Strabismus management in retinoblastoma survivors.

PURPOSE: To report the result of strabismus surgery in eye-salvaged retinoblastoma (Rb) patients. METHODS: A retrospective case series including 18 patients with Rb and strabismus who underwent strabismus surgery after completing tumor treatment by a single pediatric ophthalmologist. RESULTS: A total of 18 patients (10 females and 8 males) were included with a mean age of 13.3 ± 3.0 (range, 2-39) months at the time tumor presentation and 6.0 ± 1.5 (range, 4-9) years at the time of strabismus surgery. Ten (56%) patients had unilateral and 8(44%) had bilateral involvement and the most common w

Sensory Strabismus; A Literature Review.

Persistent unilateral or bilateral visual deprivation at any age, particularly in children, can compromise sensory fusion and result in a type of strabismus known as sensory or secondary strabismus. There are several pathologies that can induce visual impairment, such as severe anisometropia, congenital unilateral cataract, corneal opacity, retinal diseases, and optic nerve anomalies. Sensory strabismus may be horizontal or vertical or a combination of them; however, most reports indicate the development of horizontal deviation as sensory strabismus. Regardless of the direction of the sensory

Restrictive strabismus: diagnosis and management.

INTRODUCTION: Restrictive strabismus is a type of ocular misalignment with limitation of motility caused by intrinsic or extrinsic mechanical forces. The clinical spectrum of either purely or partially restrictive strabismus is very broad. Most cases are of congenital, traumatic, endocrine, post-paralytic or myopathic origin. The surgical treatment strategies are designed to correct abnormal head posture, to eliminate diplopia in primary and functional positions of gaze and to enhance aesthetic and psychosocial aspects of a patient's life. PURPOSE: The objective of this paper is to present a

Severe complications of strabismus surgery.

PURPOSE: To determine the type, incidence, and clinical outcomes of severe complications from strabismus surgery in the United Kingdom. METHODS: Cases were identified prospectively through a national surveillance unit between September 1, 2008, and August 31, 2010. Questionnaire data were requested at the time of the complication recognition and at 6 months' follow-up. Outcome was graded I to V, with a poor or very poor outcome meaning either loss of corrected visual acuity or primary position double vision. RESULTS: A total of 60 completed reports of adverse events and complications were re

Perinatal risk factors for strabismus.

BACKGROUND: Little is known about the aetiological factors underlying strabismus. We undertook a large cohort study to investigate perinatal risk factors for strabismus, overall and by subtype. METHODS: Orthoptists reviewed ophthalmological records for Danish National Birth Cohort (DNBC) children examined for strabismus in hospital ophthalmology departments or by ophthalmologists in private practice. Information on perinatal characteristics was obtained from national registers. We used log-linear binomial regression and polytomous logistic regression to estimate risk ratios for strabismus ove

Strabismus in pediatric lens disorders.

PURPOSE: To evaluate the prevalence and types of strabismus in children with cataract and ectopia lentis. METHODS: The authors retrospectively evaluated 860 children treated in the ophthalmological department of Children's Hospital "Agia Sophia" during the past decade. Their medical files with reference to strabismus prevalence, correlation between the time of strabismus occurrence and lens extraction surgery, and type of strabismus were studied. RESULTS: Of the 822 children with cataract, 337 had congenital cataract and 485 had acquired cataract. Strabismus occurred in 199 (24.2%) patients;

Strabismus complications from local anesthetics.

Strabismus developing after retrobulbar or peribulbar anesthesia for both anterior and posterior segment eye surgery may be due to myotoxicity to an extraocular muscle from the local anesthetic agent. Initial paresis often causes diplopia immediately after surgery, but later progressive segmental fibrosis occurs, and/or hypertrophy of the muscle, producing diplopia in the opposite direction from the direction of the initial diplopia. The inferior rectus muscle is most commonly affected. Usually a large recession on an adjustable suture of the involved muscle(s) yields good alignment. Using top

Genetic basis of congenital strabismus.

Strabismus is misalignment of one eye in relation to the other, resulting in failure of the 2 eyes to simultaneously focus on the same image and loss of binocular vision. Strabismus affects 2% to 4% of the population and can result in amblyopia, which is often not discovered in time to initiate effective treatment. Thus, an understanding of the genetic underpinnings of strabismus may help identify patients at risk early enough to prevent disability and may lead to new preventive or therapeutic approaches.

Strabismus and eye muscle function.

Studies of external eye muscle morphology and physiology are reviewed, with respect to both motor and sensory functions in concomitant strabismus. The eye muscles have a more complex fibre composition than other striated muscle, and they are among the fastest and most fatigue-resistant muscles in the body. However, it is not generally believed that concomitant strabismus is due to a primary abnormality of the eye muscles or the ocular motor system. The gross anatomy of eye muscles, including the shape and position of the eye muscle pulleys, was not changed in strabismus. The histology of the e

Strabismus after endoscopic third ventriculostomy.

Until recently, the treatment of choice for noncommunicating hydrocephalus included the use of valve-regulated shunts such as the ventriculoperitoneal shunt. High failure rates and multiple complications have led neurosurgeons to use other techniques. Endoscopic third ventriculostomy (ETV) is becoming popular as treatment for noncommunicating hydrocephalus when standard shunts have failed.(1) We present 2 adolescents who developed exotropia, hypertropia, and diplopia after endoscopic third ventricle surgery.

[Binocular vision and vertical strabismus].

BACKGROUND/AIM: Elevation in adduction is the most common pattern of vertical strabismus, and it is mostly treated with surgery. The results of weaking of inferior oblique muscle are very changeable. The aim of this study was to evaluate binocular vision using sensory tests before and one and six months after the surgery. METHODS: A total of 79 children were divided in two groups: the first, with inferior oblique muscle of overaction (n = 52), and the second with dissociated vertical deviation (DVD), and primary inferior oblique muscle overaction (n = 27). We tested them by polaroid mirror te

Ocular dominance columns in strabismus.

During development, the projection from the lateral geniculate nucleus to striate cortex becomes segregated into monocular regions called ocular dominance columns. Prior studies in cats have suggested that experimental strabismus or alternating monocular occlusion increases the width and segregation of columns. In the squirrel monkey, strabismus has been reported to induce the formation of ocular dominance columns. However, these studies are difficult to interpret because no animal can serve as its own control and the degree of inter-individual variability among normal subjects is considerable

Combined strabismus and lens surgery.

BACKGROUND: Simultaneous eye muscle and lens surgery in patients with strabismus and lens abnormalities offers the advantage of avoiding staged surgery. METHODS: Thirty-three combined strabismus and lens surgeries were performed on 30 patients who ranged in age from 22 months to 91 years. Fifteen of the strabismus procedures were performed for esotropia, 12 for exotropia, 4 for vertical deviations, and 2 for combined vertical-horizontal deviations. Surgical amounts often were reduced to lessen the risk of overcorrection, to minimize anesthetic requirements (when using topical rather than gene

Acquired strabismus following cosmetic blepharoplasty.

The purpose of this study was to report on 12 patients with acquired strabismus following cosmetic blepharoplasty and to identify patterns of strabismus related to the surgical procedure. Clinical ophthalmologic examinations were performed to specifically clarify the type of strabismus. Operative reports of the blepharoplasty procedures were reviewed. Patients were followed for a minimum of 3 months after the blepharoplasty procedure before surgical intervention was considered. Operative findings at corrective strabismus surgery were noted and an attempt was made to correlate these findings wi

Vertical strabismus in monofixation syndrome.

PURPOSE: The manifest angle of the horizontal deviation in monofixation syndrome (MFS) has been reported to reach a maximum of 8 to 10 PD. Review of the literature, however, revealed no studies about the vertical deviation associated with MFS. The purpose of this study is to evaluate the range of the angle of the vertical deviation in MFS. METHODS: Forty patients who had MFS with vertical strabismus in the distance primary position were included in this study. All fulfilled the criteria for monofixation, fusing Worth 4 dots at near but suppressing at distance and/or having stereoacuity betwee

PURPOSE: In this prospective cross-sectional observational study, the distribution of organic pathologies in patients initially presenting with strabismus was evaluated. METHODS: Thirty-one of 243 patients examined between May 1997 and May 1998 had strabismus due to organic causes and 28 patients had posterior segment abnormalities. RESULTS: Toxoplasma chorioretinitis, morning glory anomaly, toxocara retinopathy, retinopathy of prematurity and Coats' disease were the most common diagnoses. Eighteen patients (58%) had esotropia and 13 (42%) had exotropia. The mean age of onset of deviation wa

Scar remodeling after strabismus surgery.

PURPOSE: Patients with overcorrected strabismus (and several patients with undercorrection after extraocular muscle resection) underwent exploration of previously operated muscles, with the intention of advancing their tendons to prevent the need for surgery on additional muscles. Unexpectedly, it was found that, in many cases, an elongated scar segment of variable length was interposed between the muscle and its insertion site on the sclera. Laboratory investigations were carried out to elucidate the underlying mechanism(s) and to create an animal model of the disorder. METHODS: Lengthened s
