We studied 40 previously healthy women (median age 83 years 90% central range 77-90) 1 week after surgical fixation of their proximal femoral fracture to establish the significance of leg extensor power, postural sway, age, pre-injury mobility and fracture type in recovery of walking and stair-climbing ability. Subjects demonstrated a range of walking and stair-climbing abilities. Those not mobile all had sway greater than 40.9 cm/1 min and were more likely to have suffered a trochanteric fracture (p < 0.05). These subjects were not the oldest or those with the worst pre-injury mobility, but d

Purpose: This study aimed to quantify multi-segmental coordination using Uncontrolled Manifold (UCM) analysis to examine the effect of speed reduction on the control of stair descent. Methods: Twenty healthy participants performed stair descent at a self-comfortable pace for normal speed conditions and at a slow speed set to a metronome rhythm of 60 beats/min. UCM analysis was separately conducted for the center of mass (COM) and swing foot, with anteroposterior and vertical movements designated as task variables, and segment angles defined as elemental variables. ΔV, the normalized difference

We compared the timed performance and compensatory movements of 32 boys (mean age = 10.0 years) with Duchenne muscular dystrophy (DMD) and 38 healthy boys (mean age = 9.2 years) on 10 -meter walking and four-step stair work (climbing and descending). Dependent measures were digital scores on the Functional Evaluation Scale for DMD obtained by watching the boys on film. Groups were compared initially and after 6 and 12 months, using General Linear Models analysis (for two groups and three assessments). Results showed no significant timed performance differences between groups on 10 -m walking a

We conducted a 2-year (42 weeks of consecutive training in each year, separated by 10 weeks of testing and vacation time) randomized, controlled trial of weight training in 142 healthy male and female subjects, aged 60 to 80 years. Measurements included dynamic strength, symptom-limited endurance in cycling, treadmill walking and stair climbing, muscle size, and bone mineral density and content of the lumbar spine and whole body. One hundred and thirteen subjects completed the study (57 exercise, 56 control), with a mean attendance of 85% among the exercisers. Muscle strength was unchanged in

Whole-body vibration training using single-frequency methods has been reported to improve bone mineral density. However, the intensities can exceed safe levels and have drawn unfavorable comments from subjects. In a previous article, whole-body vibration training using multiple vibration displacements at multiple vibration frequencies (MVDMVF) was reported. This article presents the computational simulation evaluation of stress dispersion on a femur with and without the MVDMVF input. A model of bone femur was developed from a computed tomography image of the lower limb with Mimics software fro

CONTEXT: Testosterone in Older Men with Mobility Limitations Trial determined the effects of testosterone on muscle performance and physical function in older men with mobility limitation. Trial's Data and Safety Monitoring Board recommended enrollment cessation due to increased frequency of adverse events in testosterone arm. The changes in muscle performance and physical function were evaluated in relation to participant's perception of change.
METHODS: Men aged 65 years and older, with mobility limitation, total testosterone 100-350 ng/dL, or free testosterone less than 50 pg/mL, were rand

Falls during stair descent have been associated with non-uniformities in stair dimensions. A common stairway non-uniformity is the top-of-flight defect, where the rise and run dimensions of the uppermost step in a stairway differ from the subsequent lower steps. This defect has been described as hazardous, but its effect on foot placement and misstep risk in stair descent has not been studied. In this experimental study, we quantified foot overhang for adults (n = 27) descending a seven-step staircase with a top-of-flight defect compared to descending a uniform staircase. When the defect was p

BACKGROUND: In negotiating stairs, low foot clearance increases the risk of tripping and a fall. Foot clearance may be related to physical fitness, which differs between active and sedentary participants, and be acutely affected by exercise. Impaired stair negotiation could be an acute response to exercise. Here we determined acute changes in foot clearances during stair walking in sedentary (n = 15) and physically active older adults (n = 15) after prolonged exercise.
METHODS: Kinematic data were acquired during negotiation with a 3-steps staircase while participants walked at preferred spee

In cementless total hip replacement surgery the conditions for micromotion initiation at the bone-stem interface and the role of stair climbing versus gait in promoting incipient slipping deserve attention. The goal of the present paper was to propose a finite element approach for analysing the structural behaviour of hip joint prostheses under physiological loadings and boundary conditions, which allows the prediction of micromotion initiation with low computational effort. In this paper, three-dimensional (3D) finite element analyses were performed of intact and implanted human femurs in ord

Knee osteoarthritis (OA) accounts for more functional disability of the lower extremity than any other disease. We recruited 18 patients with knee OA and 18 healthy age-, height-, mass-, and gender-matched control subjects to investigate the effects knee OA has on select spatial and temporal gait variables during a stair climbing task. No group-by-direction interaction was observed; however, significant effects did occur for group and direction. Specifically, patients with knee OA demonstrated less time in single support, greater time in double support, decreased step length, greater step widt

Injuries commonly occur on stairs, with high injury rates in young adults, especially young women. High injury rates could result from physiological and/or behavioral differences; this study focuses on behaviors. The purposes of this observational study were (1) to quantify young adult behaviors during stair descent and (2) to identify differences in stair descent behavior for young adult men versus women. Young adult pedestrians (N = 2,400, 1,470 men and 930 women) were videotaped during descent of two indoor campus staircases, a short staircase (2 steps) and a long staircase (17 steps). Beha

Mucopolysaccharidosis VI (Maroteaux-Lamy syndrome) is a very rare inherited lysosomal storage disease. We evaluated the efficacy and safety of weekly infusions of recombinant human arylsulfatase B as enzyme replacement therapy for two patients in whom this condition was advanced. The primary outcome variables were the distance walked in a 6-minute walk test, forced vital capacity, and ejection fraction. The secondary outcome variables were the number of stairs climbed in a 3-minute stair climbing test, joint mobility, urinary glycosaminoglycan excretion, auto-continuous positive airway pressur

Excessive muscle co-contraction is one of the factors related to the progression of knee osteoarthritis (OA). A previous study demonstrated that pain, joint instability, lateral thrust, weight, and lower extremity alignment were listed as factors affecting excessive co-contraction in knee OA. However, this study aimed to assess the association between fear-avoidance beliefs and muscle co-contraction during gait and stair climbing in people with knee OA. Twenty-four participants with knee OA participated in this cross-sectional study. Co-contraction ratios (CCRs) were used to calculate muscle c

Prosthetic knees available to individuals with transfemoral amputation seek to restore functional ability to the user. Passive prosthetic knees are lightweight but can restore only limited, dissipative ambulation activities whereas active knees can provide energy to restore additional ambulation activities such as stair climbing and standing up from a chair. Semi-active prosthetic devices aim to only power a subset of activities and use passive components and control when that power is not necessary. Here, we outline an ambulation control system for a lightweight Hybrid Knee prosthesis that is
