Management of Lateral Thigh Pain following Cephalomedullary Nail: A Technical Note.

Peritrochanteric hip fractures are most commonly treated with proximal femoral fixation devices, such as a cephalomedullary nail or sliding hip screw. As usage rates increase for these fixation devices, complications from their insertion are becoming more prevalent. Lateral hip pain from proximal locking device insertion and prominence continues to be one of the most frequent complaints regarding hardware irritation following this surgical procedure. Conservative treatment options for this complaint include local corticosteroid injection and physical therapy, although once these treatments hav

Advantages and disadvantages of pinless external fixation.

The AO pinless external fixator (PEF) uses trocar tipped clamps to grip the outer tibial cortex rather than pins to transfix it. Its main advantage is to avoid further contamination of the medullary canal in open tibial fractures where a nail may subsequently be used. We tested the anatomical safety of this device and its effect on plastic surgical procedures compared with a standard unilateral external fixator (UEF).The PEF and UEF were placed on two amputated limbs which were then dissected. Structures at risk were traced on ten cadaver limbs. We found that important anatomical structures we

Intertrochanteric fractures: ten tips to improve results.

Intertrochanteric hip fractures are among the most common types of fractures, and the numbers are increasing as the population ages. Most intertrochanteric fractures are treated surgically. It is therefore important that the treatment methods are effective and have a minimal risk of complications. The goals of treatment include a predictable union, unrestricted early weight bearing, and avoidance of fixation failure or excessive deformity of the proximal femur. Careful attention to the fracture pattern (obliquity or other hallmarks implying instability) can guide fixation device selection. Reg

Tailor's bunion: clinical evaluation and correction by distal metaphyseal osteotomy with cortical screw fixation.

Fifth metatarsal distal metaphyseal osteotomy represents a very common surgical procedure performed for tailor's bunion deformity. The authors advocate using a cortical bone screw for fixation device offers many advantages over conventional devices (i.e., Kirschner wire). The purpose of this paper is to offer a means to thoroughly evaluate tailor's bunion deformity clinically, biomechanically, and radiographically. This will allow the podiatric surgeon to choose a procedure of choice to correct the deformity. The technique of fifth metatarsal distal metaphyseal osteotomy with cortical bone scr

In Vitro Antibacterial Efficacy of a New TiO2-Cu-Coated Titanium Surface for Biomedical Applications.

Despite advancements in surgical care, the management of surgical site infections (SSIs) associated with fracture-fixation devices is still a challenge after implant fixation, especially in open fractures. Staphylococcus aureus (S. aureus) is a common pathogen of SSIs and contaminates by penetrating the trauma itself (preoperatively) or during insertion of the fixation device (intraoperatively). A unique technology was developed to address this issue, consisting of an antibacterial surface obtained after depositing copper on a porous titanium oxide surface. This study aims to characterise and

Impact of the presence of orthopedic hardware on technical performance of major amputations.

Metallic prostheses used for joint reconstruction or open reduction of fractures were discovered pre- or intraoperatively during the performance of six above-knee amputations. The prosthesis was found fortuitously before surgery in two cases on the basis of a surgical scar or radiograph of the leg. In four patients, discovery occurred intraoperatively, with adverse impact on the procedure in two cases: more proximal amputation level to allow hip nail removal was necessary in one case, and difficult transection of the prosthesis stem was encountered in one case. Orthopedic prostheses will be fo

Risk factors for postoperative infection after non-elective femur fracture fixation in pediatric patients.

BACKGROUND: Pediatric femoral shaft fractures are common and often require surgical fixation. Postoperative infections remain a serious complication, yet risk factors specific to this population are not well defined. METHODS: We analyzed 14,037 pediatric patients undergoing non-elective femur fracture fixation in the 2019 and 2022 Kids' Inpatient Database (KID). Multivariable logistic regression identified independent predictors of postoperative soft tissue, device-related, and overall infection. Secondary outcomes included length of stay (LOS) and hospital charges. RESULTS: Infections occur

Twenty-four years of experience in management of complex mandibular fractures with low cost, custom-made mandibular external fixation: A 65-patient series.

INTRODUCTION: Ginestet introduced the first external device used to fix the mandible in 1936. In 1949, Morris introduced a biphasic fixation device. This "Joe Hall Morris fixation" design led to use of a self-crafted external fixator based on pins connected by a breathing tube filled with dental resin. The objective of this study was to present our surgical results with this device through a 65-patient series. METHODS: This retrospective study included all the patients who benefited from the self-crafted mandibular external fixator at our Oral and Maxillofacial department from 1995 to 2019. S

Duration of antibiotic treatment and risk of recurrence after surgical management of orthopaedic device infections: a multicenter case-control study.

BACKGROUND: Device-related infections in orthopaedic and trauma surgery are a devastating complication with substantial impact on morbidity and mortality. Systemic suppressive antibiotic treatment is regarded an integral part of any surgical protocol intended to eradicate the infection. The optimal duration of antimicrobial treatment, however, remains unclear. In a multicenter case-control study, we aimed at analyzing the influence of the duration of antibiotic exposure on reinfection rates 1 year after curative surgery. METHODS: This investigation was part of a federally funded multidiscipli

A new type of distractor for the surgical correction of scoliosis and spondylolisthesis.

A distractor for the surgical correction of scoliosis is described. It comprises a threaded rod that runs inside a cannula. The distraction is achieved with special distraction forceps. Fixation is then secured by screwing a nut along the rod until it is in contact with the opening of the cannula. This distractor is not as simple as the Harrington device but it has a number of advantages. It is easier to apply in difficult surgical situations. The fixation mechanism is infallible. The lengthening is achieved more gradually. Slackening and removal are simple and always possible, and it is never

Irrigation and Debridement of an Open Ankle Fracture With Open Reduction and Internal Fixation and Syndesmotic Fixation Using a Suture Button Device.

Ankle fractures are among the most common surgically treated fractures. The incidence of syndesmotic injury in ankle fractures is 13%-20%. The goal of syndesmotic fixation is a stable, symmetric ankle joint. Missing or poorly reducing a syndesmotic injury can result in diminished function and tibiotalar arthritis. Recently, a suture button-type device has been used instead of the traditional trans-syndesmotic screw-type fixation. This case-based video demonstrates a technique for using suture button-type devices to repair syndesmotic ankle injuries and presents the data regarding the outcomes

Elastic Locking Intramedullary Nails Fixation in Trimalleolar Fractures.

OBJECTIVE: Since the 1960s, although open reduction and internal fixation for ankle fractures has been widely used, it is associated with complications such as wound dehiscence, infection, prominent hardware, and failure. Closed reduction and internal fixation, on the other hand, offers greater biomechanical strength, requires minimal incisions, and features low-profile hardware. Our study compares the efficacy of elastic locking intramedullary nails (ELIN) fixation featuring minimally invasive microenvironmental protection microstress shielding versus rigid internal fixation (RIF) for trimall

[Treatment of distal radius extra-articular malunion in young active patients].

Malunion is a common complication of distal radius fractures despite new fixation devices. The authors discuss the surgical indications, preoperative management and the various surgical techniques used to treat distal radius extra-articular malunion, specifically in active young subjects. The treatment goals in this population group are anatomical restoration of the distal radius and distal radioulnar joint, short immobilization period, quick functional recovery and stable results over time, consistent with high functional demands.
