Risk perception of pregnancy promotes disapproval of gestational surrogacy: analysis of a nationally representative opinion survey in Japan.

BACKGROUND: To clarify the relationship between the general attitude towards gestational surrogacy and risk perception about pregnancy and infertility treatment. MATERIALS AND METHODS: This study analysed the data of nationally representative cross-sectional surveys from 2007 concerning assisted reproductive technologies. The participants represented the general Japanese population. We used this data to carry out multivariate analysis. The main outcome measures were adjusted odds ratios and 95% confidence intervals from logistic regression models for factors including the effect of pregnancy

Health service experiences of gender- and sexually diverse intended parents throughout the surrogacy journey: a scoping review protocol.

OBJECTIVE: This scoping review will identify and map health-related literature concerning gender- and sexually diverse individuals engaging with surrogacy in health care settings, including the stages of the surrogacy journey, and any reported barriers and facilitators. INTRODUCTION: Gestational surrogacy involves engagement with multiple health services and professionals across fertility, maternity, birth, and postnatal care, where cis-heteronormative assumptions about people's identities, family structure, and parenthood persist. Evidence indicates that gender- and sexually diverse individu

Complete pathologic response surrogacy for survival and its predictors in locally advanced rectal cancer: a retrospective cohort study.

BACKGROUND: Improved treatment techniques in locally advanced rectal cancer (LARC) with neoadjuvant chemoradiation and total mesorectal excision have resulted in higher rates of tumor downstaging and complete pathologic response (pCR). We aimed to explore the association of pCR with survival outcomes and its predictors. METHODS: We retrospectively enrolled 478 LARC patients referred to a tertiary cancer center from July 2008 to October 2023 who had received neoadjuvant long-course chemoradiation followed by definitive surgery. The patients were followed up, and the association of pCR with dis

Evaluating the Surrogacy of Pathological Complete Response at Radical Cystectomy in Neoadjuvant-based Muscle-invasive Bladder Cancer Trials.

Overall survival (OS) remains the gold standard end point in neoadjuvant trials for muscle-invasive bladder cancer (MIBC), yet its assessment requires prolonged follow-up. Whether pathological complete response (pCR) at radical cystectomy (RC) can serve as a valid trial-level surrogate remains unclear. To test this hypothesis, we conducted a systematic review and meta-analytic surrogacy analysis (PROSPERO CRD420251050357) of prospective nonrandomized trials and randomized controlled trials (RCTs) evaluating neoadjuvant therapies followed by RC in nonmetastatic MIBC (2003-2026). Eligible trials

Surrogacy of intermediate clinical endpoints for overall survival in BCG-naïve high-risk non-muscle-invasive bladder cancer.

INTRODUCTION: Recent trials in BCG-naïve high-risk non-muscle-invasive bladder cancer (HR-NMIBC) have added immunotherapy to BCG, often using event-free survival (EFS) as the primary endpoint. However, neither EFS nor progression-free survival (PFS) has been validated as a surrogate for overall survival (OS) in this setting. METHODS: We retrospectively analyzed data from 18 international centers, including patients with BCG-naïve HR-NMIBC treated between 2001 and 2025. Adequate BCG exposure (International Bladder Cancer Group criteria) was the treatment contrast for its effects on intermediat

Time-to-event surrogate end-points in multiple myeloma randomised trials from 2005 to 2019: A surrogacy analysis.

Use of surrogate end-points such as progression-free survival (PFS) and other time-to-event (TTE) end-points is common in multiple myeloma (MM) clinical trials. This systematic review characterises all published randomised controlled trials (RCTs) in MM using PFS or other TTE end-points between 2005 and 2019 and assesses strength of surrogacy of PFS for overall survival (OS). The association between OS hazard ratios (HRs) and PFS HRs was evaluated with linear regression, and the coefficient of determination with Pearson's correlation. We identified 88 RCTs of which 67 (76%) used PFS as the pri

A Comparison of the Performance of 6 Surrogacy Models, Including Weighted Linear Regression, Meta-Regression, and Bivariate Meta-Analysis.

OBJECTIVES: Several trial-level surrogate methods have been proposed in the literature. However, often only 1 method is presented in practice. By plotting trial-level associations between surrogate and final outcomes with prediction intervals and by presenting results from cross-validation procedures, this research demonstrates the value of comparing a range of model predictions. METHODS: Two oncology data sets were used as examples. One contained 34 trials and had an overall moderate surrogate association; the other contained 14 trials and had an overall strong association. The models fitted

In vitro maturation and surrogacy in patients with vascular-type Ehlers-Danlos syndrome--a safe assisted reproductive technology approach.

Ehlers-Danlos syndrome (EDS) is an autosomal dominant connective tissue disorder with one of the highest maternal mortality rates of any condition. Patients with the vascular type of EDS are prone to spontaneous arterial and visceral ruptures. The occurrence of these severe and life-threatening complications is increased in pregnancy. Moreover, these patients carry a 50% risk of having an affected child. However, little is known about the risks of assisted conception treatments on these patients. We present the case of a 33-year-old woman suffering from EDS with a history of repeated ruptures

Commercial surrogacy: how provisions of monetary remuneration and powers of international law can prevent exploitation of gestational surrogates.

Increasing globalisation and advances in artificial reproductive techniques have opened up a whole new range of possibilities for infertile couples across the globe. Inter-country gestational surrogacy with monetary remuneration is one of the products of medical tourism meeting in vitro fertilisation embryo transfer. Filled with potential, it has also been a hot topic of discussion in legal and bioethics spheres. Fears of exploitation and breach of autonomy have sprung from the current situation, where there is no international regulation of surrogacy agreements--only a web of conflicting nati

Where does a man stand on issues of assisted reproduction, surrogacy, artificial insemination within lesbian relationships and posthumous conception?

This article takes issues more commonly associated with women: assisted reproduction, surrogacy, artificial insemination within lesbian relationships and posthumous conception and considers what role a man may play in this domain. To this end the article commences with a short scenario of possible life events then traverses the current legal position to examine the extent that the law either facilitates or frustrates a man's desire and ability to fulfill the role of either father or parent in the above situations. The article considers the issues of assisted reproduction technology in both a r

Incidence and Implications of Twin Gestational Surrogacy at Two Large U.S. Military Hospitals: A 10-Year Retrospective Cohort Study.

INTRODUCTION: Individuals seeking a gestational surrogate often turn to U.S. military dependents due to favorable insurance coverage. Surrogate pregnancies, including multiple gestations, may be at increased risk for adverse outcomes. The objectives of this study were to determine the incidence of surrogacy in a twin population conceived by assisted reproductive technology (ART), assess the impact on the military healthcare system, and determine if there is an increased rate of complications in twin surrogacy pregnancies. MATERIALS AND METHODS: We conducted a 10-year retrospective cohort stud

Prey consumption and condition of endangered age-0 pallid sturgeon Scaphirhynchus albus and co-located congeners: An assessment of surrogacy.

The capture of wild age-0 pallid sturgeon Scaphirhynchus albus was rare historically. However, an unprecedented high number of captures during 2021, coupled with a more typical year in 2020, provided a unique opportunity to address diet-related knowledge gaps during early life and evaluate surrogacy relationships with abundant congener individuals. We assessed diet and body condition of 55 age-0 pallid sturgeon and 139 co-located non-pallid Scaphirhynchus individuals of similar size. Both pallid and non-pallid individuals primarily consumed Chironomidae (Diptera), with Lopescladius and Paracla

Effect of Skin to Skin Contact between the Biological Mother and Their Baby Who Born Via Surrogacy on Mother Infant Attachment.

BACKGROUND: The attachment between mother and infant, beginning from pregnancy and reaching a peak with skin-to-skin contact, has a considerable effect on infants' growth and development. As stress and anxiety in contact with infants and unwillingness to embrace them have been reported in biological mothers in surrogacy, this study was conducted to assess the effect of skin-to-skin contact between the biological mother and their baby who born via surrogacy on mother-infant attachment. MATERIALS AND METHODS: In this clinical trial, 50 biological mothers were randomly assigned to either interve

Pregnancy and birth outcomes of single versus multiple embryo transfer in gestational surrogacy arrangements: a systematic review and meta-analysis.

Multiple embryo transfer (MET) is associated with both an increased risk of multiple pregnancy and of live birth. In recent years, MET has become standard practice for most surrogacy arrangements. There is limited review of the use of MET versus single embryo transfer (SET) in surrogacy practice. The present review systematically evaluated the pregnancy outcomes of surrogacy arrangements between MET versus SET among gestational carriers. A systematic search of five computerized databases without restriction to the English language or study type was conducted to evaluate the primary outcomes: (

Surrogates 20 years on: long-term psychological health, contact with surrogacy families, and thoughts and feelings about post-birth contact.

STUDY QUESTION: What were the psychological health outcomes for surrogates 20 years after the surrogacy pregnancy? SUMMARY ANSWER: Most surrogates did not experience psychological problems 20 years after the pregnancy with many showing positive psychological wellbeing. A Strategic Roadmap to Support Communication on and Acceptance of Surrogate Endpoints: The REnal Surrogacy accEpTance in Chronic Kidney Disease (RESET CKD) Collaboration.

INTRODUCTION: Developing effective treatments in chronic, progressive diseases like chronic kidney disease (CKD) is challenging because patients may only experience relevant outcomes such as kidney failure after long periods of disease progression. Surrogate endpoints provide a valuable alternative to definitive final patient-relevant outcomes, which may accelerate clinical development processes. However, optimal utilization of surrogate endpoints for reimbursement decisions requires alignment across multiple stakeholders, including health technology assessment (HTA) bodies and reimbursement a
