Correlation of Impedance Cardiography-Derived and Cardiac Magnetic Resonance-Derived Stroke Volumes.

Cardiac output (CO) and other hemodynamic parameter measurements play an important role in the management of cardiovascular conditions; however, due to limitations of current day technologies, such measurements are either not routinely performed or incorporated into clinical practice. Moreover, measurement of these hemodynamic parameters in the outpatient setting at different time points to assess interval change is currently not feasible. We attempted to validate total-body impedance cardiography-based Non-Invasive Cardiac System (NICaS) derived stroke volume (SV) with that from cardiac magne

Infarct hemisphere and noninfarcted brain volumes affect locomotor performance following stroke.

OBJECTIVE: Brain damage within the right middle cerebral artery (MCA) territory is particularly disruptive to mediolateral postural stabilization. The objective of this cross-sectional study was to test the hypothesis that chronic right MCA infarcts (as compared to left) are associated with slower and more bilaterally asymmetrical gait. We further hypothesized that in those with chronic right MCA infarct, locomotor performance is more dependent on gray matter (GM) volumes within noninfarcted regions of the brain that are involved in motor control yet lie outside of the MCA territory. METHODS:

[Noninvasive determination of stroke volume with impulse Doppler echocardiography: comparison with the Fick method].

Pulsed Doppler echocardiographic studies were performed in 14 patients (eleven with mitral valve disease, two with coronary artery disease, one with aortic and mitral valve replacement) for determination of cardiac output and the results compared with those obtained from simultaneous measurements carried out according to the Fick principle. Determination of cardiac output and stroke volume was achieved with a pulsed Doppler instrument specifically designed in our laboratory (repetition frequency 10 kHz, maximal penetrance 7.7 cm, ultrasonic beam diameter 3 cm at a distance of 5 cm from the tra

Effects of weather conditions on endovascular treatment case volume for patients with ischemic stroke.

BACKGROUND: Weather conditions have been shown to influence the occurrence of cardiovascular events. We tested the hypothesis that weather parameters may be associated with variations of case volume of endovascular treatment (EVT) for acute ischemic stroke. METHODS: Individual data from the ETIS (Endovascular Treatment in Ischemic Stroke) French national registry were matched to local weather stations. Meteorological parameters (rainfall, humidity, atmospheric pressure, air temperature) were gathered from national online resources. Weather readings and EVT case volumes were annually standardi

Relationship of stroke volume to different patterns of Cheyne-Stokes respiration in heart failure.

STUDY OBJECTIVES: In patients with heart failure (HF) and reduced left ventricular ejection fraction (HFrEF), stroke volume (SV) falls during hyperpnea of Cheyne-Stokes respiration with central sleep apnea (CSR-CSA). We have identified two distinct patterns of hyperpnea: positive, in which end-expiratory lung volume (EELV) remains at or above functional residual capacity (FRC), and negative, in which EELV falls below FRC. The increase in expiratory intrathoracic pressure generated by the latter should have effects on the heart analogous to external chest compression. To test the hypotheses tha

Ferumoxytol administration does not alter infarct volume or the inflammatory response to stroke in mice.

Ferumoxytol is an ultrasmall superparamagnetic iron oxide (USPIO) nanoparticle that is FDA-approved as an intravenous iron replacement therapy for the treatment of iron deficiency anemia in patients with chronic kidney disease. Ferumoxytol has also been used as a contrast agent for cerebral blood volume mapping by magnetic resonance imaging (MRI), which suggests it could be used for imaging hemodynamic abnormalities after stroke. However, circulating macrophages can internalize USPIOs, and recent data indicate that the accumulation of iron in macrophages can lead them to adopt the M1 pro-infla

Stroke volume variation as a predictor of fluid responsiveness in patients undergoing brain surgery.

UNLABELLED: Changes in arterial blood pressure induced by mechanical ventilation allow assessment of cardiac preload. In this study, stroke volume variation (SVV), which is the percentage change between the maximal and minimal stroke volumes (SV) divided by the average of the minimum and maximum over a floating period of 30 s, continuously displayed by the PiCCO continuous cardiac output monitor, was evaluated as a predictor of fluid responsiveness. Fifteen patients undergoing brain surgery were included. During surgery, graded volume loading was performed with each volume loading step (VLS) c

Reduced Stroke Volume and Brain Perfusion Drive Postural Hyperventilation in Postural Orthostatic Tachycardia Syndrome.

Postural hyperventilation has been implicated as a cause of postural orthostatic tachycardia syndrome (POTS), yet the precise mechanisms underlying the heightened breathing response remain unclear. This study challenges current hypotheses by revealing that exaggerated peripheral chemoreceptor activity is not the primary driver of postural hyperventilation. Instead, significant contributions from reduced stroke volume and compromised brain perfusion during orthostatic stress were identified. These findings shed light on our understanding of POTS pathophysiology, emphasizing the critical roles o

Trimethylamine N-Oxide and White Matter Hyperintensity Volume Among Patients With Acute Ischemic Stroke.

IMPORTANCE: Although increasing evidence suggests that trimethylamine N-oxide (TMAO) is associated with atherosclerosis, little is known about whether TMAO and its related metabolites (ie, choline, betaine, and carnitine) are associated with small vessel disease. OBJECTIVE: To evaluate the association between TMAO and its related metabolites with features of cerebral small vessel disease, including white matter hyperintensity volume (WMHV) and acute lacunar infarction. Rightward imbalanced pulmonary perfusion predicts better exercise stroke volume in children after Fallot repair.

BACKGROUND: Residual lesions following Fallot repair are primarily pulmonary regurgitation and right ventricular outflow tract obstruction. These lesions may impact exercise tolerance, particularly because of a poor increase in left ventricular stroke volume. Pulmonary perfusion imbalance is also common, but its effect on cardiac adaptation to exercise is not known. AIM: To assess the association between pulmonary perfusion asymmetry and peak indexed exercise stroke volume (pSVi) in young patients. METHODS: We retrospectively studied 82 consecutive patients with Fallot repair (mean age 15.2±

Obstructive Sleep Apnea before Ischemic Stroke: Clinical Relevance to Infarction Volume and Neurological Recovery.

BACKGROUND: Obstructive sleep apnea (OSA) is a probable risk factor with speculative roles in the induction or aggravation of acute ischemic stroke (AIS). METHODS: The association between OSA and AIS severity was retrospectively analyzed using clinical data of first-onset AIS patients, admitted to our hospital between January 2013 and September 2016. Eligible patients were categorized based on the presence of OSA prior to stroke. Stroke severity and functional outcomes were evaluated using the National Institute of Health Stroke Severity Scale (NIHSS) and the modified Rankin scale (mRS), resp

Left Atrial Volume Index Predicts Recurrence of Stroke in Patients with Nonsustained Atrial Tachycardia.

BACKGROUND: Nonsustained atrial tachycardia (NSAT) is known to appear more frequently in patients with paroxysmal atrial fibrillation (AF). Enlarged left atrium (LA) is considered to be an independent risk factor for newly diagnosed AF. METHODS: We investigated the risk factors for predicting the stroke recurrence in NSAT patients. In total, 252 patients (114 women, mean 70 ± 11 years) with acute ischemic stroke and documented NSAT in 24-hour Holter monitoring were enrolled and followed. All patients underwent echo-Doppler evaluations. RESULTS: During a mean follow-up period of 35 ± 31 month

Left atrial appendage volume increased in more than half of patients with cryptogenic stroke.

BACKGROUND: Ischemic strokes without a well-defined etiology are labeled as cryptogenic, and account for 30-40% of strokes in stroke registries. The left atrial appendage (LAA) is the most typical origin for intracardiac thrombus formation when associated with atrial fibrillation. Here, we examined whether increased LAA volume detected with cardiac computed tomography (cCT) constitutes a risk factor in cryptogenic stroke patients. METHODS: This study included 82 stroke/TIA patients (57 males; mean age, 58 years) with a diagnosis of cryptogenic stroke after extensive radiological and cardiolog

Optimising stroke volume and oxygen delivery in abdominal aortic surgery: a randomised controlled trial.

BACKGROUND: Post-operative complications after open elective abdominal aortic surgery are common, and individualised goal-directed therapy may improve outcome in high-risk surgery. We hypothesised that individualised goal-directed therapy, targeting stroke volume and oxygen delivery, can reduce complications and minimise length of stay in intensive care unit and hospital following open elective abdominal aortic surgery. METHODS: Seventy patients scheduled for open elective abdominal aortic surgery were randomised to individualised goal-directed therapy or conventional therapy. In the interven

Changes in aqueductal CSF stroke volume in shunted patients with idiopathic normal-pressure hydrocephalus.

BACKGROUND AND PURPOSE: Aqueductal CSF stroke volume (ACSV) measured by phase-contrast MR imaging is a tool for selection of surgical patients with idiopathic normal-pressure hydrocephalus (iNPH). The aim of the present study was to investigate whether there is a relationship between clinical outcome and changes in ACSV in patients with iNPH who have been shunted. MATERIALS AND METHODS: Sixty-five shunted patients with iNPH underwent clinical evaluation and ACSV measurements 7-30 days before and 1, 3, 6, and 12 months after surgery. RESULTS: Two patients were excluded from the study for the

Assessing fluid responsiveness by stroke volume variation in mechanically ventilated patients with severe sepsis.

BACKGROUND AND OBJECTIVE: Our hypothesis was that stroke volume variation during mechanical ventilation of the lungs would allow accurate prediction and monitoring of changes in cardiac index in response to fluid loading in patients with severe sepsis. METHODS: This was a prospective clinical study in a university hospital. Ten mechanically ventilated patients with severe sepsis or septic shock were given fluid loading with 500 mL 10% hydroxyethylstarch 200/0.5 over 30 min. Before and after fluid loading pulmonary arterial occlusion pressure and central venous pressure were measured. Intratho

Blood pressure threshold confines stroke volume responsiveness to passive leg raising in healthy individuals.

Passive leg raising (PLR) is a common maneuver to assess fluid responsiveness in the emergency department. The effect of blood pressure (BP) on PLR-induced left ventricular (LV) stroke volume (SV) response was examined in normotensive (CON, n = 100), hypotensive (HOTN, n = 51) and hypertensive (HTN, n = 16) individuals. Limb BP and Echo measurements were completed within 1 min when the participants were transitioned from the semi-recumbent position to bilateral leg elevation at 45° and lower limb BP was reduced at PLR in all three groups. LV end-diastolic diameter (LVDD), LV end-systolic diame

Doppler echocardiography: The noninvasive method for accurately estimating stroke volume in ischaemic cardiogenic shock.

Accurate stroke volume (SV) assessment is crucial in ischaemic cardiogenic shock. While pulmonary artery catheterisation remains the gold standard, its invasive nature necessitates reliable noninvasive alternatives. However, the literature on echocardiographic SV accuracy is inconsistent. This study evaluated commonly used echocardiographic techniques-Doppler-derived and Simpson's method-against invasive thermodilution in 39 patients. SV by Doppler showed strong correlation (r = 0.91, p < 0.0001) and minimal bias, whereas the Simpson's method exhibited weaker correlation and more significant u
