Guideline Implementation: Sterile Technique.

Establishing and maintaining a sterile field to help prevent surgical site infections requires specific knowledge and skills and is among the most important responsibilities of the perioperative RN. Perioperative RNs should remain vigilant in monitoring the sterile field, communicate when a break in sterile technique occurs, and collaborate with other surgical team members to correct the break in technique. The AORN "Guideline for sterile technique" provides guidance to perioperative personnel on the principles and processes of sterile technique. This article elaborates on key takeaways from t

Predictors of pain during phacoemulsification under local sedation: A multicentric study.

PURPOSE: To assess the risk factors for pain during cataract surgery under local anesthesia. METHODS: This multicentric observational study assessed risk factors for pain during cataract surgery from June to November 2020 in a private clinic and two university hospitals (France). Adults who underwent cataract surgery under local anesthesia with conscious sedation were consecutively included. Exclusion criteria were locoregional anesthesia and an inability to communicate. Once the surgical drape was removed, patients rated the pain on a numerical rating scale (NRS) from 0 to 10, after the surg

Infection Prevention in Shoulder Surgery.

The microbiome of the shoulder demonstrates distinctive differences to other orthopaedic surgical sites. Recent studies have demonstrated that the most common organisms found in deep shoulder infections are coagulase-negative staph lococcal species and Propionibacterium acnes . Many studies support diligent hand washing, decreasing operative time, routine glove changing, minimizing operating room traffic, and covering instruments as means for decreasing the risk of deep infection. On the other hand, hair clipping and the use of adhesive drapes may have little effect on decreasing the incidence

Oculocardiac reflex: an unusual trigger during dental surgery.

The oculocardiac reflex is a trigeminal-vagal reflex that manifests as cardiac arrythmias, most often bradycardia. The reflex can be triggered by manipulation of periorbital structures and unintended pressure on the bulbus oculi maxillofacial procedures. In this brief communication, we describe an unusual trigger of the oculocardiac reflex during maxillofacial surgery that resulted in severe bradycardia. This case highlights the need for careful securement of medical devices and attention to surgical technique to avoid undue pressure on draped fascial structures.

A Variant of the Sciatic Nerve and its Clinical Implications.

Variants of gluteal neural anatomy are important to consider, especially during surgical approaches to the hip. During the routine dissection of the gluteal region, a variant of the sciatic nerve was found where the nerve left the pelvis fully split into its tibial and common fibular components. Intrapelvically and extrapelvically, there was no splitting of the two components by the piriformis muscle. Distally, the two parts of the nerve were draped over the medial and lateral edges of the ischial tuberosity. To avoid iatrogenic injury to the sciatic nerve during invasive or surgical approache

Gossypiboma discovered 24 years after prostate surgery, a forgotten but never forgiven complication.

Forgetting gauze or "a surgical drape" inside a patient after surgery is a rare medical error. It can lead to severe complications, high hospital costs and medico-legal implications. As a result, this complication is often not reported, mainly to avoid retaliation and because it can initiate extensive critical press coverage. This technical oversight may be just the tip of an iceberg concerning the reality of surgical errors; therefore, the entire surgical team must focus on prevention, continuing medical education and strict adherence to protocols and counting guidelines to minimize their inc

De-mucosalized pharyngeal flap: a modified technique for selected cases of velopharyngeal insufficiency.

BACKGROUND AND PURPOSE: Superiorly based posterior pharyngeal flap is performed via rotation of tissues of the posterior pharyngeal wall anteriorly and anchoring it to the soft palate. Unfortunately, bad healing of the donor site defect might be a considerable cause of morbidity of the surgery. With some modifications of flap elevation we could achieve better surgical outcomes. The aim of this study was to present the new modification of the conventional maneuver and its surgical/functional outcomes. SUBJECTS AND METHODS: The study design is a case series. 17 patients underwent the de-mucosal

Staphylococcus aureus surgical site infection following unilateral biportal endoscopic spine surgery: a two-case report.

BACKGROUND: Surgical site infection (SSI) is a serious complication of spinal surgery, including minimally invasive unilateral biportal endoscopic (UBE) procedures. Staphylococcus aureus (S. aureus) is a leading cause of such infections. This report analyses two cases of S. aureus SSI following UBE surgery. CASE PRESENTATION: Two patients (one male and one female) aged 56 underwent elective UBE surgery. Both patients had inadequate preoperative skin preparation. The cases were complicated by intraoperative fluid leakage, which led to soaked surgical drapes. The second case also involved prolo

Intraoperative interventions for preventing surgical site infection: an overview of Cochrane Reviews.

BACKGROUND: Surgical site infection (SSI) rates vary from 1% to 5% in the month following surgery. Due to the large number of surgical procedures conducted annually, the costs of these SSIs can be considerable in financial and social terms. Many interventions are used with the aim of reducing the risk of SSI in people undergoing surgery. These interventions can be broadly delivered at three stages: preoperatively, intraoperatively and postoperatively. The intraoperative interventions are largely focused on decontamination of skin using soap and antiseptics; the use of barriers to prevent movem

Flexion myelopathy of the thoracic spine. Case report.

The authors report a rare case of surgically treated symptomatic thoracic kyphosis caused by dynamic compression in an elderly man. Myelopathy due to thoracic kyphosis has been reported in patients with congenital kyphosis, Scheuermann dorsal kyphosis, and Cushing disease, but to the authors' knowledge this is the first report of dynamic kyphosis in an elderly person. This otherwise healthy 84-year-old man presented with a 2-year history of progressive difficulty in walking and bilateral leg dysesthesia. Despite several cervical and lumbar surgeries, his symptoms gradually worsened. A radiolog

Surgical field and skin preparation.

Surgical site infection (SSI) is the third most frequent healthcare-associated infection in France. SSI rates in total hip or knee replacement are around 2%. The main bacteria implicated in SSI in clean surgery are those of the skin flora, whence the importance of skin preparation to eliminate transient flora and reduce resident flora. Guidelines for the prevention of SSI have progressed in recent years in France: firstly in 2013, and then in 2016. That preoperative hair removal and scrubbing of clean skin ahead of cutaneous asepsis is non-contributive was confirmed in 2013. A shower with norm

Laser safety.

Safety has always been an important aspect of any laser application in surgery and medicine. In any review of the laser surgery literature, several issues continue to dominate. These issues include: wearing eye protectors, dealing with the plume of vaporized tissue, and controlling potential fire hazards. No one denies that lasers can pose a serious hazard to the eye, but the decision to wear eye protectors in all procedures has been frequently questioned. The degree of effort needed to minimize the very serious risk from chronic breathing of vaporized tissue also requires judgment. Aside from

An operative barrier system for skull base and mastoid surgery: creating a safe operative theatre in the era of COVID-19.

Within Neurotology, special draping systems have been devised for mastoid surgery recognizing that drilling of middle ear mucosa is an aerosol generating medical procedure (AGMP) which can place surgical teams at risk of COVID-19 infection. We provide a thorough description of a barrier system utilized in our practice, along with work completed by our group to better quantify its effectiveness. Utilization of a barrier system can provide near complete bone dust and droplet containment within the surgical field and prevent contamination of other healthcare workers. As this is an early system, f

The Utility of Perioperative Products for the Prevention of Surgical Site Infections in Total Knee Arthroplasty and Lower Extremity Arthroplasty: A Systematic Review.

Surgical site infections (SSIs) are among the most prevalent and devastating complications following lower extremity total joint arthroplasty (TJA). Strategies to reduce the rates can be divided into preoperative, perioperatives, and postoperative measures. A multicenter trial is underway to evaluate the efficacy of implementing a bundled care program for SSI prevention in lower extremity TJA including: (1) nasal decolonization; (2) surgical skin antisepsis; (3) antimicrobial incise draping; (4) temperature management; and (5) negative-pressure wound therapy for selected high-risk patients. Eyelash Impalement of Iris After Uncomplicated Cataract Surgery.

This report highlights the first known report of an intraocular eyelash embedded in the inferior midperipheral iris following a routine cataract surgery. A 74-year-old female presented 5 months postoperatively from a full-thickness macular hole repair with a 3+ nuclear sclerotic cataract. She underwent an uneventful left eye cataract extraction with insertion of posterior chamber intraocular lens (PCIOL). Review of surgical video revealed well-draped eyelashes and no introduction of an eyelash into the anterior chamber at any point during surgery. The first postoperative visit was notable for

Cable Organizer for Laparoscopic and Endoscopic Procedures.

BACKGROUND: During laparoscopic and endoscopic procedures, multiple devices are used simultaneously, requiring preconnection and arrangement of their cables. Traditionally, cables are bundled with gauze and fixed with drape clips, which hinders intraoperative adjustment of cable number and orientation. Thus, an innovative device for more flexible and convenient equipment line management is needed. Commercial cable-management products lack adjustability during surgery; a rotatable, angle-lockable organizer could fulfill this need. METHODS: We designed a novel surgical cable organizer and produ

Using simulation training to improve perioperative patient safety.

Simulation learning provides medical and nursing personnel with the opportunity to develop and refine their skills without putting patients at risk. Faced with ensuring the competence of a large number of new staff members, the management team at one facility implemented a simulation training program. Surgical team members are able to participate in an ongoing program of simulated scenarios involving surgical drape fires and airway fires, cardiac arrest of patients in the supine position and prone position, respiratory depression in the postanesthesia care unit, and malignant hyperthermia.
