Primary Ewing's sarcoma of the talus.

Ewing's sarcoma is a malignant round cell tumour of bones and soft tissues that usually arises from the diaphyseal or meta-diaphyseal parts of long bones and less commonly from flat bones. It occurs rarely in the foot and if occurs, the calcaneus and the metatarsals are commonly involved. We present a case of a young woman diagnosed with primary Ewing's sarcoma of the talus with local spread to adjacent tarsals and the ankle joint. Ewing's sarcoma of feet, if present with even a trivial suspicion of spread either locally or distant, makes limb salvage surgery difficult. So, the treatment with

Pediatric Osteochondritis Dissecans of the Talus.

OCDs of the ankle are the third most common lesion in the body and a physician should include this in the differential while evaluating the pediatric population with the history of injury and/or pain in the ankle. Pain, stiffness, and swelling are the most common presenting complaints about the pediatric and adolescent patients with an OCD. Conservative treatment options should be attempted before attempting surgical intervention. Multiple surgical procedures exist to manage the lesion based on location, size, and other factors.

Fractures of the Lateral Process of the Talus.

Lateral talar process fractures (LTPF) are uncommon injuries but have become more relevant with snowboarding. Currently the fractures are classified according to McCrory-Bladin into 3 types, with advice for treatment that is not ideal anymore. This article proposes modifying the existing classification by differentiating the multifragmented type III into 3 subtypes: IIIa, articular multifragmented but metaphyseal simple; IIIb, articular and metaphyseal multifragmented but reconstructable; and type IIIc, comminuted and nonreconstructable. A treatment-algorithm is presented. Undisplaced fracture

Management of Osteochondral Lesions of the Talus.

Management strategies for symptomatic osteochondral lesions of the talus are primarily surgical. Treatment options for symptomatic osteochondral lesions of the talus most commonly include bone marrow stimulation techniques, osteochondral autograft transplantation, osteochondral allograft transplantation, autologous chondrocyte implantation, matrix-induced autologous chondrocyte implantation, and particulated juvenile articular cartilage. The selection of the most appropriate treatment option should be based on the specifics of a talar lesion, in particular, lesion size.

Fractures of the Talus: State of the Art.

UNLABELLED: Talus fractures occur rarely but are often associated with complications and functional limitations. Urgent reduction of associated dislocations is recommended with open reduction and internal fixation of displaced fractures when adjacent soft tissue injury permits. Delayed definitive fixation may reduce the risks of wound complications and infections. Restoration of articular and axial alignment is necessary to optimize ankle and hindfoot function. Despite this, posttraumatic arthrosis occurs frequently after talar neck and body fractures, especially with comminution of the talar

Chance fracture of the talus and calcaneum.

Combined talus and calcaneum fractures are rare in themselves. We report the case of an unusual fracture pattern of the talus and calcaneum in an otherwise, healthy 17-year-old male. Plain radiographic images of the foot revealed a fracture through the neck of the talus. Additional computed tomography imaging showed the fracture line extending through the talus and entire calcaneum. The fracture was open and associated with rupture of the tibialis anterior and extensor hallucis longus tendons. The patient underwent debridement with open reduction and internal fixation of the talar component of

Bilateral osteochondral lesions of the talus.

BACKGROUND: Osteochondral lesions of the talus (OLT) are relatively uncommon but may be a cause of significant pain and disability. Although the majority of patients have an osteochondral lesion of the talus that is unilateral, bilateral involvement has been reported in 10% to 25% of cases. In addition, factors that cause one side to be symptomatic and the contralateral side to be asymptomatic have never been evaluated. MATERIALS AND METHODS: A database containing all patients at our institution with an OLT has been maintained for the past 23 years. This was reviewed and patients with bilater

Juvenile osteochondritis dissecans of the talus.

BACKGROUND: The literature on the best treatment of osteochondritis dissecans of the talus (OCDT) in children is scarce because of rarity of the condition. In addition, patients with this condition typically become asymptomatic long before radiographic healing is complete that might give a false perception of clinical success. We determined the healing rate after 6 months of nonoperative treatment of OCDT in skeletally immature patients. STUDY DESIGN: Retrospective review. METHODS: After institutional review board approval, a retrospective review of clinical and radiographic records of child

The extruded talus: results of reimplantation.

BACKGROUND: There is considerable debate regarding the appropriate treatment of the extruded talus regardless of the presence of a fracture. The purpose of this study was to report the clinical results, complications, and functional outcome following reimplantation of the traumatically extruded talus. METHODS: A database of 119 patients with an open injury of the talus occurring between 1995 and 2003 at a level-I trauma center was reviewed to identify patients with a complete talar extrusion. Demographic, imaging, and treatment data were obtained from a review of the medical records. Follow-u

An unusual fracture of the talus in a snowboarder.

Fractures of the talus are uncommon. However, snow- boarding and skateboarding are 2 activities that are specifically associated with talus fractures. These patients sustain occult lateral talus process fractures that present as a severe ankle injury. The diagnosis is difficult because of subtle clinical and plain radiographic findings. Computed tomography is a very useful tool for the assessment of these injuries. Although the majority of these athletes have lateral sided talus fractures, there are variants. We present an unusual case of a displaced intra-articular fracture of the subtalar jo

Intraosseous gouty invasion of the talus.

A 34-year-old man presents with a cystic lesion of the talus that has developed over the last 3 years and a history of a single first metatarsophalangeal attack of gout. Radiodiagnostic studies confirmed the cyst and an associated osteochondritic lesion but showed no evidence of avascular necrosis. At arthroscopy, chalk-like material was removed from the cyst, which was histologically compatible with tophaceous gout. The lesion was debrided, and the patient was then treated with antihyperuricemic medication; radiographic evidence of cyst resorption was seen after 9 months of treatment.

Ipsilateral fractures of the talus and calcaneus.

Nine ipsilateral fractures of the talus and calcaneus were treated at Tampa General Hospital between 1991 and 1994 and entered into the trauma registry of this level 1 trauma center. During this same period, a total of 78 talar fractures and 334 calcaneal fractures were entered into the registry. The patients who sustained this rare combined injury were studied retrospectively to characterize the fractures that occurred, examine the treatments instituted, and determine outcomes. Four patients had severe intra-articular damage of the subtalar joint surfaces and underwent either primary or delay

[Treatment of flake fracture of the talus].

This is a report on 18 osteochondral lesions of the talar dome after sprained ankles and ruptured lateral collateral ligaments of the ankle. They were classified according to Berndt & Harty. The treatment of choice in recent flake fractures is the reduction and glueing with fibrine. Very good results were obtained according to Weber. The recommended treatment of ancient flake fractures frequently occurring to elder patients is the removal of the fragment and the drilling of the subchondral bone.

Transchondral fractures of the dome of the talus.

Osteochondritis dissecans of the talus is a well documented entity, defined as a transchondral fracture of the dome of the talus. As a fracture, it should heal with immobilization; however, this lesion often causes severe disability and, more often than not, the fragment will not unite with conservative treatment. There is much debate concerning operative versus nonoperative treatment, and the purpose of this paper is to discuss the salient points of the controversy, review literature on the subject, and present a case with a follow-up of 4.5 years.

Primary subacute osteomyelitis of the talus.

In four children with primary subacute osteomyelitis of the talus seen one to five months from the onset of symptoms, the only constant complaints were of pain and a limp. All four were treated by curettage, immobilisation in plaster and appropriate antibiotics. All the bony cavities were healed within eight months of the operation and there were no growth disturbances nor any abnormalities of the adjacent joints.

Chondrosarcoma of the talus: a case report.

Although malignant cartilage tumors of the foot and especially of the talus are uncommon, the treating physician must become suspicious when a "simple ankle sprain" is resistant to standard treatment. In such a case, the comparison of serial radiographs may reveal subtle changes and the use of a radioactive bone scan may be of diagnostic value.
