Gonadotropin-releasing hormone and reproductive medicine.

The hypothalamic gonadotropin-releasing hormone (GnRH) is a key regulator of the reproductive system, mainly through its effects on pituitary luteinizing hormone (LH) and follicle-stimulating hormone (FSH) release. Gonadotropin-releasing hormone analogues are modifications of the natural decapeptide, being either agonists (GnRHa) or antagonists. GnRHa may imitate the native hormone and induce an endogenous LH surge; however, sustained treatment with GnRHa results in complete refractoriness of LH and FSH production. This aspect of GnRHa action is the main application of the GnRHa in reproductio

The mental health of staff working on intensive care units over the COVID-19 winter surge of 2020 in England: a cross sectional survey.

BACKGROUND: The COVID-19 pandemic generated a surge of critically ill patients greater than the capacity of the UK National Health Service (NHS). There have been multiple well-documented impacts associated with the national COVID-19 pandemic surge on ICU staff, including an increased prevalence of mental health disorders on a scale potentially sufficient to impair high-quality care delivery. We investigated the prevalence of five mental health outcomes; explored demographic and professional predictors of poor mental health outcomes; and describe the prevalence of functional impairment; and exp

Outbreak of NDM-1-Producing Escherichia coli in a Coronavirus Disease 2019 Intensive Care Unit in a Mexican Tertiary Care Center.

Emergency department areas were repurposed as intensive care units (ICUs) for patients with acute respiratory distress syndrome during the initial months of the coronavirus disease 2019 (COVID-19) pandemic. We describe an outbreak of New Delhi metallo-β-lactamase 1 (NDM-1)-producing Escherichia coli infections in critically ill COVID-19 patients admitted to one of the repurposed units. Seven patients developed infections (6 ventilator-associated pneumonia [VAP] and 1 urinary tract infection [UTI]) due to carbapenem-resistant E. coli, and only two survived. Five of the affected patients and fou

Ovarian regulation of postcoital gonadotropin release in the rabbit: reexamination of a functional role for 20 alphadihydroprogesterone.

In the rabbit, it has proposed that an ovarian progestin, 20 alpha-dihydroprogesterone (20alphaP), released at mating, is essential for a normal postcoital LH surge. However, we measured plasma levels of LH and 20alphaP after mating in rabbits and observed that the frequency, magnitude and time-course of changes in circulating levels of 20alphaP seemed inappropiate to account for the rapid and major surge of LH secretion. This prompted us to re-evalute the role of the ovary in regulating postcoital LH secretion. In chronically ovariectomized (greater than 30 days) does pretreated with estrogen

Forecasting Local Surges in COVID-19 Hospitalizations through Adaptive Decision Tree Classifiers.

IntroductionDuring the COVID-19 pandemic, many communities across the United States experienced surges in hospitalizations, which strained the local hospital capacity. Some risk metrics, such as the Center for Disease Control and Prevention's (CDC's) Community Levels, were developed to predict the impact of COVID-19 on the community-level health care system based on routine surveillance data. However, they had limited utility as they were not routinely updated based on accumulating data and were not directly linked to specific outcomes, such as surges in COVID-19 hospitalizations beyond local

Developing and implementing an infection prevention and control program for a COVID-19 alternative care site in Philadelphia, PA.

BACKGROUND: On March 27, 2020, the city of Philadelphia was given permission by Temple University to convert the Liacouras Center gymnasium to an alternate care site (ACS) to treat low-acuity COVID-19 patients. ACSs, especially those created to specifically care for infectious patients, require a robust infection prevention and control (IPC) program. METHODS: The IPC program was led by a physician and nurse partnership, both of whom had substantial experience developing IPC programs in US and low-resource settings. The IPC program was framed on a previously described conceptual model commonly

Design Criteria of Shelter Hospitals in Response to Biological Accidents: A Systematic Review.

BACKGROUND: The appropriate response to mass causality biological events requires well-established preparedness and providing a surge capacity. In such a situation, a practical solution is to convert large public venues into shelter hospitals. Due to the lack of a guideline for the transformation of a large public center into a hospital, the present study collected the design criteria for the transformation of public buildings into shelter hospitals in response to biological events such as epidemics or mass causality biological accidents. METHODS: The keywords were searched in Scopus, Web of

Health care workers' ability and willingness to report to duty during catastrophic disasters.

Catastrophic disasters create surge capacity needs for health care systems. This is especially true in the urban setting because the high population density and reliance on complex urban infrastructures (e.g., mass transit systems and high rise buildings) could adversely affect the ability to meet surge capacity needs. To better understand responsiveness in this setting, we conducted a survey of health care workers (HCWs) (N =6,428) from 47 health care facilities in New York City and the surrounding metropolitan region to determine their ability and willingness to report to work during various

Preparedness and Reorganization of Care for Coronavirus Disease 2019 Patients in a Swiss ICU: Characteristics and Outcomes of 129 Patients.

OBJECTIVES: In many countries, large numbers of critically ill patients with coronavirus disease 2019 are admitted to the ICUs within a short period of time, overwhelming usual care capacities. Preparedness and reorganization ahead of the wave to increase ICU surge capacity may be associated with favorable outcome. The purpose of this study was to report our experience in terms of ICU organization and anticipation, as well as reporting patient characteristics, treatment, and outcomes. DESIGN: A prospective observational study. SETTING: The division of intensive care at the Geneva University

Use of a GnRH antagonist in conjunction with low amplitude, high frequency LH pulses to induce follicular growth without an LH surge and ovulation in ewes.

The present study was undertaken to develop an experimental sheep model which could be used to investigate the abnormal follicle growth that is associated with the absence of the LH surge. On Day 10 of the oestrous cycle, 16 ewes were treated with an analogue of prostaglandin (cloprostenol; PG) and blood sampled every 4 h thereafter to determine the normal timing of the preovulatory LH surge. Three oestrous cycles later, all ewes were simultaneously treated with PG and a gonadotrophin-releasing hormone (GnRH) antagonist ([Ac-DNal1, D4C1Phe2, DTrp3, DArg6, DAla10] GnRH.HOAc; 50 micrograms kg-1

An insulin receptor activity surge in follicle cells drives vitellogenesis by upregulating CrebA.

Folliculogenesis is a process that requires accurate interpretation of female physiological cues and elaborate coordination between the growing oocyte and its surrounding follicle cells, each being capable of responding to external signals. Here, we investigate the role of insulin signaling in Drosophila follicle cells. Using a phase separation-based reporter system, we observe a surge of insulin receptor activity in follicle cells during vitellogenic stages, a surge that is disrupted by a maternal high-sucrose diet. Single-cell RNA-seq reveals a diet-sensitive subpopulation of stage-8 follicl

Hospital evacuation: Exercise versus reality.

INTRODUCTION: The Dutch Major Incident Hospital (MIH) is a standby, highly prepared, 200-bed hospital strictly reserved to provide immediate, large-scale, and emergency care for victims of disasters and major incidents. It has long-standing experience training for various major incident scenarios, including functioning as a back-up facility for the Netherlands. In 1995, the MIH had experience with overtaking an evacuated hospital when that hospital was threatened by flooding. In November 2014, an exercise was performed to transfer an evacuating hospital to the MIH. The scenario again became re

Roadmap: one anesthesiology department's strategic approach to COVID-19.

PURPOSE OF REVIEW: The COVID-19 pandemic has driven transformation in every aspect of the healthcare delivery system. The unpredictable onset and magnitude of COVID-19 infections resulted in wide gaps in preparedness for healthcare systems. The development of protocols to address both scarcity of resources and staff protection continues to be essential for risk mitigation. RECENT FINDINGS: The northeast region of the United States had a rapid early surge of COVID-19 infections leading to the exhaustion of critical care capacity. In addition, northeastern hospitals experienced decrease in elec

Incident command system: Situation unit leader and county public health liaison roles during the federal medical station, Santa Clara, during the COVID-19 response.

Federal medical stations (FMS) and alternative care sites are used to provide surge capability and capacity for short-term inpatients with healthcare needs that cannot be accommodated or provided for in a general shelter or general acute care facilities. In March and April 2020, an FMS was deployed to support the management of the COVID-19 pandemic in the 'hotspot' of Santa Clara County, CA. This paper describes how the flexibility of the incident command system allowed for the positions of situation unit leader and liaison officer to be combined at the FMS, supporting the effectiveness and ef

Preovulatory serum estradiol concentration is positively associated with oocyte ATP and follicular fluid metabolite abundance in lactating beef cattle.

Cattle induced to ovulate a small, physiologically immature preovulatory follicle had reduced oocyte developmental competence that resulted in decreased embryo cleavage and day 7 embryo quality compared with animals induced to ovulate a more advanced follicle. RNA-sequencing was performed on oocytes and their corresponding cumulus cells approximately 23 h after gonadotropin-releasing hormone (GnRH) administration to induce the preovulatory gonadotropin surge suggested reduced capacity for glucose metabolism and oxidative phosphorylation in the cumulus cells and oocytes from follicles ≤11.7 mm,

Species-specific wave attenuation and cost-effectiveness of mangroves for hybrid coastal defense.

Rising sea levels and intensifying storm surges are amplifying flooding risks in tropical and subtropical coastal regions under climate change. Conventional concrete structures provide reliable flood protection but they are increasingly criticized for their low ecological value and high maintenance costs. As an alternative, nature-based hybrid coastal protection systems, which combine engineering dikes with mangrove foreshores, are gaining recognition for providing both flood mitigation and ecological benefits. However, the wave attenuation performance and cost-effectiveness associated with di

Components of the rat conceptus that account for prolactin inhibition.

The secretions from developed concepti in the rat appear to inhibit both the rhythmic nocturnal secretion of prolactin (Prl) during the second half of pregnancy and the neurally mediated Prl release in response to suckling stimulation. In order to identify the parts of the concepti capable of inhibiting Prl secretion, the components of the concepti were systematically removed and their effect on plasma Prl levels was examined. In the first experiment, female rats which were simultaneously pregnant and lactating were tested for Prl secretion in response to suckling stimulation after one of the

Hospital Disaster Preparedness and Response to a Pediatric Mass Casualty Incident in Central India: A Retrospective Quality Improvement Analysis.

Background Mass casualty incidents (MCIs) place extreme demands on health systems, and pediatric victims introduce additional physiological and psychosocial challenges. Although disaster preparedness principles are well described, reports of coordinated pediatric MCI responses from India remain limited. On 26 November 2024, a school bus accident near Nagpur led to the simultaneous arrival of 49 casualties at a tertiary trauma center. This study uses retrospective operational data to evaluate hospital surge capacity, triage, imaging workflow, and clinical outcomes and interprets these findings
