OBJECTIVE: To examine head growth in macrocephalic infants with benign expansion of subarachnoid spaces (BESS) relative to those without imaging abnormalities and World Health Organization (WHO) standard curves.
STUDY DESIGN: Patients with macrocephaly who underwent intracranial imaging at St. Louis Children's Hospital between 2012 and 2022 were identified via radiology records search. Records were retrospectively reviewed to identify patients with isolated macrocephaly (without intracranial abnormalities) and BESS (defined by macrocephaly and concurrent imaging diagnosis of BESS by neuroradi

The function of cerebrospinal fluid (CSF) is to protect the brain and optic nerve from mechanical damage, provide nutrition for axons/neurons, and remove of toxic metabilites. CSF is produced mainly by the choroid plexus epithelium and ependymal cells of the ventricles and flows into interconnecting chambers; namely, the cisterns and the subarachnoid spaces. Based on studies of CSF circulation and direction of flow using radioisotopes and other tracers injected into the CSF, it is thought that there is a bulk circulation of fluid from the sites of production in the third, fourth, and lateral v

A 70-year-old woman was examined because of increasing problems with cognition. She had a history of a cerebral shunt placed surgically 10 years previously. Introduction of Tc-99m DTPA directly into the ventricular cavity revealed good ventricular distribution, followed by progression downward, as though into a previous ventriculoperitoneal shunt. However, a chest radiograph revealed what appeared to be a shunt tube in the right atrium. Delayed lateral images showed activity in proximity to the vertebral column, indicating migration of tracer and cerebrospinal fluid into the dorsal and lumbar

To study the immune response within the subarachnoid space in patients with neurocysticercosis, we measured the cerebrospinal fluid contents of immunoglobulins A, E, G, and M in 38 patients and the contents of the proinflammatory cytokines TNF-alpha, IL-1b, IL-6 and IFN-gamma in 17 patients. The same measurements were made in 30 neurological patients without inflammatory or immune-mediated disorders. Each immunoglobulin and cytokine, including the gender and age of the patient, was compared by multiple regression analysis with the CSF contents of cells, protein and ELISA for cysticercal antige

Subarachnoid haemorrhage (SAH) often leads to subarachnoid fibrosis and resultant normal pressure hydrocephalus; however, how subarachnoid fibrosis occurs is unknown. We examined the changes within arachnoid granulations (AGs) and the subarachnoid space (SAS) chronologically at the parasagittal region obtained from patients with SAH at autopsy and made comparison with controls by immunostaining for cytokeratin, specific marker for leptomeningeal cells and by the elastica Masson-Goldner methods. Within a week some AGs were torn, and many inflammatory cells filled the AGs and SAS. Cytokeratin po

Background The prevalence of incomplete disproportionately enlarged subarachnoid space hydrocephalus (DESH) and its cognitive relevance remain unclear. Purpose To determine the prevalence of complete and incomplete DESH and their associations with baseline cognitive impairment and follow-up cognitive decline. Materials and Methods This prospective community-based cohort (age ≥ 65 years) included participants who underwent baseline brain MRI and were followed for 5-7 years. Two radiologists assessed baseline MRI scans for core DESH features: Evans index greater than 0.3, enlargement of the Sylv

A patient with gait instability underwent I-123-ioflupane SPECT/CT imaging for suspected Parkinson's disease. DaTQUANT analysis was negative for Parkinson's disease; however, evaluation of the SPECT images revealed low counts within the bilateral parietal region. Further review of the CT images demonstrated the classic appearance of disproportionately enlarged subarachnoid space hydrocephalus (DESH), a supportive imaging feature of idiopathic normal pressure hydrocephalus (iNPH), highlighting the importance of correlation of cross-sectional CT and DaT SPECT imaging.

Although the deficiency forms are the most common manifestations of rickets, there are other forms of rickets that are resistant to vitamin D. Of these, the most common is X-linked hypophosphatemic rickets. Rickets represents a group of multiple cranial bone disorders-craniosynostosis and the presence of Chari I malformation being the most notable-that explain the increase in intracranial pressure. We present a 4-year-old patient with an unusual association of X-linked hypophosphataemic rickets, bilateral proptosis, and prominent bilateral widening of the optic nerve sheaths. Although the asso

A patient with traumatic brain injury showed incomplete oculomotor nerve palsy in the subarachnoid space. A 12-year-old girl was hospitalized after a head injury. Neuro-ophthalmic examination showed that the left eye had a ptosis and pupillary involvement. An MRI indicated an intracranial hematoma at the basilar portion of the left temple. The ptosis and pupillary involvement improved after elimination of the hematoma. The presentation patterns are best explained by topographic organization of the third nerve fiber within the subarachnoid space. This case suggests that the topographic organiza

BACKGROUND: Intradural parenchymal involvement (IPI) in the spinal subarachnoid space associated with primary lung cancer is rare. A retrospective study was undertaken to investigate the clinical and pathologic features of IPI.
METHOD: A total of 1215 cases of primary lung cancer were studied at autopsy; the results were reviewed retrospectively.
RESULTS: Twenty (1.65%) of the cases revealed IPI in the spinal subarachnoid space. The histologic diagnoses were small cell carcinoma in ten cases, adenocarcinoma in eight cases, and squamous cell carcinoma in two cases. In 14 (70%) cases, the IPI

BACKGROUND AND OBJECTIVE: Several studies have shown that idiopathic normal-pressure hydrocephalus (iNPH) can mimic other neurodegenerative disorders, particularly progressive supranuclear palsy (PSP). In this study, we investigated iNPH clinical and neuroimaging features in patients with autopsy-confirmed PSP or Lewy body disease (LBD) by assessing the normal pressure hydrocephalus (NPH) triad of symptoms and imaging features of disproportionately enlarged subarachnoid-space hydrocephalus (DESH) and Evans index (EI) on antemortem MRI scans.
METHODS: Among our study participants (N = 190), th

We report on an 80-year-old woman who had shunt-responsive idiopathic normal pressure hydrocephalus (iNPH). Her brain MRI showed an initial change in the subarachnoid space (a disproportional narrowing of the subarachnoid space at the cerebral high convexities) at the age of 70 years, followed by enlargement of the ventricles (Evans index: 0.31) at the age of 78. The observation suggests that changes in the subarachnoid space may precede ventricular enlargement in iNPH.

OBJECTIVE: Benign Enlargement of the Subarachnoid Spaces in Infancy (BESSI) is a common finding during workup for progressive macrocephaly. BESSI has been associated with slightly higher prevalence of subdural (SD) spaces and a risk for developing subdural hematoma. This study utilizes fast brain magnetic resonance imaging (MRI) to investigate the prevalence of visible SD spaces in BESSI.
METHODS: A retrospective review was performed for all pediatric patients who underwent brain MRI for macrocephaly. Patients with a diagnosis of BESSI were included in the study. A total of 109 patients met t

BACKGROUND: Traumatic injury to the spinal cord disrupts cerebrospinal fluid (CSF) flow in the spinal subarachnoid space (SAS) and can precipitate cystic accumulation of fluid within the tissue - syringomyelia. The dynamic processes that lead to syringomyelia from an obstruction to CSF flow are not clearly defined. Investigations into CSF circulation have revealed that physiological variables, including respiration, heart rate, and blood pressure, influence CSF flow. Spinal cord injury affects each of these variables, however it is unknown how these variables influence CSF flow when the SAS is

BACKGROUND: Individuals with both Alzheimer’s Disease (AD) and cerebrospinal fluid (CSF) dynamics disorders, such as idiopathic normal-pressure hydrocephalus (iNPH), exhibit reduced CSF Aβ42 levels, complicating the interpretation of AD biomarkers. However, the influence of CSF dynamics on blood-based AD biomarkers remains unclear. This study investigated whether immunoprecipitation mass spectrometry (IP-MS)-based AD plasma biomarkers were associated with disproportionately enlarged subarachnoid space hydrocephalus (DESH) on MRI. METHOD: This retrospective, cross-sectional study included 509 p

Macrocephaly is one of the most frequent reasons for referral to a pediatric neurologist. Benign enlargement of subarachnoid space (BESS) in infancy is the most common cause of macrocephaly and characterized clinically with large head circumference, normal or mildly motor and language delay and increased cerebrospinal fluid (CSF) in the subarachnoid space with normal ventricles or mild ventriculomegaly. In this review, we describe the etiology, epidemiology, clinical presentation, pathogenesis, neuroimaging, differential diagnosis, treatment and outcome of this entity from current literature w
