Novel Surgical Approach for Wound Closure of Large Livedoid Vasculopathy Wounds: A Case Series.

BACKGROUND: Livedoid vasculopathy (LV) is a rare, thrombotic disorder of the dermal capillaries characterized by recurrent, painful ulcerations in the lower extremities. Diagnosis is challenging and typically based on clinical presentation and distinct histopathologic features. Although systemic anticoagulation often leads to ulcer resolution, large, nonhealing wounds may require surgical intervention. In such cases, split-thickness skin grafting (STSG) may be considered. However, LV's prothrombotic pathogenesis can threaten graft viability in the immediate postoperative period. Thus, perioper

Negative pressure wound therapy for skin grafts and surgical wounds healing by primary intention.

BACKGROUND: Indications for the use of negative pressure wound therapy (NPWT) are broadening with a range of systems now available on the market, including those designed for use on clean, closed incisions and skin grafts. Reviews have concluded that the evidence for the effectiveness of NPWT remains uncertain, however, it is a rapidly evolving therapy. Consequently, an updated systematic review of the evidence for the effects of NPWT on postoperative wounds expected to heal by primary intention is required. OBJECTIVES: To assess the effects of NPWT on surgical wounds (primary closure, skin g

Pseudo-stab wounds: putrefactive dehiscence of remote surgical incisions masquerading as stab wounds.

Artifacts due to decomposition can be mistaken for antemortem injury leading to an incorrect suspicion of foul play. We describe an instance of postmortem wound dehiscence that mimicked antemortem stab wounds. A man with a history of colon cancer and substance abuse was found dead. There were advanced putrefactive changes and multiple defects of the anterior torso that resembled stab wounds. Subsequent investigation revealed that 11 months earlier, he had a laparoscopic-assisted colon resection that involved surgical incisions corresponding in location and size to the above defects. Putrefacti

Antiseptic wound ventilation with a gas diffuser: a new intraoperative method to prevent surgical wound infection?

Postoperative wound infections are often a result of peri-operative contamination by Staphylococcus aureus. With a new insufflation device, a gas diffuser, it has become possible to establish a local micro-environment of almost 100% carbon dioxide in an open surgical wound. The device enables ventilation of the wound with an antiseptic agent, which in gaseous form can be delivered as a low uniform dose to all parts of the wound. The use of carbon dioxide (CO2) as a carrier gas eliminates possible inflammability of an antiseptic agent and helps to concentrate it to the site of interest by gravi

Retained Surgical Cotton Ball Leading to Chronic Calf Wound.

Retained surgical items, such as cotton balls (gossypibomas), are rare yet preventable complications that may cause chronic infection and diagnostic difficulty. We report the case of a 65-year-old man with a non-healing right calf wound and sinus tract persisting for 1 year following orthopedic surgery. Magnetic resonance imaging (MRI) revealed an abscess characterized by T1-weighted hypointensity and T2-weighted spiral-like low-signal fringes, prompting surgical exploration. A retained cotton ball was identified and excised, followed by debridement and local rotational flap closure. The patie

Tension-Free Management of Surgical Wound in Paramalleolar Bypass.

Objective: In paramalleolar bypass for critical limb-threatening ischemia (CLTI), excessive skin tension may occur for the closure of surgical wounds around the ankle. Furthermore, these surgical incisions are often proximal to infectious ischemic ulcers. Wound dehiscence caused by skin tension and surgical site infection carries a risk of graft exposure, anastomotic disruption, or graft insufficiency. Patients and Methods: Tension-free wound management was adopted in eight patients who underwent paramalleolar bypass for CLTI. Tension-free closure was adopted for surgical incisions for distal

Surgical wound dehiscence: a conceptual framework for patient assessment.

This paper presents a conceptual framework which outlines the risk factors associated with surgical wound dehiscence (SWD) as identified in the literature. The purpose for the development of the conceptual framework was to derive an evidence-based, informed understanding of factors associated with SWD, in order to inform a programme of research on the aetiology and potential risk factors of SWD. Incorporated within the patient-centric conceptual framework are patient related comorbidities, intraoperative and postoperative risk factors related to SWD. These are categorised as either 'mechanical

Tensile strength of surgical knots in abdominal wound closure.

Abdominal wound dehiscence is a surgical catastrophe that can be attributed to patients or technical factors. The technical properties of the monofilament sutures and knots that are commonly used in abdominal closure are poorly understood. The aim of this study was to compare the tensile strength of monofilament sutures tied with conventional knots. To do this, the knot-holding capacity of four types of knots (square, surgeons', Aberdeen and loop) were tested using three types of gauge 1 monofilament suture, namely nylon, polyglyconate and polydioxanone, using a tensiometer. We found that the

Surgical wound mucormycosis necessitating hand amputation: a case report.

We report an unusual case of cutaneous mucormycosis in a 17-year-old man who had no risk factors for fungal infection. The aggressive nature of cutaneous mucormycosis is illustrated. A high index of suspicion is crucial for identifying and preventing progression of this disease, which can lead to limb amputation, even death. Extra vigilance should be given to those who are immunocompromised, including those receiving short courses of steroids. Early recognition, prompt surgical intervention and initiation of an appropriate antifungal treatment are crucial in the management of this rare but pot

Long-term prognosis of patients with surgical wound infections.

This study examined if surgical wound infections (SWI) result in a severe prognosis regarding general health and increase the consumption of social resources. A group of 1301 patients were interviewed by self-administered questionnaires during 1993-1994, while operated during hospitalization in seven Danish hospitals. These patients were followed up at least once by similar questionnaires at a median time of 5.5 and 10.0 months postoperatively. The consequences of surgically diagnosed SWI were analyzed in a hospital cohort of 58 infected patients and 648 controls. Postdischarge infections were

Reduction of wound infection in high-risk surgical patients.

During a 9-year period, 183 consecutive patients underwent total colectomy and the endorectal ileal pull-through procedure (ERIPT) for ulcerative colitis (UC) (n = 156), familial polyposis (n = 25), or Hirschsprung's disease (n = 2). The average age was 29.4 years (range 7.1-59.1 years). All patients with UC were steroid-dependent at the time of operation. Two groups were retrospectively reviewed based on the management of their midline abdominal wounds. Ninety consecutive patients underwent the ERIPT procedure between 1983 and 1987 with stapled skin closure and perioperative intravenous antib

Neurodermatitis: a skin condition that affects surgical wound healing.

Neurodermatitis, or lichen simplex chronicus, generally found in females between the ages of 25 and 55 years, is highly inflammatory and may become generalized. The patients are hypersensitive and are not good candidates for surgery as a Köbner-like phenomenon may occur at the site of the incision with consequent breakdown of the incision, wound dehiscence, and infection. If these patients are to respond normally to treatment, it is sometimes necessary for the surgeon to treat their tensions.

Inguinal Vascular Surgical Wound Protection by Incisional Negative Pressure Wound Therapy: A Randomized Controlled Trial-INVIPS Trial.

OBJECTIVE: A randomized controlled trial (RCT) was undertaken to determine the effect of negative pressure wound therapy (NPWT) on closed incisions after inguinal vascular surgery regarding surgical site infections (SSIs) and other wound complications. BACKGROUND: SSIs are a major concern in open vascular procedures involving the inguinal region. Prophylactic NPWT on closed incisions has shown promising results, but the quality of evidence can be debated. This study aims to objectively evaluate whether NPWT on sutured inguinal incisions after elective vascular surgery can decrease the inciden
