Tarlov cysts are an uncommon and often incidentally noted source of low back pain in women. Because these cysts can be asymptomatic, they can be overlooked on radiological imaging. This case is of a 49-year-old woman who presented with a chronic history of low back pain and bilateral radiculopathy who on magnetic resonance imaging (MRI) was found to have multiple Tarlov cysts. This case illustrates the need for large observational studies to show the incidence of Tarlov cysts as a cause of low back pain in women.

BACKGROUND: Perineural cysts of the sacrum, or Tarlov cysts, are cerebrospinal fluid (CSF)-filled sacs that commonly occur at the intersection of the dorsal root ganglion and posterior nerve root in the lumbosacral spine. Although often asymptomatic, these cysts have the potential to produce significant symptoms, including pain, weakness, and/or bowel or bladder incontinence. We present a case in which the sacral roof is removed and reconstructed via plated laminoplasty and describe how this technique could be of potential use in maximizing outcomes.
METHODS: We describe technical aspects of

BACKGROUND AND OBJECTIVES: Intrasacral meningoceles (IM) are a type of spinal meningeal cyst that can develop within the sacral spinal canal and cause sacral radiculopathy symptoms. The research on IM is sparse, comprised largely of case reviews lacking standardization with validated measures. However, a disease-specific health-related quality-of-life scale has been published for Tarlov cysts to measure symptoms before and after surgical treatment. The purpose of this study was to validate the Tarlov Cyst Quality-of-Life (TCQoL) survey as a disease-specific health-related quality-of-life measu

OBJECT: Symptomatic sacral perineural cysts are extremely rare. The aim of this retrospective study is to investigate the outcome of 15 consecutive patients treated by microsurgical resection of the cyst and to review the literature.
METHODS: The authors retrospectively reviewed their clinical data archive from 2002 to 2014. Fifteen patients who were operated on due to symptomatic sacral perineural cysts were enrolled in the study. Patients' symptoms, radiographs, intra-operative findings, and clinical results were evaluated. All 15 patients underwent microsurgical excision of the cyst. The l

Extradural meningeal cysts are rare CSF-filled dural diverticula, which may complicate the provision of neuraxial anesthesia, and there is a paucity of literature to guide anesthetic management of pregnant patients presenting with extradural meningeal cysts. We report the safe provision of labor epidural analgesia to a parturient with a low-thoracic type I extradural meningeal cyst and review the literature describing this pathology.

OBJECTIVE: Perineural cysts, also known as Tarlov cysts, are cerebrospinal fluid-filled growths that develop at the intersection of a dorsal root ganglion and posterior nerve root. They are typically an asymptomatic and incidental finding during routine spine imaging. For symptomatic perineural cysts, there is little evidence on which treatment is most effective or when it is indicated. The aim of this study was to review our experience from a population-based cohort of patients with symptomatic perineural cysts and to propose an algorithm that could be used in the selection of surgical candid

Tarlov cysts commonly occur in women but are often thought to be asymptomatic findings. However, recent studies have suggested that Tarlov cysts can be a contributing factor to various pain and neurologic dysfunctions with a significant effect on health-related quality of life. Tarlov cysts and various intrapelvic pathologies can result in compression or impairment of peripheral and spinal nerve roots and can potentially affect women's sexual function, particularly by causing persistent genital arousal disorder. Persistent genital arousal disorder is characterized by spontaneous and often unre

Tarlov or perineural cysts are lesions of the nerve root most often found in the sacral region. Several authors recommend surgical treatment of symptomatic Tarlov cysts. However, successful surgical treatment is dependent on appropriate patient selection. In this article, we report three cases of a sacral perineural cyst, causing sciatic pain, and emphasize the usefulness of CT-guided percutaneous aspiration as an important diagnostic and prognostic procedure prior to definitive operative treatment.

BACKGROUND: Spinal perineural Tarlov's cysts (TCs) are considered incidental findings that occasionally might exert pressure upon nerve roots and correspond with patients' signs and symptoms. Purpose of this meta-analysis is to deliver global incidence and characteristics (location, size, and shape) of TCs.
METHODS: Following PRISMA checklist, all major databases were searched by two authors for radiologic studies reporting incidence and morphologic features (location, size, and shape) of TCs. Anatomical Quality Assessment tool was applied for risk of bias evaluation. Meta-analysis of random-

BACKGROUND: Giant Tarlov cysts are always symptomatic and mimic many serious pathologic entities. We present the largest, to our knowledge, reported Tarlov cyst.
CASE DESCRIPTION: A 33-year-old woman with Marfan syndrome suffered from right kidney hydronephrosis because of ureter obstruction, for which she was treated with nephrostomy. Her neurologic examination was unremarkable. The role of magnetic resonance imaging in the management of this case is described. Absence of intractranial hypotension symptoms after cerebrospinal fluid (CSF) overdrainage suggested the presence of a valve-like me

This article presents a case of unsuccessful pharmaceutical and invasive pain treatment for 6 years without any adequate diagnostics in a female suffering from unilateral thoracic radiculopathy (Th8, right) leading to severe disability and unemployment. The origin was an undetected Tarlov cyst. After resection of the cyst the pain and other complaints disappeared (follow up: 8 months) without need for further pain medication. This case underlines the necessity of adequate diagnostics ahead of long-term pain treatment. Thoracic Tarlov cysts are very uncommon but should be included in the differ

This illustrative case shows a meningocele created by erosion of the sacrum by a perineurial cyst. It is the first report of a large presacral mass consisting of a perineurial cyst and a meningocele simultaneously. We describe a hypothesis for the pathogenesis of this particular constellation of circumstances and present a successful surgical option based on the assumed pathogenesis.

BACKGROUND Tarlov cysts are rare, with a prevalence of 3.3% in the Asian population, and symptomatic cases are even rarer. Here, we report a case of a young woman with multiple Tarlov cysts presenting in primary care with severe low back pain. CASE REPORT A 23-year-old Malay woman presented to a primary care clinic with sudden-onset, severe, and persistent low back pain for 1 week, affecting her activities of daily living (ADL), especially as a medical student, as she could not stand for more than 10 minutes. There were no other associated symptoms or recent trauma prior to the onset of back p

Tarlov cysts, also known as perineural cysts, are cerebrospinal fluid-filled sacs that typically form near the dorsal root ganglion, most commonly at the sacral levels S2 and S3. While most are incidentally identified on imaging, symptomatic cysts, particularly in the lower lumbar and sacral regions, can produce neurological symptoms similar to those of a herniated disc and may have a clinically significant impact on urogenital function. We present the case of a 38-year-old woman with a long-standing history of chronic pelvic dysfunction characterized by recurrent urinary tract infections, uri

BackgroundTarlov cysts (TC), also known as perineural cysts are meningeal dilations of the posterior nerve root sheath that typically affect sacral nerve roots. TC are usually asymptomatic and found incidentally. We present the case of a patient with an enlarging sacral TC causing pain from spinopelvic instability secondary to extensive bone erosion. Such illustrative case is intended to increase awareness of the potential need for complex spinopelvic reconstruction in atypical instances of large TC.Case descriptionA 29-year-old female presented to clinic reporting progressive bilateral sacroi

BACKGROUND: Tarlov cysts (TCs) are expanded nerve root sheaths that occur near the dorsal root ganglion and result from increased intraspinal hydrostatic pressure. TCs most frequently affect the lumbosacral plexus and therefore may cause specific symptoms such as perineal pain and neurogenic bladder, bowel, and sphincter problems. It has been estimated that 1% of the population has symptomatic Tarlov cysts (STCs). However, STCs appear to be underdiagnosed, with the pain reported by patients commonly attributed to degenerative alterations seen on MRI. The aim of the present study is to investig
