Prevention of early infective complications after laparoscopic splenectomy with the Garamycin sponge.

INTRODUCTION: Surgical site infection (SSI) appear to be more frequent in splenectomized patients than might be expected and its incidence can be explained neither by the extent of surgery nor by the risk of bacterial contamination of the operating field. AIM: Evaluation the local antibiotic prophylaxis using a gentamicin surgical implant in order to reduce SSI, particularly subphrenic abscesses. MATERIAL AND METHODS: WE CONDUCTED A PROSPECTIVE, RANDOMIZED STUDY OF TWO GROUPS OF PATIENTS UNDERGOING LAPAROSCOPIC SPLENECTOMY WHO WERE CONSIDERED AT HIGH RISK OF INFECTIVE COMPLICATIONS: patients

Ultrasound and computed tomography in the diagnosis and drainage of abscesses and other fluid collections.

The indications, technique and results of percutaneous drainage in 20 patients with abscesses and other fluid collections are presented. Special emphasis is placed on the complementary use of ultrasound and computed tomography and the relative indications for the use of each. Of the 20 patients, 5 had hepatic abscesses, 7 had subphrenic or subhepatic abscesses, 3 had pancreatic pseudocysts and the remaining 5 had loculated fluid collections in either the abdomen or chest.

CT evaluation of pancreatic injury following splenectomy.

A fluid collection in the left subphrenic space immediately after splenectomy is often associated with pancreatic injury. The configuration, location, and vascular supply of the tail of the pancreas explain this postoperative complication. Depending on the degree of injury, the CT findings may show swelling of the tail of the pancreas, ill-defined fluid collections, or a well-encapsulated pancreatic pseudocyst. The diagnosis is confirmed by percutaneous aspiration with amylase determinations and the demonstration of a pancreatic fistula. Failure to diagnose this complication promptly may lead

Sonography in complicated hepatic amoebic abscess.

Ultrasound examination was conducted on 150 patients with hepatic amoebic abscesses. Twenty of 150 patients were found to have various complications such as direct thoracic involvement, peritoneal rupture, subphrenic collection, obstructive jaundice and ascites. Ultrasound examination of the liver is not only useful in demonstrating the abscess cavity but it also depicts the complications extremely well.

An intrahepatic abscess as a rare complication of a pelvic fracture from blunt trauma to the abdomen.

An otherwise healthy young male sustained a non-displaced fracture of the pelvis in a blunt trauma from a motor vehicle accident. A fever developed on his second post injury day which led to discovery of an intrahepatic abscess by CAT scan. After blunt trauma to the abdomen, one must consider not only a retroperitoneal hematoma but also, subphrenic, subdiaphragmatic and intrahepatic abscess as its complication.

Ultrasonic evaluation of intra-abdominal abscesses.

This study describes the role of ultrasound in the evaluation of abdominal abscesses in children. A total of 41 abscesses were encountered in 36 patients (21 boys and 15 girls). The youngest patient was 1-month-old. Abscesses were localised at various sites (hepatic, renal, perirenal, paracolic, appendicular, pelvic, psoas, subphrenic, parietal), and of varied sonographic patterns (complex, anechoic, hypoechoic and containing uniform low level echoes). Most of them had irregular walls.

Giant horseshoe intra-abdominal abscess.

A study of 12 patients with giant horseshoe abscess of the abdominal and pelvic cavities seen at the Surgical Services of the University of Cincinnati Medical Center has emphasized the complexity and bizarre nature of these lesions. These infections represented a huge abscess or series of communicating abscesses extending from one subphrenic space along the corresponding paracolic gutter into the pelvis, up and along the opposite paracolic space, and into the other subphrenic space. Since these lesions occurred infrequently, they were often not recognized until they had become far advanced and

The clinical characteristics and outcome of intraabdominal abscess in Crohn's disease.

BACKGROUND: We aimed to elucidate the incidence and natural course of abdominal abscess complicating Crohn's disease (CD). METHODS: Of 352 patients with CD who were observed at our hospital between 1985 and October 2001, we studied 35 patients (9.9%) with abscesses in the mid-abdominal region (the abdominal wall, peritoneal cavity, retroperitoneum, and subphrenic region). RESULTS: The cumulative incidence of complication with an abscess was 9% and 25%, respectively, 10 and 20 years after CD onset. Of the 35 CD patients with abscess, 60% had had surgery by the time of the present study. The a

[Computer tomographic findings in splenic ruptures, subcapsular haematomas of the spleen and perisplenic abscesses (author's transl)].

The computer tomographic criteria of splenic rupture, new or old haematomas of the spleen (pseudocysts) and perisplenic or subphrenic abscesses are described with reference to three cases. It is shown that all the relevant problems in the pre-operative stage can be clarified by computer tomography, making arteriography unnecessary. The differential diagnosis between old haematomas and perisplenic abscesses can only be made by taking account of the history and other clinical information.

Short report: Hyperammonaemia in critically ill septic infants.

Three infants with subphrenic abscess, pyonephrosis, and obstructive ureterocoele respectively had grossly increased concentrations of plasma ammonia. This was considered to be a result of infections with urea splitting organisms. All died in spite of intensive care support, including specific measures to reduce plasma ammonia.

Thoracobilliary fistula as a rare complication of Roux-en-Y cholangio-jejunostomy stricture after iatrogenic bile duct injury correction: A case report.

INTRODUCTION: Thoracobiliary fistula is a rare presentation of biliary obstruction which is secondary to anastomotic Roux-en-Y cholangiojejunostomy stricture. CASE PRESENTATION: A 52-year-old man presented with fever, fatigue, and anorexia. He was jaundiced. He had a history of a laparoscopic cholecystectomy complicated by iatrogenic bile duct injury which was corrected by Roux-en-Y cholangiojejunostomy. The CT revealed a subphrenic abscess. Therefore, we established CT-directed percutaneous drainage and antibiotics treatment. Four months later, he presented again with fever, fatigue, and jau

[Chilaiditi syndrome].

The paper deals with a patient who was admitted through the surgical emergency service under the suspicion of subphrenic abscess. From the moment of admittance the patient was carefully followed. Laboratory tests were repeated several times. The following examinations were performed within a very short period of time: a posteroanterior X-ray of the chest and both subphrenic spaces, an ultrasonogram of the upper abdomen, a CT scan of the upper abdomen, roenthgenography of the gastroduodenum with gastrografin, an esophagogastroduodenoscopy and irrigography. Staging laparotomy in Hodgkin's disease.

Staging laparotomy (multiple liver and lymph node biopsies and splenectomy) was performed at the North Carolina Baptist Hospital in 123 patients with Hodgkin's disease. There were no deaths. Wound, pulmonary, intra-abdominal or urinary tract complications developed in 17 patients. Of those complications, one case each of pelvic abscess, subphrenic abscess, small bowel obstruction, staphylococcal peritonitis and subphrenic hematoma required a major intra-abdominal operation. The patients were classified on the basis of histopathology; nodular sclerosis--40, mixed cellularity--44, lymphocyte pre

Bronchoperitoneal fistula from a lung abscess.

Fistula formation between the bronchi and peritoneal cavity is extremely rare. In previous reports, fistulas have occurred secondary to thoraco-abdominal trauma, subphrenic abscess, suppurative biliary tract obstruction, malignancy and iatrogenically through procedures such as biliary surgery or percutaneous biliary drainage. The direction of fistula formation has always been thought to be from the peritoneal cavity to the bronchi: there are no reports of a fistula with a bronchial origin. This case report presents a patient who presented with sepsis and a bronchoperitoneal fistula and pneumop

[Surgical approach to the posterior surface of the liver (author's transl)].

Anterior incision does not afford a direct approach to lesions located in the posterior part of the liver, or to right subphrenic and retrohepatic abcesses, and may contaminate the peritoneal cavity in suppurative collections. Posterior incision is a more direct approach. It can be extrapleural and extraperitoneal, or transpleural with exclusion of the pleura when resecting a lower rib; a posterior thoraco-phreno-laparotomy gives a wider exposure. When the pleural cavity has been opened it should be drained and excluded before dealing with the subphrenic lesion. Such an incision is advocated i

[Abscess of the spleen: diagnosis and treatment. Apropos of 12 cases].

Twelve cases of splenic abscess, seen at our hospital between January 1980 and June 1987, were reviewed retrospectively. The most common causes of splenic abscesses were subacute endocarditis and intra abdominal sepsis. Diagnosis was suspected on clinical grounds and was always confirmed by sonography and/or computerized tomography. Two patients were drained unsuccessfully under CT scan guidance and underwent splenectomy. The other patients were operated primarily. One patient developed a subphrenic abscess postoperatively. One patient died from intractable cardiac failure due to subacute endo

Bare area abscess: imaging findings and potential communication with the mediastinum.

The bare area of the liver is not usually visualized by standard cross-sectional imaging techniques except in patients with ascites or subphrenic collections where this area is spared of fluid. We present a case of an abscess in the bare area of the liver with imaging findings and demonstration of a communication with the mediastinum, not previously described.
