Osteomyelitis of the sternoclavicular joint is a rare yet challenging problem. It is commonly treated by resection and a muscle flap. We report a case that was treated with single-stage resection of both the sternoclavicular joint and part of the sternum followed by implantation of calcium sulfate beads impregnated with antibiotics and primary closure without a muscle flap. A single-stage debridement with resection for osteomyelitis of the sternoclavicular joint and primary closure with calcium sulfate beads impregnated with antibiotics may be an option when dealing with this difficult clinica

Eight middle-aged women with spontaneous atraumatic subluxation of the sternoclavicular joint were evaluated with radiography and computed tomography. All patients were employed in occupations involving moderate to heavy physical labour, and no patients could recall a specific traumatic incident associated with onset of symptoms. In seven of the eight patients, the displacement of the medial clavicle was in a cranial direction; in four of the eight patients, there was an associated anterior subluxation, and in one patient, the subluxation was purely anterior. All five patients with an anterior

Septic arthritis of the sternoclavicular joint (SCJ) is a rare disorder, and is usually associated with predisposing factors such as contiguous foci of infection, heroin addiction, rheumatoid arthritis and diabetes mellitus. Three cases in previously healthy adults are reported here. The aetiology, clinical manifestations and treatment are briefly reviewed. The considerable difficulty in diagnosing this disorder in adults is emphasized. In summary, diagnosis of septic arthritis of the SCJ in adults requires a high index of suspicion, and must be considered not only in patients with predisposin

Posterior sternoclavicular dislocation accompanied by clavicular end fracture is an extremely rare injury according to the available literature. We present a case of a young male with this injury, resulting from a vehicle accident, and additionally complicated by a clavicular bone fragment in the proximity of the aortic arch. An initial X-ray showed no signs of bone trauma, which was, however, revealed on a CT scan. Stable fixation of the fragment and sternoclavicular joint was achieved utilizing a plate and screws. The osteosynthetic material was removed after eight weeks due to the plate bre

Sternoclavicular joint (SCJ) injury represents 3%-5% of all shoulder girdle injuries, yet can produce significant pain and disability. While conservative management improves symptoms in most cases, surgical intervention may be indicated for patients with symptoms recalcitrant to nonoperative treatment. A wide range of surgical stabilisation techniques is documented in the literature; however, the scarcity of SCJ pathology has hindered development of a 'gold standard'.We present a minimalistic medial clavicle osteoplasty and SCJ reconstruction using semitendinosus autograft anchored with unicor

Although rare, ectopic salivary gland fistula should be considered in the differential diagnosis when treating a fistulous lesion on the neck because its natural history and the level of difficulty of surgery are different from those of the branchial fistula.

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2017eng
PURPOSE: To describe the results and functional scores in a group of patients who underwent arthroscopic excision of a symptomatic sternoclavicular joint (SCJ) disk tear with a minimum follow-up period of 2 years.
METHODS: Between April 2010 and December 2014, 14 patients underwent arthroscopic excision of a torn SCJ disk. Patients whose intended surgery was an isolated diskectomy and underwent that surgery only, with no additional procedure, were included. The minimum follow-up period was 24 months. All patients underwent an arthroscopic SCJ diskectomy. Postoperatively, no immobilization was

BACKGROUND: Surgical stabilization of the sternoclavicular joint (SCJ) is infrequent, and cardiothoracic surgery assistance is often recommended. Patient safety and surgeon efficiency may be improved by greater understanding of the anatomic relationships near the SCJ. The purpose of this study is to determine the distances from the SCJ to critical structures in the superior mediastinum.
MATERIALS AND METHODS: Distances from the posterior SCJ to adjacent mediastinal structures were recorded using contrast computed tomography scans of 49 consecutive patients. Patient sex, height, body mass inde

This case report details the diagnosis and treatment of a previously unreported complication of a congenital chest wall anomaly. Our patient presented with a painful anterior sternoclavicular joint subluxation secondary to a bicipital rib. Thoracic magnetic resonance and computed tomographic imaging provided the diagnosis. Complete resolution of symptoms was achieved after resection of the bicipital rib.

Ten patients with unilateral spontaneous swelling of the sternoclavicular joint are presented. Plain radiographic examination of the joints revealed no abnormalities. Technetium scintigraphy was positive. After surgical biopsy the swelling and the symptoms disappeared. Patho-anatomical examination of synovialis from these joints revealed serous synovitis as the only pathological finding. Seven other patients with clinical symptoms undistinguishable from the above-mentioned and with normal plain radiographic findings showed, patho-anatomically: osteomyelitis (2 cases), ganglion (2 cases), osteo

The posterior sternoclavicular joint dislocation is a rare and potentially life-threatening injury, as massive haemorrhage can occur at the time of trauma, during reduction manoeuvres and drilling. These injuries are rare and a collective experience of managing them is of paramount importance. We present our multidisciplinary experience of managing several of these injuries in our centre, with learning points we have identified. Assessment should include Computerised Tomography Angiography (CTA) to assess the anatomy of the joint including the proximity to the underlying innominate vein and to

A 34-year-old man was admitted for surgical treatment following traumatic posterior sternoclavicular joint dislocation and anterior mediastinum hematoma with compression of brachiocephalic vein (A, B). Intra-operative positioning consid- ered access for possible sternotomy and control of upper vena cava territory in case of bleeding from venous confluent (C). Patient was submitted to open reduction of sternoclavicular joint through an oblique supraclavicular incision (D), fixation with high strength orthopedic suture (E), mediastinal drainage and closure with absorbable suture (F).

CASE: We report a rare variant of sternoclavicular joint (SCJ) dislocation, namely locked anterior-inferior dislocation, with unique clinical, radiographic, and intraoperative findings. In this variant, the medial clavicle was displaced anteriorly and inferiorly and locked in the manubrial-intercostal space, with corresponding mechanical dysfunction of the ipsilateral shoulder girdle joints. Symptoms unique to this variant included painful neck spasms and limited glenohumeral elevation. Nonsurgical treatment was not successful, and open reduction and ligament reconstruction resulted in correct

Invasive infections due to Streptococcus dysgalactiae are uncommon in the adult population, and sternoclavicular joint septic arthritis (SCSA) is usually caused by other organisms such as Staphylococcus aureus and Pseudomonas aeruginosa. We hereby report a case of SCSA caused by this organism. The patient responded well to intravenous antibiotics and recovered fully without any surgical intervention.
