determine the cause, the distribution, the stability, and the prognosis of cervical spondylolysis.
RESULTS: Cervical spondylolysis may affect any level of the lower cervical spine with the C6 isthmus being the most commonly affected region (48%). In 40% of cases, there were radiologic features consistent with congenital cervical spondylolysis and in 74% of cases there was a history of acute tr

STUDY DESIGN: Retrospective cohort study.
PURPOSE: To examine bone healing with conservative treatment in cases of adolescent bilateral L5 spondylolysis.
OVERVIEW OF LITERATURE: We classified bilateral L5 spondylolysis, which is the most affected spinal level, by fracture stage and aimed to compare the bone healing rate according to the fracture stage and evaluate the presence of a preexisting contralateral terminal fracture at the diagnosis of fresh spondylolysis.
METHODS: We evaluated 48 patients (38 boys and 10 girls) with bilateral L5 spondylolysis diagnosed during or before high school

Lumbar pars interarticularis (PI) injury or spondylolysis occurs only in humans. This represents a stress fracture of the PI. Excessive loading in repetitive hyperextension is a significant risk factor and occurs most commonly at L5 followed by L4. It is bilateral in 80% of symptomatic cases but can be unilateral defect as well which runs a more benign course. Symptoms of low back pain relating to this lesion are more common in young athletes involved in trunk twisting sports. Like other stress fractures, the pain may come on abruptly or more insidiously over time and only related to certain a

Spondylolysis, defined as injury to the pars interarticularis, is the most common identifiable cause of back pain in children. Historically, treatment has primarily been nonoperative, including physical therapy, activity modification, and occasionally bracing1. In instances in which the condition is refractory to nonoperative management, however, surgical treatment may be an efficacious alternative. Persistent pain following nonoperative management is described as occurring in 6% of the general population, but is reported to be as high as 15% to 47% in the pediatric population who participate

STUDY DESIGN: Retrospective non-experimental study.
OBJECTIVE: To investigate variables associated with active spondylolysis.
DESIGN: A retrospective study audited clinical data over a two year period from patients with suspected spondylolysis that were referred for a SPECT bone scan. Six exploratory variables were identified and analysed using uni- and multi-variate regression from 82 patient records to determine the association between symptomatic, physical and demographic characteristics, and the presence of an active spondylolysis.
SETTING: Tertiary level multidisciplinary private pract

SUMMARY OF BACKGROUND DATA: Symptomatic spondylolysis resulting from a stress fracture of the pars interarticularis is a cause of low back pain in the juvenile and adolescent patient. Treatment is conservative in the majority of cases.
OBJECTIVE: To analyze the outcome of patients with symptomatic isthmic spondylolysis treated nonoperatively with a custom fit thoracolumbar orthosis and activity cessation for 3 months followed by an organized physical therapy program.
STUDY DESIGN: Retrospective case series.
PATIENT SAMPLE: Four hundred thirty-six juvenile and adolescent patients with spondy

The general aetiology, diagnosis, and treatment of spondylolysis, a bone defect in the pars interarticularis of the vertebra, are reviewed. A retrospective study of young tennis players diagnosed between 2000 and 2004 with spondylolysis with or without spondylolisthesis, assessed by radiography and planar bone scintigraphy, is described. If the radiographic results were negative, computed tomography was performed. Sixty six cases were evaluated: 53 L5 lesions, eight L4 lesions, two L3 lesions, and one bilateral lesion at the L2 level. Two more lesions at two levels were found (bilateral L5 and

Summary. Spondylolysis involves disruption of the bone structure at the vertebral arch at the isthmus. Various methods are utilized in diagnosing spondylolysis, such as plain radiography, computed tomography (CT), magnetic resonance (MR), and scintigraphy. Plain radiography with anteroposterior (A-P) and lateral views is a useful method in the initial diagnostics of low back pain. Although lateral views have great value in the assessment of the progression of slippage, dynamic views are useful for the determination of vertebral stability, while oblique views seem be less precise. There is evid

A scintigraphic pattern of increased uptake was observed in the pedicles of the affected segment and a triangular pattern of uptake in the sagittal projection of tomographic studies of pars fractures. It was hypothesised that these observations were specific for pars fractures. A retrospective study of 25 young athletes (age 9-16 years) with a variety of spinal disorders was undertaken. Back pain was present for an average of 4 months. Diagnosis was confirmed by radiologic studies and/or response to treatment. All 15 children with spondylolysis had evidence of increased uptake in the ipsilater

Spondylolysis and Spondylolisthesis present with typical age and activity-related issues: in newborns the pars interarticularis is always intact. Only bipedal ambulators develop spondylolysis, mostly during early childhood. Corresponding to the mechanical etiology, the incidence of spondylolysis is higher in athletes who repeatedly have to hyperextent and rotate their lumbar spine for example gymnasts, javelin throwers etc. Spondylolysis is one of the most frequent diagnosis among adolescents with lumbar back pain. However, most of the people with an interrupted Pars interarticularis (about 6%

An unusual case of a combination of multiple bilateral spondylolyses (L2, 3 and 4), spondylolisthesis at L3/4, spondyloptosis at L4/5 and sacralization of L5 in a teenage female is described. The patient had severely increasing lower back pain radiating to the left lower limb. Radiography identified the abnormalities and myelography revealed complete obstruction and compression of the thecal sac at the L4/5 level. The case was treated surgically by posterior decompression, corpectomy and fusion in a three-stage operation. The follow-up was extended to 2 years with no complications. No similar

Lumbar spondylolysis is a common cause of low back pain in adolescent athletes. The early diagnosis and treatment of this condition will result in decreased morbidity and an earlier return to full activity for most patients. We report a case of lumbar spondylolysis in an adolescent athlete and review current diagnosis and management of this condition.

In 66 patients with back pain and suspected spondylolysis, the results of bone scintigraphy have been correlated with operative findings and clinical follow-up. Although bone scintigraphy is of little value for primary diagnosis, it helps to distinguish between those patients with established non-union of the defect, and those in whom healing is still progressing and who may benefit from immobilisation. We also found that increased uptake on the contralateral side to a unilateral spondylolysis is suggestive of impending fracture.

A technique for repairing the defect in spondylolysis by wiring is presented together with the results achieved in seven patients; the follow-up ranged from 2 to 12 years. The relative simplicity of the technique is stressed; and the technical problems likely to be encountered, together with the modifications of the basic technique necessary to cope with them, are noted. When conservative methods have been tried without success and the patient has a lesion at one or more levels and is less than 30 years old at presentation, then the technique described will give predictable results with regard
